The Islamic Republic of Iran, a lowmiddle income oil exporting country, with an area of 1.648 million sq. kilometers is fourth largest country in Asia. The country has 28 provinces, 293 districts, 885 cities and 68,000 villages. Total population, which doubled compared to 1975, was estimated at the end of 2001 at 64.5 million, of which 63% live in urban and the remaining 37% in rural areas (table-1).

Generally economic growth rate and investment level have been stable in the recent past, but no substantial growth was witnessed in terms of human development. The main problems, in this regard, are inappropriate investment and saving policies and continuing high inflation rate. Further, there is high unemployment rate, although women’s employment rate has increased compared to that of men’s. Resultantly, the real income of an average Iranian household has been under severe pressure, and 15% to 25% of population lives in poverty. 3

On the other hand, health and social indicators depict a fairly good picture. Basic literacy rate is estimated at 84.4% - well above the regional average. Over 94% of population has access to health services, while 95% has access to safe drinking water and 86% to sanitation. These factors together with improved coverage of maternal and child health care and continued commitment for self-reliance in vaccines and pharmaceutical productions contributed to the reduction of incidence of communicable diseases, although there has been a rise in the non-communicable life style related diseases. General Aspects

2.1. Office Management The implementation of 2002/03 Joint Programme Review Management (JPRM) started in January 2002, while the Country Office was still busy finalizing the collaborative programme pertaining to 2000/01. That is, activities like fellowships, procurement of supplies and equipment, Agreements for Performance of Work (APW) for various projects continued till mid 2002. Thus, the Ministry of Health and Medical Education (MOH&ME) could benefit fully from the allocated funds. Whereas the implementation of JPRM continued during year 2002 (see section 3), a number of issues regarding management of Country Office were addressed. Mainly, these were: In compliance with the requirements of Minimum Operating Security Standards (MOSS), Field Officer of United Nation Security Coordination (UNSECOORD) in Iran arranged two sessions for the training of staff on security issues. Subsequent to that, necessary telecommunication equipment was procured and installed. Further, for the efficient operation of this equipment, the IT Assistant underwent essential training organized by a United Nations (UN) consultant. Nonetheless, the requirements for MOSS cannot be complied fully while the Country Office remains in the existing building. The office registering and filing system as well as the work distribution amongst staff has not been satisfactory for sometime. Responding to the request made by the Country Office for streamlining the routine, the Regional Office (EMRO) assigned Ms Ikram Hassan of the EMRO Registry. She worked for one week in January 2002 and thankfully the office routines set up are much better.

Furthermore in June 2002, after having received PMDS forms from EMRO, staff in the Country Office was trained in developing work plans and the product of this exercise was submitted to the Regional Office.

As indicated above, the current office space is not secure as well as is much less than needed. The MoH&ME was therefore pursued during 2002 for speeding up the construction of new building. Thus, the hope for acquiring new building appears to be materializing in 2003. Another related logistic development during 2002 was the sale of three old vehicles, replacing these with the new ones in agreement with the Regional Office. Thus, the Country Office has three new vehicles for office use and has been particularly valuable in organizing the aforementioned activity. A sub-office was established in Mashhad in coordination with UN Agencies and MoH&ME for dealing with any health related emergency arising out of the crisis in the neighboring Afghanistan. This was closed with the return of peace, but with another crisis looming in Iraq Informal and social meetings are useful in creating an environment conducive to work - Country Office staff in their monthly meeting measures were taken for emergency preparedness. Although not much emergency and human displacement took place, the Country Office was over stretched given its meager resources. Last but not the least, staff at Country Office continued organizing monthly gatherings. Whilst in this forum the issues of common interest are discussed, this has been useful in strengthening social cohesion amongst them - an environment conducive to work.

2.2.

