Setting up a Portal Account and accessing your Occupational Health Portal to complete a Questionnaire The portal is the new electronic system for completing forms that are sent to occupational health. Once you have set up your portal account please keep the details safe as you may need to login in the future to complete another questionnaire for another process or to run an immunisation history report for yourself. If you have received any immunisations or blood tests in occupational health you can access this information for yourself through the blue report tab in the portal.

Before you start your questionnaire, please ensure you have given yourself enough time to sit and complete the task in one sitting. If you stop and start the system will lock you out and you may have to start again. If you have any difficulties please email [email protected]

PORTAL SETUP The link below will take you to the front page of the Occupational Health Portal. https://northampton.warwickicsystems.com/GenohsisPortal

From here if you have not already set up an account, please click the Sign up now button.

You will need a valid email address, that you have access to, in order to create a new account. The system may not open on all hand held devices a laptop or desktop PC is preferable. For the Company Reference please copy and paste the following code U51624C1

Your Password will need to be at least 6 characters long including one upper case letter and one special character (!£$%&@* etc.). Once you have completed the details click continue, you will now be sent a verification email.

If the verification email does not come back to you within a short time please check your spam email box, if you still do not receive it please email [email protected] who can verify it for you. (There are sometimes issues with Hotmail and Gmail accounts)

A link in the email will return you to the Portal Login page where you will need to enter your newly created Password

You will be directed to complete your personal details, please complete as many fields as you are able, and then click Next.

It is important you use standard English with upper and lower case letters, as how you enter this data will populate our database and be used on any future correspondence with you. Please do not use capital letters for all the text.

Complete your address details and then click Next.

You should be asked to complete your employment details. If so please enter todays date as the Start Date as it will lock you out if you put a date in the future

Then click Save Changes

You can now use the Create New option on the left to start the University Health Questionnaire.

Once you have clicked create new online form the questionnaire should appear.

Questionnaire details Please go through the questionnaire systematically answering all the questions – there are some mandatory questions that you need to answer before it will let you continue.

If you close the cross in the picture below (in the red circle) the side bar will close and give you a bigger screen

Once you have completed the questionnaire attach any immunisation information using the attachment button on the left this can be scanned or photographed and attached. Once you have finished you will need to save it and then the questionnaire is automatically sent to our live system. If you cannot attach your supporting information please scan or photograph it and email it to [email protected] Once you have completed your form – press the save form button and the form will come directly to Occupational Health.

Other features of the portal once you have started your course, and had appointments booked and immunisations done. Future Appointments If you click onto the blue tab (click the X to close the side bar and you will see the full page) you can run a report/preview using the drop down menu on the left which will show you your future appointments in occupational health

Vaccination History You can also choose to view your vaccination history by choosing the personal inoculation history report and run/preview report (this will only show new vaccinations and blood tests you have had since starting at NGH and using the portal for health clearance – it does not show any historical data)

Change Personal Details/Contact Details /Change Password By clicking on the purple tab you can chose to change your personal details or contact details or change your password

Frequently asked questions:

1. Why have I not received my verification email? The verification email to set up the login has trouble with some email addresses such as Hotmail and Gmail. Check your ‘spam folder’ and if you haven’t received it within an hour ( normally a few minutes’) then email [email protected] and she will verify it for you.

2. Why has the system locked me out and told me I don’t have an employment record? You need to put your employment start date as the day you started the questionnaire as the system won’t recognise you until that date. If you do this accidentally then email [email protected] and she will be able to reset the date.

3. Why am I not allowed to write everything in capitals? What you write in your personal details fields will automatically populate your details in our system; this means that any emails or letters sent to you will use those details in the form you have written it in. 4. If I have a problem setting up my login can I start again with another email address? Please use your personal email address and follow the process through as per the instructions. If you have a problem please email us, please do not start another login with a different email. Each time you do that the system will create another personnel record for you and creates work for the admin team to clean out unwanted data.
