Login Note: Your SESA ID must be in capital letters, no spaces in between the letter A and number.

Your username is your SESA ID (ex: SESA0000). If this is the first time you are accessing the site, your password is defaulted to your last four digits of your SSN (1234). You will be prompted to change your password and create answers to two security questions. The password must have one Capital letter, one numeric number, and one symbol. If you have accessed the site previously, you will use the password you created when you first logged in.

1. The home page includes a message center that reminds you of when Open Enrollment ends and other important information you may need to know. 2. Click on “Enroll Now” on the left navigation bar to begin your enrollment.

2 1. Click on the link “To make your 2012 elections, click here.” to proceed with your enrollment. 2. You will be taken to another screen that states that you are now eligible to enroll. Click “Enroll” to continue.

After clicking “Enroll” on the previous screen, you will be taken to the Instructions Screen. After reading the instructions, click “Next” to continue.

After clicking “Next” from the Instructions Screen, the Event Actions screen appears. It has 3 tabs across the top that you will use to update specific information. They are (1) Employee Info, (2) Dependents, and (3) Elections. If your personal or contact information on the Employee Info Tab is incorrect, login to Bridge HR self service to update your information You can print an enrollment worksheet which shows the benefit options available to you and associated costs To continue enrolling, click “Next” to move to the Dependents Tab or you can click the Dependents Tab itself

2 3 1 On the Dependents tab, you can add additional dependents to the system or modify existing dependent data. 1. If you do not have dependents that you wish to add, click the “Next” Icon to go to the Elections Tab. 2. Click “Add Dependent” to begin adding new dependents. New dependents must be added to the system prior to making elections so they can be enrolled in coverage. 3. To edit an existing dependent’s information, click “Edit”. To remove a dependent, click “Remove”

Adding a Dependent

Enter your dependent’s information including: First name, Last name (change only if different from yours), Relationship (ex: spouse, child, domestic partner, etc.), Social Security Number*, Date of Birth, and Gender *If your dependent does NOT have a Social Security Number, leave this field blank (do not enter a fake SSN) Other Fields:  If your dependent child is over the age of 19, not disabled and enrolled as a full-time student, check the “Student Status” box  If your dependent is disabled, regardless of age, check the “Disability Status” box  If you dependent is a domestic partner or DP child, and he/she is a tax dependent, check the “Tax Dependent” box  Click “Submit” to save your dependent’s information

After adding a dependent, you will be returned to the Dependent Tab. You will now see your existing and/or newly added dependents. • Verify that the name, date of birth and relationship are correct. • To remove a dependent, click “Remove” and he/she will be removed from the screen. (It is preferred to remove covered dependents from the elections tab.) • To make changes to a dependent, click “Edit” to update. Once you have made your changes, click “Submit” to continue enrolling. Repeat the process until you have all your benefit eligible dependents in the system. Once all dependents have been added and/or modified, click “Next” to continue enrolling in your benefits.

On the Elections Tab, you will see instructions and information on enrolling in your benefits. • The red “X” indicates you must review and update that benefit in order to complete your enrollment. Click “Modify” to review or update that benefit. • If a benefit is grayed out and the “Coverage” column shows “Not Enrolled”, you must click “Enroll” in order to complete your enrollment. • The green “√” check next to a benefit, means you do not need to review the benefit unless you want to change the “Coverage” that is currently showing. To change “Coverage”, click the “Modify” link to go to the detail screen for that benefit.

Making Your Elections 1

1. As an example, if you would like to elect or change your medical benefit, click “Modify” to be taken to the medical elections page. 2. On the medical page, select the coverage category and option you would like to elect.  You can only elect the coverage level based on your eligible dependents.  Make your election by clicking on the radio button for the desired option. 3. Once you have made your election, click “Next” to continue. You will be taken to the dependent selection screen if you elect any option other than “Employee Only”

After electing coverage, an additional enrollment screen will appear. This screen is to add your dependents to coverage. • Click on “Dependent Coverage”. This will open a new window that will give you the opportunity to select the dependents you want to include for benefit coverage

Note: Newly added dependents, are NOT automatically assigned to coverage. You must assign coverage through the above screen.

