Hospitalist Program In August 2010, Danville Regional Medical Center partnered with Apogee Physicians, one of the nation’s leading hospitalist groups. A hospitalist is a specialist in internal medicine who solely treats patients in the hospital around the clock. Uniquely, Apogee is completely owned and operated by physicians who understand the business of medicine inside and out. The company is obsessively passionate about being the best and is guided by their mission statement, “What’s best for the patient is best for the practice.”

Hospitalist FAQs

What is a hospitalist? A hospitalist is a medical doctor who specializes in caring for hospitalized patients. Hospitalists are trained in general internal medicine and other medical specialties. Because hospitalists do not maintain outside clinics, they can focus their attention on your medical care inside the hospital. What are the benefits of having a hospitalist? Your hospitalist specializes in the kinds of medical conditions that need to be treated in the hospital. Because the hospitalist does not have an outside clinic, he or she has added knowledge of other departments and specialties and is very accessible to hospital nursing staff. The hospitalists can help to speed your recovery and shorten your hospital stay by following up on tests and adjusting your treatment plan throughout the day. Hospitalists

have systems in place to communicate with your family and your primary care doctor as needed, especially at the time of admission and discharge.

Why might I see several Hospitalists physicians during my hospital stay? Hospitalists are a small group of specialists who cover admitted hospital patients 24 hours a day, seven days a week and 365 days a year. Because no one person could do this on his or her own, they work as a team. This may result in you seeing more than one hospitalist during your stay at DRMC. What is the relationship between the hospitalists and my primary care doctor? The hospitalist partners with your primary care doctor to administer and oversee your treatment while you are in the hospital, serving as your regular doctor until you are discharged. The hospitalist will see you daily and manage your conditions, contacting your primary care doctor as appropriate and at the time of discharge. When you are discharged, you will return to your regular doctor or other communitybased caregiver to continue the care plan coordinated between your primary care provider and the hospitalist. Can the hospitalist become my primary care provider? No. Patients retain their primary care provider and return to their regular caregiver after their release from the hospital. We recognize that an important part of this is ensuring the availability of tasty and nutritious meals, carefully prepared and quickly served to our patients at their bedside. That’s why we’ve partnered with Sodexo, an international food services expert, to bring to our patients a new personal meal service option called “Expressly For You.” “Expressly For You” gives patients individual service in choosing meals that are tasty and nutritious, but also supports a patient’s nutritional goals as set for him or her by his or her physician.

The Way It Works nU  sing our new menu, a DRMC food services host/hostess will assist you by taking your breakfast, lunch and dinner meal order(s) at your bedside. n Th  e host/hostess will visit your room every morning to get your lunch order, and visit with you again in the afternoon to get your dinner and next day’s breakfast meal order(s). nY  ou can select from our “Chef ’s Special” of the day. You also may choose a lighter meal option.

Our entire nutrition and food services team is here for our patients, family members and visitors. Questions? Please speak with your nurse or host/hostess.

For Diabetes Management

Your meals will be delivered on a schedule to allow for appropriate blood testing and medication dosage. Diabetic guidelines are available upon request. Some items may not be allowed or may be modified. Please let us know if you have any food allergies or questions about your diet.

Interpreter Services

If it’s difficult for you to communicate for any reason, you can request interpreter services. We have interpreters for foreign languages or for sign language. A webcam is also available for face-to-face interpretation. Please communicate with your nurse if you require this service.

Connect To Guest Network Our Acceptable Usage Policy will appear. After reviewing the policy, you will need to click the “I Agree” button to continue. For assistance, the DRMC Help Desk is available Monday through Friday from 8:00 a.m. to 5:00 p.m. by calling extension 4400. Our goal in Environmental Services (EVS) is to provide you with a clean, germ-free and safe environment, as well as ensure you are satisfied with the cleaning of your room and bathroom daily. If you have any cleaning concerns, please call 3821 and an EVS Tech will gladly come and take care of them.