Informatics and Telecommunication activities

A number of developments regarding informatics and telecommunication were registered during 2002. In addition to undertaking measures to comply with the MOSS requirements, following steps were taken to upgrade the access to Internet and IT equipment: The ordinary dial-up system was replaced with a leased line system. That is, with the assistance of United Nations Development Program (UNDP) a dedicated line was leased from an internet service provider. Also, some additional equipment needed for this up-gradation was procured. Thus, with the new system in place, the Country Office network has un-interrupted, high speed and reliable 24 hours internet connection. 6

A new email server system was procured and installed, with the hope of making Country Office independent of UNDP. That is, with new system, the individual office staff is able to correspond with EMRO using their own email address using Local Area Network of the WHO Representative Office in the I.R. Iran. Another major achievement during 2002 was regarding IT equipment added to the inventory. Six powerful Pentium-4 computers were procured in two phases, replacing those old and worn out. The same has been installed and operationalised along with the updated software and applications. Collaborative meetings were held with other UN Agencies for increasing coordination and cooperation, particularly for operating joint radio telecommunication system, as part of the plan for establishing and maintaining communication between UN Agencies. 2.3. Major Events During 2002 The 2002 saw a number of major events, likely to have long-term impact on collaboration of WHO Representative Office with the MoH&ME. Some of these events were as follows:

2.3.1. Integrated Medical Education and Health Care Delivery Following a parliamentary debate on the future of the integrated medical education and health care delivery, and the possibility of dividing the Ministry of Health and Medical Education raised concerns in the Regional Office. Consequently, a high level delegation headed by Dr H.A. Gezairy, Regional Director of EMRO visited Islamic Republic of Iran at the invitation of HE Dr. Pezeshkian, Minister for Health and Medical Education to attend an international seminar on this subject held in September 2002. The delegation comprised Dr Haythem Khayat, Senior Policy Adviser, Dr Ghaneim Al Sheikh, Regional Advisor and distinguished experts on medical education, Dr Charles Boelen and Prof. Henry Walton. The seminar was successful in presenting to the high-ranking policymakers and government/ university officials the international experiences and expressed their

viewpoints with respect to the integrated medical education and health care delivery. In addition, the Regional Director, making full use of his visit, met the Parliamentary Committee on Health and the following dignitaries (see pictures): Thus, WHO was effective in advocating the continuation of the integrated system of medical education and health care delivery by influencing the policymakers in an important member state. It was decided that the MOH&ME and WHO would together assess the present system for its cost H.A. 2.3.2. Vision 2020 – the Right to Sight Launched in Geneva on 18 February 1999, the ‘Vision 2020 - the Right to Sight’ envisages global partnership aiming to eliminate avoidable blindness by the year 2020. To commence the initiative in the I.R. Iran the WHO/EMRO Medical Officer for the Control and Prevention of Blindness visited the MoH&ME in January 2002. Based on his recommendations, a National Committee was constituted and a National Focal Point was also nominated. Later, a national study protocol was developed followed by data collection. The WHO Country Office participated in the activities, sharing technical information and fact sheets. To launch the initiative, the Regional Director visited I.R. Iran accompanying HRH Prince Abdul Aziz Bin Ahmed Bin Abdul Aziz Al Saud, the

Chairman of the International Agency for Prevention of Blindness, Eastern Mediterranean Region on 22 September 2002. The ceremony held for the purpose was marked by the signing of an agreement by the MoH&ME with the objectives of the ‘Vision 2020 – the Right to Sight’ and the presentation of the findings of a national survey on ‘low vision and blindness’ as well as a preliminary work plan for the control of preventable blindness in the Islamic Republic of Iran.

2.3.3. World Health Day 2002 The WHO Country Office was fully engaged in preparing and celebrating the ‘World Health Day 2002’. The main activities included organizing a painting competition announced by the Regional Office, distribution of message from the Regional Director and supporting various ceremonies. Paintings highlighting the theme ‘Physical Activity’ were received and selected for prizes by EMRO. Two participants were successful: one receiving first prize amounting to US$150 and the other one won the fifth prize amounting to US$90. In addition, certificates were delivered to winners in a ceremony. Also, snapshots from the I.R. Iran formed part of the documentary included in the message of the Regional Director. Support was provided for holding ceremonies marking the occasion in terms of translation of materials for mass media, billboards, placards, etc. The occasion was used to distribute message from the Regional Director and World Health Day material.

