THE LEADER IN WHITENING FOR MORE THAN 25 YEARS From one smile to millions “Dad, I need my teeth whiter. Dad, I need my teeth whiter.” Jaleena was persistent. She would cut tooth whitening advertisements out of dental journals and leave hand-written reminders on her dad’s nightstand. As Dr. Fischer’s 14-yearold daughter, Jaleena was determined to keep her dad‘s attention focused on developing what would later be known as Opalescence. “Be patient, love. I’m working on something,” Dr. Fischer would say. In Ultradent’s R&D department, Dr. Fischer worked tirelessly to get his whitening product just the way he wanted it. He knew it needed to have a sticky, viscous composition to keep the gel in place and to prevent it from lea​ching out of the tray onto the soft tissue. It needed to provide sustained peroxide release for superior whitening results. And it needed to be delivered via a soft, thin scalloped tray with reservoirs to ensure the maximum amount of gel stays in contact with the teeth during the entire treatment. One night in 1991, after Dr. Fischer was finally satisfied with the whitening gel he and his team had come up with, he took it home to Jaleena. He had her load the new Opalescence® gel in the tray before bed and asked her to wear it throughout the night. “When you wake up in the morning,” he said, “don’t take the tray out. Come find me, and we’ll take it out together.” The next morning, Jaleena and her dad took the tray out. After just one night, she had gone from an A3 to an A1. 2

In an in vitro study researchers looked at whether treatment with tooth whitening products with different concentrations of carbamide peroxide or hydrogen peroxide would increase the susceptibility for caries. A tooth whitening product with a neutral pH and 10% carbamide peroxide did not lead to a higher caries risk. In addition, Opalescence tooth whitening gel contains PF (potassium nitrate and fluoride), which helps maintain the health of enamel throughout the whitening process.

Instructions

Press all mixed gel into the RED syringe. Separate the two syringes and attach the Micro 20ga FX Tip onto the red syringe. Check the flow on a cotton gauze or mixing pad prior to applying it intraorally. If you encounter resistance, replace the tip and recheck the flow.

Place Ultradent IsoBlock and self-supporting plastic cheek retractors. Completely rinse and air dry teeth. Securely attach a Micro 20ga Tip to OpalDam Green and check flow.

Express a continuous bead along the gingival margin, overlapping approx. 0,5mm onto the enamel. Begin and finish the bead one tooth beyond the most distal tooth that is being bleached, building the barrier 4-6mm high and 1,5-2,0mm thick.

Express the resin through any open embrasures. Light cure the OpalDam resin barrier with VALO® using a scanning motion for 10 seconds and 20 seconds for other curing lights. Carefully check the resin cure with an instrument.

Apply a 0,5-1,0mm thick layer of Opalescence Boost to the labial surface of the tooth and slightly onto the incisal/occlusal surfaces. Allow the gel to remain on the teeth for 20 minutes.

Suction gel from teeth using the Luer Vacuum Adapter and SST™ Tip. Do not use water. Repeat application two times or until desired results are achieved. Do not exceed 3 applications per visit.

After the final application is complete and all visible gel is removed, thoroughly rinse the teeth. Gently slide the tip of a dental instrument beneath the OpalDam and lift it off. Check for and remove any interproximal remnants.

Evaluate the shade change. If additional whitening is desired and no significant sensitivity is noted, reschedule the patient in 3–5 days for repeat treatment or to dispense take-home whitening trays.

WHITENING QUESTIONS BEHIND TOOTH WHITENING There are many causes of tooth staining. Some affect the full arch – these discolorations are mostly caused by food, beverages and tobacco – stains and colors migrating into the enamel. Aging also leads to darker teeth. Such discolorations are generic and can affect everybody. Lightening of these discolorations is cosmetic and can be achieved with proven cosmetic whitening products formulated for superior results in our Opalescence PF whitening gels that are used with custom trays or in our pre-filled disposable trays offered in Opalescence Go® with the new UltraFit tray. A brighter, whiter smile is the result. In cases of re-darkening, a short touch-up restores the perfect smile. TM

