Introduction This guide provides information for parents, carers and families of people with learning disabilities. Professionals with an interest in learning disability issues may also find this guide helpful. Organisations that provide relevant local services are included along with national organisations that have helplines and websites offering information and support. Telephone numbers, postal addresses, email addresses and websites are included wherever possible so that you can get more information about each organisation. If you do not have access to the Internet at home you can visit your local library and access it free of charge. This guide has been produced by Rhondda Cynon Taf County Borough Council in consultation with parents, carers and professionals from the Council, the Local Health Board and the Rhondda Cynon Taf Learning Disabilities Partnership Group. Introduction to Council Services (see individual chapters for other services) We believe that everyone with a learning disability is capable of considerable personal development with the correct support and opportunities. People with learning disabilities have the right to: • an ordinary pattern of life within the community. • be treated as an individual. • get extra help and support to achieve their full potential.

Learning Disability Services We have two Community Support Teams for adults, which are made up of groups of people with a variety of skills from a Health, Community Care and Education background. Community Support Team West serves the Rhondda Valleys and Pontyclun areas. Community Support Team East serves Cynon Valley, west as far as Llantwit Fardre and south as far as Taffs Well. Disabled Children’s Team based in Tonypandy that covers the whole of Rhondda Cynon Taf.

Transition between children’s and adult services All of us have to make transitions in our lives: child to adult; school to work; adult to parent. For many people with learning disabilities and their carers, these transitions are particularly big steps, which can provoke worries and anxieties. We know this and so we try to make these changes manageable for all concerned. To do this the Disabled Children’s Team (who work with young people up to the

age of 18) and the Community Support Teams (who work with adults) work together to plan for the transition of each young person as they move into services provided for adults. This involves discussion with the young person, their carers and the professionals involved in their support, to work out the best and most appropriate way forward. These plans start early, particularly for young people who have many needs, so that we can encourage young people to try out new opportunities which we hope will encourage them to develop their skills and independence, and promote their role in the community. The Community Support Teams and the Disabled Children’s Team have specialist Transition Workers to help this process along. They can advise on the range of options that are available and encourage all interested parties to work together to plan for changes. A booklet ‘A Parent / Carer’s Guide to Transition and Transition Planning’ is available from schools and social workers.

Assessment of Need Services are provided to individuals and their carers on the basis of an assessment of their needs. In other words a social worker, nurse or assessment care manager will gather information about the needs and circumstances of the person with learning disabilities in order to establish if he/she is eligible for services, and if so, to advise about the services we can offer. This will involve looking at the things the young person enjoys doing, their strengths and the difficulties they or their carer may be experiencing. Nothing will be arranged unless the individual agrees to it.

Individual Care Plan The Care Plan is the documents that we use to set out the services that we have agreed to provide. They are put together on an individual basis in order to ensure they meet the specific needs of each person as best we can. This Plan is reviewed on a regular basis to ensure it continues to meet those needs. As needs change, the plan will change.

How Can I Get Help? Anyone can ask for our services. You can contact us directly through the Community Support Teams or Disabled Children’s Team (see end of this chapter for the telephone numbers). It will then be arranged for a member of staff to complete an assessment to confirm eligibility for services and, subject to eligibility, determine the most appropriate support.

What Help is Available? Services are arranged by our two Community Support Teams and the Disabled Children’s Team. Each team is made up of staff with a variety of skills from a Health, Community Care and Education background. This way of working enables individuals and their families to benefit from a broad range of specialist services, designed to support anyone eligible to receive our services in leading an independent and enjoyable life. We can help with things like: • practical advice at home • arranging short stays away from home (for example with Adult Family Placements or in Respite Care Houses) • offering support in the home • planning for the future • coping with difficult behaviour • planning for daytime opportunities • meeting specialised health needs, like psychiatry, clinical psychology, physiotherapy and speech therapy • advising on leisure time pursuits • advising on play opportunities • discussing educational and / or training needs • providing supported living • helping to find and hold down a job, with support if needed • supporting the development of independent Advocacy Services which help people with a learning disability make their views and wishes known • providing information about other things that might be of interest to you • supporting the people that care for our service users

Services for Children and Young People The 1 in 7 and Family Support team is part of the Disabled Children’s Service and works in partnership with Health, Education and voluntary organisations. The support provided by the service aims to assist disabled children and young people – and their families – to overcome barriers, be aware of their rights and take a more active role in their community. The service focuses on support after school, on weekends and during school holidays enabling children and young people to access facilities in their local community. The service has also developed short break schemes, sitting services (a person to sit with a child for family and carers to take a break) and provision of aids and equipment.

