MSW PATHWAY LEARNING CONTRACT & PROGRESS REPORT GUIDELINES This document is used by MSW students to complete the Pathway learning contract and end of semester progress reports.  Full time students (3 semesters) and part time students (6 semesters) complete the learning contract in consultation with their mentors in the first semester of the program. Part time students (9 semesters) complete the learning contract in consultation with their mentors in the fourth semester (typically Fall semester of Year 2) of their program.  Learning contracts must be completed and submitted to the mentor by the end of the 8th week of the first Pathway semester.  Progress reports must be completed in each subsequent semester and submitted to the mentor on the last day of the semester.  The MSW Pathway Learning Contract and Progress Report has expandable textboxes to allow adequate space for your description and comments.  After completing the learning contract, save it as Word document twice, first under the file name Pathway Contract and then under the file name Progress Report 1. Submit the Pathway Contract document to your mentor. The learning contract must be signed by the student and the mentor.  At the end of the first semester, open Progress Report 1 and use it to complete your first progress report.  When completing progress reports you should indicate when (month and year) goals/actions are completed or if work is on-going. You may add or delete actions/goals and record/update the number of hours you have completed to date. Provide a brief description of your progress and work to date in the space provided.  Save Progress Report 1 and save it a second time under the file name of Progress Report 2. Submit Progress Report 1 to your mentor. You will use Progress Report 2 to complete your next progress report.  Repeat the process of saving and renaming the document at the end of subsequent semesters to complete progress reports 3-6.  The learning contract and final progress report only must be signed by the student and the mentor.  The mentor will give the signed learning contract and final progress report to the secretary for graduate programs for placement in the student’s file.

Action Plan: When completing your action plan, some of the goals will be the same or similar to your learning goals. You should also include additional goals that will help you complete your Pathway Scholarship product (e.g. seeking project approval if working with an organization; seeking ethics approval; securing funding or in-kind support, completing data collection, etc.). For each goal, describe the actions or tangible steps you will need to take to complete your Pathway Scholarship product (e.g. meetings; communication with stakeholders; writing proposals or submitting applications; readings or skill acquisition etc.). At the end of each semester, you will need to provide a brief progress report on your actions in the space below. This process should allow you to easily document your progress.
