Residential Living Agreement This Residential Living Agreement is entered upon on by and between JCR Enterprises, LLC d/b/a Skip Murphy’s (hereinafter “Skip Murphy’s”) and hereinafter “Resident”), and shall govern Resident’s living and participation in the sober living community known as Skip Murphy’s at 544 Ocean Ave, 32 Deering Ave, and 21 Whitney Ave, all of which are located in Portland, Maine. 1. Zero Tolerance Policy Concerning Use of Drugs and Alcohol – While living at Skip Murphy’s, Resident is to remain clean and sober at sober at all times and shall refrain from the use of alcohol, illegal drugs, or prescription drugs for which the Resident does not have a valid prescription. Any violation of this rule will result in immediate termination of the Resident’s tenancy, subject to the provisions and requirements of Maine law. 2. Resident agrees that, at request of Skip Murphy’s, he shall be subject to a drug test and/or Breathalyzer test upon move in and randomly thereafter. Resident can expect to submit a urine sample at random at the request of the medical director of NorthEast Wellness Group. The frequency of testing will be determined based on medical necessity. NorthEast Wellness Group will conduct the urinalysis screenings. DO NOT EAT ANYTHING WITH POPPY SEEDS ON THEM OR YOU WILL BE DISCHARGED IN THE EVENT YOU TEST POSITIVE FOR OPIATES. Any violation of this rule will result in immediate termination of the Resident’s tenancy, subject to the provisions and requirements of Maine law. There is a 2-hour limit to produce a urine sample. If resident has insurance, NorthEast Wellness Group reserves the right to bill the Resident’s insurance policy for urinalysis services. 3. All medications to be taken by the Resident (prescription and non-prescription) must be pre-approved by Skip Murphy’s management staff prior to admission. Any violation of this rule will result in immediate termination of the Resident’s tenancy, subject to the provisions and requirements of Maine law. 4. The first (30) days of residency at Skip Murphy’s are a probationary and orientation period. This probationary period will allow both the staff and client to assess if the program is a good fit for the Resident. 5.

Fees. The Skip Murphy’s fee schedule is as follows: Stage One: $2600.00 per month (approximately 2 months) Stage Two: $1500.00 per month (approximately 4 months) Stage Three: $900.00 per month (approximately 6 months) Stage Four: $500.00 per month (6 month commitment) Stage Five: $100.00 per month (3 months) The resident must have a financially responsible party who assumes ownership and responsibility for these fees for the entire duration of the Resident’s stay. Please see the Financial Agreement. *If the Resident is discharged for any reason, or chooses to go AWOL from the program, a $500.00 discharge fee will be deducted from the current month’s fees. The remaining

Residential Living Agreement balance will be returned to the financially responsible party in the form of a check. 6. The Resident must attend and be on time for the weekly in house peer recovery meeting, held each Sunday evening at 7:00 p.m. 7. Substance abuse counseling is offered by NorthEast Wellness Group and will be conducted at the discretion of the client and family. 8. The Resident is required to attend 12-step recovery meetings in accordance with their current Stage in the program (AA/NA/CA etc.). One of these meetings will be their home group. Stage One: Resident must attend one meeting per day, Monday through Friday, plus the house meeting on Sunday. Stage Two: Resident must attend four meetings per week, Monday through Friday, plus the house meeting on Sunday. Stage Three: Resident must attend four meetings per week, Monday through Friday, plus the house meeting on Sunday. 9. The Resident shall work with all other residents to keep kitchen clean, wash all dishes and put things back where they belong. Quiet time is at 12 midnight. No TV after that. 10. The Resident is required to have chores done and bed made by 8:30 a.m. and keep room clean and neat throughout the day. 11. Disruptive behavior by the Resident will not be tolerated and will result in immediate termination of the Resident’s tenancy, subject to the provisions and requirements of Maine law. Disruptive behavior includes, without limitation, the following: • • • • • •

Physical or verbal abuse/violence Destruction of property Disrespect to other house members/staff Drug talk/romanticizing High risk behaviors Gambling in the house

12. Skip Murphy sober house is designed to provide a clean, safe, sober environment for those who wish to learn how to live a program of recovery. The only approved visitors are the Residents’ sponsors. NO Females are allowed at Skip Murphy’s. 13. If you are not currently in a romantic relationship, we do not allow our residents to get into romantic relationships within the first 6 months of your stay. 14. No smoking in the house. You may smoke out back in driveway area and must use an ashtray. No chewing, spittoons, e-cigarettes or vaping in the house.