Elections Note: If you added a dependent that does NOT meet the eligibility requirements, that dependent will be grayed out from this screen. If you newly add a dependent to coverage, documentation will be required to verify dependent eligibility.

From this screen check the box in front of the dependents you wish to add to this coverage. Then click “Submit” After clicking “Submit” you will be taken to the screen below. You should now see a green “√” next to dependent coverage Click “Complete” to save your elections for this benefit and continue enrolling

Elections

 

After clicking “Complete”, you will be taken to the main Elections Tab to continue electing benefits. Click “Modify” next to each benefit line you wish to elect or modify. The process will be similar to the medical election process.

Note: You must select coverage for dependents in each option: medical, dental, vision, etc. The following benefits will not ask for dependent information: • Basic Life & AD&D*, Supplemental Life and AD&D*, STD, and LTD * These benefit options require beneficiary information

EOI and Beneficiary Screen Note: The “Complete” button at the bottom of the page will be grayed out until all links on this page are showing a green “√”.

 



You will be taken to the above screen after you elect Basic Life and AD&D Insurance, Supplemental Life Insurance, Spouse Life or Child Life Insurance If “Evidence of Insurability” appears on this screen, this means EOI is required for the life insurance coverage amount you elected.  Click “Evidence of Insurability” to be taken to the EOI screen to continue your enrollment. (We will review this on the next page) If “ Life Insurance Beneficiary” appears on the screen and there is a red “x” to the left of it, you must designate a beneficiary for that benefit

Once you click on “Evidence of Insurability” from the previous screen, the above box will pop up. 1. Click “Check to confirm you have read this information”, then 2. Click “Continue” at the top of the screen to continue your enrollment

After clicking “Continue” from the EOI pop-up box, you will be taken back to this screen. If there is a red “x” to the left of “Life Insurance Beneficiary”, click that link to designate a beneficiary.

Adding a Beneficiary



Once you click “Life Insurance Beneficiary” from the previous screen, you will be taken to this screen to designate your life insurance beneficiary. You have the option of adding new beneficiaries by clicking “Add” or selecting an existing dependent that appears on the screen. • The total percentage at each level can not exceed 100% (Primary 100%, Secondary 100%) • After making your elections, click “Submit” to continue

1 1. Once all of the links are showing a green “√”, you may click “Complete” at the bottom of the page to continue. 2. You are then returned to the Elections Menu. Please review to ensure that each benefit line is marked with a green “√”. You will not be able to complete your enrollment until all benefits are marked with a green “√”. If all lines are showing a green “√ “, click “Next” to continue.

Note: If any benefit lines show a red “x”’, you will need to make an election for that benefit before you can complete your enrollment

Elections

From the final enrollment screen (shown above), you have the opportunity to review your elections and make any changes before finalizing your enrollment. • If there is a red “x” to the left of “Life Insurance Beneficiary”, click that link to designate a beneficiary. You cannot complete your enrollment until all links on this page show a green “√” • Click the “Confirmation Statement” link to see all of your elections and price tags. If you do not wish to make changes to your elections, save/print the statement for your records. • If you wish to change your elections, click “Back” to return to the elections tab. • To complete and finalize your enrollment, click “Complete” at the bottom of the page to save and submit your elections. • If you do NOT click “Complete” within the Open Enrollment period, your elections will not be saved and you will default to the coverage indicated in your enrollment guide.

Confirmation Statement

When you click on “Confirmation Statement” from the previous screen, another window will open (as shown above) To view your confirmation statement, click “Click here to open the document” Save or print a copy for your records. A confirmation statement will not be e-mailed to your or mailed to your home.

Complete

This screen is confirmation that your elections have been saved. Your enrollment now is Complete! If you do not see this screen after clicking “Complete” on the final enrollment screen, contact PeopleLink at 1-877-248-2998 for support.
