Indications for Use The Magna-FX Cannulated Bone Screws are indicated for fractures of: • The intracapsular hip, femoral condyles, tibial condyles, ankle, acetabulum, pelvis, and other areas where accurate screw placement is required. • The screw can also be applied to slipped capital femoral epiphysis if image intensification fluoroscopy is used to ascertain the proper position of the screw. The Mini Magna-FX Cannulated Bone Screws are indicated for: • Fractures of the medial malleolus, distal tibia, distal radius, calcaneus, talus, patella, and pelvis. • It can also be used in the reduction of small bone fragments, reduction and fixation with cerclage wire, the fixation of bone prominences (i.e., lateral end of clavicle, coracoid process of scapula).

The Magna-FX and Mini Magna-FX Cannulated Screw systems share a common approach to fracture fixation. Each system has it’s respective purpose, but is applied in the same fashion.

1. Guide Pin Placement

3. Drilling Using the cannulated reamer, drill to a depth of no more than 5 mm shorter than the depth gauge reading per the surgeon’s preference of screw purchase. (See Table 1)

Following open or closed reduction of a fracture site, use a guide pin under fluoroscopy to ensure proper screw placement. A 1.7mm, 2.5mm, or 3.2mm guide pin should be used with respect to bone size and density in order to achieve maximum fixation(See Table 1). For larger fractures, multiple guide pins can be used.

4. Tapping (Optional)

If multiple guide pins are required, position the parallel guide pin on the bone over the guide pin. This will ensure accurate parallel guide pin placement.

5. Screw Insertion

2. Measuring Place the depth gauge over the guide pin and read the actual depth of the pin in the bone. The surgeon may elect to use a screw 5mm less than the depth gauge reading per his/her fracture reduction techniques for increased bone purchase.

The self-cutting threads of Magna-FX and Mini MagnaFX screws allow tapping to be optional. Place the cannulated tap over the guide pin and tap the proximal cortex (See Table 1). In patients with hard bone, it may be necessary to tap the entire reamed length.

Drilling manually, or using a power drill, with a cannulated screw driver bit, insert the proper length Magna-FX or Mini MagnaFX Screw over the guide pin. A washer may be used to distribute stress over the cortex of the bone. When inserting screws, if the cortex is thin or damaged, washers must be used to prevent the screws from pulling through the cortex. Using the manual cannulated driver, finish seating the screw and check fracture impaction with x-ray. Threads must not extend across the fracture site. Remove guide pin. Repeat steps 2-5 for additional screws, and close.

Femoral Neck Fractures - Alignment Guide Technique

The Magna-FX Cannulated Screw Fixation System is appropriate for use in Stabilization of femoral neck fractures with multiple screws, as well as for fixation of other metaphyseal fractures. Exposure and reduction are performed appropriate to the site and alignment technique used.

1. Guide Pin Placement* A. Following open or closed reduction of the fracture, use the Variable Angle Drill Guide under fluoroscopy. Insert a 3.2mm, 9” long Guide Pin through the center of the femoral neck and across the fracture site, engaging the subchondral bone.

Through the placement holes, drive C. three to four 3.2mm, 9” long Guide Pins in a triangular pattern until they engage the subchondral bone. A minimum of three screws is required to adequately secure femoral neck fractures. Remove the Parallel Pin Guide and the center pin.

2. Measuring* Place the Depth Gauge over the Guide Pin and read the actual depth of the pin in the bone. The surgeon may elect to use a screw 5mm less than the Depth Gauge reading.

B. Using the manual Cannulated Driver, finish seating the screw and check fracture impaction with x-ray. Threads must not extend across the fracture site. Remove Guide Pin. Repeat steps 2-5 for additional screws, and close.

Freehand Technique

The Magna-FX Cannulated Screw Fixation System is also appropriate for freehand insertion.

1. Guide Pin Placement* A. Following fracture reduction under fluoroscopy, insert a 3.2mm, 9” long Guide Pin across the fracture site, either freehand or using the Freehand 3.2mm Pin Guide, engaging the opposite cortex if necessary

B. If additional fixation screws are to be used, reposition and repeat A.

2. Measuring* Place the Depth Gauge over the Guide Pin and read actual depth of the pin in the bone. The surgeon may elect to use a screw 5 mm less than the Depth Gauge reading.

3. Drilling* (Optional in poor bone) Using the Cannulated Reamer, Drill to a depth 10mm less than the actual depth of the pin. (Optional in poor bone)

4. Tapping* (Optional) The self-cutting threads of the Magna-FX Screw allow tapping to be optional. Place the Cannulated Tap over the Guide Pin and tap the proximal cortex. In patients with hard bone, it may be necessary to tap the entire reamed length.

*PRECAUTION: During placement of the guide pins, measuring, reaming, tapping, and screw insertion, check the guide wire position frequently using fluoroscopy to prevent unintended guide wire advancement and/or penetration into the surrounding tissues.

5. Screw Insertion* A. Using a power drill with the Cannulated Screwdriver bit, insert the proper length Magna-FX Screw over the Guide Pin. A washer may be used to distribute stress over the cortex of the bone. When inserting screws, if the cortex is thin or damaged, washers must be used to prevent the screws from pulling through the cortex.

B. Using the manual Cannulated Screw- driver, finish seating the screw and check fracture impaction with x-ray. Remove Guide Pin. Repeat steps 2-5 for additional screws, and close.