Residential Living Agreement 15. Be respectful of the house and furnishings. No hanging pictures on the wall. Damage to our property will not be tolerated. 16. The use of anabolic steroids or pro-hormones is strictly prohibited. Use of either of these substances will result in immediate termination of the Resident’s tenancy. 17. Good relations are to be maintained with our neighbors and the community, and Resident shall conduct himself accordingly. He is a representation of who we are. If we feel that the Resident is not representing Skip Murphy’s in a positive manner, we reserve the right to terminate the tenancy. 18. Resident is expected to stay at Skip Murphy’s for a minimum of 6 months unless the Resident’s tenancy is otherwise terminated. 19. All residents are required to give (30) days notice before moving out, and failure to do so will result in a $500.00 discharge fee. 20. Assumption of Risk, Release & Indemnification. Resident agrees to assume and accept all risks arising out of, associated with or related to Resident’s participation in Skip Murphy’s sober program and residence, 544 Ocean Ave, 32 Deering Ave, or 21 Whitney Ave in Portland, Maine (the “Premises”), including but not limited to those risks which may have been caused in whole or part by the negligent actions or omissions of Skip Murphy’s or its agents. Skip Murphy’s and its agents shall NOT be liable for any damage or injury of or to the person or property of the Resident, Resident’s family, guests, invitees or agents entering the premises at 32 Deering Avenue in Portland or the building of which said premises are a part (the “Premises”) – including but not limited to damage or injury (to person or property) caused in whole or in part by the negligent actions or omissions of Skip Murphy’s or its agents – unless such damage or injury is proximately caused by the reckless or intentional misconduct of Skip Murphy’s or its agents. Resident hereby releases Skip Murphy’s and its agents, including but not limited to Joseph C. Riley, from all such liability, and further agrees to indemnify, defend and hold Skip Murphy and its agents harmless from any and all claims or assertions of damage or injury to the person or property of Resident occurring on or about the Premises, with the sole exception of claims for damage or injury proximately caused by the reckless or intentional misconduct of Skip Murphy’s or its agents. 21. Subject to Skip Murphy’s approval of Resident’s application and Resident’s execution of a separate Residential Lease Agreement with Skip Murphy’s, Resident’s move-in date at Skip Murphy’s shall be

Residential Living Agreement Breakdown of Stages Stage One: • 9:30pm nightly curfew • Minimum of 60 days at this Stage • Resident must attend all groups: o 9:15am – Morning Medication Call o 9:30am - Upon Awakening Group o 11:30am - Big Book Group o 3:00pm - Big Book Group o Nightly outside 12-step meeting (time varies based on the meeting) plus this inhouse meeting on Sunday o 9:45pm – Nightly Medication Call o 10:00pm - Nightly Review Meeting • Individual counseling once per week • Obtain an approved 12-step sponsor • Join and commit to a home group and sign up for a service position in that group • Complete at 5th step with approved sponsor • Complete a resume • Obtain full-time employment or as an alternative, part-time work with part-time school • Attend weekend recreational activities offered: CrossFit, Yoga, hiking, service work Stage Two: • 11:00pm nightly curfew • Approximately 90-120 days in this Stage • Resident must attend all groups: o Four outside 12-step meetings, Monday through Friday, plus the in-house meeting on Sunday o Weekly meeting with approved sponsor o Individual counseling once every two weeks o Actively work steps 9, 10, and 11 o Meet with staff weekly for individual support o Consistency with balance of house responsibilities, work, and recovery o Formulate a budget and financial contract to work with the financially responsible party in order to participate in the payment of monthly fees. Stage Three: • 12:00am curfew on weeknights, 1:00am curfew on weekends • Maximum of six months during this Stage • Resident must attend all groups: o Four outside 12-step meetings, Monday through Friday, plus the in-house meeting on Sunday o Weekly meeting with approved sponsor o Individual counseling once per month

Residential Living Agreement o Continued 12-step work with a focus on helping others and carrying a message of recovery to those who still suffer o Meet with staff bi-weekly for individual support o Continued follow through and demonstration of commitment to the financial contract and budget formulated during Stage Two Stage Four: • Resident needs to apply for this halfway though stage four, all applicants will be considered as long as they are in compliance. • Resident will have scheduled hours to look after the house and its residents during the staff’s unscheduled coverage • Resident will facilitate and/or co-facilitate peer group in-house meeting on Sunday evenings • Resident will attend individual counseling for treatment plan review once every 60-90 days • Resident will work with staff to be available to help with newly admitted housemates get accustomed to the area and house rules. • Resident must be willing to commit to a 6 month commitment as a Stage Four mentor Stage Five: • Resident is living on his own within the community • Resident & family members will draw up a monitoring program to fit the needs of the client & family • Resident will be now available & expected to, sponsor new comers at Skip Murphy’s • Counseling & drug tested on as needed basis, determined by family of client Restrictions: • Staff will provide the Resident with a verbal warning prior enforcing restrictions. Staff will hold the Resident to a higher level of expectation the further the Resident is into the program. Restrictions may result in loss of overnights, loss of recreational activities funded by the program, earlier curfew, and stricter expectations for improvement. • Multiple restrictions may result in moving back to Stage One or even termination of the Resident’s tenancy.