2.3.4. World AIDS’ Day Celebrated later than the actual date on 11th of December 2002 due to the ‘World AIDS’ Day’ falling in the holy month of Ramadan, was marked by a number of activities. The Regional Director at the invitation of the MoH&ME visited the I.R. Iran to attend the ceremony which attracted a large audience including the Minister of Education and representative from UN Agencies and Organizations such as Presidential Office for Women’s Participation Affairs and Prison Organization. The message of HE the President of the I.R. Iran was read by HE the Minister of Health and Medical Education. The Country Office provided to the MoH&ME the materials for ‘World AIDS’ Day’ comprising video tapes, CDs for the package of campaign against AIDS, The Regional Director with a group of Nationals on World AIDS’ Day celebrations high resolution reproducible CD-ROM and copies of the Regional Director’s message for the day. Furthermore, an exhibition was organized in collaboration with other members of the UN HIV/AIDS Theme Group. The Regional Director also visited a ‘Triangular Counseling Clinic’ in West Tehran providing counseling and treatment including ARVs and laboratory tests to the positive cases, addicts and those suffering from sexually transmitted diseases.

2.3.5. WHO Consultative Meeting on Cardiovascular Diseases A meeting was held in Isfahan on 5 - 8 January 2002 to launch the ‘Integrated Community-Based Program for the Prevention of Cardiovascular Diseases’ in the Region. The Regional Director in addition to Dr Osama Khateeb, Regional Adviser for Non-Communicable Diseases and Dr Atarod Mojtabaie, WHO Adviser/Consultant attended this occasion. Further, inter-alia, a product of this consultative meeting was a press conference held by the Regional Director where he emphasized the importance of continuing Anti-tobacco Campaign. A mention was also made of a research into the involvement of industry in smuggling of tobacco (see more in section 6.2).

2.4. Collaboration with other United Nation Agencies WHO Country Office, as a member of UN family, kept close liaison with sister organizations for coordinated and synergistic activities are undertaken. Two areas in this respect are worth mentioning: a Common Country Assessment (CCA) in order to identify Millennium Development Goals (MDG); and Theme Groups established by UN for soliciting stakeholders’ involvement in priority areas.

2.4.1. The Common Country Assessment and the Millennium Development Goals The terms of reference for establishing Common Country Assessment (CCA) were drafted and a Steering Committee was constituted to overview the process. The latter, identifying the data collection and analysis needs, formed four groups, respectively focusing at: (i) economic development; (ii) social development; (iii) environmental sustainability and food security; and (iv) governance. While these areas were identified based on national priorities and goals set by various international conferences and conventions, the membership of these groups has been drawn from senior UN staff and government officials who have already begun deliberations. WHO Country Office in Iran is the member of social development group, but also actively participated in the meetings of the Steering Committee. Different groups so constituted have reached agreement on a minimum set of indicators, collected the required data, and produced reports. A national seminar was planned to review these reports, but was postponed due to other pressing priorities of the National counterparts. Instead draft CCA report was distributed among National and International partners for seeking their comments. Revisions were made and a second draft report was circulated. The process continues during 2003. Likewise a number of meetings have been held regarding work on MDGs report. Agreement was reached on the sections of report and various concerned agencies were assigned to develop relevant parts. For example, United Nation Office for Drug Control was assigned to prepare a section on HIV/AIDS (target 7), while WHO was asked to work on malaria and other communicable diseases (target 8). The process for developing the assigned component of report is underway, albeit, in close coordination with the National Departments, particularly malaria and tuberculosis.

2.4.2. Theme Groups on Priority Areas The WHO Country Office remained an active partner on various Theme Groups on priority areas, e.g. HIV/AIDS; Poverty Alleviation; and Gender and Development. In this section the WHO’s role in the progress made by different groups is presented. 2.4.2.1.Theme Group on HIV/AIDS Following to the WHO assuming the chair of UN Theme Group on HIV/AIDS, meetings were held on 5-6 March 2002 with the outgoing chair (UNFPA) in order to review the ongoing activities and to seek advice on future undertakings. The crosscutting issue identified was regarding the resource guide for the Theme Groups used as a monitoring tool. It revealed that there was (i) a lack of clear understanding of the 11

roles and priorities; (ii) no work plans; (iii) no regular assessment; and (iv) no advocacy activities. To address these issues, a draft work plan indicating the roles of different members was prepared. The Group’s chairmanship was rotated to UNDCP in July 2002, but WHO remained active partner by contributing the following: 1. 2. 3. 4.

