TO ALL PLANHOLDERS: Notice is hereby given to all ITB Holders of record on this date that the following ADDENDUM NUMBER 1 shall supplement and supersede (if in conflict) all documents previously issued for the information of the respondents to the ITB. Questions: Q. Is low slope roof (patio) at garage getting re-roofed with shingles? A. No. It is getting pressure washed and repairs as per scope. Q. What is lead based paint process? A. All painted surfaces will be repainted as per scope and specs. Alternate Pricing: See Revised Bid Form In consideration of replacement of sewer line – see revised bid form. Alternate Price #1: See Revised Bid Form Revised Bid Form Attached NOTE: An additional walk-through will be held at the site on Tuesday, December 23, 2014 at 10:00 a.m. There will be no questions addressed at this walk-thru.

The Bidder declares that he has carefully examined the Contract Documents for the construction of the project, that he has personally inspected the site, that he has satisfied himself as to the quantities involved, including materials and equipment, and conditions of work involved, including the fact that the description of the quantities of work and materials, as included herein, is brief and is intended only to indicate the general nature of the work and to identify the said quantities with the detailed requirements of the Contract Documents, and that this Proposal is made according the provisions and under the terms of the Contract Documents, which Documents are hereby made a part of this Proposal. The Bidder further declares that he has exercised his own judgment regarding the interpretation, of subsurface information and has utilized all data, which he believes pertinent from City and other sources and has made such independent investigations as the Bidder deems necessary in arriving at his conclusions.

The Bidder further proposes to accept as full payment for the work proposed herein the amounts computed under the provisions of the Contract Documents and based on lump sum and unit price amounts, it being expressly understood that the unit prices are independent of the exact quantities involved. The Bidder agrees that the lump sum prices and the unit prices represent a true measure of the labor, services, and materials required to perform the work, including all allowances for overhead and profit for each type and unit of work called for in these Contract Documents. If any material, item, or service required by the Contract Documents has not been mentioned specifically, the same shall be furnished and placed with the understanding that the full cost to the Owner has been merged with prices named in the proposal.

Deductive Alternate #03: Delete: Remove and replace kitchen cabinets with new cabinets. Add: Repair existing cabinets; add shelving, doors and drawer fronts as needed. Replace hardware; hinges, handles and brackets. Refinish; sand, stain and apply two coats of clear lacquer. Color selection by owner.

ALTERNATE PRICING: A.

Alternate Price #01: Remove and replace patio roof and roof framing in lieu of pressure washing and repairing existing roof and framing. Post replacement will remain in Scope of Work. Roofing panels specified as Palruf clear corrugated PVC or equal. Fasteners to be hot galvanized gasket type. Refer to Section 00700 (General requirements) for a description of the Liquidated Damages associated with failure to complete specific work within the stipulated time frames.

Sales Tax: The Bidder is required to pay sales tax, on material only, in accordance with state law and as described elsewhere in these documents. The Bidder shall include sales tax in any Unit Costs and Amounts for each of the bid items. 5. If the undersigned Bidder is notified of the acceptance of this bid within Sixty 60) days after the time set for opening bids, it agrees to execute a contract for the above work, for a compensation computed from the above-proposed sum, in the form of the agreement bound in the contract documents, and to furnish performance and payment bond(s) as required by the contact document.

[FOR BIDS OVER $25,000.00, the paragraph below applies] The undersigned Bidder further agrees that the check, money order or surety bid bond accompanying this proposal will be left with the Owner, that its amount or penal sum will be the measure of liquidated damages which the Owner will sustain by the failure of the undersigned Bidder to execute and deliver the abovenamed contract and performance and payment bond(s), and that if the undersigned Bidder fails to deliver and said documents within ten (10) days of written notification of the award of the contract to it, then the check or money order shall become the property of the Owner or the sum of the surety bid bond shall be forfeited to the Owner; but, if this proposal is not accepted within thirty (30) days after the time set for opening bids, or if the undersigned Bidder executes and delivers said contract and performance and payment bond(s), the check or money order shall be returned to it or the surety bond shall become void.
