The Health Care committee [Vård- och omsorgsnämnden] In Sollentuna, the Health Care committee is responsible for support for people with some form of disability. However, the committee's remit does not include supporting people under 65 years of age with a mental disability. This is the responsibility of the Social Committee. The Health Care committee has a dual mandate: firstly, the committee is a client committee, which means that the committee uses different contractors to provide support, service or care to those who need it. Secondly, it is responsible for the municipal provider SOLOM.

The Health Care office [Vård- och omsorgskontoret] LSS case workers work at the Health Care office, receiving your application and answering your questions. (Although the municipality's contact centre can answer general questions about disabilities.) You can reach your Health Care office's LSS case worker and contact centre via the municipal switchboard, see the contact information on page 14.

Support in daily life At the Health Care committee in Sollentuna Municipality, you can apply for assistance under the Act concerning Support and Service for Persons with Certain Functional Impairments (LSS), the Social Services Act (SoL) and the Act on Housing Adaptation Grants (BAB).

The Act concerning Support and Service for Persons with Certain Functional Impairments (LSS) LSS is a law of individual rights that guarantees people with major and permanent disabilities good living conditions. LSS provides you with additional support and service beyond what you can get as a result of other legislation. Who can get help and support under LSS?

You are entitled to receive help under LSS if you fall under one of the following categories (see the points below). It also requires that you need support in your everyday life, and that the need is not met in any other way. 1. P  ersons with a developmental disorder, autism or autismlike condition 2. P  ersons who have suffered brain damage in adulthood as a result of external violence or a physical illness and thus have an intellectual impairment 3. Persons who have major and permanent physical or mental disabilities that are not due to normal aging and therefore require extensive support and services to cope with everyday life.

LSS assistance 1. Counselling and other personal support

Advice and support under LSS is a complement to other support measures provided by the health service and social services and may relate to support from, for instance, a psychologist, social worker, occupational therapist, speech therapist, physiotherapist, educational specialist or dietician. This type of support is available from the county council habilitation service. They also provide support for family members. Contact information can be found on page 14. 2. Personal assistance [Personlig assistans]

Personal assistance entails individual support if you, as a result of a major disability, need a lot of help and support daily. This might include help with personal hygiene, meals, dressing and undressing or communicating with others. You cannot apply for personal assistance or get an extension after you have turned 65. If you need personal assistance for basic needs for less than 20 hours per week, you should apply for it to the municipality. If you need more than 20 hours per week, apply for an assistance allowance from the Swedish Social Insurance Agency. Contact information can be found on page 14. 3. Companion service [Ledsagarservice]

The companion service will make it easier for you to participate in community life if you have a severe disability. This may involve, for example, visiting friends, participating in recreational activities or cultural life or going for a walk. The aim is to reduce the isolation that can be a result of a severe disability.

4. Contact person [Kontaktperson]

A contact person should be able to advise you in everyday situations. An important task is to help reduce isolation through social contact and helping with recreational activities. A contact person must be a fellow human being who can replace or supplement contact with relatives. Such efforts should be seen as non-professional support given by a person with great enthusiasm and an interest in other people. 5. Respite service at home [Avlösarservice i hemmet]

The respite service at home means that a person temporarily takes over your care responsibilities as a parent. If you are a parent of a child with a disability, you may need assistance with respite to be able to relax or to complete activities in which your child is not involved. Respite may be a prerequisite for you as a parent to be able to dedicate time to the siblings of a child with a disability. Respite also helps you as a parent to attend training and to participate in treatment. This type of support is primarily offered in the evenings and at weekends. 6. Short stays away from home [Korttidsvistelse utanför det egna hemmet]

Short stays are for children or adolescents with a disability. Short stays can be offered away from home, with another family, in a respite home or at a camp. This type of help can serve as relief for family members, as well as act as a stimulus for the child/adolescent. Short stays can also be an aid in breaking a dependency relationship between children and parents. 7.Temporary supervision for school children over 12 years [Korttidstillsyn för skolungdom över 12 år]

School children over 12 years who have a disability and who have working parents can get temporary supervision before and after school hours and during holidays. Children will be offered a safe environment and meaningful activities.

8. Accommodation in a family home or housing with special services for children [Boende i familjehem eller bostad med särskild service för barn]

Most children and adolescents with a disability live with their family. If daily care becomes too difficult for the family, the child/adolescent can stay in a family home or a children's home and get the care and attention they need there. This should be seen as complementary to the parental home. 9. Housing with special services for adults [Bostad med särskild service för vuxna]

If you are an adult with a disability that means you are unable to live in an ordinary apartment, you may be offered accommodation in housing with special services. This type of accommodation takes the form of high-quality apartments where you pay for rent, food, clothing, hygiene items and activities yourself. There are communal areas. Staff are on site 24 hours a day to provide care and support for recreational activities. • Group home [Gruppbostad]

Group homes are for people with extensive care needs.

• Service home [Serviceboende]

A service home is a half-way point between a group home and private accommodation.

Other specially-adapted housing [Annan särskilt anpassad bostad]

Other specially-adapted housing is intended for people who can cope with living on their own, but who need an apartment which has been adapted according to their physical needs. There are no staff attached to the apartment. 10. Daily activities [Daglig verksamhet]

Daily activities is a form of employment that corresponds to going to work every day. To be entitled to daily activities, you must fall within category 1 or 2 (see page 5). In addition, you must be of working age and not be available to the labour market or be studying.

