Research and Innovation Back in 2011, I was asked by Professor Brendan McCormack to write the preface for the review of the Institute of Nursing Research (INR). Two years have passed and I am again privileged to be asked to write this preface. A preface to later editions of a review should explain in what respects that review differs from the previous one. Well, since 2011, the membership of the Institute has been expanded to include health science researchers and readers will note how the research within the new Institute continues to advance both quantitatively and qualitatively. Health care is an applied discipline and one where research findings are of little use if they have no impact on practice. I note from this review that the processes of knowledge transfer ensure that the dissemination of research findings encourage and enable clinical impact. This review has arrived at an opportune time. The Institute’s members are preparing for the 2014 Research Excellence Framework. Results from RAE2008 showed that much of the research was world leading. This placed it in the top three research intensive UK nursing departments. Going forward with the new configuration,

Preface

the University seeks to sustain and enhance the Institute’s excellent reputation by undertaking further high quality research. Through its research, the Institute is committed to support the quality and safety of patient care. Institute members welcome the opportunity to work in partnership with the wider healthcare community in Northern Ireland to generate, test, disseminate, apply and, where possible, commercialise new knowledge. However, we are also aware that members must take their place among the national and international community of researchers and innovators. This is being achieved and is bringing back reflected glory on local healthcare. The Magna Charta Universitatum, was signed by the rectors of European universities in Bologna in 1988. According to its second principle teaching and research in universities must be inseparable if their tuition is not to lag behind changing needs, the demands of society, and advances in scientific knowledge. The Institute members undertake high quality research that informs their high quality teaching. They are also involved in ensuring that students have the opportunity to study and do research

and appreciate its importance for the care of patients, families and communities. Members believe that being focused on specific research themes is important; they don’t try to do everything, but what they do, they do well. So to my concluding remarks: I recommend with enthusiasm this review to would-be readers. It is a solid and significant contribution to the ongoing development of nursing and health science’s research and innovation at Ulster. I will watch with interest to see how it is regarded regionally, nationally and internationally.

Professor Hugh McKenna CBE, FAAN, FRCSI, FEANS, FRCN Pro-Vice-Chancellor (Research and Innovation)

Foreward

Faculty of Life and Health Sciences As Dean of the Faculty of Life and Health Sciences, I am delighted to write a foreword for this INHR research review. As has already been noted and celebrated, Nursing at the University of Ulster is in the premier league of universities in the UK and is ranked among the top 3 universities for nursing research. Rehabilitation sciences research at Ulster brings together the larges grouping of allied health professional researchers in the UK and Ireland. Since 2011 nursing and Health Sciences have been working together as an integrated unit. This combination of leading nursing and allied health researchers working collaboratively with researchers from health services research, public health research and an array of other disciplines, makes this Institute a vibrant and exciting community with many successes to celebrate – and which are highlighted in this publication. Despite these successes, we know that the research environment has changed considerably over the lifetime of the INHR and we have been fortunate to work in a macro political context that has been largely supportive of research. However, as we move forward, this context has changed considerably and the pressures on university, research council, regional and charitable funding is enormous. However, at the same time, the expectations on research units in the Research Excellence Framework (REF) to perform as well-as if not better than the previous

RAE are considerable. Despite such ‘gloomy forecasts’ the need for visionary and lateral thinking in addressing these challenges and a need for diversification in the way research is conducted in universities is greater than ever. Research income targets, publication standards and multidisciplinary competitive grants are the order of the day and the INHR will play its role in this agenda at Ulster. The INHR has been at the forefront of innovative research practices and its commitment to creative and integrated ways of working whilst simultaneously being uncompromising about quality will serve it well for the future. Research is a central part of the business of the Faculty of Life and Health Sciences and the INHR plays an important role in the overall research successes of the Faculty. I know it will continue to do so into the future. For now, I congratulate the whole team on their incredible successes and look forward to supporting and enabling even more success in the future.

Professor Carol Curran Dean, Faculty of Life and Health Sciences

Introduction

School of Nursing The School of Nursing offers a wide range of educational provision, which has been developed to permit flexibility in relation to admissions, modes and levels of study, and exit routes. The entire range of programmes available at undergraduate and postgraduate level, seeks to engender life-long learning and opportunities for continuing professional development. Areas of educational provision encompass the following three broad categories, which recognise the wide range of academic backgrounds among potential students: the need for newly qualified nurses through the provision of pre-registration Adult and Mental Health nursing; opportunities to further develop skills, knowledge and competency in the provision of a wide range of post registration specialist courses in nursing and health promotion; opportunities for updating and continuing education of those people working in interdisciplinary fields related to health and/or social care. The research undertaken within the Institute of Nursing and Health Research is embedded within all of these courses.