Children under 4 years The Under 4’s Surestart Scheme operates in partnership with Rhondda Cynon Taf Early Years and the Surestart team. The team works in partnership with social workers, health workers, preschool projects and community based projects and can provide practical help and advice with: • • • • • • • • •

meeting a child’s health, learning and social needs managing behaviour speech and language development play skills supporting physiotherapy programmes supporting children to communicate their needs more effectively preparation for full-time education participation in community activities planned rehabilitation programmes

A home visiting educational service is also available for pre-school age children who have special needs. This is known as the Portage Service. This service assesses young children with disabilities and then, in partnership with parents, builds on the abilities the child already has, teaching skills the child has yet to master.

Disabled Children’s Index We are required by law to keep a register of disabled children and young people that we refer to as an Index. This provides information about disabled children and young people living in Rhondda Cynon Taf and helps us to: • plan and develop future services • provide parents, carers, children and young people with up-todate information about services that we can offer and about other activities available in your area. • include you in consultations about services You will be asked if you would like to register your child but registration is not compulsory and families cannot be included in the Index without their written consent. Your services will not be affected if you decide not to be included in the index.

Daytime Opportunities Our Daytime Opportunity Service works to enable people with learning disabilities to develop their skills and abilities in many areas of their lives, including building relationships, making choices, achieving independence and becoming valued members of the community.

The service can offer: • work experiences which give people with learning disabilities opportunities to try out a range of work placements to help them decide where their skills are best suited • work placement with the required support arranged • leisure activities which are arranged at a variety of centres, offering indoor and outdoor pursuits Our Daytime Opportunities community units and tutor groups discuss, agree and develop programme plans to meet each individual's needs. These plans will help us to: • promote individual development and self esteem through taking part in meaningful, age appropriate, community based activities • offer a variety of activities to everyone using the service, regardless of their individual skills level • develop skills teaching programmes which increase individual independence and confidence, enabling a fuller participation in the running of their homes and more integration in community activities • encourage awareness of everyone's rights and responsibilities in the community Personal programmes are revised and reviewed regularly to ensure effective work continues with each individual service user.

Day Centre Facilities Our day services are provided either at our three main day centres or in a number of smaller local units. Service users also have opportunities to visit leisure centres, adult education classes and work experience opportunities from these units. In recent years, considerable progress has been made in moving from a large day centre based service to a more community based service, enabling our service users to make use of local facilities. The day service also works closely with the Education Service, Pontypridd and Rhondda College, Coleg Morgannwg and Glamorgan University's summer school in supporting individuals to attend a range of life skills classes. A number of people with learning disabilities wish to go on to find paid employment or work based activities. We have, in partnership with other agencies and local employers, made good progress in developing employment and training opportunities for some of our service users. Our three main day centres are : Llwynypia Day Centre, Llwynypia Road, Llwynypia, Tonypandy Cynon Valley Independent Living Resource Centre, Maelgwyn Terrace, Gadlys, Aberdare Treforest Day Centre, Factory G7, Treforest Industrial Estate, Treforest, Pontypridd

Community Living Service The Community Living Service comprises three schemes: The Independent Living Service provides targeted support to individuals who live in a home of their own without the direct support of their families. The Supported Living Service, provided in partnership with a number of independent agencies, helps people to live in their own homes. Each individual has their own tenancy of their house and receives the support they need, including overnight support, to enable them to be good tenants, pay their bills and generally take part in running their home. Some people receive support to enable them to continue to live in their family home after the death of a carer. The Respite Care Service gives access to five respite care houses in Rhondda Cynon Taf. A short stay in one of the houses can provide a welcome break for both our service users and their carers. During their stay individuals are encouraged to learn and practice new skills that may be used to develop greater independence in readiness for moving on from the family home to a more independent living situation.

Your Rights We know that your circumstances may be very personal. Everything you tell us will be treated in strict confidence, in accordance with the Data Protection Act. We will keep records of our meetings with you on file, which you have a right to see. We can tell you how to go about this. We try hard to meet your expectations by providing the best possible care to meet your needs. We understand that you may not agree with everything we say or do and sometimes things might go wrong. You have every right to question any decision we make

and to let us know if you are not happy with anything. Finding out if you are pleased or disappointed with the service we provide will help us to improve the way we help you and other people in the future. Ask any member of staff about letting us know how you feel about our services. Alternatively, we have a leaflet that can tell you more about our Complaints Unit and how to make a comment or complaint about our services.

Charges for Services Some of our services may involve a charge (children under the age of 18 are exempt from charges). The amount we ask you to contribute towards our Community Care services will be determined under our Fairer Charging Scheme – a leaflet explaining this is available from your local Social Services office. You will always be informed before we provide any service so you won’t have any unexpected bills to pay.

