Laboratory Hazard Assessment Tool (LHAT) This form was adopted with permission directly from UCLA’s Office of Environment, Health and Safety (http://www.ehs.ucla.edu ). It was completed for each chemistry laboratory course offered at Santa Monica College by the full time faculty teaching each course. The tool identifies hazards to employees and students and specifies personal protective equipment (PPE) to protect employees and students during lab activities.
