Alcohol/Drug Abuse Recovery Center Phone: (270) 765-5145 OFFICE HOURS: 24 Hours per day SERVICES: To provide the first step in helping an individual attain and maintain freedom from alcohol and other drugs through a 30 day inpatient program. Education, self-help groups, individual and group therapy, and family counseling, outpatient services, DUI treatment. SERVICE AREA: Lincoln Trail Area and outreach throughout the U.S. ELIGIBILITY: Anyone who suffers from alcohol or drug dependency. FEES: Services are covered by most major insurance companies, Medicaid & Medicare. A sliding fee scale also offers another payment option.

Communicare OFFICE HOURS: 8:30 AM - 5:00 PM (Monday-Friday). Some evening hours. SERVICE AREA: Clinics located in Breckinridge, Grayson, Hardin, Marion, Meade, Nelson, and Washington counties. ELIGIBILITY: No requirements. FEES: Many services are covered by insurance, Medicaid, or other third party payor. Sliding fee scale, based on income, for those without insurance. Adult Crisis Stabilization Unit 100 Gray Street Elizabethtown, KY 42701 Phone: (270) 737-1360 Fax: (270) 737-1920 8-bed, short-term, crisis stabilization unit offers psychosocial and psychiatric assessment for consumers with mental health difficulties that cannot be managed in their home environment. Individual and group therapy is available daily, along with case management and medication management, as needed. Clinic Services For new appointments call: 888-344-8066 This service is available for persons of all ages who are experiencing emotional difficulties. The outpatient clinics offer individual and group therapy, drug/alcohol counseling, family and marital counseling.

Communicare Services (continued) Consumer Directed Option (CDO) Support Brokerage services are now being offered by Communicare for Kentucky Medicaid Waiver members. CDO allows members to choose who provides their non-medical waiver services which allows them greater freedom of choice, flexibility, and control over their supports and services. Members can choose to direct all or some of their non-medical waiver services. Youth, age 12-17, who have drug or alcohol related offenses are eligible for services. A variety of tools are used to place each youth in the appropriate prevention education curriculum, or to refer them for further assessment. If a youth demonstrates the need for education, those services will be provided by the Early Intervention Specialist. Youth needing treatment for chemical dependency will be given appropriate referral options, which may include clinical assessment and group or individual counseling. Staff collaborates with a variety of community agencies in order to identify women to invite to universal prevention services, which are open to all pregnant or up to 60 days-postpartum women. The Zero Tolerance (ZT) Program is for young persons under the age of 21 charged with driving with a blood-alcohol content of .02-.08. Admission criteria includes: Kentucky resident over the age of 18; homeless; have demonstrated self-acknowledgment of a chemical dependency problem; a limited annual income; have received chemical dependency treatment within the past six months prior to application, and have maintained 30 days of sobriety prior to admission.