To send an e-card just fill out an online form with as much information as possible, and we will do the rest! Registered Dietitians

Diet is an integral part of the patient’s treatment program. Registered dietitians work closely with the patient’s physician to develop a dietary program based on each patient’s nutritional requirements. Dietitians also provide patient education and guidance for meal planning.

Family Counseling/ Social Work Services

Discharge planning involves a multidisciplinary team of healthcare professionals who evaluates the patient’s capacity to self-care, the possible need for post-hospital services and the availability of the anticipated post-discharge services. In collaboration with the patient, family and/or other healthcare decision-maker, the hospital can assist in the assessment for eligibility for services such as home health, hospice, nursing home placement and personal care services.

Leave Your Valuables at Home

If you have jewelry or other valuables, please give them to a relative or friend to take care of during your stay. Store contact lenses, eyeglasses, hearing aids and dentures in cup provided when not in use. Please don’t put them on your bed or food tray—they may be damaged or lost.

Valet Parking

Valet parking is available at no charge for our patients and their families. The valet podium is located at our main entrance from 6:00 a.m. – 4:00 p.m. on Monday and Friday.

Escorts to the Parking Lots

Escorts to parking lots are provided upon request. Dial 7941 or 0 (for the operator) from any hospital phone to reach a security officer for an escort.

Visitation Guidelines

n Patients will be able to continue

interactions with family and significant others. The patient or his/her support person has the right to chose who may visit during the hospital stay. A support person may be a family member, friend or other individual who is there to support the patient during the course of the hospital stay. Any restrictions to visitation are solely based on clinical reasoning or safety concerns and not on the basis of race, color, national origin, religion, sex, gender identity, sexual orientation, or disability. During Your Stay continued employees. DRMC operates under a “reasonable expectation rule.” The patient care staff generally establishes reasonable expectations, based upon the needs of the patients. Therefore, the expectations will not necessarily always be the same but will be based upon the needs of individual patients. There are several expectations that are uniform as listed in this policy. n Children less than 16 years of age must be accompanied by, and remain in the presence of, a non-patient adult at all times while in any area of the medical center. n The hospital discourages visitation by children under the age of six years. n Please do not bring food, drinks or medications to the patient until you check with nursing staff. n Please wash your hands before and after you visit every patient. If there is a Precautions sign on the door, please see the nurse before you visit for the first time. n We ask that you refrain from visiting patients if you have a stomach bug, flu or any illness with a fever. n While visitors are encouraged to comfort and support patients, we ask that you not sit or lie in a hospital bed or use the bathroom facilities in a patient’s room. Contact your nurse to see if a sleeper chair is available. n O vernight visitors are permitted in private and semi-private rooms with approval from the nursing staff. n Private Rooms – Only one overnight visitor (18 years or older) is permitted. 10

n All hospital doors, with the exception

of the entrance at the Emergency Department, will be locked or monitored from 9:00 p.m. until 5:00 a.m. Visitors with family emergencies must check in at the Emergency Room for a visitor’s pass. n Danville Regional Medical Center is a tobacco-free facility. Use of tobacco products is not permitted on campus. n

DRMC reserves the right to deny access to any or all persons. Visitors and/or patients may not possess firearms or other weapons nor illegal drugs or alcoholic beverages on the premises. Visitors found to be under the influence of drugs or alcohol will be asked to leave the premises. In order to ensure safety for patients, all associates and visitors’ packages are subject to being searched upon entering and leaving the hospital.

n A “No Visitors” sign may be placed

on the patient’s room door upon request of the patient, family or physician. n As a courtesy to our patients, we can re-label any multi-dose medication prescribed to you during your stay for a minimal cost of $3. This includes medications such as insulin pens, inhalers, eye drops, etc.

Our Chapel is open at all hours to all people, patients, families and staff. The chapel is an ideal place for sanctuary, to refocus, to find a sense of place, to pray, meditate or just be alone with your thoughts.

Directions The Chapel is located on the third floor in the “B” building. Take the A or B elevator and turn right, going directly straight ahead to the end of the hallway, past the waiting area.