Other types of stains can penetrate into enamel and dentin from the inside, as a consequence of diseases, injury or medical treatment, e.g. congenital, systemic, metabolic, pharmacological, traumatic, or iatrogenic factors such as dental fluorosis, jaundice, tetracycline, adult minocy-cline stains, porphyria, trauma, and erythroblastosis fetalis. To treat staining from these causes, a medical, in-office whitening system is needed. In many cases such focused whitening may make restorations, veneers or crowns unnecessary or postpone them for a long time. Professional whitening is the best and most minimally invasive option to safely lighten discolored teeth. How does whitening work? The whitening process is possible due to the ability of the carbamide peroxide and hydrogen peroxide to freely pass through enamel and dentin and to permeate to all parts of the tooth.1 For cosmetic whitening concentrations up to max 6% hydrogen peroxide are sufficient to pass the outer area of the teeth, removing stains caused by food, beverages, and tobacco. For teeth discolored by disease, injury or medical treatment, higher concentrations may be required to pass deep into the discolored area of the teeth. These peroxides break down into oxygen radicals, which migrate between the enamel prisms,

breaking down any colored molecules that result in tooth discoloration. The structure of the tooth is not altered; the internal tooth color is simply made lighter. Whitening agents break down into tiny molecules and move in all directions so even if the entire tooth is not covered with bleach, the entire tooth is whitened.2 Existing restorations will not be whitened. Numerous studies have proven the effectiveness of peroxides in whitening teeth. Enamel, dentin, existing fillings, and bonding materials are not affected by the whitening agents, nor are they harmed by the whitening materials. Will whitening affect bond strength? Even though whitening agents release oxygen into the tooth, existing bonds are not weakened.3,4 If whitening before bonding, allow a period of 7–10 days after whitening. The high concentration of oxygen in the tooth could significantly and adversely affect polymerization of the resins.5,6 How long does the whitening last? Whitening results are very stable, but depending on the patient’s nutrition and lifestyle habits, the procedure may need to be redone periodically. Due to the safety of the whitening agents, this should not concern the dentist or patient. Will whitening cause tooth sensitivity? Tooth sensitivity is a relatively common side effect of whitening. If sensitivity occurs, it is transient and disappears after the completion of whitening. However, most Opalescence® products include PF (potassium nitrate and fluoride) desensitizing agents for additional comfort. Research has shown that just as hydrogen peroxide penetrates through the enamel and dentin and to the pulp, so does potassium nitrate. Potassium nitrate acts more like an analgesic or anesthetic by keeping the nerve from repolarizing after it has depolarized in the pain cycle.7 Fluoride acts primarily as a tubule blocker, plugging the holes and slowing down the fluid flow that causes the sensitivity. Will whitening weaken the tooth’s enamel? Opalescence® tooth whitening gel contains PF (potassium nitrate and fluoride), which helps maintain the health of enamel throughout the whitening process.8-12

• During whitening treatment, your teeth become slightly „dried“. This is now compensated by increased moisture uptake from the oral cavity. Thus you should avoid the intake of staining substances, such as coffee, black tea, coke, red wine, red fruits, red beetroots etc., as well as smoking during the treatment breaks and the first few days after whitening of the teeth. During the following period, avoiding or limiting these foods, drinks and tobacco is beneficial for keeping the bright color of your teeth. • It is also very important to keep your teeth free of deposits. We therefore recommend thorough personal oral hygiene. We would be pleased to inform you about suitable aids and techniques for keeping these „problem areas“ clean. You should also visit us for a „professional dental cleaning“ at regular intervals. • However, should your teeth darken slightly over the course of the next few years, which cannot always be avoided, – a short refreshment treatment by us can restore the brightness of your teeth. Fill the whitening gel into the tray. Use approximately 1/3 to 1/2 of a syringe. To fill the tray, please only use the gel you received from us for this purpose.

Clean your teeth, then insert tray in your mouth and lightly tap it to adapt the sides to your teeth.

Use a finger or soft toothbrush to clean any excess gel. Rinse your mouth with water twice.

Remove a new Go-UltraFit Tray from the blister pack (U = Upper = maxilla; L = Lower = mandible). You can wear the foils for the upper and lower jaws at different times.

Clean your teeth, then place the UltraFit Tray over the row of teeth by aligning it according to the tooth arch.

Gently suck down on the tray to bring the inner foil with whitening gel into contact with the teeth.

Please follow our instructions on wear time and duration.

Remove tray and use a finger or soft toothbrush to clean any excess gel off your teeth. Rinse your mouth twice being sure not to swallow rinsed gel. Clean tray with soft brush and cool tap water.

Store the tray in case provided after use and cleaning.

Remove the outer tray – it only serves as insertion purposes and can now be disposed of. The inner foil with gel remains on the row of teeth.

Now adjust the flexible inner foil to your row of teeth with your tongue and pouting your lips, if necessary, lightly adjust with a finger.

Please follow our instructions on wear time and duration. Then remove tray and brush your teeth, being careful not to swallow the remaining gel. The foil can now be disposed of.

• When wearing the trays, you should not smoke, eat or drink.
