Each of its 3 sites (Nashville at Southern Hills, Franklin and Columbia) provides healthcare services to the underserved and uninsured population. Services Offered: Primary Care (Adults and Children); Urgent- and Walk-In Care Requirements: All amounts due from the patient whether insured or uninsured can be discounted based on provided proof of household size and income. Offers pay-in-full discounts and deferred billing. Fees: Uninsured patients with income less than 200% of the federal poverty scale will pay $40 for the office visit and a minimum of $17 for lab work. If unable to pay, patient will be charged a minimum of $20 for services. If insured with income less than 200% of the federal poverty scale, patient will pay a $7 co-pay. Traditional Medicare patients with income less than 200% of the federal poverty scale will not pay a deductible and co-insurance will be reduced to $15.