Individual plan [Individuell plan] If you are granted LSS assistance, you can request an individual plan. Your case worker will draw up the plan in consultation with you. The aim is to coordinate or provide an overview of the assistance and support you have, or may need in the future. The aim is also to help you to improve or change that support if required. What does it cost? In most cases, LSS assistance is provided free of charge, but there are a few exceptions: • If you live in housing with special services, you pay the rent and the cost of food yourself. • If you go on a short stay and/or require temporary supervision, you pay the cost of food yourself. • If children receive care in a home other than their own, the municipality will charge a fee. This will be assessed individually.

The Social Services Act (SoL) The Social Services Act provides basic support to people who need support and services in their everyday life. The municipality may, for instance, offer you home-help service and housing assistance. Home-help service [Hemtjänst] The home-help service provides you with support in your own home. The goal of the home-help service is to make it easier for you to live an independent life. Home-help service may include: • Personal care • Meals • Looking after the home • Shopping and errands • Activities

To qualify for the home-help service, you must have a need for help. How much help you can get may vary from a few hours a month to help several times a day. The help is adapted to your needs. What does it cost to use the home-help service?

The municipality charges a fee for home-help service. Information on fees is available in a separate brochure. Please ask your case worker. Living support [Boendestöd] The purpose of living support is to increase your ability to function and help you become more independent. Living support is carried out together with you and is primarily provided in or close to your home. The support can include motivation and activation measures to provide support for dressing, eating, taking care of hygiene or communicating. The support can also help you to structure daily life and to develop social skills to cope in society. You can get help running errands and shopping, hanging out with friends or going off on activities.

Housing adaptation grants [Bostadsanpassningsbidrag]

(In accordance with the Act on Housing Adaptation Grants, BAB) If due to illness or disability you need your home adapted, you can apply for a housing adaptation grant. The purpose of the grant is to allow you to live independently in your own home. In order to get the grant, your disability must be permanent. There must also be a clear link between your disability and the purpose of your application for a housing adaptation grant. Housing adaptation grants can be granted for the adaptation of the permanent features of the home, such as the removal of thresholds and bathtubs, installing ramps and handrails, or installing stair lifts or automatic door openers.

You should send your application to the municipal housing adaptation officer. Along with the application, please enclose a certificate from an occupational therapist or other expert stating that the measure is necessary with regard to your disability. Mobility service [Färdtjänst] If, as a result of a disability, you cannot travel by public transport, you may be able to get help from the mobility service. The county council makes decisions on and is responsible for the mobility service, but it is the municipality's contact centre that receives your application and examines your needs. You can contact the contact centre through the municipal switchboard. Contact details can be found on page 14.

National mobility service [Riksfärdtjänst] The national mobility service can provide help if you have a significant and permanent disability, and therefore need to travel in a particularly costly way. Your LSS case worker at the municipality investigates and decides on national mobility service requirements. Contact information for Sollentuna Municipality can be found on page 14.

Your application How to apply If you want to apply for help and support or get more information, contact your LSS case worker at the Health Care office. What happens to your application? The LSS case worker, in consultation with you, will investigate what your needs are and what help you need. In order to obtain LSS assistance, the LSS case worker must receive a medical certificate confirming your diagnosis. Often, a certificate from a psychologist, speech therapist or occupational therapist may also be necessary. In addition, we often visit the home, preschool, school, or habilitation centre. The case worker will then assess whether or not there is a need for the help you have applied for. The decision will be sent to you by post. If your application has been granted, the LSS case worker will contact the provider who will be responsible for your support. For some types of assistance, you can choose which provider will be responsible for your support. Read more about this in the brochure Information till dig som ska välja eller byta utförare. Implementation plan [Genomförandeplan] Once you have been granted assistance, the provider will draw up an implementation plan setting out how your help will be implemented. The implementation plan is an agreement between you and the provider and will serve as a tool for staff in their work. The purpose of the implementation plan is to create a clear structure for the practical implementation and monitoring of the assistance provided to you. You have the right to appeal the decision If you are not satisfied with the decision reached by your LSS case worker, you may appeal the decision. This can be done at the Administrative Court. The appeal must be submitted in writing and sent to the Health Care office within three weeks of the date you received the decision. Even if you are not entitled to LSS assistance, you may be entitled to help under the Social Services Act (SoL).

Useful information Dental care support [Tandvårdsstöd] If you have been granted LSS assistance, you are entitled to a dental care support certificate. This provides high-cost protection for necessary dental care if you are over 19 years old. The dental care support certificate entitles you to receive an oral health assessment and individual advice. Contact your LSS case worker to receive a dental care support certificate. Support for relatives [Anhörigstöd] If you are a relative of a person with a disability, the municipality can offer various forms of support. Contact the municipal relations liaison officer or talk to your LSS case worker for more information. Contact information for Sollentuna Municipality can be found on page 14. Confidentiality Everyone working within the health and social care sector is bound by a duty of confidentiality. This means they cannot talk to any unauthorised person about you or your personal circumstances. Any written document about you, the support you receive and your health always has to be dealt with in confidence. This is prescribed in the Swedish Secrecy Act. Comments and complaints If you have any comments or complaints, you can contact the manager of the department involved. You can also contact the Health Care office, who will then pass your comments on to the relevant department. Comments can be submitted both in writing and verbally.