Completed implementation of the UNAIDS PAF for 2000-2001; Documented the Kermanshah Triangular Clinic as a best practice; Supported nationals in conducting a study tours to Australia and Thailand; and Developed a new PAF Proposal for 2002-2003.

Moreover Dr Jihane Tawilah, Regional Adviser for HIV/AIDS and Sexually Transmitted Diseases (STDs) visited I.R. Iran on 20-24 October 2002. She reviewed the implementing progress of UNAIDS’ sponsored national project on harm reduction and discussed technical needs of the National AIDS Program (NAP). Also, she assisted the NAP and WHO office in their plans regarding the reprogramming of JPRM and future projects. 2.4.2.2.Theme Group on Poverty Alleviation With UNDP acting as the chair of group of which WHO is a member and under the joint auspices of the United Nations and Government of the I.R. Iran an initiative is being undertaken in the provinces of Sistan and Baluchestan. The United Nations, however, advocates an integrated, inter-sectoral, community-based and participatory approach to poverty alleviation and employment generation. Accordingly, a project is being implemented targeting the poor and vulnerable in two areas of Zahedan provincial capital. WHO allocated funds for building the capacity of Community Based Organizations and implementing a Healthy City/Village Programme in two areas of Zahedan - one of these is common with that of the Theme Group’s intervention area. The activities include building the capacity of communities for improving their health and education; income-generation; creating conditions enabling joint communitygovernment approaches to poverty reduction; and establishing linkages with the local government efforts. 2.4.2.3.Theme Group on Gender and Development Hitherto working for issues surrounding gender and development under the chairmanship of FAO, UNICEF took its charge in 2002. WHO is one of its members, and contributed in many ways to the achievements of the group during the year. Mainly a delegation from Regional Office led by Senior Program Adviser and comprising the Regional Advisors of Healthy Life Promotion (HLP) and Women in Health and Development (WHD) visited the I.R. Iran. They met members of the Steering Committee for Gender Mainstreaming and exchanged ideas for further collaboration and planning. Further the visiting delegation discussed ways and means of commemorating International Women’s Day and supporting Afghan women. Accordingly, an NGO based in Zahedan and Mashhad, which is involved in Afghan affairs, was chosen to operate three micro-credit funds in Zahedan. The visiting dignitaries from EMRO also participated in a one-day seminar on violence against women.

3. Regular Program of Cooperation The 2002-03 joint collaborative program of WHO Country Office and the I.R. Iran focuses on a number of priority health policy and strategic planning issues as outlined in the Third 5-year National Development Plan and selected by the MoH&ME. These include the following: • • • • • • • •

Health sector reform Management effectiveness program Health information system Non-communicable diseases including healthy lifestyles and health promotion Human resources for health Essential drugs Quality improvement Health system research

Major effort went into ensuring that the WHO’s technical support has a catalytic effect, and is seen as an international role contributing to the implementation of different national health strategies and programs. For this purpose, a number of consultants well qualified in their filed were recruited for various programs, in addition to carrying out a number of national workshops and contracting various activities to the Nationals. Further, a range of fellowships was awarded for sending eligible candidates abroad for training in specialized fields. Equipment was purchased either by the Regional or Country Office and supplied to the counterparts for supporting various national programs. Moreover, many Nationals were sponsored to participate in the international and national conferences and other events. Since the progress on different collaborative program currently underway will be provided in the next biennium report, hence it is not given here. However, while in the section below, an overview of the mid-term review and reprogramming exercise conducted for JPRM 2002-03 is given, various activities carried out during 2002 are listed in the annexes, which are introduced as follows: • • • • •

Consultants – lists different consultants and WHO staff who visited IR Iran for technical support to the National counterparts Fellowships, study tours and training activities – lists fellowships, study tour and training activities sponsored by Country Office both for Nationals in other countries and those from member countries of region coming to the IR Iran Printing/ translation of material – lists the support provided to the National Counterparts in terms of printing and translation of different material Participation in meetings – lists the Nationals who were sponsored by the Country Office for attending technical meetings Agreement for consulting services – list the agreements made with different Nationals for performing specified work

