Smile Design & Worn Dentition: Recipes for Repeatable and Predictable Everyday Dentistry

Treating tooth wear has increased over the last two decades. Nevertheless, the treatments involved have not been satisfactory to most patients, nor have they achieved the expected goal in some worn teeth scenarios. New approaches have emerged to reconstruct full arches in a minimally invasive way. These new concepts constitute a revolution in dentistry and their application requires adapted technique and training to achieve optimal results.

Dr Stefen Koubi graduated from the Marseille University School of Dentistry, France, in 1998. From 1998 to 2001, he worked as a clinical assistant at the Hôpital Louis Mourier (Hôpitaux de Paris) in the Department of Stomatology headed by Prof. Poidatz. In addition, he was research assistant at the University of Paris V, School of Dentistry in Montouge. In June 2001, he received the AEA diploma (clinical traineeship) and the DEA diploma (postgraduate study program) at the department headed by Prof. de Lumley of the Hôpital Nord in Marseille, France. He earned a doctorate degree in dental surgery in 2001. In the same year, he became an assistant at the department of Conservative Odontology headed by Prof. Koubi at the University Clinic of Marseille, France. Dr Koubi opened a private dental practice in Marseille in 2002. In 2004, he obtained a university diploma in oral implantology. In April 2004, he was appointed assistant professor at the Department of Conservative Odontology of the Marseille University School of Dentistry. He became Associate Professor in 2008 and earned his PhD in 2011. Moreover, he was awarded the Gold Molar by the students of Marseille University. In 2009, he established a private dental practice in Paris, France. Dr. Koubi is the founder of “L'institut de la facette”, a private training centre established in 2013 which mainly focuses on laminate veneer restorations and the minimally invasive rehabilitation of worn dentitions. Dr Koubi is the author of various national and international publications and reports. He has lectured internationally on the topics of esthetic dentistry, smile design, wear and erosion.

The purpose of this session is to suggest a precise and reproducible method that simplifies the treatment of worn dentition. A wax up and a mock-up is mostly used for designing aesthetic outcomes, and then used as a guide (preparation through the mock up or the wax upapt for the preparation of anterior teeth). The technique can also be used on the posterior occlusal surfaces of the teeth. This will not only provide the exact new vertical dimension of the occlusion but also let the dentist use it as a guide for precisely controlling occlusal preparation simultaneously with buccal preparation The classical cavity design for a partial bonded restoration on posterior worn dentition will also be reviewed. During the preparation of the occlusal surfaces of the posterior teeth, the marginal ridges must be preserved to reduce the biological cost and the mechanical stress leading to fracture. A new type of thin and reduced restoration called “table top” will be presented.

Dr Belograd is the author of the training courses Endodontology According to the Rules. Since 2010 he has been the ambassador of the European Society of Microscope Dentistry in Ukraine, and in 2012 was a speaker for this organisation, as well as an editorial board member of the Russian Journal of Aesthetic Dentistry. In 2013 he became an official trainer of the Carl Zeiss Dental Academy. He also addressed national societies of microscopic dentistry in Germany (DGmikro) and the Czech Republic (ProMikro), the Academy of Microscope Enhanced Dentistry (AMED), co-organised the international community MicroVision Group and founded the MixFight Dentistry project. In 2014, Dr Belograd became an international member of the StyleItaliano group and a team member of the DSD Masters group. Since 2016 he has taught and provides CE programs in Ukraine. He also launched such projects as The Formula of Endodontics, and From Endo to Restoration, United Endodontics, and Restorative Dentistry in the New Direction - Microdentistry.

The Prosthodontic Treatment Plan for Long-term Success of Dental Rehabilitation ABSTRACT

When it comes to treatment planning, sometimes dentists have no idea where to start or how to build a treatment plan. In reality, the most difficult clinical cases are those in which the teeth or bone is out of position, or gingival contours are poorly situated, or there is a problem with the occlusion. The thorough analysis of four main areas: Aesthetics, Function, Structure and Biology following a systematic approach starting with history-taking, to the patient examination, and model analysis in the articulator using a risk evaluation system allows us to make a proper diagnosis and to create a plan.

Dr Gadzhy Dazhaev graduated in Dentistry in 1993 and completed his specialisation in prosthodontics at the Medical Academy of Postgraduate Education, Moscow, in 1996. He runs a private practice in Moscow limited solely to prosthetics on natural dentition and on implants. Dr Dazhaev has been involved in dental education since 2010. He lectures and gives courses both in Russia and internationally. He is president of the Eurasian Association of Esthetic Dentistry.

Despite the fact that the determining factor in treatment planning is aesthetics, the need for proper examination of the TMJ, muscles, periodontium and teeth is recognized to achieve the long-term success of dental rehabilitation.

Sleep Apnoea: Parafunction… or Protective Function

For decades we have blamed grinding and clenching of the teeth on: 1. Stress, and 2. Malocclusion. While certainly emotional stress and occlusal factors can play a role in the etiology of bruxism, it is becoming clear that in many of our patients, obstructive airway issues may be a major driving factor.

You may know Jamison Spencer as the "Sleep Apnoea and TMD Guy." Jamison Spencer, DMD, MS, is the Director of the Center for Sleep Apnea and TMJ in Boise, Idaho, and the Director of Dental Sleep Medicine for the 28 practices of Lane and Associates Family Dentistry in North Carolina. He is adjunct faculty at Tufts University, University of the Pacific, and the University of North Carolina at Chapel Hill.

