Executive Summary The Wyoming Mental Health and Substance Abuse Services Division (the Division) strives to provide a broad array of mental health and substance abuse services to meet the needs of the citizens of Wyoming. These services are designed as a continuum of care to address each client’s unique needs. In order to provide continuous quality improvement, the Division utilizes client satisfaction surveys to ensure access, quality of care, and client outcomes. Analyzing client feedback promotes access to services and the development of quality services. This information helps managers, staff, and clients understand the service delivery system and demonstrate that clients are achieving positive outcomes. Clients from across the state provided input in the summer and fall of 2009 on their perception of access to services, quality of services, treatment planning, cultural sensitivity of staff, and overall satisfaction with services received. They also provided information on outcomes achieved as a result of the services that they received. This report provides information collected from youth and adults receiving substance abuse services. Overall, clients are satisfied with the substance abuse services they received from the Community Mental Health Centers/Substance Abuse Centers (CMHC/SACs) throughout Wyoming. The centers collected surveys from 881 substance abuse clients. Clients reported good access to services, positive responses to quality and appropriateness of services, and high ratings for social connectedness.

Methodology The Wyoming Mental Health and Substance Abuse Services Division (the Division) is transforming the service delivery system to better meet the needs of the citizens of Wyoming. As the service delivery system expands and transforms, the Division is utilizing client satisfaction surveys to obtain information on the client’s perception of services. This information provides feedback to stakeholders to help ensure access, quality of care, and clientlevel outcomes. The Division strives to assist clients and family members to be the ultimate decision makers regarding the services that they receive. Each year, mental health and substance abuse clients are asked to provide feedback on services received from Community Mental Health Centers (CMHCs) and Substance Abuse Centers (SACs). To gather this information regarding client satisfaction, the Division developed a survey process in collaboration with two contracted agencies. The Division contracted with UPLIFT (a family-based organization) for survey administration and data entry, and with IDEA Consulting for survey analysis and reporting. Survey instruments were developed through an extensive review process to ensure that quality indicators of specific relevance to Wyoming’s public mental health and substance abuse systems were measured, and to ensure data comparability with national quality benchmarks. Wyoming, as well as other states receiving federal block grant funding through SAMHSA, adapted the most recent versions of the federal Mental Health Statistics Improvement Program (MHSIP) surveys as the foundation for the Wyoming substance abuse services surveys. The substance abuse survey was administered to youth and adults enrolled in substance abuse services. A sample survey instrument is provided in Appendix B. Survey questions cover several different domains, which are described in further detail below. Survey respondents answered several demographic questions before being asked to agree or disagree with statements about the services that they received (e.g., “services were available at times that were good for me”) and personal outcomes (e.g., “as a direct result of the services I received in the past 6 months, I do better in social situations”). Statements were rated on a fivepoint Likert-type scale: Strongly Agree, Agree, Neutral, Disagree, and Strongly Disagree. Respondents could also choose “N/A” for those survey statements that did not apply to them. There are twenty-one CMHCs/SACs in Wyoming, and all of them participate in the annual client survey process. In some CHMCs/SACs, surveys were administered by peer advocates and volunteers who received training on survey administration. In other CMHCs/SACs, clinical or clerical staff administered the surveys. Survey administrators asked clients to complete the survey when they arrived at the center to receive services during a specific block of time between July and September, 2009. The target population for survey participants included all clients receiving services at the CMHC/SAC, regardless of funding source. Completed surveys were sent to UPLIFT, who provided data entry services. The data was then submitted to IDEA Consulting for analysis and report preparation.

Substance Abuse Domains The client survey obtains client feedback on a broad range of areas which have been categorized into several different domains. These domains are outlined below. Access. Questions regarding access to services provide information on the timeliness of services, convenience of service location, and waiting time to get an initial appointment. Quality and Appropriateness of Services. Clients provide feedback on the quality of services and if the services met their needs. Treatment Planning. Information on the level of involvement with treatment planning provides feedback on the client and family member’s participation in helping plan service goals and making decisions regarding treatment. Cultural Sensitivity. Respondents provide feedback on the respect and sensitivity of staff regarding the client’s culture. Outcomes. Clients provide information on whether the treatment they receive is helping them get along better with others, live in a safe and stable environment, obtain education or employment, and improve their health. Substance Use. Respondents rate statements regarding alcohol and/or drug use. Social Connectedness. These statements provide information regarding the availability of a social support system and network of family and friends who are supportive of the client’s recovery. Satisfaction. Questions in this domain provide a general overview of the client’s satisfaction with services received. The following report summarizes data on the youth and adult client’s perception of services across these different domains. The results from the substance abuse surveys (across all ages) are presented here. This data is shown by fiscal year. For example, FY 2010 reflects information collected between July 1, 2009 and June 30, 2010. Please note that the number of respondents for each question may vary slightly. This is due to the fact that some clients did not respond to all survey questions.

