ANNUAL APPROPRIATIONS – ALL FUNDS - FY 2016 Upon the recommendation of the Treasurer, the South Euclid Lyndhurst Board of Education approves the permanent appropriations for all funds for fiscal year 2016 as submitted. Also the Board of Education approves the appropriation resolution for the general fund (function/object level). The appropriations by fund type are reflected as follows:

5-YEAR PROJECTION DOCUMENTS Upon the recommendation of the Treasurer, the South Euclid Lyndhurst Board of Education approves the State Department of Education required (pursuant to HB 412) 5-year projection document including assumptions for fiscal years 2016-2020 as submitted. Furthermore, upon the recommendation of the Treasurer, the South Euclid Lyndhurst Board of Education approves The Zero Point Analysis as an internal planning spreadsheet as submitted.

3.8

SET-ASIDE REQUIREMENTS – FISCAL YEAR 2016 Upon the recommendation of the Treasurer, the South Euclid Lyndhurst Board of Education approves the set-aside requirements established by S.B. 345 pertaining to related expenditures in the areas of textbooks and instructional materials and capital and maintenance as defined by H.B. 412.

TAX ADVANCES – CALENDAR YEAR 2016 Upon the recommendation of the Treasurer, the South Euclid Lyndhurst Board of Education approves a resolution to the County Treasurer’s Office requesting advances of all tax revenue collected in calendar year 2016.

DONATIONS Upon the recommendation of the Treasurer, the South Euclid Lyndhurst Board of Education approves the following donations: From Roger & Sally Nicely, $250 for the Class of 1965 gift.

2015-2016 CALENDAR CHANGE Upon the recommendation of the Superintendent, the South Euclid Lyndhurst Board of Education approves an amendment of the 2015-2016 school calendar to include November 3, 2015 as a Professional Development Day for staff. There will be no school for preK-12 students.

SCHOOL FEE FOR BRUSH HIGH SCHOOL SHOW CHOIR (15-16) Upon the recommendation of the Superintendent, the South Euclid Lyndhurst Board of Education approves a non-instructional costume fee of $110 for students participating in the Brush High School Show Choir for the 2015-2016 school year. This non-instructional costume fee is not waiverable.

BELLEFAIRE JCB (MONARCH SCHOOL) AGREEMENT FOR PROVISION OF SPECIAL EDUCATION AND CERTAIN RELATED SERVICES FOR THE 20152016 SCHOOL YEAR Upon the recommendation of the Superintendent, the South Euclid Lyndhurst Board of Education approves the agreement with Bellefaire JCB (Monarch School) for the 2015-2016 school year at a yearly tuition rate of $78,000.00 per student.

SOLUTIONS BEHAVIORAL CONSULTING AGREEMENT FOR BEHAVIORAL INTERVENTION SERVICES FOR THE 2015-2016 SCHOOL YEAR Upon the recommendation of the Superintendent, the South Euclid Lyndhurst Board of Education approves the agreement to provide consulting services at $150.00 per hour and tutoring services at $45.00 per hour for the 2015-2016 school year.