3.1. Mid-Term Review At the end of year 2002 a mid-term review exercise for monitoring the progress of various priority programs covered by the JPRM was undertaken, and a report was submitted to the Regional Office in January 2003. It revealed that several program were implemented in accordance with the work plans, except for few requiring changes, according to the needs of the MOH&ME. Salient features of the mid-term review report are as follows: 1- The Community Based Initiative financed through last biennium’s funds has been fully implemented. Accordingly, small loans were distributed in three pilot sites to start a number of income generation projects. 2- For Management Effectiveness Program (MEP) a team of consultants along with staff of HQ/EMRO visited Iran for assisting the Nationals in holding workshop in Tabriz aimed at reviewing the progress of program. The implementation of different programs was rated as 100%. 3- The program for ‘vaccine production and quality improvement’ has been continuously supported through regular visits made by teams from HQ/EMRO. In addition, training courses for the staff of MOH&ME and Razi Institute were arranged. The program was mainly covered through extra-budgetary provisions. 4- The I.R. Iran hosted numerous fellowship programs, especially from Afghanistan and Sudan. Also, study tours were received, arranging visit by guests to the Iran’s successful health programs, especially in the primary health care. 5- The performance of health system has been a major concern for the MoH&ME. To address the same, the work plan for health sector reform was carefully reviewed and changes were made, incorporating the recruitment of the Nationala as short-term consultants and APWs for reviewing the national health accounts, health insurance schemes, and other health care financing options. As seen above, though the implementation of various program has been satisfactory, few issues were faced and attention of the Nationals had to be drawn for keeping in line with the original work plans. Further, to expedite implementation close assistance had to be provided, in most cases for preparing terms of reference for the required consultants. Similarly, a close follow-up was necessary with EMRO for their response to requests, especially issuance of APWs. Some of the factors that possibly led to the slow take off and/or slow implementation during the first year of the biennium include: 1- The Health Sector Reform constitutes over 20% of JPRM. It was reprogrammed, but it took almost a year to negotiate and finalize with the EMRO (DHS). Nevertheless, implementation of some of its components, e.g. fellowships was processed promptly. 2- Several components of MEP appeared unimplemented, while actually activities were planned for the visiting STCs. And when such activities were implemented

the HQ absorbed the cost, thus leaving funds planned in the JPRM for reprogramming. 3- The implementation level of GPE and IMD programs was low, because the concerned departments did not request for the envisaged input despite several reminders. That is, the counterparts in the MoH&ME were not proactive in program implementation. 4- The majority of the unimplemented activity reflected in the JPRM for 2002 were to be covered by the MoH&ME, and no allocation was made by WHO. In such cases, even if the activities are implemented by the MoH&ME there is no mechanism of informing the Country Office. 5- Many activities reflected in the joint collaborative program are for implementation in 2003. Although not an excellent way of planning, the National partners argued strongly because of their own constraints in allocating resources during 2002. 6- During 2002 extra-budgetary resources were receive in several priority areas such as RBM, STB, TFI, HIV/AIDS, CBI, EDB, MEP, POL etc, covering several activities planned out of regular budget leaving funds for reprogramming. Also, these added activities (especially HIV/AIDS) resulted in shifting the time frame towards 2003. 7- The APWs constituted major part of collaborative program. Since these take long to set the objectives, prepare terms of reference, seek clearance from the MoH&ME and EMRO, a delayed implementation rate was evidenced in most cases. 8- Another issue that hindered the progress on program particularly fellowship component was the selection of candidates, clearance by the government, application to and seeking placement in universities and visa processing. Inordinate delays in completing these formalities led to many fellowships actualizing in 2003. Finally, the Country Office with its key skeleton staff upon emergency be it across borders in Afghanistan or Iraq or internally due to earthquakes or floods remains overstretched, often compromising the implementation, monitoring and reporting of the regular programs of cooperation. Further, the overwhelming demand by colleagues in technical units of EMRO for holding inter-country meetings and consultations in Iran, although a welcome sign for the appreciation of sincerity and commitment of the MoH&ME and Country Office as best achievers in the region, adds to the workload of the latter.