An Alternative to Receiving Services Directly from Us If you are eligible to receive our services but prefer to make your own arrangements, you might want to find out more about our Direct Payments Scheme. Here, instead of providing a service ourselves, we would pay you to arrange your own services to meet your assessed need. An independent organisation, the Dewis Centre for Independent Living, runs a support service that provides information and training to help service users make full use of the scheme.

Advocacy and Legal Advice Advocacy Advocacy is a process of supporting and enabling people to: • • • •

express their views and concerns access information and services defend and promote their rights and responsibilities explore choices and options

Mental Capacity Act The Mental Capacity Act (2005) legislates the process for caring for people who may lack capacity and how decisions should be made. It sets out when you should be consulted about decisions made on behalf of the person you care for and how that person is protected when others are making decisions on their behalf. The Act also gives every person with a learning disability a right to access independent advocacy. It is the duty of local authorities and health boards working together to make independent advocacy services available to those with a learning disability free of charge. Free booklets are available that provide introductory information on the Act. Advocacy Resource (ARX) A not-for-profit organisation which works in partnership with advocacy schemes, projects and providers, to support the provision of independent advocacy for a wide range of disabled and disadvantaged people. Carers Support You are a carer if you look after your partner, parent, child, other family member, friend or neighbour who needs help because they are ill, frail or have a disability. The care you provide is unpaid.

Carers Assessments Carers assessments are for people who provide ‘regular and substantial’ care for a partner, friend or relative. No definition of ‘substantial’ is given, so if you feel you devote a lot of time to caring for someone and you do this regularly, you have the right to a carers assessment. You can have a carers assessment whether or not the person you care for has had an assessment or wants one themselves. Your carers assessment should look at: • how you feel about your caring role • whether you are coping with the level of care you are providing now and how this might change in the future • what help you need to be able to carry on providing care • your health – is it suffering because you are a carer? • work, training and leisure interests – are you working and caring? • Whether you would like to work. Are there hobbies or interests you would like more time to pursue? Following the assessment, you should be sent a copy of the ‘care plan’ social services have produced which will outline the services and support they can give to help you in your caring role.

Child Protection What is child abuse? Physical Abuse is where someone injures or hurts a child – this includes all forms of physical harm. Sexual Abuse is where someone forces or entices a child to take part in sexual activities. Emotional Abuse is where adults treat a child over time in ways that damage their emotional development, making them afraid, insecure or feel worthless and unloved. Neglect is where adults fail to meet a child’s basic needs, this includes not providing adequate food, shelter, clothing and medical care, not protecting a child from danger, leaving a child alone or not sending a child to school.

Anyone who has a concern about the safety of a child must – without delay – pass on those concerns. The Council has a duty to investigate any report that a child might be abused or neglected.

How does abuse affect children? We know that many children are badly affected, physically and emotionally, and that this can last throughout their lives. It is therefore very important that we are mindful of children’s needs and problems, and that we take action where we have concerns. It is always tempting not to get involved, or to be afraid of causing problems, or we just don’t want to believe that a child is being hurt. Remember! If we do nothing about a child who is being harmed, that child may continue to suffer. Telling the right people may make sure that the child and their family get the help and services they need.

What can I do? Who can I talk to? If you are a child or young person and you are unhappy about your life or if you are worried about a friend, or someone else at school or if you or a friend is being bullied tell someone that you trust - a friend, someone in your family,your teacher, your school nurse or ring one of the numbers at the end of this section. If you are an adult who has concerns about a child, you can ring one of the numbers at the end of this section. You can ask that your name is not given to the family you are worried about, and this will be respected. You should also be told later whether your call has been followed up – you probably cannot be given any other information, as this will be confidential. In Rhondda Cynon Taf, schools and other support services will have the protection and well-being of children as their priority.

Communication Estimates suggest that 50% to 90% of people with learning disabilities have communication difficulties. People with learning disabilities do not have one recognised tool for communication and are often dependent on professional help to develop an individually tailored communication plan. They may need different ways of communicating which require all the people around them to learn their language e.g. sign language. Counselling Many of us use some counselling skills in our daily lives and often appreciate support from friends, colleagues and family members. Sometimes however, our usual sources of support can be too close, inappropriate or sometimes part of the problem. Counsellors who have been trained over many years have been shown to be particularly effective in helping, especially in difficult or sensitive situations. They respect privacy and are independent, neutral and professional. People use counselling for a wide variety of reasons: stress, heavy workload, keeping up with change, conflict at work, bereavement, depression, loss of confidence or motivation, health problems. Counselling can help us come to terms with issues that we may face on a daily basis and counsellors can help us to understand our situations, review the options and decide upon actions. People often find that knowing they have a plan they have considered carefully from all angles improves self-confidence and optimism about the future. Counsellors can help people to make important changes in their lives, but equally can help people to live more creatively with situations that cannot easily be changed. Some people find that the ability to discuss their difficulties openly with someone they can trust is sufficient reason on its own to enter counselling; just explaining things to someone impartial can often clarify the situation.

