Process Safety Information (PSI)

Process Hazards Analysis

The PSI provides key information related to the hazards of the HHCs used or produced in the process. As well, the PSI considers technology and equipment deployed in the process. The PSI enables the operators to identify, understand, and control the hazards posed by the processes involving HHCs.

Federal PSM Regulations require that all PSMcovered facilities shall perform an initial process hazard analysis (PHA) on processes covered by this standard. The PHA shall be appropriate to the complexity of the process and shall identify, evaluate, and control the hazards involved in the process.

Compliance Audits are intended to verify that the procedures, practices, and documentation required under the PSM standards are adequate and being followed at your facility as per OSHA 1910.119 (o).

PSM regulations require development and maintenance of written operating procedures that provide clear instructions for; tasks to be performed, data to be recorded, operating conditions to be maintained, samples to be collected, and safety and health precautions to be taken. Operating procedures should be reviewed by engineering staff and operating personnel to ensure accuracy. Operating pocedures should also provide practical instructions on how to safely perform job duties.

In-house audits are discouraged and often challenged by OSHA inspectors due to recognized potential for biases.

Mechanical Integrity

Training

PSM regulations require that a mechanical integrity (MI) program be in place to ensure that all equipment used to process, store, or handle highly hazardous chemicals is designed, constructed, installed, and maintained to minimize the risk of releases of such chemicals.

Good employee training is one of the most important steps to improve employee safety. Training helps employees understand what type of issues they may encounter while operating their process and what may cause these problems. Training can improve employee awareness of the process hazards involved in their jobs.

Requirements: • Inspections and Tests Periodically • Frequency of inspections and tests should be consistent with manufacturers’ recommendations, follow good engineering practices, and occur more often, if necessary based on prior operating experience.

Incident Investigation PSM regulation requires that all incidents, including near misses, involving HHCs that could reasonably have resulted in a catastrophic release be investigated. The investigation should determine causal factors so that appropriate corrective actions can be developed and implemented to prevent similar events from occurring.
