Is the working area around the rotary provided with non-slip decking or mats? 18. Is the mouse hole kept covered when not in use? 19. HYDRAULIC CATWALK 1. Is housekeeping in area adequate? 2. Is catwalk in good condition? 3. Is the lighting adequate and properly snubbed or bolted to a hard surface? 4. Are cylinders in good operational order on indexers. 5. Is V-Door chute in good condition? 6. Are shear pins in good condition and tightened? 7. Is skate aligned with chain and in good working condition? 8. Are hoses in good working condition and free of leaks? 9 Are guards in place? 10 Are daily inspections being done? 11 Are all security bolts, nuts and safety pins in place with keepers? 12 Are all grease fittings in place and kept lubricated? 13. Is the catwalk properly grounded? 14 Are bumper guards in good condition on gull-wings? 15 Is the pull-back cable system in good working condition?

Is each layer of pipe properly chocked with factory plastic chocks; front and back? If not, this must be corrected prior to pre-spud approval!

2. Do pipe racks have proper stops on both ends? 3.

Is the Geronimo line anchored properly on both ends with the distance being 2 times the height. If not, this must be corrected prior to pre-spud approval!

Is Geronimo landing area clear? (6 ft. on each side). If not, this must be corrected prior to pre-spud approval!

Are the boards on location in good condition and no large gaps or loose boards? 4 Is the distance to the first rung greater than 12" on any of the fixed ladders? 6. Are whip-checks installed on all fittings (unions, quick connects)? 7. Are all fire extinguishers mounted at least 1" off the deck in a proper bracket? 8. Are all employees complying with the policy not carry a cell phone, cigarettes or lighter to their work areas? 9. Are all ladder safety cable systems tight with thimbles on both ends? 10. Are all skids properly grounded?