This report provides a welcome update on the considerable work undertaken within the Institute of Nursing Research. This work is important to providing effective educational opportunities to nurses and other professionals and within the School we very much value the importance of making the use of research evidence in order to advance our understanding of peoples’ experiences and developing effective nursing interventions. Students in the School of Nursing benefit from the direct involvement of active researchers within their courses, alongside other staff within the School and the opportunities available to learn about the latest research findings. The synergies between the teaching and research within the School also provide real opportunities for students to become actively involved in research and students are actively encouraged to consider the role of research within their future careers.

Professor Owen Barr Head of School of Nursing

School of Health Sciences I am delighted to have been asked by Professor Brendan McCormack, to provide an introduction to the School of Health Science research for this review. This is quite unusual in that there will be two introductions provided by the two Heads of Schools which make up the Institute of Nursing and Health research (INHR). This came about when I joined the School of Health Sciences in November 2011 as the Head of School, at that time the research undertaken in our School was based in the Rehabilitation Sciences Research Institute (RSRI). At that time we made a decision to join with the Institute of Nursing Research in late 2011 as the INR, as it was then, complemented the research undertaken by the RSRI and permitted us to establish a critical mass of experienced researchers in both quantitative and qualitative research. We have since refocused and renamed our center as the Centre for Health and Rehabilitation Technologies (CHaRT). This is the largest multidisciplinary grouping of allied health professionals, clinical scientists, and PhD students in the UK and Ireland, with many Cochrane Fellows in our center. We have an excellent track record in the education and training of high caliber PhD students with over 40 completing PhDs since 2002. Our undergraduate and master modules have been commended as being excellent examples of research informed teaching, underpinned by internationally leading research produced by active researchers within CHaRT.

The School of Health Sciences is one of the oldest Allied Health Professional academic schools in the United Kingdom, with a distinguished history of producing graduates who then become leaders in research, health and social care services and education. The School supports continuing professional development; contributes to knowledge through world-leading multiprofessional translational research, centered on the physical needs of people with chronic conditions; transfers this knowledge to key stakeholders in order to directly impact on healthcare policy and practice; and contributes to social and economic development through technology and knowledge transfer.

Dr Mary Hannon-Fletcher Head of School of Health Sciences

Striving for Excellence

Institute of Nursing and Health Research Public health, nursing, midwifery, and allied health professionals research at the University of Ulster have continued to grow since 2008. This growth reflects the broadening of our discipline base and an on-going commitment to multiprofessional working. In terms of critical mass, the Institute of Nursing and Health Research (INHR) is one of the largest centres in the UK combining public health, nursing, midwifery and allied health professionals research. Institute membership (full or associate) is based on well-defined criteria reflecting excellence in research performance (or strong potential to do so). Currently there are 39 full members and 23 associate members (those who meet some but not all the criteria for full membership). Institute members have protected time for research and the system ensures compatibility between high quality research and pedagogic activities. The INHR’s mission is to undertake excellent research and development that has high impact and contributes to the achievement of the University Vision. We do this by undertaking high quality strategic and applied research through a range of creative and appropriate methodologies; by contributing towards the development of models, conceptual frameworks and theories relevant to policy, practice and education; by providing consultancy on service evaluation and developments, in partnership with statutory, private and voluntary organisations; by educating and training high calibre research students, within the context of providing a high profile community

for seminars, master classes, workshops, lectures and conferences; by engaging actively in knowledge transfer at local, national and international levels; and demonstrating the impact of new knowledge through innovation and development. The overall vision is to be world-leading in health research and development that enhances knowledge, expertise and engaged scholarship within public health, nursing, midwifery, and allied health professionals in order to improve the health and wellbeing of people. Staffing policies in the Schools of Nursing and Health Sciences and the INHR are designed to deliver the twin objectives of high quality research and high quality education/learning. We aim to ensure that our research, teaching/learning and clinical scholarship activities are integrated as much as possible with the intention of facilitating, co-ordinating and producing nurses and allied health professionals who are committed to the highest quality healthcare practice, education and research. Our commitment to the integration of these elements of our work ensures that we continue to grow and maintain a vibrant culture of research and scholarship within the University of Ulster and in partnership with health providers, commissioners and other centres of excellence.

