Ms. Trela has over 25 years experience in the health information management (HIM) profession, and specializes in healthcare billing, coding, and reimbursement issues with a focus on rehabilitation facilities. Code accurately and ask the following questions • Could there be a coding issue in cases from 2004, 2005? • Did you know then what you now know about coding?

• Do these cases need to be recoded and resubmitted?

RACs and the Coder Example: Patient with fractured hip replacement reported with Impairment Group for joint replacement, should have been reported as fractured hip RAC reviewing joint replacements, denied case for medical necessity. If accurately coded, case might not have been reviewed. 24