Pastoral Care

At Danville Regional we are committed to bringing people to health and wholeness. The staff of our pastoral care department provides services that are designed to address the spiritual dimensions of illness. Spiritual health is a vital component of our overall sense of health and well-being. To provide these spiritual support services, we have a staff of chaplains drawn from many different denominations. These compassionate men and women have extensive training in theology, psychology and healthcare ministry. Chaplains are available 24 hours a day for patients and families of all faith traditions. To arrange for a chaplain, please ask your nurse or call the Pastoral Care Department at 434.799.3731.

Volunteers

Danville Regional has a dedicated group of adult and teen volunteers who donate thousands of hours each year. Their cheerfulness and dedication add a personal touch, contributing to the comfort and happiness of our patients and families. By volunteering, you become an important and integral part of Danville Regional. It’s easy to begin as a volunteer. your hospital will treat you and your family as partners in your own care. One important way that you can be involved is to speak up. Ask questions, voice your concerns, and don’t be afraid to raise any issues relating not only to your care and treatment, but also to overall hospital services. Stay Safe You can contribute to healthcare safety. While you are in the hospital, many people will enter your room, from doctors and nurses to aides and orderlies. The following information will help make your hospital stay safe and comfortable.

Employee ID All hospital employees wear identification badges. Feel free to ask any staff member for his or her name and job classification. If you do not see an ID badge on an employee, please call your nurse immediately.

Don’t Be Afraid to Ask… A number of people may enter your hospital room. Be sure to: n Ask for the ID of everyone who comes into your room. n Speak up if hospital staff doesn’t ask to check your ID. nA  sk if the person has washed his or her hands before he or she touches you. n I f you are told you need certain tests or procedures, ask why you need them, when they will happen and how long it will be before you get the results. 16

PATIENT IDENTIFICATION Any time staff members enter your room to administer medications, transport you or perform procedures and treatments, they must verify your identity. At times, you may be asked the same questions repeatedly. We are aware that this may be annoying. Please understand that this verification process is a critical component in our patient safety program in order to guarantee that all of our patients receive the correct medications and treatments.

Know Your Meds While you are hospitalized, your doctor may prescribe medications for you. Be sure that you understand exactly what they are and why they are being prescribed. Use this checklist to help you get the information you need from your doctor: What is the name of the medicine? What is its generic name? Why am I taking this medicine? What dose will I be taking? How often, and for how long? When will the medicine begin to work? What are the possible side effects? Can I take this medicine while taking my other medications or dietary supplements? Are there any foods, drinks or activities that I should avoid while taking this medicine? Should I take my medicine at meals or between meals? Do I need to take the medicine on an empty stomach or with food or a whole glass of water? What should I do if I forget to take the medicine and miss a dose?



Medication Safety Be sure that all of your doctors know what medications you have been taking, including prescription drugs, over-thecounter medications, herbal and vitamin supplements, natural remedies and recreational drugs. Be sure that all of your doctors know of any allergies you may have— to medications, anesthesia, foods, latex products, etc. When you are brought medications or IV fluids, show that person your ID bracelet to verify your identity. Remember—you play an important role in medication safety.

Preventing Falls Hospital patients often fall because they are weak or unsteady due to illness or medical procedures, their medications make them dizzy, or they’ve been sitting or lying down for too long. Danville Regional cares about our patients’ safety. Please help us keep you safe by following these guidelines during your hospital stay: n Do not get out of bed by yourself. Your hospital bed is probably higher and narrower than your bed at home, and you can easily fall while trying to get in or out of it. Please use your nurse call button and ask for assistance. n Keep often-used items—call button, tissues, water, eyeglasses, telephone, TV remote— within easy reach. n Do not walk in bare feet. Wear non-skid socks or slippers. n Make sure your robe or pajamas don’t drag on the floor; they can cause you to trip. n Use the handrails in your bathroom and throughout the hospital at all times. n Ask your nurse to show you how to properly walk with your IV pole, drainage bags or any other equipment. n Be sure your wheelchair is locked when getting in or out of it. Never step on the footrest. n If you see a spill on the floor, report it at once.