Professor Brendan McCormack Director, Institute of Nursing and Health Research

Structure of the Institute of Nursing and Health Research

The INHR Research Strategy is focused on undertaking research connected with global health priorities in public health, chronic conditions and user-focused perspectives. Our research strategy is underpinned theoretically by the theory of ‘Engaged Scholarship’. Engaged scholarship has an explicit intention, to co-produce knowledge, i.e. to work in partnership with key stakeholders at all stages of the knowledge production process. The adoption of this theoretical approach enables a specific commitment to the integration of research, teaching/learning, innovation and scholarship and the co-production of knowledge with our partners. This is reflected in our strong partnerships for clinical research with local health and social care trusts, in our commitment to service-user engagement and in our international collaborations. Our strategic framework underpins all research activities in the INHR, and is delivered through five Research Centres with each one being led by an internationally recognised expert in the field: Centre for Intellectual and Developmental Disabilities (Head of Centre, Professor Roy McConkey) Centre for Health and Rehabilitation Technologies (Head of Centre, Professor Suzanne McDonough) Managing Chronic Illness Research Centre (Head of Centre, Professor Vivien Coates) Centre for Maternal, Fetal and Infant Research (Head of Centre, Professor Helen Dolk) Person-centred Practice Research Centre (Head of Centre, Professor Brendan McCormack)

Centre for Maternal, Fetal and Infant Research

Professor Helen Dolk Head of Centre The Centre for Maternal, Fetal and Infant Research (MFIR) focuses on public health, primary care, perinatal epidemiology and midwifery research through its two programmes of work: the Perinatal Epidemiology Cluster; and the Midwifery Research Cluster. The Centre has an established international reputation for research concerning health and wellbeing in the preconceptional, pregnancy, and postpartum period. MFIR research tackles the multiple interconnecting environmental and socioeconomic influences on maternal, fetal and infant health and wellbeing. An important focus of the work is influencing appropriate use of technology. The Centre is the European co-ordinating centre of EUROCAT European Surveillance of Congenital Anomalies (www. eurocat-network.eu ), funded by European Commission DG Sanco Public Health Programme and a WHO Collaborating Centre for the Surveillance of Congenital anomalies.

Centre for Intellectual and Developmental Disabilities

Professor Roy McConkey Head of Centre The Centre for Intellectual and Developmental Disabilities is one of the leading research centres on disability in Ireland and internationally. The Centre focuses on three main strands of work within a multi-disciplinary perspective: Vulnerability and complex needs; Promoting the health and wellbeing of people with intellectual and developmental disabilities; and Social inclusion and advocacy. In 2010 it became the Regional Collaborating Centre for Special Olympics in Europe/Eurasia (jointly with the Sports and Exercise Research Institute at Ulster). It has strong links with similar centres around the world and is an active member of the International Association for the Scientific Study of Intellectual and Developmental Disabilities, the Royal College of Nursing and the Royal Society of Medicine.

The Centre has secured substantial external funding totalling over £2.6m from competitive sources in the last four years. Many of these research projects have been in collaboration with other leading research centres nationally and internationally, and across disciplines. A core focus of the Centre has been the translation of their research findings into practice. Hence, the Centre also undertakes research and evaluation that informs the development of policy and the organisation of services for people with disabilities through partnerships with statutory and voluntary agencies as well as advocacy groups. The Centre members have written over 150 peer-reviewed papers in a range of international journals, over 20 book chapters along with four books. Ten PhD students have successfully completed their doctoral studies and nine are currently enrolled.

Managing Chronic Illness Research Centre

Person-centred Practice Research Centre

Professor Vivien Coates Head of Centre

Professor Brendan McCormack Head of Centre

The Managing Chronic Illness Research Centre conducts rigorous research into holistic care, responsive to the needs of those living with a chronic condition. The focus of work in this Centre is upon Palliative Care; Diabetes & Cardiovascular Disease and Cancer Care. Managing chronic conditions is a major global health care problem with cardiovascular disease, cancer and diabetes amongst the five most common chronic conditions. We collaborate with a wide network of patient groups, service providers and academics from a range of disciplines and institutions. In partnership with such groups and individuals, members of this Centre aim to undertake research to bridge gaps in our understanding of the management of chronic conditions from patient, carer and health care provider perspectives. We inform educational curricula and translate research results into appropriate and effective service and practice improvements.

The Person-centred Practice Research Centre (PcPRC) has its focus on the enhancement of knowledge and expertise in person-centred practice (an approach to practice that is focused on ensuring that care services are provided in respectful, dignified, compassionate and caring ways and that the cultures of practice support these ways of working). The PcPRC works with three themes that reflect the expertise of the research team – provision of care to older people, person-centred practice development (across a variety of health care settings) and methodology development. The work of the PcPRC is known internationally with models, frameworks and tools developed by PcPRC researchers being replicated and implemented in countries around the world.

