1 Select the MyPolkSchools Internet icon from your desktop. Or In the Internet Address line: •

Type http://www.mypolkschools.net



Press Enter

Under Account Login:

Type your First Name. Last Name into the username field



Type your Network (Outlook) password into the password field



Click the Login button

Once you’ve logged on: •

Move the cursor over Student Services



Move the cursor over Discipline Referral



Click Discipline Referral

To View / Edit existing Student Referrals •

Click the My Referrals button

The program will display the Referral List screen where you can Enter the appropriate information to retrieve previously entered referrals •

The program will display the Referral List screen. From left to right you will see the following Column Headers: View Edit Student Id Last Name First Name Referral num Referral Date

Referral Status Description Course Period TCHR ONLY

To View referrals for a specific student: •

Click the View button into the far left of that students information A printable Student Discipline Referral document is displayed. •

Click the Printer icon to print



Click the X in the top right corner to close the PDF document

To Edit referrals for a specific student: •

Click the Edit button to the far left of that students information

The same fields are available to Edit as are available on the initial Student Referral screens: See steps 8-10 •

Click the Back button twice to return to the Student Lookup screen.

From the Student Lookup screen, there are several different ways to search for a Student enrolled at your school. To search for a Student enrolled at your school by name In the Selected by name field •

Type all, or part, of the Students Last Name



Type all, or part, of the Students First Name



Click the Get Students button

6 A list of all the Students at your school matching those criteria is displayed. •

Click the Student Id number to view that Students Referral information

Or To search for a Student in one of your classes by period: In the Selected by Period field

Click the drop down arrow to view the Selected by Period list



Click the appropriate Period



Click the Get Students button

7 A list of students in your class for the selected period is displayed in Student Listing

To View / Edit / or create a new Discipline Referral for a specific Student •

Locate the appropriate Student on the list



Click the Student Id number for that Students Referral information

8 To Create a New Referral for the Student selected •

Click the Create New Referral button

NOTE: The current date and time are assigned by the system as default and can be changed. •

Type in the Referral Date:



Click the Calendar icon to use the Calendar



Type in the Referral Time:

9 In the Brief Description field •

Type a Brief Description of the incident

Examples: (tardy, continuously disrupts class, fighting) In the Detailed Description field •

Type a detailed and specific account of the incident explaining all of the reasons for the referral

NOTE: If more than one student will be included into this student referral their name (s) will need to entered into the Detailed Description. After the descriptions are completed, there are 2 options to save the referral.

10 NOTE: The default option is Teacher Use Only/Teacher Actions (Click Save)

Teacher Use Only/Teacher Actions: This option allows the teacher the ability to track actions taken when there is a discipline concern with a Student that could result in sending a referral to the office ex: (Tardies, talking in class, dress code violations, etc.). Once Save is Clicked, using the Teacher Use Only option, the form will be displayed with these additional options: Follow Up Note:

Click Save

Teachers Actions: •

Click the drop down arrow to the far right of the Action taken by teacher prior to referral: field



Click the line that defines the action taken to select that action

Action Date: Or



Type in the Action Date:



Click the Calendar icon to use the Calendar Click a Date to select it

• Time: • Type the Action Time Comment: • Type in Comments •

Click the Add Action button

After you have Clicked Add Action the Teacher Action will display in the Action Taken Section •

Click the Delete button to remove an Action

If you are sure you want to delete the referral: •

Click the Yes button

The program will return to the Referral List screen where you can Enter the appropriate information to retrieve previously entered referrals or begin a new Student Referral.

13 Send to Office: If the Student’s action is severe enough to result in an immediate referral without any prior teacher actions Enter the information as described in the Teacher Use Only/Teacher Actions: steps 8-12 •

Click the Send to Office option



Click Save

14 NOTE: Student Referrals can be edited until they are viewed by an Administrator in Genesis, then they are placed into a Pending Status and cannot be edited.