Hospital Discharge Procedure The Council works together with the Health Trusts to make sure that your return home from hospital is safe and comfortable. This process is called the Hospital Discharge Procedure. When you go into hospital you will be assigned a person who will be your main link during your stay. This person is your named nurse. As well as all the medical questions, your named nurse (or their deputy) will enquire about your circumstances at home. This is to find out how well you will be able to cope and whether you will need further care when you leave hospital. Do not be afraid to mention any concerns or fears you have about your return home. These are very common and are easier to sort out before you leave hospital. It is not uncommon for this discharge plan to be discussed on the same day as you are admitted to hospital. By looking at your needs as early as possible, we will have more time to make sure that you get the most appropriate support when you return home. It will also mean that leaving hospital need not be delayed unnecessarily.

Home and Housing Rhondda Cynon Taf Council provides services to help individuals to continue to live in their own home or to help family members care for another person living at home. Protection of Vulnerable Adults (POVA) What is Adult Abuse? This is any non-accidental act, which is harmful to a vulnerable adult.

What different kinds of abuse exist? • Financial - theft, misuse of benefit books, withholding money for bills • Physical - punching, slapping • Sexual - sex without consent • Psychological - threats, persistent name-calling • Neglect - ignoring need for medical help, failing to give prescribed medication

Am I, or is a person I care for, a Vulnerable Adult? Any adult, who is unable to care for themselves without support because of a disability or serious illness, may be more vulnerable to being abused.

Where can abuse happen? Anywhere, even in a person’s own home, a nursing home or a hospital.

What kinds of people abuse others? All kinds of people have been known to abuse vulnerable adults. These include care workers, nurses and relatives.

Why does abuse happen? There is no single explanation for why abuse happens. Reasons can include: • care staff being inadequately trained or supervised • carers not being adequately supported • people wishing to deliberately harm a vulnerable adult

How will I know if someone is being abused? Abuse can be difficult to spot and quite often the best option is simply to listen carefully to what a vulnerable adult is telling you. The following may be signs of abuse: • • • • •

a series of unexplained injuries malnutrition, untreated ulcers or bedsores disappearance of personal possessions sudden appearance of unpaid bills bruising or bleeding in the rectal or genital areas

What can I do to stop abuse from happening? If you suspect a vulnerable adult is being abused then you should try and discuss this with the adult to find out what their views are and what they wish to happen. The adult may feel uncomfortable talking about things for a number of reasons. For example being afraid of what they believe will happen if they tell someone they are being abused. Remember! Abuse is not acceptable and there are people who can help to stop this happening.

Reporting Your Concerns If you feel a vulnerable adult is unable to make decisions for themselves and is likely to remain at risk of abuse, or you yourself are being abused, you should call Social Services on 01443 849944 or your local Police Station. If you believe the situation is very serious and the vulnerable adult is at immediate risk of harm you should report the matter to the Police – ring 999.

Respite and Holidays Respite (short breaks) Day care away from home for the person you care for can give you a complete break. You may want a regular break, perhaps on a daily or weekly basis, or you might want an occasional break. Day Care for Adults There are centres where the person you care for could meet friends, have a chat, enjoy a meal or undertake a social activity. Sometimes voluntary groups run these centres. Day Care for Children Voluntary organisations, local authorities, local education departments and government schemes provide a range of day care for children. Many of these offer places for children with special needs. Day Care Services at Home Care can be provided in your own home to give you, the carer, a break. See also section Carers Support.

Support to Access Recreation Scheme (STARS) The aim of this scheme is to assist service users to take up the benefits of participating in enjoyable exercise. The person will receive a 45-week activity programme specifically designed around their needs with guidance and regular reviews from a highly qualified member of the leisure services team. In this time all activities within Rhondda Cynon Taf leisure facilities will be charged at a subsidised rate of £1 per session. Cinema Ticket Scheme A national card scheme aimed at disabled people that entitles the holder to one free ticket for a person accompanying them to the cinema. The card applies in almost every UK cinema. To apply for the card you will need to meet one or more of the following criteria: • Be in receipt of the disability living allowance or attendance allowance. • Be a registered blind person. The card is valid for 3 years from the date of issue and costs £5. It usually takes about three weeks for the card to arrive from the time you send off your application.

Cwm Cycling This is a bike hire scheme that offers bikes and trikes which are adapted to suit the needs of the individual. The scheme allows cycling in a tranquil environment away from traffic.
