3. Are the relief valve shear pins from the manufacture and the proper size and are the covers in place?

Are all bolts in place at the Discharge Manifold and Module? If not, this must be corrected prior to pre-spud approval!

5. Are the weep holes on pumps modules properly guarded?

Are all the handles on choke manifold installed and are they in good condition? 12. 3. Is the PPE storage box clean? 4. Is the PPE box labeled with "Safety Equipment Inside"? 5. Is only authorized equipment inside? 6. Is the caustic mixing barrel properly labeled? RIG FLOOR Is the "Crown-o-Matic" function tested each tour and documented on the IADC report and set for 1. drilling or tripping mode? Must visually verify the High and Low EDS set points. If not, this must be corrected prior to pre-spud approval! 2. Is the drill line in satisfactory condition? 3. Is air hoist line in good condition with factory eyes and inspection tags? 4. Does the hoist line have a proper swivel and hook? 5. Does air hoist have drum guard and line guide and in good condition? 6.

Are the tong lines in satisfactory condition with inspection tags installed and are they properly secured? If not, this must be corrected prior to pre-spud approval!

7.

Change all jagged ends of wire rope on the tong lines, Mud Bucket etc. to factory eyes to prevent injury to personnel?

8. Do tong lines have 4 part shackles? 9. Are all cables using wire clamps equipped with factory eyes with a 3 clamp minimum? 10. Are the tong dies in satisfactory condition and are retainer pins installed properly? 11. Are the tong jaws retaining pins secured with cutter keys, nuts or spreader type pins? 12. Are all adjusting weights kept inside the tong counterweight buckets? 13. Is all wire rope protected from rubbing against any derrick structural member? 14. Is the Pipe Spinner guarded and are nylon tethers attached to use as handles? 15. Is the Pipe Spinner suspended to prevent personal injury? 16. Is the rig floor decking in satisfactory condition? 17. Is the working area around the rotary provided with non-slip decking or mats? 18. Is the mouse hole kept covered when not in use? 19. Are all guard rails around the rig floor in place and in good condition? (No more than 6" gap) 20. Are all floor openings properly guarded? If not, this must be corrected prior to pre-spud approval! 21. Is V-door handrail opening protected with a gate closed when not handling pipe? 22.

Is the lighting adequate, properly snubbed or bolted to a hard surface; and are all lights equipped with guards and globes?

Are the rotary and kelly spinner hoses in satisfactory condition and is rotary hose snubbed-off with 23 safety chain or wire rope to the derrick leg and the swivel? If not, this must be corrected prior to prespud approval! 24 Are all hand tools in satisfactory condition and properly stored?

DERRICK RIG MANGER / TOOLPUSHER REQUIRED TO INSPECT DERRICK PRIOR TO DERRICK BEING RAISED Date

Is the derrick ladder securely mounted with proper equipment? No altered equipment allowed. If not, this must be corrected prior to pre-spud approval!

2.

Is the derrick ladder equipped with a Lad-Safe Cable System, two Lad-Safe sleeves and Climb Assist system and good working condition? If not, this must be corrected prior to pre-spud approval!

3. Is the derrick ladder clear of all obstructions? 4.

Does the derrick ladder have a minimum clearance of seven (7) inches between the derrick and the back of the ladder?

5. Are all sections of the derrick ladder properly aligned? 6.

Is the derrickman's work belt in good condition and equipped with both a belly line and a tail line? If not, this must be corrected prior to pre-spud approval!

7. Is the upper derrick free from loose tools or other materials which could possibly fall? 8. Are all derrick sheaves and lines properly secured and located to avoid rubbing? 9. Are derrick fingers snubbed? 10.

Are crown sheaves in satisfactory condition? If not, this must be corrected prior to pre-spud approval!

11.

Is the aircraft warning light at the crown in operating condition? If not, this must be corrected prior to pre-spud approval!

12.

Are all girts, legs and braces in satisfactory condition, properly secured and free from damage? If not, this must be corrected prior to pre-spud approval!

13.

Is the standpipe properly secured to the derrick with all bolts and nuts installed on clamps? If not, this must be corrected prior to pre-spud approval!

14.

Is the lighting adequate and properly snubbed or bolted to a hard surface? If not, this must be corrected prior to pre-spud approval!

15.

Are all personnel required to wear a safety harness with lanyard when working in or on the derrick at elevations of six feet (or more) above the rig floor? Is 100% tie-off being utilized at six feet (or more)?

16. Are decking and handrails on walkarounds in good condition? 17.

Are all assembly pins in place and secured with keepers? If not, this must be corrected prior to prespud approval!

18.

Is secondary fall protection installed from the scorpion / hangman? If not, this must be corrected prior to pre-spud approval!

19.

Is the SALA Block secured with a nylon strap or shackle? If not, this must be corrected prior to prespud approval!

20.

Are all bolts in the derrick in place, tightened and have nylon-locking nuts? If not, this must be corrected prior to pre-spud approval!

Are all discharge lines properly snubbed on both ends e.g., Kill Line, Fill-up Up Line and Injection Line if present?

4. Are diverter valves properly installed? 5. Is the conductor or BOP's properly braced and turnbuckles snubbed? (Do all cables have thimble eyes?) 6. Are 4-part shackles in place with nut & cutter-pin? 7.

Are substructure members free from damage and are all securing bolts, nuts, pins and safety pins in place with keepers?

8. Is housekeeping in the BOP and substructure area satisfactory? 9. Are the snubs cables for B.O.P. in good condition and installed properly? 10. Is the SALA Block protected? 11. Is there a 1/4" rope connected to the clip/hook on the SALA Block? 12. Is all electrical wiring and outlets in good condition? 13. Is the cellar cover available and in place? 14.