DVT: Lower Your Risk Deep-vein thrombosis (DVT) occurs when blood clots form in the legs and block circulation. The clots can lodge in your brain, heart or lungs, causing damage or even death. When you’re hospitalized and in bed with limited physical activity, your risk of DVT increases. Ask your doctor about using compression boots or stockings and/or blood thinners to prevent DVT during your stay. And be sure to tell your doctor or nurse if you have any of the following symptoms: DVT Pain Swelling Discoloration (bluish, purplish or reddish skin color) Warmth Pulmonary Embolism Shortness of breath Chest pain (which may be worse with deep breath) Unexplained cough (may cough up blood) Unexplained rapid heart rate

Fighting Infections The single most important thing you can do to help prevent infections is to wash your hands and make sure that everyone who touches you—including your doctors and nurses—washes his or her hands, too.

You, your family and friends should wash hands: 1. after touching objects or surfaces in the hospital room 2. before eating 3. after using the restroom

It is also important that your healthcare providers wash their hands with either soap and water or an alcohol-based hand cleaner every time, both before and after they touch you. Healthcare providers know to practice hand hygiene, but sometimes they forget. You and your family should not be afraid or embarrassed to speak up and ask them to wash their hands.

Happy Birthday to You! Wash your hands with soap and warm water for 15 to 20 seconds. That’s about the same amount of time that it takes to sing the “Happy Birthday” song twice.

Be sure to:

n Cover your mouth and nose to prevent the spread

of infection to others. Many diseases are spread through sneezes and coughs. n Use a tissue! But be sure to throw the used tissue away and clean your hands after coughing or sneezing. If you don’t have a tissue, cough/sneeze into the bend of your elbow. n Make sure that your vaccinations are current—even for adults. Check with your doctor about shots you may need.

Doctors, nurses and other healthcare providers come into contact with a lot of bacteria and viruses. So before they treat you, ask them if they’ve cleaned their hands.

No Soap? No Problem. Alcohol-based hand cleaners are as effective as soap and water in killing germs. To use, apply the cleaner to the palm of your hand and rub your hands together. Keep rubbing over all the surfaces of your fingers and hands until they are dry.

Prevent the Spread of

Multi-Drug Resistant Organisms When you are admitted into the hospital you may have a nasal swab done to determine if you have MRSA (methicillinresistant Staphylococcus aureus). This is a multi-drug-resistant organism (MDRO) that is frequently found in patients. MDROs are bacteria that are resistant to antimicrobial drugs (antibiotics), so precautions must be taken to ensure that the MDRO isn’t spread. If your culture is positive, you will be placed on isolation. This means staff will wear gowns, gloves or masks when they treat you. Your family and visitors are expected to do the same to help prevent the spread of bacteria. These are the most common types of MDROs:

MRSA: Methicillin-Resistant Staphylococcus Aureus

MRSA is a bacterium that is carried on the skin or in the noses of healthy people and is a common cause of minor skin infections. It can also cause surgical wound infections, bloodstream infections and pneumonia. To prevent the spread of MRSA: n Cover your wound: Keep areas of affected skin covered with clean, dry bandages. n Clean your hands: Anyone in close contact should clean hands with soap and warm water or an alcohol-based sanitizer. n Do not share personal items: Avoid sharing clothing, towels or razors. n Talk to your doctor: Tell any of your healthcare providers that you have a MRSA infection. If you have a wound that does not heal or a boil or “spider bite” that looks infected, see your doctor.

E. coli: Escherichia coli E. coli is a type of bacteria found in your intestines and is usually harmless. This bacteria, highly resistant to antibiotics, can spread outside of your gut, and cause an infection. In hospitals, E. coli infections spread through contact with wounds or stool. To help prevent E. coli infections: n Tell your medical team if you were treated in another hospital or country recently. n Make sure that all healthcare providers clean their hands before and after caring for you. n Be sure to clean your hands often. n Take antibiotics only as prescribed.