In this interactive presentation, Dr Spencer takes the audience on an evidence-based journey and makes a strong case that the so-called “parafunction” that wreaks havoc on our patients’ teeth and on our best restorations may actually be an attempt by the brain to “protect” the airway. This understanding will help with the diagnosis and actual treatment of the underlying etiology of bruxism.

Dr Spencer teaches mini-residencies in Dental Sleep Medicine at Tufts and UOP, and lectures around the world on Dental Sleep Medicine and TMD. He is the inventor of an FDA approved oral appliance, and recently launched the Spencer Study Club as an online education and mentoring tool to help dentists take Dental Sleep Medicine and TMD to new levels in their practices. In this lecture, however, Dr Spencer draws on his many years of being a student of "self help" and “success” literature and experiences. Following the principles he will discuss in this presentation, Dr Spencer has been able to create an incredible lifestyle for himself and his family.

With the case made that clenching and grinding may be the brain’s way to protect itself from suffocation, the lecture will then focus on how to protect the patient, protect their teeth, protect their restorations and protect your practice.

Part 2. Infection Control: Are You Achieving Consistency in Infection Prevention? ABSTRACT

This lecture is designed to help dental practices implement quality assurance measures in their everyday implementation of infection control, and follows the journey from the initial patient contact through to the end of the appointment and subsequent reprocessing of instruments and surgery changeover.

Laurence Walsh is Professor of Dental Science at the University of Queensland School of Dentistry, where he recently completed a 10-year term as the Dean of Dentistry. He leads a research program in advanced technologies and biomaterials, and works clinically as a specialist in special needs dentistry.

The lecture will highlight critical moments where mistakes can be made, to increase awareness around these. Examples of mistakes and the processes which can prevent these happening will be given. The lecture is suitable for all clinical staff working in a dental practice.

Laurence received his undergraduate education and his PhD from UQ and then studied at the University of Pennsylvania and Stanford University. He recently completed a six-year term as the chair of the ADA Inc. Infection Control Committee.

LEARNING OBJECTIVES

Laurence is the editor and principal author of the Australian Dental Association Infection Control Guidelines (2012 and 2015 editions) and he serves on committees of Standards Australia in the areas of personal protective equipment, instrument reprocessing and sterilisation. He has lectured widely nationally and internationally on infection control in dentistry.

Clinical Assessment Process and Clinical Decision Making in the Patient Centred Care — Rational Dental Care (Useful for Dsability, Aged Care etc) ABSTRACT

There is widespread acceptance that a “patient-centred” approach is an important aspect of the provision of quality dentistry. The recurring theme of patient-centred care is the importance of a humanistic, non-judgmental approach based on good communication, empathy and understanding. However, what does this actually mean, particularly in the context of an aged patient or a patient with special needs? Can we ensure that a patient-centred approach will adequately serve the needs of the patient and enhance the quality of his or her life?

Assoc Professor Liberali completed her undergraduate dental degree at the University of Adelaide in 1986 and was awarded a Doctor of Clinical Dentistry in Special Needs Dentistry at the University of Adelaide in 2009. In 2010 she became a Fellow of the Royal Australasian College of Dental Surgeons in the special field of Special Needs Dentistry. In 2011 she was awarded a Fellow of the International College of Dentists (FICD) and in 2013 a Fellow of the Pierre Fauchard Academy. Assoc Professor Liberali is the Director and Senior Consultant of the Special Needs Unit, Adelaide Dental Hospital, SA Dental Service, and holds an honorary Associate Dental Consultant position at the Royal Adelaide Hospital. She is the Program Director of the specialist training program in Special Needs Dentistry at the School of Dentistry, Faculty of Health Sciences, University of Adelaide and is the Principal Dentist at Health Partners Dental.

The concept of “rational dental care” was developed 30 years ago by Dr Ronald Ettinger. Rational dental care is a framework of clinical decision-making that allows a clinician to develop a treatment plan and provide the most appropriate care in the best interests of the patient after taking into consideration all the underlying or modifying factors. It can apply to any patient, as it ensures a comprehensive and holistic approach to oral health; considering not only the patient’s current oral health but also the potential environmental, social, medical, physical and cognitive factors that may have an impact on their oral health, oral function and ability to receive dental treatment.

Assoc Professor Liberali is a past President of the SA Branch of the Australian Dental Association and Immediate Past President of the Australian and New Zealand Academy of Special Needs Dentistry. Assoc Professor Liberali currently chairs the national ADA Infection Control Committee and is the ADA representative on the HE-023 committee for Standards Australia and the NHMRC Infection Control Guidelines Advisory committee.

This lecture will discuss the concepts of patient-centred care and rational dental care and how to incorporate these approaches into the clinical decision-making process for your patients.

There is so much (mis) information on nutrition these days, it’s no wonder many of us are confused! The most common diseases we see our patients being treated for are Heart Disease, Diabetes, Cancer, and Autoimmune Disease. With these diagnoses come a variety of medications to treat the disease, but often with medication comes the burden of side effects. What if the food we ate could actually help prevent or even cure disease? What if Hippocrates was right when he stated “Food is medicine”? In this course you will learn about a variety of popular diets and what they are intended for. You will learn about GERD, food allergies/sensitivities, leaky gut, pH, glycemic index/load, inflammatory foods, and more. Dental professionals are often times the first to see systemic disease signs. We can be educators beyond the “brush and floss” and empower our patients to make healthier choices for their TOTAL health.

Kris Dowling, RDHAP, BA, CNE is a practising dental hygienist in Berkeley, California.
