Organisational context Neami National is a community-based recovery and rehabilitation service supporting people living with mental illness and psychiatric disability to improve their health, live independently and pursue a life based on their own strengths, values and goals. We provide services in more than 50 locations, ranging from the inner-city and suburbs to regional and remote parts of Australia. Our vision is for full citizenship for all people living with a mental illness in Australian society. Our mission is to improve mental health and wellbeing in local communities across the country. We use an approach called the Collaborative Recovery Model (CRM) to support people through our services. PIR Consortium and position overview Central Adelaide and Adelaide Hills Partners in Recovery was established by a consortium. Under a new structure Primary Health Networks replaced the Medicare Locals on 30 June 2015. Neami National was nominated as the lead agency. The ultimate objective of PIR is to improve the system response and outcomes for people with severe and persistent mental illness by: •

Facilitating better coordination of services;



Strengthening partnerships with clinical and community support organisations;



Improving referral pathways and advocating for system change to meet individual client needs; and



Promoting a community based recovery model to underpin all clinical and community support services.

The Support Facilitator is responsible for facilitating the engagement of services and support for individuals who are eligible for PIR. The primary role of the position is to support clients to develop a PIR support plan based on each client's individual needs using a recovery oriented approach. The Support Facilitators actively engage with clients to coordinate existing services and supports to address the needs of people who meet the eligibility of the PIR program. This is primarily a facilitation and coordination role to seek solutions to complex systems in a resourceful manner.

Conditions and period of employment The Partners in Recovery program will run until the 30 June 2016. Successful applicants will be employed directly by the agencies in the PIR consortium, not Neami. The agencies are: •

The base salary will be in the range $62,244 - $65,010 pa (pro rata), to be negotiated with the employing agency. •

Accountability The Support Facilitator (PIR) is accountable to a line manager within the host agency and also has a reporting relationship to the Partners in Recovery (PIR) Coordinator(s) employed by Neami as the lead agency.

Operating in a way that is consistent with the principles of recovery Developing productive and supportive relationships with clients and their existing support network Undertaking ongoing review of the needs of the PIR clients Developing an Action Plan with clients of PIR and conducting regular reviews of the Action Plan in consultation and collaboration with clients, service providers, carers and families, as appropriate, The PIR Action Plan will guide the necessary engagement and integration of required services Developing and maintaining local partnerships and relationships Identifying and connecting a PIR client to ongoing case-management early in the implementation of the PIR Action Plan Being a point of contact for PIR clients, their families and carers when service arrangements are not working or the client becomes disconnected from required support Engage with potential PIR consumers and their support networks Utilise different strategies to support engagement with consumers disconnected from the service system Assess individual consumer needs and collaboratively develop a PIR Action Plan that is implemented, monitored and reviewed regularly Engage with and work collaboratively with the existing care coordination activities that may already be in place Involve carers, family and friends as identified by the consumer in the development and implementation of the Action Plan Facilitate the engagement of services for PIR consumers, including primary health care services. Utilise conflict resolutions skills and participate in cross program/service coordination to resolve systemic issues Advocate for the consumer as needed or requested Contribute to service mapping and gap analysis in your area

About this role: The successful applicant for the Support Facilitator position will bring solid experience in the human services environment. In particular, they will demonstrate an understanding of how best to access, advocate for, and maintain services for consumers living with a mental illness who have complex needs. Degree level qualifications



Experience working with Aboriginal and Torres Strait Islander people



Knowledge of relevant State and Commonwealth Acts and Legislation relevant to people living with mental illness and/or disability



Knowledge of supported decision making for people who may live with a level of mental incapacity as defined by the Guardianship and Administration Act 1993



Knowledge of National Mental Health Standards

In addition you will need: •

Computer literacy



Current Australian driver’s license



Tertiary qualifications in Mental Health, Disability or Community Services desirable



Demonstrated skills in working with clients with complex needs, and capacity to see consumers at the core of their focus.

About Neami National Neami National is a community mental health service supporting people living with mental illness to improve their health, live independently and pursue a life based on their own strengths, values and goals. We were founded in 1986 by a group of concerned residents wanting to support people in their local community.
