HUDSON, NH SCHOOL DISTRICT REQUEST FOR PROPOSALS Alvirne High School Design/Build Services with Elevator Installation I. INTRODUCTION The Hudson, NH School District is seeking a qualified, professional firm to provide design/build services as outlined herein to install a passenger elevator at Alvirne High School, 200 Derry Road, Hudson, New Hampshire. This solicitation is open to all qualified firms eligible to do business in the State of New Hampshire. The existing high school is a three-story masonry wall bearing structure with steel floor framing in the original core, and in the additions,. The building is presently used 12-months per year with less daily activity in the summer months. Various events, conferences, summer school and athletics may take place in or around the building at the same time, and the building may be partially occupied during the anticipated construction duration. There is presently no elevator available for construction in the building. II. SPECIFICATIONS The District intends to enter into a contract with one (1) firm that will supply both design and construction services relative to the elevator addition and associated interior renovations. Submitting firms may be in-house design/build firms or firms with subcontracted design entities. It is the intent of the District to receive proposals from the most qualified firms for further consideration. Final decisions will be made based on proposals, references and the criteria below. Upon selection of the successful Design/Builder, the District will negotiate fees and reimbursable expenses. If a mutual agreement is not reached within thirty (30) days between the District and the selected Design/Builder, the District reserves the right to reach mutual terms with the second most qualified firm. The District reserves the right (1) to shortlist the team (or teams) it deems most suitable for further consideration based on personnel named to the project, New Hampshire experience, high school experience, elevator addition experience and other qualifications and criteria deemed to be in the best interest of the District, (2) to reject any or all submissions, and/or (3) to waive any of the requirements of the selection procedures outlined in this document. The District also reserves the right to retain all copies of proposals submitted by responding teams. The District will examine and evaluate each proposal and identify teams that would contribute most positively to the District and to meeting its objectives. The District will use evaluation criteria it feels is most appropriate for the review process. The proposal should include the following: -

Price Proposal Form Design concept plans based upon site visits; Cover letter and interest in the project; Management structure and organization; Personnel resumes and qualifications; New Hampshire experience/high school experience; Similar elevator installation experience; Client references; and Ability to complete the project within the necessary schedule. Proof of Insurance (See section VI)

All respondents must demonstrate experience in: working around high concentrations of people, fast-track construction, design and construction of comparable elevator projects, NH school experience, NH experience, as well as “on time” and “on budget” construction of comparable projects. All qualified parties should adhere to the following proposal schedule: A. All questions regarding specifications/bid process/site inspection should be

Proposals must be received at the above address no later than 11:00 AM on Friday, April 11, 2014. The District anticipates the execution of a Design/Build Contract on Tuesday, April 22, 2014. III. PROJECT DESCRIPTION Construction of the project may begin on June 23, 2014 (the current last day of school is scheduled for June 26, 2014). School officials are considering an early start but all firms proposing on the project should base response on a June 23th start. Criminal background checks will be required for staff that will be working in the school buildings. The foregoing notwithstanding, necessary site work and construction preparation may be performed upon receipt of all required permits and approvals for said work, and upon prior approval by Hudson School administration. Proposals must include a substantial completion date as well as dates for final inspections and approvals. Design and construction will include:  Cut opening in foundation wall and excavate to build an elevator pit. This opening will be the half floor stop required to get to the main first level.  Cut opening in foundation wall and build an elevator machine room under the existing enclosed stairwell. Machine room size will be constructed according to the requirements of the elevator system as outlined below.  New elevator shaft will incorporate two existing CMU walls. Also included will be shaft siding for elevator shaft.  New corridor walls will be constructed at each level to provide new elevator shaft wall construction, hallway access, and subdivision of existing classrooms. Classroom walls will include an accessible classroom passage door. (Completion of classroom walls is critical to the start of the school year and will be subject to completion penalties as outlined below).  Construction on a new entrance roof, existing shed roof wall enclosures, and relocation of existing double passage exit doors equipped with exit panic hardware.  Installation of a ThyssenKrupp Endura Twinpost Above-Ground 2 stage hydraulic elevator system (3500 pound capacity passenger elevator) in the high school to service the basement through third floor and to provide better HCP access to all floors. This will also help bring the building closer to ADA compliance. The District anticipates that the design/builder will pre-purchase materials and installation of the elevator system. The NH State Fire Marshal’s Office and Hudson Fire Department will approve construction documents prior to construction. Alvirne High School is a fully sprinkled building. The contractor will be required to follow NFPA13 code reference Section 8.15.5.