Statewide Substance Abuse Client Survey Results The following pages show the results of the surveys completed by clients of all ages who receive substance abuse services. As shown in Figure 1 below, substance abuse clients completed 881 surveys throughout the state of Wyoming. Every Substance Abuse Center (SAC) had clients complete surveys. Figure 1 also shows the number and percent of substance abuse participants by agency. Curran-Seeley had the most survey respondents, with 99 surveys completed (11% of the survey population). In contrast, Cloud Peak had nine survey respondents (1% of the survey population). In all of the graphs in this report that present results by agency, the agencies are arranged by the size of the county population, from smallest to largest population. Figure 2 shows the number of substance abuse clients who responded to the survey from each CMHC/SAC for the last three fiscal years. As shown, participation at most centers was lowest in FY 2008, and highest in FY 2009 (e.g., Volunteers of America, Solutions for Life, Carbon, Curran-Seeley, Cedar Mountain, Fremont, Southwest, Northern, and Peak). The total number of surveys completed was highest in FY 2009 with 1,006 surveys; this number dropped to 881 in FY 2010. Figure 3 shows the number and percent of substance abuse participants by age. Of the 858 respondents who provided their age, most were adults between the ages of 25-34 (272 participants; 32%), ages 18-24 (200 participants; 23%), and ages 35-44 (151 participants; 18%). Youth ages 0-17 made up seven percent, with 60 respondents. Finally, there were 5 participants who were older adults ages 65+. Figure 5 shows the number and percent of substance abuse participants by race/ethnicity. As shown, the majority of participants were Caucasian/White, with 702 participants (80% of the population). Latinos made up 10% of respondents with 91 participants; similarly, Native Americans made up 7% of respondents, with 57 individuals. African Americans made up 1% of the population with 12 participants and Others made up 1% of the population (9 respondents). The smallest group were Asians, with five respondents (1%). Figure 6 shows the survey results of substance abuse participants by domains. The highest rated domain was Social Connectedness, with 87% of substance abuse participants agreeing with the statements in this category. Figure 7 shows the survey results of substance abuse participants by domains across Fiscal Years 2008, 2009, and 2010. The graph shows the percent of clients who agreed with the statements in each domain. As shown, the results were relatively stable across the three fiscal years, although they were highest in FY 2008. Figure 8 (on pages 11 and 12) illustrates the number of substance abuse participants and the percentage who responded that they “agree” or “strongly agree” with each survey statement. Ninety-one percent (91%) of substance abuse clients agreed with the statement “Staff here believe I can grow, change, and recover”. In contrast, only 57% of substance abuse clients agreed with the statement “I, not staff, decided my treatment goals”. Summary and Conclusions Overall, clients are satisfied with the substance abuse services received from the CMHC/SACs throughout Wyoming. The centers collected surveys for 881 clients across all age groups. Clients reported positive feedback about staff, with high agreement on statements such as “Staff here believe I can grow, change, and recover” (91%), “Staff treat me with respect” (87%), and “Staff respected my religious/spiritual beliefs” (82%). Clients also reported positive responses to quality and appropriateness of services and social connectedness statements. Several client outcomes were also measured using this survey. Results showed that the statement “I am better able to handle things when they go wrong” received the highest rating, with 79% agreement. “I spend more time with people who do not use alcohol and/or drugs” also received a high rating, at 78%. In contrast, the statement “I get along better with my teachers” only received 59% agreement. Appendix A shows the results of these surveys for each CMHC/SAC, by domain. Across all CMHC/SACs, there was a low rate of agreement with the statements in the treatment planning domain. CMHC/SACs and stakeholders can utilize these results to help improve services.
