Eligibility Requirements for GR Services † Must have a diagnosis of mental retardation, as defined

by Texas Health and Safety Code §591.003* † and/or a diagnosis Pervasive Developmental Disorder (PDD) including Autism as defined in the current edition of the Diagnostic and Statistical Manual „

What to expect from a Service Coordinator † Develop and monitor the Person

Home and Community Based (HCS ) Waiver Program † The HCS Program provides services to people with mental

retardation who live with their family, in their own home, in a foster/companion care setting, or in a residence with no more than four people who also receive services. The HCS Program provides id services i tto meett th the person’s ’ needs d so th the person can maintain himself or herself in the community and have opportunities to participate as a citizen to the maximum extent possible. Services consist of case management, adaptive aids, minor home modifications modifications, counseling and therapies therapies, dental treatment, nursing, residential assistance, respite, day habilitation, and supported employment. Persons living in a foster/companion care setting or an HCS residence pay for their own room and board with either their monthly Supplemental S Security it Income I (SSI) check h k or other th personall resources. Th There is a limit to the yearly cost of services provided through the HCS Program. † LMRA’s manage the local interest list for this waiver program.

Texas Home Living (TxHmL) Medicaid Waiver Program † The TxHmL Program provides essential services and supports

so that people with mental retardation can continue to live with their families or in their own homes. TxHmL services are intended to supplement, not replace, the services and supports th may receive they i ffrom other th programs, such h as th the T Texas Health Steps Program, or from natural supports such as family, neighbors, or community organizations. Services consist of community support, nursing, adaptive aids, minor home modifications specialized therapies modifications, therapies, behavioral support support, dental treatment, respite, day habilitation, employment assistance, and supported employment. Service coordination is provided by the MRA. TxHmL Program services are limited to a yearly cost of $13,000 per participant. Additional information about the TxHmL † TxHmL services are offered to individuals on the HCS Interst List there is not a separate interest list for TxHmL.

Intermediate Care Facilities for persons with MR (ICF/MR) † An ICF/MR, which may be operated by a private or public

entity, provides residential and habilitative services, medical services, skills training, and adjunctive therapies with 24 24-hour hour supervision and coordination of the individual program plan. These residential facilities range from six beds to several hundred beds for people who have mental retardation or a condition related to mental retardation (related condition). „ Two categories: State Supported Living Centers and Community-Based ICF/MR Facilities. „ LMRA’s LMRA’ assist i t with ith the th ICF/MR search h and d placement l t