Centre for Health and Rehabilitation Technologies

Professor Suzanne McDonough Head of Centre The Centre for Health and Rehabilitation Technologies (CHaRT) is the largest multidisciplinary grouping of allied health profession (AHP) researchers in the UK. This Centre undertakes world-leading multidisciplinary translational research that underpins evidence-based practice, centred on the physical needs of people with chronic conditions, in order to improve health and wellbeing; and to transfer this knowledge to key stakeholders in order to directly impact on healthcare policy and practice. Partnerships at a strategic level e.g. Northern Ireland Regional Respiratory Forum and the NI Clinical Research Networks (NICRN, primary care and respiratory health), and with local Trusts (e.g. joint posts) and services users ensures the clinical relevance of the work. CHaRT makes a significant contribution to the Cochrane Collaboration (via reviews and training) and to the infrastructure and training required for high quality clinical trials both locally and Europe wide. CHaRT

focuses its research in three main areas: Musculoskeletal Disorders; Respiratory Health and Technology for Enablement and Connected Health.

05

Structure of the Institute of Nursing and Health Research In addition to the five research centres, two research groups have been established:

Education and Professional Issues Research Group Lead by Neal Cook and Professor Vivien Coates The overall aim of this group is to work to develop a national and international reputation for research and scholarship related to nursing and allied health education and professional issues. This will be through enhancing research undertaken within this area through development of novice researchers and support of more experienced researchers. The Education and Professional Issues group developed an action plan to structure the group and target activities over the year. There are now three theme groups which members align themselves to in order to support and enhance research activities that complement each other. These are: 1. Supporting Practice Learning 2. Innovative approaches in Education 3. Enhancing Graduate and Professional Qualities/attributes

Research Capacity

Institute of Nursing and Health Research Mona Grey Pre-Registration Research Award 2012 From left: Professor Owen Barr, Head of School of Nursing, Patrick McBrearty, winner and Chancellor Dr James Nesbitt.

Building research capacity is a key activity for the INHR as we know that we need to continuously develop high calibre researchers who can work independently and who can contribute towards the advancement of knowledge and in delivery of health Services – we do this in three ways, 1) the supervision of PhD students, 2) the support of academic staff to develop research expertise, and, 3) the identification of talented graduates from our undergraduate programmes and enabling them to ‘fasttrack their research careers. The INHR has a large and vibrant community of doctoral students, including many international students. Currently there are more than 80 doctoral students studying in the INHR. The postgraduate research programme provides students with transferable skills in research and serves as the foundation for a career as an investigator and scholar. It also gives students the opportunity to work with internationally renowned scholars engaged in innovative

research that will make a difference in people’s lives and in healthcare. We are committed to offering excellent supervision and support to our doctoral students and our PhD supervisors are drawn mainly from the five research centres in the Institute. For academic staff we have an internal ‘Doctoral Studies Support Scheme’ that enables academic staff to complete doctoral studies and to compete for funding to provide them with protected time for research. So far we have supported 12 staff to develop their research profiles, complete doctoral studies, secure external funding and build international partnerships. We support a ‘new researcher’ award for undergraduate and postgraduate students. These awards recognise excellence in research and provide small scholarships to enable further study that will contribute towards a PhD.

New Publication on Rare Chromosome Abnormalities (RCA) “Rare chromosome abnormalities, prevalence and prenatal diagnosis rates from population based congenital anomaly registers in Europe” has now been published in European Journal of Human Genetics. This paper, using data from 16 EUROCAT registers, is one of the first to offer epidemiological data on the full range of rare chromosome abnormalities. In our study there were 10,323 cases with a chromosome abnormality giving an overall prevalence of 43.8 per 10,000 births. 71% were trisomy 21, 18 or 13, 5% had a sex chromosome trisomy and 8% 45,X. The remaining 1737 cases (17%) had a rare chromosome abnormality giving a prevalence of 7.4 per 10,000 births. These included triploidy, other trisomies, marker chromosomes, unbalanced translocations, deletions and duplications. There was a wide variation between the registers in both the overall prenatal diagnosis rate of RCA, an average of 65% (range 5-92%) and the prevalence of RCA (range 2.4-12.9/10 000 births). In all, 49% were liveborn. These data provide the only baseline prevalence figures currently available for health service planning for the management and care of people with a rare chromosome abnormality. The live birth prevalence rate of 3.7/10 000 births of long-term survivors with an RCA is significant and may be used to guide long-term healthcare for affected individuals.

Views and Experiences of PhD students PROJECT TITLE Reducing radiation dose in paediatric interventional cardiology: An investigation into the effects on image quality and DNA integrity The aim of my project is to determine the feasibility of implementing a method for reducing ionising radiation to children undergoing cardiac procedures. In addition to this I will also be investigating the effect that these levels of radiation have upon DNA by observing the amount of double strand breakages which are known to occur from ionising radiation.

