Saturday, May 3, 2014 Stonegate Conference Centre (Hoffman Estates) Crystal Lake Central Junior, Seniors and their guests may attend “Puttin’ on the Ritz” Black, Red, White, and Gold

Tickets:

Tickets will be $84 per person or $168 per couple. They will be on sale on Monday, April 14th, Tuesday, April 15th, Wednesday, April 16th, Thursday, April 17th, 2014 and Monday, April 21st, 2014 before school or during the lunch hours in the bookstore. The LAST day for ticket sales will be April 21, 2014.

*Once again we encourage you to pay for your tickets using our online system. If you would like to pay by credit card using RevTrak, login to your family access account and follow the prompts for Prom. You must print a receipt and bring it with you when you register for Prom to show proof of payment. Checks can be made out to Crystal Lake Central High School. The last day to purchase tickets will be Monday, April 21, 2014. Everyone (no matter what age) must have a permission slip signed by a parent or guardian in order to purchase a ticket. Prom packets will only be available on the school website. They can be found on the Junior Class webpage located under the Activities heading. (http://ww2.d155.org/clc/actdirectory/juniorclass/default.aspx). *Any Junior or Senior attending Prom MUST have all discipline served before being able to attend Prom. If a ticket is purchased and discipline is not completed, you will be unable to attend Prom and WILL NOT receive a refund*

There are no refunds on Prom Tickets!!! No Tickets will be sold after Monday, April 21, 2014 Buses:

The fieldhouse will be open at 5:30 p.m. Those attending Prom should be here at 5:30 p.m. Buses will begin loading at 5:45 p.m. The first set of buses will leave CLC at 6:00 p.m. Stonegate will open its doors at 6:30 p.m. Dinner will be served at 7:15 p.m. There will be a “parade” to the bus if your parents want to come and take pictures. We will leave Stonegate at midnight and return to Central about 12:30 a.m. You will be assigned a bus when you purchase your ticket. You cannot change bus

assignments. Do not be late to meet your bus. We will leave without you. You must ride the bus to and from Prom or you will not be admitted. Table Reservations: Table reservations can be made when you buy your tickets, if you have all 5 couples. A table reservation form is included in this packet and should be filled out completely. You can pick your table when you purchase your prom tickets. All couples must purchase a ticket before your table can be reserved. All students will have an assigned table. If you do not have 5 couples to fill a table, you can pick where you would like to sit from the tables remaining. Anyone not choosing will be assigned by the sponsors to a table.

Menu:

The menu this year will be a chicken dish with vegetables and a potato. A vegan/gluten-free dish will also be available upon request. You must state this on your reservation form when buying your tickets. The vegan meal is usually a spinach stuffed portabella mushroom. Each meal is usually served with pasta (fruit salad for vegan/GF), salad, and a gourmet dessert. The Junior Class Officers and Advisors tried to accommodate a wide variety of palates when picking the meal for Prom.

Hint:

The best way to ride and sit with all of your friends is to have one person collect your tables’ money (checks payable to Crystal Lake Central) and the permission slips. This one person can then purchase all the tickets, sign up everyone on the same bus, and fill out the table form properly.

Prom Favor: This year the Juniors are very excited to have 2 photo booths and a green screen available at Prom. Both the photo booths and green screen will be available for use starting at 8:00 PM until Midnight. Gather your friends or your date and take unlimited photos during that time. The photo booth holds up to 6 people. The green screen is just that, a green screen, where you can pose for a picture and feel like you are a movie star! After Prom night, you can go online to view your photos and download them for free. www.photoboothexpress.com and www.greenscreenexpress.com Parking:

Only students attending Prom will be able to park in the student parking lot. All parents and guests watching the promenade will need to park in the teacher parking lot or Husmann Elementary. This change is for the safety of our students when they return from Prom late at night.

Prom Table Reservation Form In order to reserve a table you must have 5 couples. This form must also be completely filled out. Please make sure that you check with the people you’ve listed on your form. If a couple appears on two table forms, both tables will be cancelled. The table may also be canceled if all 5 couples have not purchased a ticket. 1. 2. 3. 4.

Guest Form Dear Guest and Parents: Junior/Senior Prom should be a memorable and special event in the life of high school students. The following information is provided in order to help plan for the 2014 Prom, which will be held at the Stonegate Conference Centre in Hoffman Estates on May 3, 2014. The price of the ticket will be $84 per person. Once the tickets have been purchased, there will be no refunds. All students and guests who attend the Prom are required to ride District #155 buses to and from the site. No one using private transportation will be admitted. The buses will load at school at 5:30 p.m. and will return after the Prom at the school at approximately 12:30 a.m. Bus assignments will be made when the tickets are purchased. Only 1 guest allowed per CLC student. School District #155 policies prohibit the possession of, control of, use or consumption of, or being under the influence of drugs or alcohol. Smoking is prohibited as well. Any violation of these rules will result in the parents being called to pick up the student from Prom. The local police agency may also be called. Suspension may result as well. Any guest attending the Prom must be under the age of 21 years old. In the past, we have sometimes had problems with students coming to Prom after drinking at activities. If you have a Pre-Prom party for any of the students, please keep in mind that the students will be held accountable for all school rules and regulations for the evening. Any student or guest who shows evidence of being under the influence of alcohol or drugs will not be allowed to board the bus for Prom. If the student is denied permission to board the bus, there will be NO refund. We want the Prom to be a very enjoyable and safe evening for our students. We will not allow a few students to disrupt the evening for the entire group. We merely expect the students and their guests to demonstrate behavior and conduct of which we can all be proud.