3.2. Reprogramming Given a number of activities lagging behind schedule on one account or the other explained above preparations were made in December 2002 for conducting an exercise for the reprogramming. Based on the information shared with the National Committee on JPRM, it was envisaged: (i) reviewing the implementation status of

different programs envisaged in the current JPRM; (ii) identifying priority areas with capacity to absorb inputs and not requiring activities extending beyond biennium for the reallocation of funds; and that (iii) 10% funds were set aside as contingency for later release as needed. In addition, following decisions were taken regarding reprogramming: That adequate fund is allocated for the assessment of integrated medical education and health services (see more in section 2.3.1); Regular monthly meetings of WHO and Directors General of MoH&ME will be held for remedying the possible problems in implementation of programs components; and Some provision is made for supporting NGOs working in areas that are also priority for the MoH&ME.

4. Other Program Areas 4.1. Global Fund to fight AIDS, Tuberculosis and Malaria The Global Fund to fight AIDS, Tuberculosis and Malaria set up on 28-29 January 2002 issued its first round of call in February 2002 to the governments around the world asking for proposals to fight these menaces of mankind. Given the deadline of March 10, 2002 several shortcomings remained, therefore the two proposals submitted by the MoH&ME in the first round could not meet the criteria for support. Second round was announced on February 8, 2002 marking September 27, 2002 as deadline for the submission of proposals. Since there was adequate time, the WHO Country Office extended full assistance to the MoH&ME. In addition to providing technical material and appointing a full time national consultant, an international consultant was recruited to assist the Nationals in developing the Country Cooperation Mechanism (CCM) and writing the proposals. Further, Dr Jihane Tawilah, Regional Adviser for HIV/AIDS and STDs and Dr Andrew Ball from WHO, Geneva visited Iran for facilitating the MoH&ME in finalizing the proposals that was submitted in time, and have been funded.

4.2. The Commission on Macroeconomics and Health At the invitation of Commission on Macroeconomics and Health established in Geneva, the WHO Representative for Iran, Deputy Minister for Health and (late) Dr. Nouri - Head of the Commission on Health in the Iranian Parliament - attended briefing session held in Geneva. Dr. Vaez Mahdavi from Management and Planning Organization could not attend the meeting due to other pressing commitments. The discussion in the meeting focused on the impact of investment on health and economic development. In this regard, evidence was brought from the contribution of primary health care towards health and development. The group agreed on the centrality of healthy human for sustainable development.

4.3. Production and Quality Improvement of Vaccines WHO played a major role in introducing ‘Good Manufacturing Practices’ in local vaccine production; and for this purpose a consultant Dr Nadir Harjee was recruited: firstly in March 2002 and then in May 2002. He developed a proposal for building capacity at the National Regulatory Authority (NRA) and National Control Laboratory (NCL) for submission to the World Bank for financing out of a Second Primary Health Care and Nutrition Project of the MoH&ME. The proposal was approved and implementation started according to work plan. Later a team comprising Mr Lahouari Belgharbi, WHO/HQ, Mr Alejandro Costa WHO/HQ, Dr Chung Keel Lee, University of Seoul, and Dr Anne Carter, NIB SC visited Iran from 7-12 September 2002 to assess the impact of the aforementioned project. Discussions with the officials of Pharmaceutical and Narcotic Division of MoH&ME; NRA; NCL; Inspection Office and Center for Disease Control, and a visit to the manufacturing units, i.e. Razi and Pasteur Institutes revealed that regulatory

functions, particularly clinical evaluation of safety and efficacy were not fully enforced. This led to employing Dr Harjee for another 6 months starting December 2002 to assist the NRA/NCL in improving the quality of vaccines and enforcing Good Manufacturing Practices in production.

4.4. Polio Eradication Salient activity regarding polio eradication was assessing the country’s progress in the implementation of National Action Plan for Laboratory Containment of wild Polioviruses in order to prepare for the Global Certification Phase. For this purpose, Dr Soad Farid Hafez, WHO/STP visited Teheran from 5-12 September 2002 to review the situation. She visited the MoH&ME, Centre for Disease Control, National Polio Laboratory, Razi Institute, Alborz Research Laboratories. Furthermore to review AFP surveillance system the Regional Office sent a team comprising Dr Peter Figueroa STC, Dr Faten Kamel WHO/EMRO, Dr Malek Afzali, WHO Temporary Advisor, Dr Relian Hafiz, MOH Pakistan, Dr Waquar Ajmal, WHO Afghanistan, Dr Yehya Seida, WHO Sudan and Dr Tahir Mir, WHO Pakistan from September 7-14, 2002. The team expressed its satisfaction on the effectiveness and comprehensiveness of AFP surveillance system and that it was sensitive to detect any transmission or importation of wild poliovirus.