Richard Gould (HSC R&D Fellow/1st Year PhD Student

Having worked as a radiographer in general radiography, magnetic resonance imaging and cardiology in Belfast over a 10 year period, beginning a full-time PhD at Ulster has provided me with new challenges as I prepare for my 1st year initial assessment. The completion of my MSc part-time last year at Ulster investigating DNA damage from low dose ionising radiation has helped provide me with some of the skills required for my PhD project. I am now competent in performing laboratory work, which is an entirely new skill from radiography, and I am learning further about the principles and application of digital radiography. Although at an early stage, I am already gaining skills in time management, problem solving and communication. My project has been funded by the HSC Research and Development division who have provided me with the opportunity to undertake a worthwhile project and to receive doctoral level education. I am the first radiographer in Northern Ireland to have attained an R&D Fellowship.

I am aware that there may be a lot of setbacks, problems and hard work ahead but I am fully committed to achieving the aims and objectives of my work. I would not be in this position it were not for the help and support of both clinical and academic staff that have supported my funding application and project so far. I have recently started to collect results from preliminary work at Ulster which has provided me with fulfilment and I look forward to the overall outcome and dissemination of the project. Supervisors Dr Ciara Hughes Dr Sonja McFadden Richard is a member of the Centre for Health and Rehabilitation Technologies (CHaRT)

Postgraduate Research wonderful people became my supervisors when I eventually became a PhD student in early 2009. Through their guidance I gained confidence in my study and developed an intellectual understanding of the theories and principles underpinning the research and outcomes of my work. As a mature student and a non-academic there were times when I doubted my ability to achieve my goal, as I was out of my natural environment of hospitals and away from the patients. The supervisors encouraged and supported me through my many crises and Professor Mc Cormack, in particular was always there to give generously of his time and wisdom. The roller coaster of the journey could not have been tolerated without the unconditional love of the family but also the friendship of my fellow students.

As a former Chief Nurse and Patient Safety Officer, in the Gulf region, I found that the greatest challenge that leadership faced in this region, in relation to patient safety, was that the evidenced based standards set by the organization were not always followed through in the patient care areas and staff were reticent to report non-compliance or difficulties with the overarching policies, procedures and practice guidelines. My practice interests centred on critical care nursing but my career has opened me up to the vast and complex world of healthcare as a senior administrator, and even though in my last organization we introduced a non-punitive response to error policy and what I believed was a nonthreatening medical error reporting process, there was still under-reporting of errors and reporting of safety issues in general. The literature could not help me, as I had applied all the relevant strategies advocated by the patient safety experts. In 2007 I met Professor Carol Curran and discussed possible ways of investigating this issue ending with a suggestion that perhaps I should consider studying the problem myself. Subsequently Carol introduced me to Professor Brendan McCormack. These two

My thesis presents and discusses findings of an ethnographic study exploring how the culture of a multicultural Middle Eastern healthcare institution facilitates patient safety. The conceptual framework is based on an adaptation of Schein’s organizational culture assessment model informed by discourse analysis influenced by the French philosopher Michel Foucault’s approach to governmentality, to provide for a micro and macro level analysis of the study. The two key findings include: The routine non-conformity with set standards and patient safety related policies originate from the interconnectedness between the organization’s adaptation to its external environment and the internal relationship among its group members. The unintended consequences of the subsequent strategic position of changing the vision focus, to becoming internationally recognised for healthcare excellence through accreditation, has led to internal and external regulatory and disciplinary technologies and the subsequent inherent resistive actions. As I concluded in my thesis, I believe that the lessons learned through my research are invaluable to the leaders in this organization and other organizations in the Gulf. I also believe that the challenges faced by the staff and managers on a daily basis are a global problem but the solution is firmly embedded in local contexts. I am negotiating, presently, for a position as Director of Quality and Patient Safety in the region and hope that together we can work on those solutions in the future. What our students have to say ‘Investigating the decision making process in patients with non-curative cancer who have been invited to join a clinical research trial’. I am now in my third and final year of study. There is no doubt that the last two years have been full of challenges and hurdles. However, I have learnt so much along the way, not just relating to my study, but also during training courses provided by the university. The support and encouragement from my supervisors and peers has been wonderful. I have also felt very privileged that patients with incurable cancer have given up some of their very precious time, to chat with me about their decision regarding a clinical trial. I do believe that I will emerge at the end of this as a more rounded, knowledgeable and resilient individual. Purpose of study: To address the gap in knowledge and to increase understanding of the real issues patients consider when making decisions regarding trial consent. Part of its aim is to provide explanations for non-participation by some patients. It also aims to make known the issues that research doctors and nurses feel are relevant for their patients, and by doing so, potentially finding common themes and indeed differences in what staff and patients feel are important. By conducting this study we have the potential to influence service improvement by contributing new knowledge on decision making by patients with incurable cancer, who have been invited to join a clinical trial. This knowledge should help clinicians base their decisions on sound research, during the recruitment process for clinical trials.