VRE: VancomycinResistant Enterococci Enterococci are bacteria that are normally present in the intestines and in the female genital tract. Vancomycin is an antibiotic that is often used to treat infections cause by enterococci. When enterococci become resistant to vancomycin (not killed by it) it is called VRE. VRE can cause infections of the urinary tract or the bloodstream, or of wounds. Most VRE infections can be treated with antibiotics other than vancomycin. Smoking Cessation In the United States, tobacco use is responsible for nearly one in five deaths. Tobacco use accounts for at least 30 percent of all cancer deaths and 80 percent of lung cancer deaths. Besides lung cancer, tobacco use also increases the risk for other cancers. Each year, about 3,400 non-smoking adults die of lung cancer as a result of breathing secondhand smoke. (Source: Cancer Facts & Figures 2014) When you quit smoking, the risk of lung cancer, other cancers, heart attack, stroke, and chronic lung disease decreases.

Rapid Response What You Can Do If You Have Concerns about Your Condition

Our staff at DRMC strives to provide the highest quality of care to you and your loved ones. During your hospital stay, your condition may change and additional resources may be needed. The Rapid Response Team is a group of clinicians who will bring critical care expertise to your bedside to assist your nurse in determining the best treatment for your symptoms. Preparing to Leave the Hospital About Your Stay

n Your Physicians and Nurses will coordinate

scheduling of any final tests, such as blood work, X-rays, etc., as may be needed before your departure. Billing Billing: Insurance and Billing Information We apologize for requesting your insurance information each time you register at the hospital, but we have found this to be the best way to ensure accurate billing. Our staff submits claims for all verifiable and assigned health insurance including Medicare and Medicaid. It is important that you provide all related insurance information at the time of registration such as policy number, group number and the correct mailing address for your insurance company. If your insurance company delays settlement of your claim, we may look to you to assist us in encouraging them to pay or for payment. We request payment of co-pay, coinsurance and deducible amounts at the time of service. Discounts are available if you pay within two days of service. To take advantage of this discount offer, please contact our financial counselor at 434-799-2288. Once your insurance company has paid, we will bill you for any remaining amount. If you paid the estimated amount due up front, any remaining balance after insurance will be discounted at the same rate. If you are unable to pay your balance in full, it is important that you contact our Business Office to discuss payment arrangements. You can reach a representative at 434-799-3780.

If you would like for one of our financial counselors to visit you in the hospital to address questions or concerns with your insurance or payment of your account, please dial 2288 on your room phone. You also may visit our office on the first floor of the hospital near Outpatient Registration.

Other Bills You May Receive

During your visit, there may be services provided by independent physicians operating under contract. Hospitals and doctors charge separately. Therefore, services such as anesthesia administration, emergency room physician care, pathologist, radiologist, cardiologist and hospitalist services will be billed separately.

Precertification

Most insurance plans now require precertification for hospital stays, certain diagnostic tests and outpatient procedures in order for you to be eligible for your full policy benefits. It is important that you verify that this information has been completed prior to receiving your services. If you are unsure of your precertification requirements, we recommend you contact your insurance company as soon as a procedure or test has been scheduled. This could prevent a reduction in the amount your insurance pays.

Coordination of Benefits

Coordination of Benefits, referred to as COB, is a term used by insurance companies when you are covered by more than one insurance policy. This usually happens when spouses or partners are listed on each other’s insurance policies or both parents have medical insurance and automobile insurance benefits available for payment of your hospital service. Most insurance companies have COB provisions that determine who is the primary payer when medical expenses are incurred. This prevents duplicate payments. COB priority must be identified at admission in order to comply with insurance guidelines. Your insurance company may request a completed COB form from you before paying a claim. The hospital cannot provide this information to your insurance company. You must respond to any request from them before your claim will be paid.

