Hello. My name is Jennifer Fraker. I will be the presenter for this Introduction to the  Individual Support Plan (ISP) Manual training course. I am a Human Service Program Specialist Supervisor for the Office of Developmental  Programs. I supervise the Individual Support Planning Unit in the Bureau of Supports for  People with Intellectual Disabilities. A few of the responsibilities of the Individual Support  Planning Unit are to oversee and manage the supports coordination organization  qualification and monitoring process, Individual Support Plan, and the Supports Intensity  Scale® and PA Plus.

As we begin this training on the ISP Manual, it is important to note that the manual is one  attachment to the ISP Bulletin.  In order to have a thorough understanding of the  requirements for the development and implementation of Individual Support Plans, your  review of the bulletin and listed attachments is necessary in addition to completing this  training. This training supports but does not replace the actual content and information  contained in the ISP Manual.  In July 2012, Informational Memo # 056‐12 was distributed to identify the upcoming  changes to the ISP Manual.  This Memo explained that the ISP Manual was updated based  on the approved waivers and 55 PA Code Chapter 51.  Also included in this Memo was the  fact that a separate bulletin would no longer be issued for Service Definitions.  Service  definitions are now found in the ISP Manual and the approved waivers. All documents and resources identified throughout this 5 lesson webcast, can be found on  the DPW website with the other ODP bulletins. There are also links to these documents  available where you launched this webcast.

This course consists of five lessons. ‐ This first lesson is an introduction to the ISP Manual and this online course. ‐ The next lesson is about ISP development. This lesson addresses identifying, gathering, and using  available information in the ISP, roles and responsibilities, outcome development and actions,  identifying services and supports, choosing qualified providers, and how to effectively and correctly  complete the ISP Signature Form. ‐ The third lesson focuses on service selection, service definitions, ISP approval and authorization and  the ISP Review Checklist that now includes Residential Habilitation Criteria, One‐Person Home  Guidelines, Supplemental Habilitation/Additional Individualized Staffing, and the six‐month review  process for Residential Habilitation, Pre‐Vocational, and Job Finding. ‐ The fourth lesson concentrates on service implementation, service utilization and monitoring of  services. ‐ The fifth lesson provides information on specific waiver policies and resources for your continued  use. All five lessons must be viewed for the entire course to be considered completed. If you are  participating in this course for credit towards the 2012 required Supports Coordination training hours,  be sure to also take the post‐test after viewing all lessons in order to receive credit for completing the  course. You must print a certificate of achievement in order to receive credit for this course in the SCC  Reporter.

As you know, Pennsylvania implemented the ISP to have a consistent statewide plan format. This  does two things. First, the ISP allows for the service system in Pennsylvania to be seamless for individuals receiving  services. Individuals should have the same experience across the state in regards to ISP  development. Second, in order to meet federal waiver requirements, an ISP that is developed and implemented  consistently across the state helps to meet many of the CMS assurances specific to service plans.   The Pennsylvania ISP documents the safeguards that will be taken to: ‐ protect the health and welfare of persons receiving waiver services ‐ develop plans in accordance with ODP’s policies and procedures ‐ update and revise the plans annually or when warranted by changes in need ‐ assure that services are delivered in accordance with the plan including  type,  amount, duration  and frequency  ‐ afford choice to participants between waiver between/among waiver services and providers

In developing the manual, the values and vision expressed in Everyday Lives provided a  framework. The Everyday Lives Principles were developed by people with an intellectual  disability, family members and their allies in 1991. When asked ‐ what is important to you,  what do you want in your life – people answered, I want relationships, choice, control,  quality, stability, safety, individuality, freedom, success, the opportunity to contribute to  their community, accountability, mentoring, community inclusion and collaboration. Everyday Lives continues to be the guiding philosophy of Intellectual Disabilities Services  within the Office of Developmental Programs.

As the intellectual disabilities system in Pennsylvania evolved with the Everyday Lives  philosophy, concepts such as Person‐Centered planning, Positive Approaches and Self‐ Determination were put in place as foundational principles and practices. Individual  support planning is based on all of these concepts and captures the true meaning of  working together to create a shared commitment to planning with a team in support of  individuals. Therefore, these concepts were also part of the framework in developing the  ISP Manual.

The ISP Manual is divided into 19 sections. The intent of the manual is to pull together  multiple pieces of information that encompass the required ISP development and  implementation process including service definitions. The ISP Manual has been revised to  include all of the current processes, forms and information needed to develop and  implement an ISP.  Throughout the manual specific SC required documentation instructions  are identified to meet waiver requirements.  Some of these instructions will be highlighted  throughout this training corresponding to each activity. The first four sections of the manual cover the ISP Process, ISP Preparation, ISP  Development and ISP Signature Form. These areas of the manual will be covered in lesson two of the ISP Manual Training course  titled ‐ Individual Support Plan Development and Process.

Next, let’s look at what is in Sections: 5, 6, 13, and 16 of the ISP Manual. Section 5 provides information on ISP Approval and Authorization by the Administrative Entity (AE). Section 6 contains information on the ISP Review Checklist (formerly called the AE ISP Checklist).   Originally, the AE ISP Checklist was completed by AEs in their review of ISPs for waiver participants for  whom intensive staffing was being proposed. ODP is expanding the use of the checklist to ensure that  annual and critical revision ISPs are being developed and reviewed using consistent, statewide  standards. ODP will use the checklist as a source to assess and verify compliance with ISP requirements.  In Section 13 you will find information on each specific service included in Appendix C of the approved  Consolidated and P/FDS Waivers as well as the corresponding procedure codes.  The information  contained in this section replaces the separate Service Definition Bulletin previously issued.  Each service definition identified contains a service description, suggestions for determining need,  documentation requirements and service limits. There are questions in this section that should be  answered and documented in the ISP for each particular service. The manual identifies where in the ISP  this information should be documented. This section is a resource to you when supporting the selection  of a service or reviewing the need for a service.  Base‐funded service definitions and procedure codes are located in Section 16 of the ISP Manual.   The  information contained in this section replaces the separate service definition bulletin that included  base‐funded services and descriptions.   The information in these sections is covered in the third lesson of this course titled ‐ Service Selection, Service Definitions and ISP Approval.

Lesson #5 in the ISP Manual training covers Sections: 14, 15, and 17, and is titled ‐ Specific  Waiver Policies and Resources.   Included in this lesson is information on two policies.   The service policy on Waiver Relatives, Legal Guardians and Legally Responsible Individuals  is unchanged. The Travel policy has changed to a limit of 30 total days per fiscal year.  Previously this limit  was 30 days per travel event.  There are no restrictions on the place of travel. In addition, we will review the Waiver Service Request form process, the Prioritization of  Urgency of Need for Services (PUNS), Independent Monitoring for Quality (IM4Q) and the  Positive Practices Resource Team (PPRT).