The Town of Hudson provides all building inspections and final construction plans are to be approved and permitted by the Town of Hudson, NH. Winning bidder will be responsible for the securement and payment of permits required by the Town of Hudson.

Drawings and specifications of existing high school conditions will be available for the chosen design/builder. No documents are available for review and study prior to design/build submissions. Mandatory site visit is required (Section IV Pre-Bid Conference). Geotechnical surveys of the area under the proposed location for the elevator have not been completed to date; the District will execute a contract directly with a geotechnical engineer, and the District anticipates that the design/builder will coordinate the selected geotechnical work as part of the guaranteed maximum price. Design/build team must design the elevator installation to be in compliance with all current local, state and federal code requirements. Contractor must be compliant with all OSHA regulations for construction. If the design/build contractor encounters any potentially hazardous materials during construction, the contractor will inform the District and stop the construction until the material is (i) identified and, (ii) if necessary, removed from the site. The District will be responsible for proper removal and disposal of any hazardous materials. After the job is completed; all construction materials shall be properly removed at the Design/ Build team’s expense. This project is being sold as a turnkey project and will be ready for use upon final inspection of the State Elevator Inspector. Final State Elevator inspection will be the responsibility of the winning bidder. The complete project must be completed no later than August 25, 2014. The completion of interior classroom walls must be no later than August 15, 2014. Penalty assessments for interior classrooms will begin on August 16, 2014, at the rate of $500.00 per day. NH RSA XII 155:66 does prohibit smoking on public educational facilities at any time. IV. PRE-BID CONFERENCE A mandatory pre-bid conference will be held at Alvirne High School, 200 Derry Road, Hudson, NH 03051 at 10:00 A.M., Friday, April 4, 2014. All meeting participants will sign in and meet in the main office. V. PROPOSAL Bids must be submitted with sufficient construction literature and/or drawings to allow determination that specifications are met. Bids must contain; bidding firm’s current New Hampshire licensing, proof of liability and workers compensation insurance. Final bid pricing statements must be signed by a principal officer of the proposing firm. Each Design/Builder’s written proposal shall address the following: Provide a cover letter indicating the name of the firms(s) making the submission, indicating their interest in being selected for the project and the firms’ overall general understanding of the critical elements necessary to be addressed to deliver a successful project. If the respondent is a collaboration of firms, the letter should briefly describe the intended organizational structure of the collaboration. Also note how the architects, engineers and construction partners interact during the functional day-to-day process (is) for design and/or construction delivery. The letter should be signed by the responsible principal of the firm or the collaborative.

Provide a brief description of your firm and the organizational structure of your design/build team. If the submitting firm includes design or other consulting firms, please indicate all key team members. Provide resumes of key personnel and describe their role on the project.

List four (4) clients or other clients who would provide references for your work including names, organization, address and telephone.

Describe the experience your firm has had during the past five (5) years including: school projects your team has managed over the last five years (list the name of the school, the dollar amount and the date of completion; design/build projects; elevator projects completed within occupied areas; New Hampshire experience.

Discuss your team’s experience assisting other School Districts in implementing their design and construction programs. Also, describe how that experience is indicative of your firm’s abilities to assist the Hudson School District in implementing this project.

5.

Provide a demonstration of successful management systems for the planning, design and construction of similar projects. Among these are: design, value engineering, scheduling, estimating, constructability reviews, budgeting, on site construction supervision, commissioning and “as-built” documentation.

6.

Describe your team’s safety program.

7.

Provide financial references and current aggregate and single project bonding capacity and insurance limitations as listed below.

Outline the key tasks, durations, approximate start/complete dates for the project assuming a contract is in place by April 22, 2014 and construction begins no later than June 23, 2014.

Complete attached Proposal Form for Design/Build Services.

10. Other information, qualifications and/or exceptions each Design/Build Team may consider

helpful to the selection process. VI. INSURANCE REQUIREMENT The Contractor must obtain and maintain during the entire contract period, at the Contractor’s expense, insurance coverage as set forth below: 

Comprehensive Liability Insurance (Bodily Injury, Personal Injury, and Property Damage, coverage to include contractual liability coverage) with the Hudson School District named as other insured with the minimum policy limits of: Per Occurrence: Per Aggregate:

Umbrella Coverage to supplement all above insurance, with the Hudson School District named as other insured to provide the coverage to at least $5,000,000 per occurrence.



In addition, during the entire contract period, the Contractor, at its own expense, shall maintain for its employees all Workers Compensation coverage required by New Hampshire Law.