4.5. Integrated Management of Childhood Illnesses The initiative for the Integrated Management of Childhood Illnesses (IMCI) was introduced in 1998, and since then placed under the Family Health and Population Department of the MoH&ME. Different clinical guidelines have been adapted to meet the needs of both the general physicians and Behvarzes. The WHO Country Office at the request of the MoH&ME arranged a visit of Dr Suzanne Farhoud, RA/CAH and Dr Samira Aboubaker, CAH/HQ to review the implementation status of initiative in the I.R. Iran. The team held briefing sessions with the Minister for Health and Medical Education and the Deputy Minister for Health and other National counterparts. They also visited an implementation site, Mobarakeh in Isfahan province and were impressed with the continuing commitment of the Nationals towards IMCI initiative.

5. Emergencies and Humanitarian Assistance Iran is a country prone to disasters, and year 2002 saw four major disasters. WHO worked effectively in these situations by providing technical support in situation analysis, assessing needs, monitoring and disease surveillance. Further, the UN provided assistance in developing a National health sector contingency plan. In the following section the role of Country Office in assisting the MoH&ME in combating particular disasters is explained.

5.1. Floods In August 2002 – the first anniversary of the last year’s devastating flood – torrential rains caused heavy flooding of Gorgan and Doogh Rivers in Golestan province, affecting also some areas of Khorasan and Semnan provinces. Caspian Sea Golestan

As a result, about 200,000 people were affected, killing 43 people in Golestan province. About 4,000 households suffered due to lack of safe drinking water and sanitation facilities. Over 500 houses needed reconstruction and other 1,160 required repair of varying degree. The WHO Country Office was proactive in this emergency and participated in meetings along with other UN Agencies, and was prepared to meet any demand. However, the government made no request for any material help.

5.2. Earthquake An earthquake struck Qazvin, Hamadan and Zanjan provinces on 22 June 2002. Reportedly 266 died including 29 injured who died in hospitals while in total 1,300 were injured. Physical destruction was wide spread, i.e. in Qazvin 53 villages turned to rubbles, some completely. Similarly, 11 villages were hit in Hamadan with the

destruction rate ranging from 10 to 100%. In Zanjan province where damage was moderate, 4 villages in Khodabande district were affected with a varying degree of destruction.

World Health Organization as a member of the UN Field Assessment Team participated in the visits made to the affected area including the one by the UN Resident Representative in the I.R. Iran in order to assess the situation of destruction. The situational report along with visuals was shared with the EHA at EMRO and HQ. Additionally, the Country Office provided material help to the provincial health authorities in terms of 5 Emergency Health Kits, 5 Emergency Trauma Kits Type-A and 5 Emergency Trauma Kits type-B.

5.3. Afghanistan Crisis Given the war in neighboring Afghanistan a sub-office of WHO was established in Mashhad anticipating a mass human displacement. Although not many refuges crossed the border, this stretched the resources of the Country Office. Further, even after closure of the sub-office in March 2002 the Country Office continued taking part in consultation aimed at facilitating the returning refugees. Moreover, WHO was of the view to continuously providing primary health care like prenatal care, immunization, treatment for tuberculosis, malaria and other chronic illnesses to the repatriating refugees. Likewise, a training program for Afghan Health Workers was proposed for the interim period. However, UNHCR was opposed to both these proposals, because they thought it might be a disincentive for repatriation.

5.4. Iraq Crisis With the war looming in Iraq and the possible human sufferings the Country Office undertook the preparations for meeting eventualities in partnership with other UN Agencies on the Task Force established for the purpose.

In addition to the above, in November 2002 the WHO Representative in Iran and the Medical Officer attended induction briefing organized by Emergency Humanitarian Action held in the WHO/HQ in Geneva. This event was useful in terms of briefings as well as the material provided to the participants. The experience so gained was used in further developing the Contingency Plan and the WHO Plan of Action as well as the Flash Appeals.

