PURPOSE To appropriately recognize members of the association, who perform acts of distinct bravery and courage, exceptional service, or achievement to their Department, or public safety activities in the community?

AWARDS COMMITTEE Association members are appointed by the President to the committee and will review the applications and acts of each individual nominated. The committee will then select the person or persons who are to receive an award, and determine under which category they will be honored utilizing the criteria outlined in this program. The nominations shall be reviewed by the committee once a year between the last meeting in March and prior to the annual SMVFA convention in early May. The committee is not required to select an individual(s) for each award. The committee report will be due to the President on or before 30 April.

NOMINATIONS FOR AWARDS The President or Vice President or a Chief Officer of the member Department may make application for a member or citizen to receive an award. The application must include a statement, which outlines the person's eligibility for that award. The award recommendation shall be forwarded to the Awards Committee on or before the last SMVFA meeting in March. The Awards Committee will acknowledge receipt of the application if requested by the sender.

PRESENTATION OF AWARDS Awards will be presented at the annual SMVFA convention by the Awards Committee. There is no limit placed on the number of awards that may be made to any one individual, or the number awarded during any one year.

FIRE PREVENTION CLASS AWARDS AND ROLAND B. DUKE AWARD The Fire Prevention "Class" Awards are presented each year at the Annual Southern Maryland Volunteer Firemen's Association convention, to a department that does the most by "Class" for Fire Prevention in their community. The awards are for the department's effort and therefore, submission of the entry must be for the department, rather than an individual. The intent of these awards is to recognize the Fire Prevention activities performed by the department in their particular community. There are two categories for the Fire Prevention Awards.  

The first category is for a year round program in fire prevention. The second category is for a special project related to fire prevention. RULES

1. The recipient must be a member in good standing of the Southern Maryland Volunteer Firemen's Association. 2. Portfolios entered must be of the standard notebook/report size. 3. The first page must be on company letterhead, a protective cover is recommended. 4. The department shall forward complete detailed information on the work done by the department. Photographs and newspaper clippings must be dated and the source indicated. 5. The Letter of entry with "Class" (see classification table below) and related information must be submitted to the Awards Committee no later than last meeting March. 6. Dates of EMS activities shall cover the calendar year from January 1 through December 31. 7. The winner of the Roland B. Duke memorial award will be based on the best and most complete fire prevention entry of all "Class" submitted. CLASSIFICATION TABLE BASED ON POPULATION Year Round Program

EMERGENCY MEDICAL SERVICE AWARENESS AWARD The EMS Awareness "Class" Award is presented each year at the Annual Southern Maryland Volunteer Firemen's Association convention, to departments that do the most by "Class" in EMS awareness for their community. The awards are for the department's effort and therefore, submission of the entry must be for the department, rather than an individual. The intent of these awards is to recognize the EMS activities performed by the department in their particular community. There are two categories for the EMS Awareness Awards. 

The first category is for a year round program related to EMS Awareness.



The second category is for a special project related to EMS Awareness. RULES

1. The recipient must be a member in good standing of the Southern Maryland Volunteer Firemen's Association. 2. Portfolios entered must be of the standard notebook/report size. 3. The first page must be on company letterhead, a protective cover is recommended. 4. The department shall forward complete detailed information on the work done by the department. Photographs and newspaper clippings must be dated and the source indicated. 5. AWARD CRITERIA - INDIVIDUAL

W. David Gott Memorial Fire Prevention Award

The W. DAVID GOTT MEMORIAL AWARD is awarded each year at the annual Southern Maryland Volunteer Firemen's Association convention, to an individual who does the most in fire prevention for his or her community or county. This award is for individual effort and therefore, submission of the entry must be for a single individual, rather than a group or department. In addition, the intent of the award is to recognize fire prevention leadership and activities that the individual demonstrated within his or her particular community or county. RULES 1. The recipient must be a volunteer in a fire department or ladies auxiliary in the Charles, St. Mary's, or Calvert Counties. 2. He/she must be nominated by their fire department. Nomination must be on company letterhead and signed by either the president or chief of said department. 3. His/her fire department shall forward complete detailed information on the work done by the individual. Photographs and newspaper clippings must be dated and the sources indicated. 4. The nomination and related information must be in the hands of the Awards committee no later than the March meeting. 5. Nominees are subject to investigation by the committee. 6. Date of fire prevention activities should cover the calendar year from January 1 through December 31.

AWARD CRITERIA - EMS

LaVerne Stewart Memorial, EMS Excellence/Awareness Award

The LaVerne Stewart Memorial, EMS Excellence/Awareness Award is awarded each year at the annual Southern Maryland Volunteer Firemen's Association convention, to an individual who does the most in EMS excellence and awareness for his or her community or county. This award is for the individual effort and therefore, submission of the entry must be for a single individual, rather than a group or department. In addition, the intent of the award is to recognize EMS excellence and awareness leadership and activities that the individual demonstrated within his or her particular community or county. RULES 1. The recipient must be a volunteer in a Rescue Squad/ALS Unit or ladies auxiliary in the Charles, St. Mary's, or Calvert Counties. 2. He/she must be nominated by their Rescue Squad/ALS Unit. Nomination must be on company letterhead and signed by either the president or chief of said Rescue Squad/ALS Unit. 3. His/her Rescue Squad/ALS Unit shall forward complete detailed information on the work done by the individual. Photographs and newspaper clippings must be dated and the sources indicated. 4. The nomination and related information must be in the hands of the Awards committee no later than the March meeting. 5. Nominees are subject to investigation by the committee. 6. Date of EMS Excellence/Awareness activities should cover the calendar year from January 1 through December 31.

