COMPLETING YOUR APPLICATION The following documents are required for EACH scholarship application to be complete. If you do not wish to have your financial need considered in your scholarship application, you may omit the FAFSA SAR requirement, although you should be aware that most scholarships using this CSA do consider financial need in their selection process. Remember that each scholarship also has supplements that are required in addition to the following documents. Check the scholarship requirements carefully each time you apply! If I have previously completed a scholarship application, I have reviewed my information, updated as necessary, and I agree that it is correct to the best of my knowledge.

I have attached my High School Transcript. Current HS transcript. For MCSD, this can be obtained from your Student Portal account, under ‘Reports.’ I have attached my FAFSA SAR (Student Aid Report). Provide ONLY your one-page SAR and NOT the entire package from FAFSA. I have attached my résumé. Please limit résumé to one page. Use a quality, current résumé that includes school activities, athletic involvement, community involvement and activities, work experience, and achievements. I have attached my Letter of Recommendation from School Personnel This could be a teacher, coach, counselor, administrator, etc. I have attached my Letter of Recommendation from Non-School Personnel This could be an employer, volunteer work supervisor, music teacher, pastor, scout leader, etc. (Optional) Financial Aid Package If available, please attach the financial aid package from the college you are planning to attend.