6. Priority Areas Within the context of WHO visualization a number of initiatives have been launched in the Islamic republic of Iran. Progress made during 2002 in addressing these priority areas is given below:

6.1. Community Based Initiatives Health is widely understood to be a central goal as well as an important outcome of any development intervention. That is, the reduction in poverty should be taken as investing in health, and for this purpose WHO advocates using community based approaches. This initiative is in line with the Millennium Development Goals that include call for reduction in poverty and the consequent improvement in the health of the poor.

During the year 2002 the Country Office continued supporting three programs aimed at improving the quality of life, hence also the health of the people. This section provides an overview of the contribution made by the Country Office towards these programs.

6.2. Basic Development Needs Program The Basic Development Needs (BDN) Development at the grass root level – Dr. Marwat Abu-Shabana Regional Advisor/EMRO in a village with the members of a Village Development Committee Program is currently supporting income generating projects in 8 villages of three provinces: Charmahal; Bushehr and WestAzarbayejan. The WHO Country Office supported this initiative out of the contractual fund transferred from previous biennium in the following manner:

Providing small loans for income generation and social projects; Supplying equipment and procuring leased line for internet connection of relevant office;



Organizing training workshops for improving knowledge and skills of concerned agencies in terms of planning, financial management, monitoring, supervision and reporting; and



Conducting advocacy campaigns by preparing brochures, pamphlet, and videotape.

In addition to the above, two APWs were signed with the National Focal Points for the follow-up of different activities being undertaken through Community Based Initiatives. Further, to review CBI activities the Regional Advisors for CBI and WHD visited the I.R. Iran and traveled to the field sites. Another important event was attending a 10 days’ BDN Management course held in Pakistan.

6.3. Healthy City /Village Project This project has been implemented in 56 cities of the I.R. Iran, and a Coordination Council has been set up to overview different activities undertaken. To support the project, the Regional Advisors for CBI and WHD visited two such projects: one in Saveh and the second in south of Tehran. The former is since a success story for harnessing community participation and intersectoral collaboration, the Mayor of city was invited to share his experiences about health and A gymnasium built under Health City/Village Initiative

sustainable development projects in the “World Summit on Sustainable Development” held in Johannesburg their success stories. However, due to certain domestic commitments he could not attend the Summit. The Country Office in order to review and document the process for project implementation from initial stages recruited an expert. Further, a Short Term Consultant was appointed to develop a Work Plan common to different project sites in consultation with the members of the Coordination Council of the project. The Consultant also shared his experience and the success stories about health and sustainable development projects.

6.4. Women in Health and Development A delegation on Gender Development from EMRO under supervision of Senior Program Adviser came to Iran with the aim to review the achievements made in relation to Gender Mainstreaming in different fields particularly related to health and to exchange ideas for future collaboration and planning. They held meetings with the Head of Environmental Organization, Senior Officials in the Presidential Office for Women’s Participation Affairs, Authorities in the MOH&ME, and the Members of UN Gender Theme Group. Moreover, the Country Office participated in different seminars held on the issue, e.g. ‘violence in the family’, ‘women empowerment’ and ‘violence against women’. Further, certain income generation projects, e.g. carpet weaving launched under BDN initiative were allocated for supporting women in health and development.

6.5. Tobacco Free Initiative Year 2002 saw many activities regarding Tobacco Free Initiative (TFI). The Country Office facilitated the preparation of Country Profile on Tobacco, which was shared with the National Focal Point and the amendments and comments conveyed to the Regional Office. WHO supported different ceremonies held concerning ‘World Notobacco Day’ and participated actively in various events organized for the purpose. At this occasion, the message of the Regional Director was communicated to the National Authorities and its Farsi translation was distributed among the audience gathered to celebrate the day. The Regional Director in his speech delivered in the ceremony held to launch Integrated Community-Based Program for the Prevention of Cardiovascular Diseases highlighted the importance of Tobacco Free world. In addition, at the request of MoH&ME, EMRO advocated the results of a research conducted by Mr. Eric Le Gresly and Mr. Luk Loossens, Consultants, International Union against Cancer into the industry’s activities in smuggling tobacco. The report was given to the Director General of Environmental and Occupational Health – the Focal Point for TFI. Also the same was translated into Farsi, released in the press conference held in Isfahan on January 5, 2002 by the Regional Director. Among others, Dr. Derek Yach, Executive Director of Non Communicable Diseases and Mental Health were also present at the occasion (see also section 2.3.5).