‘Pre-hospital delay in patients presenting with myocardial infarction in Kingdom of Saudi Arabia’. I am Hassan Alshahrani, from Saudi Arabia and I am currently a PhD student in the INHR. My study is using a mixed methods approach and thesis is entitled: “Prehospital delay in patients presenting with myocardial infarction in Kingdom of Saudi Arabia”. I took the opportunity to study in this prestigious University after reading about it and I took into consideration their high ranking that the INHR achieved in the previous Research Assessment Exercise and the good reputation they have in the UK and internationally. Studying in the INHR is a good choice for any student to develop their research expertise and personal skills. The INHR allows all students to build a good relationship with people who are interested in the same area of research. As a student in the INHR I have received a lot of support and valuable feedback from my qualified supervisors, which encouraged me to overcome the difficulties encountered. Honestly, the INHR team helped me to look for a better future.

Postgraduate Research Success of INHR PHD Students at Collaborative Action Research Network Conference The work of Institute of Nursing and Health Research doctoral students was commended at a major Action Research Conference in November. The conference combined the Collaborative Action Research Network (CARN) and International Practice Development Collaborative (IPDC) and was hosted by the England Centre for Practice Development at Canterbury Christchurch University (CCCU) in Kent. PhD student, Famke van Lieshout, delivered an outstanding keynote presentation on action research which was very well received by the conference.

“My presentation was entitled: Taking Action for Action: A Praxiological Analysis of the Interplay between Contextual and Facilitator Characteristics in Developing an Effective Workplace Culture in a Dutch Hospital Setting, through Action Research. It was a great honour to be invited as a keynote speaker to share my key study findings, in relation to what conditions are essential in facilitating participatory action research. My greatest challenge was to articulate my message in a clear and inspiring way to the audience that would trigger them to reflect on their own practices in supporting researchers or students that are doing action research, and to find confirmation for my newly developed model. I think I succeeded in this, as I got an unexpected applause after I shared my personal story of being a facilitator of PAR in a turbulent context through the use of a sailing metaphor. Also, there was a lot interest in the model. Further encouraging responses from a variety of attendants was an added boost for me to continue the writing of my thesis and the publication of this work. This opportunity feels more of a beginning than a closure, and definitely has put the wind in my sails!”

Fellow doctoral student, Catherine Buckley, won the student poster prize and the overall conference poster prize for her poster entitled: “Working in a Storied way: Changing practice through narrative inquiry.”

“My poster was presented in the format of a daily newspaper because I wanted to showcase my work in a more narrative way. I was delighted when I was awarded both the student and overall conference prizes. The support and direction I receive from my supervisors, Professor Brendan McCormack and Dr Assumpta Ryan and engaging with other doctoral students in the INHR Action Research Community of Practice (AR CoP) has enabled me to be both creative and critical in my PhD work.”

Caroline Williams collected the student runner-up prize on behalf of the AR CoP with a poster entitled: “Creating a safe space for growth with a Community of Practice.”

The poster was about our Action Research Community of Practice (CoP) which was set up in 2010 by PhD students from within the Person-centred Practice Research Centre. Our aim is to enable mutual support, and to develop and share knowledge around action-oriented methodologies, facilitation, and researching practice development. As a Community of Practice we meet in the University twice a year, with our members travelling from The Netherlands, Norway, Wales, England and the Republic of Ireland to join up with those in Northern Ireland. A Professor and a PhD

student from Canterbury Christ Church University are also part of our membership. Members include Ulster PhD students: Catherine Buckley, Shaun Cardiff, Donna Frost, Brighide Lynch, Famke van Lieshout, Tone Elin Mekki, Jill Murphy, Orla Watt and Michele Hardiman (CCCU PhD student). Professors Brendan McCormack, Jan Dewing (Canterbury Christ Church University) and Angie Titchen (Fontys University of Applied Sciences), Although I designed the poster and organised the contributions, the content was truly a collaborative effort – no mean feat when some of the members were in Australia at the time! Preparing the poster was undertaken as a learning opportunity for me, so to win a prize at the end of it was a real bonus! I was very lucky that the CoP began when I was just coming towards the end of the first year of my part-time PhD. It has been a lifeline for me, and I have made friendships, received support, learned loads, and as I gain in experience and confidence I am now starting to offer support to others. The University of Ulster acted as a collaborator in the event. Professor Brendan McCormack, Director of the Institute, commented:

G N I S S I M

The University of Ulster is proud to be a member of the International Practice Development Collaborative (IPDC). Postgraduate Research The Person-Centred Practice Research International Community of Practice The Person-centred Practice Research Centre has developed a formal partnership with Canterbury Christ Church University England, Fontys University of Applied Science The Netherlands, Buskerud University College Norway, West Park Healthcare Toronto Canada and The University of Technology Sydney, to develop a ‘Person-centred Practice Research International Community of Practice’ [ICoP] The focus of the ICoP is to establish a collaboration in order to strengthen a programme of research and practice development between the parties with the intention of advancing knowledge, skills and expertise in person-centred practice, practice development and research (with a particular focus on practitioner research). Person-centred practice is currently a dominant focus in policy, strategy, education and practice developments. Whilst a variety of ‘alternative’ perspectives to person-centred care have become prominent (such as relationship-centred care, compassionate care, dignified care), a broad focus on person-centredness encapsulates all these agendas and ensures that we never lose sight of developing services that have ‘persons’ at the centre of their attention. The ICoP is enabling collaborators to actively participate in the development of collaborative and mutual communities of learning about person-centred practice, person-centred practice development and methodologies for research into person-centredness. In addition it will support collaborative working between facilitators, lecturers and professors in the development of resource materials, publications and research activities. It is further envisaged that the exchange of staff for developmental purposes will be facilitated as well as the exchange of visiting students/staff who wish to study or engage in collaborative practice at one of the collaborating institutions. One success of the ICoP is the hosting of an international action research residential school. Pictured below are facilitators and participants at the 2011 school hosted by Buskerud University College and The University of Stavanger, Norway and in association with the University of Southern Denmark.

Research Week 2012 The Institute is delighted to have held its first ever ‘research week’ in May 2012 to profile and develop the Institute’s work. For the first time, our visiting professors from all over the world came together to give master classes, seminars and workshops, at the University’s Jordanstown campus. Our distinguished visiting speakers included: Prof Mi Ja Kim, Prof Mike Clarke, Prof Domhnall MacAuley, Prof Debra Moser, Prof Ruth Northway, Prof Jan Dewing, Prof Angie Titchen, Dr Nicola Armstrong, Prof Val Wilson, Dr Janice Bailie, Prof Soo Downe and Prof Jo RycroftMalone. Researchers from across the university and colleagues in health and social care services joined us in seminars, conferences and master classes alongside partners from across healthcare in sharing knowledge, expertise and networks. Dr Sonja McIlfatrick, lead for the palliative care research strand also works half time as Head of Research for the All Ireland Institute of Hospice and Palliative Care (AIIHPC). AIIHPC launched a €1.25 million Structured Research Network for palliative care research in September 2012. AIIHPC was awarded funding for the Network by the Health Research Board and it brings together experienced researchers from the Republic of Ireland and Northern Ireland from a broad range of disciplines, academic institutions and service sites, to work together on research and capacity-building initiatives which aim to improve the future quality of care for patients across the island. Within the Network, there are two complementary research strands, each incorporating several work projects. The first strand takes a social justice approach while the second strand focuses on measurement and evaluation.

Current projects include evaluation of: the Regional Palliative and End-of-Life Care Learning and Development Programme for Nursing Home staff; the Carers Support Programme; Projects in Development include: An Exploration of Existential Concerns of Hospice Patients and staff; An Uptake of Interventions Offered at the Day Hospice Service; An Evaluation of Volunteering within Adult Services. Northern Ireland Hospice has a strong culture of service evaluation and audit and links have been established with its Audit Steering Group to consider the suitability of completed audits for publication and to inform potential research work A newsletter outlining the activities of the committee entitled Research Update is produced twice a year: http:www.nihospiceeducation.org

Research News Person-Centred Practice Research Centre (PcPRC) Research into person-centredness undertaken by Ulster researchers has resulted in the development of a Personcentred Practice Framework (McCormack & McCance 2010) and this framework continues to be developed and tested in a range of projects locally, nationally and internationally. The uniqueness of our work lies in the integrated approach we have adopted to teaching/ learning, research and development activities through the use of the person-centred practice framework alongside a framework for the continuous development of practice. The practice development framework of McCormack and others (McCormack & Garbett 2002 and 2004; Manley et al 2010) is an internationally recognised framework for developing practice and is used in large scale change programmes in Northern Ireland, the Republic of Ireland, Canada, Australia, Norway and the Netherlands. The central philosophy of the framework is that of creating the conditions for nurses and multidisciplinary teams to become empowered to transform the cultures and context of practice in order to ensure that it is more person-centred and informed by the best available evidence. One example of the framework being used is that of ‘The Older Person’s National Practice Development Programme’ in the Republic of Ireland.