FRANK R. COTRUFO JR. MEMORIAL SAFETY AWARD

RULES

1. The recipient must be a member in good standing of the Southern Maryland Volunteer Firemen's Association. 2. Portfolios of accomplishments must be documented and preferably in a report format. 3. The first page must be on company letterhead, a protective cover is recommended. 4. The department shall forward complete detailed information on the work done by the individual and not the department. Photographs and newspaper clippings must be dated and the source indicated. 5. The letter must be submitted to the Awards Committee no later than last meeting March. 6. Dates of Safety activities shall cover the calendar year from January 1 through December 31. 7. The winner of the FRANK R. COTRUFO JR. MEMORIAL SAFETY AWARD memorial award will be based on the best and most complete entry of all nominees.

FIFTY YEARS of SERVICE AWARD 1.

This award is established for the recognition of Fifty (50) Years of Continuous and Faithful Service to the Fire Department and Rescue Squads of Southern Maryland.

2.

The names of individuals that are to be honored are limited to the Member Companies of the Southern Maryland Volunteer Firemen's Association.

3.

The names of individuals to be honored for this recognition may be received from the individual's Fire Department or Rescue Squad, any Company, Department or Squad in good standing in the Southern Maryland Volunteer Firemen's Association, the Executive Board of the Southern Maryland Volunteer Firemen's Association, or from an individual member of the Association.

4.

Names of individuals must be submitted to the Hall of Fame Committee in writing, with a brief description of why you feel the individual should be honored.

5.

Names of individuals submitted for this honor, must be submitted to the Chairperson of the Hall of Fame Committee no later that the March Meeting of the Southern Maryland Volunteer Firemen's Association.

6.

Each individual receiving this honor will receive a two certificates and a Commendation Ribbon.

FATHER ALOYSIUS NEWMAN CHAPLAIN'S AWARD

RULES 1.

Contribution of a spiritual nature to the department/squad/ALS Unit.

2.

Involvement with SMVFA i.e. attending meetings and supporting programs.

3.

Service in the community through church, community service, and charitable organizations.

4.

Other involvement with department/squad/ALS Unit.

5.

Length of service as member and as chaplain.

The citation will be submitted to Awards Committee of the Southern Maryland Volunteer Firemen's Association (SMVFA) at its business meeting on the last Monday of March prior to the annual SMVFA convention. The citation will be submitted on Fire Department, Rescue Squad or ALS letter head and signed by the President, Chief or both.

Past President Award This award will be given to each member who has served in the office of President. The award will be presented at the end of the President's term of office by the incoming President.

President's Awards The President's Award may be awarded to Department member based on criteria determined by the outgoing President.

Firefighter/EMT/Paramedic Lifesaving Award The Firefighter/EMT/Paramedic Lifesaving Award may be made to any member (s) of the Southern Maryland Volunteer Firemen’s Association who has been involved in a lifesaving effort at an emergency scene. This nomination will be by the Fire Department/Rescue Squad/ALS unit to which the personnel is assigned and must be on Department letterhead signed by a chief officer or president. A Life saving uniform ribbon will be presented at the SMVFA convention banquet.

Citizen Lifesaving Award The Citizen Lifesaving Award may be made to a person outside the Southern Maryland Volunteer Firemen’s Association who has been involved in a lifesaving effort at an emergency scene before the arrival of the Fire Department or who has provided extraordinary assistance to Fire Department members in a lifesaving effort. This nomination is normally made by Fire Department/Rescue Squad/ALS personnel assigned to the incident and must be on Department letterhead signed by a chief officer or president. A plaque will be presented at the SMVFA convention banquet.

This evaluation form is for all Fire Prevention/EMS Projects and the Duke award and the Gott, Stewart and Cotrufo individual awards. The scale of 1-5 (5 being the highest) will be used. The evaluation will be based on the following criteria:

Fire Prevention/EMS Projects and the Duke Award (Department Level)           

FP/EMS activities performed from Jan 1 - Dec 31. Number of entries submitted (per district). Class Level for Fire & EMS - (number of households served). Layout Quality - proper format - [book size, on 8 1/2x11 paper, on company letterhead with cover, table of contents, tabs, photos (dated), neatness, organization. etc.]. Photos and newspapers clippings dated. Photos backed up with documentation (see FP activity report). Sources cited. Number of projects/services performed. Variety of projects/services performed. Innovation and creativity in projects/services performed. Overall quality and level-of-performance of projects/services performed that address fire prevention and/or EMS activities.

Gott, Stewart and Cotrufo Award (Individual Award)  FP/EMS/ Safety activities performed from Jan 1 - Dec 31  Nominated by their Department on letterhead signed by President and Fire/Rescue/ALS Chief.  Layout Quality - Proper format - [book size, on 8 1/2x11 paper, on company letterhead with cover, table of contents, tabs, photos (dated), neatness, organization. etc.].  Documentation of their activities conducted, SOP's/manuals developed, etc.).  Photos and newspapers clippings dated.  Photos backed up with documentation.  Sources cited.  Number of projects/services performed.  Variety of projects/services performed.  Innovation and Creativity in projects/services performed.  Overall quality and level-of-performance of projects/services performed that address fire prevention/EMS/safety activities.