VII. TERMS AND CONDITIONS Each team shall submit five (5) copies of the proposal (marked Design/Build Proposal – Alvirne High School Elevator) to: Karen Burnell Business Administrator Hudson School District 20 Library Street Hudson, NH 03051 Faxed or e-mail submissions will not be accepted. Submission packages must be sealed. The name and address of the respondent as well as the project name (Design/Build Proposal Alvirne High School Elevator) should be clearly indicated on the outside of the package, and it must be addressed to the designated contact individual. Each respondent is solely responsible for the timely delivery of its package by the deadline stated above. Please allow sufficient time with regard to weather, traffic and parking to deliver the submissions. Failure to meet the deadline for submission may result in rejection. No team shall have any cause of action against the District or Alvirne High School arising out of failure by the District to consider a team’s proposal, or the methods by which the District evaluated the proposals received. The selection of the prospective teams, the decision to interview those teams and the final selection shall be at the sole discretion of the District. Revisions, Amendments, and Cancellation The District may extend the deadline date for qualifications submission if, in the District’s opinion, the revisions make this necessary. The District may also cancel this solicitation, in whole or in part, or reject all submissions when this action is deemed to be in the best interest of the high school project. Acceptance of Submissions The District reserves the right to accept or reject any or all submissions received `as a result of this solicitation, to waive minor irregularities, or to negotiate with any respondent, in any manner necessary, to serve the best interests of the District. Incurred Expenses Any costs incurred by the respondent in preparing and submitting a response to this RFP will be the sole responsibility of the respondent and will not be reimbursed by the District. Economy of Preparation Submission packages should be prepared simply and economically, providing a straightforward, concise description of the respondent’s ability to fulfill the requirements of this solicitation. Discrepancies and Clarifications The District reserves the right to request clarification of any aspect of materials received or to request further information that might be required to adequately evaluate credentials and qualifications. Respondent’s Inquiries Questions and inquiries concerning this solicitation must be received in writing, either as a signed original, legible facsimile, or electronic mail transmission.

Award The award shall be made to the responsible and responsive bidder whose bid is determined to be the most advantageous to the District based on the evaluation of price, value, quality, and timeliness of delivery. Bids are expected to be opened on the bid closing date and a successful bidder recommended to the Hudson, NH School Board at its next regularly scheduled meeting, April 21, 2014. The awarding authority is the Hudson, NH School Board which reserves the right to select the bid which, in its totality, is in the best interest of the District. The winning bidder will be required to enter into a contract with the District which will require a performance bond in the amount of the bid, conditions and allowances for contract changes, negotiated penalties for untimely completion, and structural and leak warranties.

VII. GENERAL INFORMATION There is no expressed or implied obligation for the District to reimburse responding firms for any expenses incurred in preparing proposals in response to this request. The District reserves the right, where it may serve the District’s best interest, to request additional information or clarification from bidders, or to allow correction of errors or omissions. The District reserves the right to reject, in whole or part, any or all proposals and to accept deviations from the requirements of this Request for Proposals when it is in the best interest of the District. The District reserves the right to retain all proposals submitted and to use any ideas in a proposal regardless of whether that proposal is selected. Submission of a proposal indicates acceptance by the firm of the conditions contained in the Request for Proposals, unless clearly and specifically noted in the proposal submitted and confirmed in any resulting contract between the District and the firm selected.

THIS PROPOSAL IS SUBMITTED TO: Ms. Karen Burnell, Business Administrator Hudson School District 20 Library Street Hudson, NH 03051 The undersigned Submitter proposes and agrees, if this Proposal is accepted, to enter into a Contract to deliver the product as specified or indicated for the prices and within the times indicated in this Proposal in accordance with the other terms and conditions of the Request for Proposal Documents. The Submitter accepts all of the terms and conditions of the Request for Proposal Documents. The Proposal will remain subject to acceptance for ninety (90) days after the proposal opening, or for such longer period of time that Submitter may agree to in writing upon request of OWNER. GUARANTEED MAXIMUM COST PROPOSAL. It is anticipated that the Design-Build Entities will agree to a Guaranteed Maximum Price (“GMP”) to be included in the Design-Build Agreement. The Hudson School District, at its discretion, may negotiate with the selected Design-Build Entity for a Project an initial GMP subject to modification and negotiate a final GMP after execution of the Design-Build Agreement. Pursuant to the Design-Build Agreement, the GMP shall equal the sum of: the (1) Construction Costs; (2) Design (Architect and Engineer) Fees; (3) Design- Build Entity’s General Conditions and Reimbursables; (4) the Design-Build Entity’s Contingencies for Construction Costs; and (5) the Design-Build Entity’s fee.