The Older Person’s National Practice Development Programme’ in the Republic of Ireland (2008-2011) The overall aim of the programme was to implement a framework for person-centred practice for older people across multiple settings in Ireland, through a collaborative facilitation model and to carry out an evaluation of the processes and outcomes. Eighteen residential units for older people were involved in the programme. Practice development programme groups were established. The groups represented staff from different areas within the units and different grades i.e. Clinical Nurse Mangers, Staff Nurses, Health Care Assistants, Housekeeping, Catering and Administration staff. The participants from the sites met with an internal facilitator from within their unit and an external facilitator from the University on a regular basis. The programme had a number of visible activities that took place on a regular basis. Overarching these ‘events’ the programme activities principally involved: (i) developing an understanding of what the work/practice development involves and the competence and confidence to role model the processes to be used (ii) becoming familiar with the Person-centred Nursing Framework and Practice Development Model (iii) developing an understanding of workplace culture and ‘change’ processes (iv) awareness raising activities for different staff groups, older people and families in the programme sites (v) developing a shared vision using Values Clarification Exercises involving the Residents/Patients Families/Carers and all staff within their work place (vi) Active Learning in the workplace (vii) structured reflection (viii) facilitation skills development (iv) developing greater appreciation of and skills in effective group and team working and (v) working with evaluation methodology and methods.

Significant progress was made in achieving numerous attributes of a person-centred culture. There is no end point to person-centred cultures, thus further development is still needed. There were significant outcomes for nursing and care staff and were statistically proven. These changes are significant in terms of developing a personcentred culture. Statistically significant changes included: Better staff preparation for their role Improved staff support Better knowledge among staff of treatment decisions More effective communication and support Greater role satisfaction More stable staffing and resources Staff feeling more committed to their work Better workload management For residents and families, the data demonstrated significant changes in care practices that resulted in an impact on four key areas of care experiences for older people and their families, with each of them showing qualitatively a change in the practice culture: A greater sense of hope and less feeling of hopelessness. Residents and families having a greater range of choices. Families feeling a greater sense of belonging and connectedness. Residents having a greater range of activities to engage in. More meaningful relationships between staff, residents and families.

Some of these changes are shown in these quotes: “The staff are friendly and they would do anything for you, even back a horse”. “I love to get up at six. When I was at home I would go to bed at midnight and get up at 6am. I would hate it if someone told me I couldn’t get up till 10am- I like my routine”. “Feels like a hotel here, you have everything you want. It is the staff really, they leave their troubles behind and focus on you, have a laugh and a joke and attend to your questions, you can ask them anything and they are very attentive”. “We made cream slices and were barely hot out of the oven before they were eaten”. “I like my own room and love watching the telly. I am a real telly addict”. “I like the company in here, you always have someone to talk to when you want. The nurses are brilliant, especially the wee blonde Sister”.

Promoting person-centred practice within children’s hospital services through the use of key performance indicators: an international study The aim of this international pilot study is to test the use of 8 key performance indicators (KPIs) across twelve practice settings in Australia and Europe that care for children and young people to see if the information gathered is a useful way to help teams identify areas for development to improve the quality of care they deliver. The 8 KPIs are: 1. Consistent delivery of nursing care against identified need 2. Patient’s confidence in the knowledge and skills of the nurse 3. Patient’s sense of safety whilst under the care of the nurse 4. Patient involvement in decisions made about their nursing care 5. Time spent by nurses with the patient 6. Respect from the nurse for patient’s preference and choice 7. Nurse’s support for patients to care for themselves, where appropriate 8. Nurse’s understanding of what is important to the patient.

Right: KPI International Paediatric Project Workshop on 5 December 2012

The study involves the use of different approaches to obtain information on the KPIs, which will include patient surveys, stories with parents and children/young people, periods of observation in practice settings, and reviewing patient records. The results of this study have the potential to provide robust information to nursing teams with the purpose of implementing changes in practice in order to improve care within children’s services.

EUROCAT Swine Flu Response

Dr. Rhonda Curran (MFIR) represented the University of Ulster and EUROCAT by delivering a presentation The challenges of health inequalities in the treatment and prevention of birth defects in Europe and being a panel member at an information workshop for MEPs and NGOs at the European Parliament, Brussels, October 2012.

EUROCAT is responding to the Swine Flu pandemic by analysing congenital anomaly data in relation to swine flu, neuraminidase inhibitors and pandemic vaccine. As part of this study, we conducted a survey of European pandemic influenza vaccination campaigns. This survey, based on questionnaires sent to EUROCAT registry leaders and European national authorities via the European Medicines Agency found the vast majority of European countries had pandemic influenza vaccination policies in force and all of the countries with pandemic influenza vaccination policies in force considered second and third trimester pregnant women a priority group for vaccination. Differences between countries in pandemic vaccines used for pregnant women and in vaccination of first trimester pregnant women were identified. Congenital anomaly risk, if any, would be related to first trimester use.