If you are in need of assistance with your hospital bill, call our MedAssist representative at 434-799-2285. He or she will provide information about programs available for people without insurance, including Medicaid and the Health Insurance Marketplace. He or she also can assist you with the application process. Danville Regional provides financial assistance to patients based on their level of income, total acquired assets and general need. In addition, we will assist you in applying for medical assistance programs or arranging a manageable payment plan. Patients meeting our charity care guidelines could have their hospital bill waived in full for non-elective care. The most important thing you can do is let us know if you will have trouble paying your medical bill.

While you are making sure that your loved one’s needs are being met, don’t neglect your own. Caregiving is a stressful and time-consuming job. You may neglect your diet, your normal exercise routine, and your sleep needs. You may find that you have little or no time to spend with friends, to relax, or to just be by yourself for a while. But downtime is important. Don’t be reluctant to ask for help in caring for your loved one. Take advantage of friends’ offers to help and look into local adult daycare programs. Understanding Restraints A Guide for Patients and Families

A restraint is any device that limits or restricts freedom of movement, physical activity or normal access to the body. Even raised bed rails (on all sides) used to prevent a patient from getting up from bed is considered a form of restraint.

Why a Restraint May Be Necessary

When a patient is unable to cooperate with his or her care, restraints may help prevent injury to the patient or others. Restraints might be indicated when a patient: n Attempts to remove or dislodge tubes and lines n Picks at surgical wounds n Threatens or carries out violent or self-harm actions that put the patient, healthcare workers, other patients or visitors in jeopardy. Restraints are NEVER used for reasons of convenience, to punish a patient or to reduce the need for frequent patient checks.

Alternatives First

Because we value patient rights and dignity, the healthcare team tries alternative actions before using restraint. First and foremost, we recognize the importance of adequately managing pain, nausea, shortness of breath and other bothersome symptoms. We also want to make sure that the patient’s hunger, thirst and toileting needs are met.



Other actions will be specific to the patient. For instance, reducing noise in the room may help one patient while another might be better served through the distraction of a favorite TV program or music. Some patients who pull at lines or tubes may be redirected by encouraging simple activities that keep hands busy (folding towels, for instance). If alternative methods are not successful, we use the least-restrictive restraint approach for the shortest time possible.

During Restraints

For safety reasons, members of the healthcare team frequently check on any patient who requires restraint. We make sure the restraint is not constricting circulation or causing any other problems. We also provide the patient with nourishment, liquids, medications and other treatments in keeping with the patient’s medically prescribed plan.

Removal of Restraints

Restraints will be removed as soon as this can be accomplished safely. Our goal is to be restraint free, but shortterm use of restraints may be necessary for the safety of a patient or others. This flyer will explain why restraints may be used, how alternatives may help prevent the need for restraint, and ways that family and friends can help.

If you have questions or concerns about restraints, please bring them to the attention of your nurse or other healthcare team member. We encourage you to write down your questions— whether about restraint usage or any other concern—because this will help you remember everything you need to discuss with your doctor or nurse.

What Family And Friends Can Do

Family and friends are important to helping patients remain calm and comfortable: n Some patients, especially those who are elderly or mentally disabled, become confused when placed in an unfamiliar environment. Having a family member nearby can help reduce anxiety and confusion. n It is important to give the staff members contact names and telephone numbers of family members or friends so they can be called if needed. n If possible, have someone familiar stay with the patient as much as possible. In addition, let the nurse know when the patient will be left alone. n Tell the healthcare team about the patient’s usual behavior, alertness and ability to follow directions. Let the team know if he/she seems more confused than usual or if you notice something different. Also tell the healthcare team about things you feel may help calm the patient.

hearing aids if needed.

n Talk calmly to the patient. Remind

him or her of their surroundings, time of day, etc. and that he or she has tubes, drains, dressings, etc. in place. n Ask him or her if he or she is comfortable and tell the nurse or physician if you feel he or she may be in pain or have other needs. n Offer activities the patient may find enjoyable such as listening to music, watching television, looking at a magazine, knitting, putting together a simple puzzle, etc. n NEVER remove or loosen restraints yourself. n Do not bring potentially harmful items to the patient (cigarettes, matches, knives).
