Mission & Impact Statements Mission The mission of the Interfaith Dental Clinic is to create a healthier community by providing transformational oral health care for those experiencing poverty. Interfaith Dental Clinic was founded with the vision that every individual have the opportunity to achieve and sustain a healthy smile. Interfaith Dental Clinic’s “Change a Smile...Change A Life” Video: https://vimeo.com/53516345 Background In 1994, Dr. Tom Underwood, the Outreach Commission of West End United Methodist Church and the Nashville Dental Society founded Interfaith Dental Clinic to provide affordable dental care to working poor families and the elderly who do not have dental insurance. The clinic began operation with two dental chairs and an entirely volunteer staff, in the basement of the West End United Methodist Church. Soon after, the clinic hired its first staff person and current CEO, Dr. Rhonda Switzer-Nadasdi. Dr. Switzer-Nadasdi's vision and skills led her and the tiny clinic to quickly outgrow the two-chair broom closet. In 1998 IFDC successfully launched the clinic’s first capital campaign. That year, we moved into our own building and expand our capacity for delivering care. In 2006, the agency completed a second capital campaign to expand into the other half of our building and increase its patient services capacity by 30%. The $1.2 Million expansion campaign resulted in what is now a ten operatory, state of the art dental facility. The clinic continues to grow. In February 2012, we opened our second site in Murfreesboro -Interfaith Dental Clinic of Rutherford County. We currently employ a staff of 35 and hold the support of an established volunteer network of more than 300.We accommodated over 14,000 patient encounters and provided a market value of $4,540,821 in services last fiscal year. The clinic has also increased its emergency program to help deter emergency room use for dental needs.

Impact Interfaith Dental Clinic creates a healthier, happier population. People who have suffered from oral disease for years before coming to the clinic experience a life-altering transformation in their overall physical, social, psychological and spiritual health through our services. By providing comprehensive and emergency dental care the burden of the tax-payer is reduced as emergency room use for dental needs declines. Absenteeism from work and school due to dental pain and infection decreases and employment opportunities increase as people restore their smiles and their confidence. The effect of Interfaith's program is felt for generations to come as families learn proper oral hygiene and the importance of preventive dental care, breaking the cycle of poor oral health in their families and moving many out of poverty. This saves our community health dollars in the future. The goals of the Interfaith Dental Clinic are: 1.To increase employability and work attendance by lessening pain and improving appearance. 2.To educate patients about proper oral hygiene and nutrition so that they can keep their teeth for a lifetime and pass good habits onto family members. 3.To improve patients’ overall health, self esteem and quality of life through improved oral health. Our top accomplishments of the past year: - In order to effectively address the needs of the community and to affect broader societal change, Interfaith Dental Clinic launched the Interfaith Education Center for Community Dental Care (housed within our existing clinics.) As a pillar of the Education Center, Interfaith Dental Clinic is a clinical training site for an Advanced Education in General Dentistry (AEGD) residency program, through a partnership with NYU Lutheran Medical Center and University of Tennessee Health Science Center (Memphis). We are also now enrolling students for the Interfaith Registered Dental Assistant Program, a full-time, eight month training program to create highly-skilled and highly sought after dental professionals with excellent clinical skills, compassion and the Interfaith patient-centered approach. - Our CEO, Dr. Rhonda Switzer-Nadasdi, was also awarded as the Bank of America CEO of the Year at the 2015 Center for Non-Profit Management Salute to Excellence. - CEO, Dr. Rhonda Switzer-Nadasdi, and Chief Officer of Education, Andrea Stilwell, were awarded as a winner for the Nashville Business Journal's 2016 Women of Influence Awards in the Dynamic Duo category. Needs 1.Interfaith Dental Clinic provides affordable dental care for low-income families on a sliding fee scale, based on income and family size. It is therefore essential that we seek generous groups and individuals who want to partner and contribute funds to subsidize the cost of delivering this care. 2.Volunteer dental professionals who are willing to provide care for our patients either in-house (at Interfaith Dental Clinic) or in their own office are so valuable to us. We actively seek those professionals looking for an opportunity to serve in a way that creates long term impact by providing compassionate care to the most vulnerable in our community. Other ways to donate, support, or volunteer Interfaith Dental Clinic relies on the support of our community to help deliver affordable, comprehensive dental care to low-income, uninsured families, their children and the elderly in Middle Tennessee. We are always in need of community volunteers who provide development and front desk assistance as well as volunteer dental professionals who generously donate their time to care for patients at the two clinics or in their own dental offices.

Board Chair Statement As the Board Chair this year for Interfaith Dental Clinic, I wanted to share with you the story behind my smile. You see, I wore braces growing up and I had to have 10 teeth pulled when I was in third grade. My dentist, orthodontist and oral surgeon changed the way I felt about myself – thanks to them and the transformation they gave me, I did not have to go through the rest of my life being called “Buck tooth Crawford” – because of that experience I have always been amazed and appreciative of what dentistry can do for people. I joined the Interfaith Board to give back because I can empathize with the patients. I was fortunate enough to be able to get the oral healthcare I needed at a young age and it changed my life forever. Many are not as lucky. I love to see firsthand how Interfaith provides people with the opportunity to feel confident and secure with themselves. I have personally witnessed how Interfaith’s mission has helped so many patients in need; and how oral health problems are deeper than we might think. Oral health affects self-esteem, confidence, the health of your entire body, employment opportunities and the next generation. When you help someone get their oral health back on track, it really can, as we like to say…change everything! For patients like Cindy, everything changed when her new healthy smile helped her to land a better paying job. For Remonda everything in her family's future health changed when oral health education brought toothbrushes into her home for her family for the first time. For Fred, everything changed in his quality of life when his diabetes came under control without medication because his oral health improved. Interfaith Dental Clinic addresses whole body and whole mouth health. It takes more than a few teeth to chew, speak, and of course smile. And it takes more than just a few partnerships with others to meet the many needs of the disadvantaged and vulnerable. I am honored by the commitment of so many to these efforts. Together everything can change. For twenty years now I have been part of the movement that is the Interfaith Dental Clinic. It has grown but it has stayed (sorry for the pun) rooted in its mission. Programs Programs Affordable Dental Care for Working, Low-Income Families & The Elderly Description

Interfaith’s care is comprehensive, aimed to eliminate disease and restore a person’s mouth in function and aesthetics, while teaching skills to maintain it. Patient Accountability - Every patient pays on a sliding fee scale according to income and family size. Most pay 20% of usual and customary industry fees. Patients must demonstrate their commitment to getting well by practicing good oral hygiene, contributing their portion, and keeping their appointments. Prevention - IFDC strongly emphasizes education, teaching every patient about what caused their oral health to deteriorate. Providers teach oral hygiene and nutrition, as well as encourage smoking cessation. Patients pass these new habits onto their children, stopping the cycle of poor oral health in their family. Emergency Care - The clinic reserves emergency appointments for people who call suffering from pain and infection in need of urgent treatment. Emergency patients who also qualify for the Comprehensive Care Program are encouraged to apply.

Interfaith Dental Clinic measures the short-term success of its patients by the number of milestones they achieve while participating in the program. Milestones are as follows:

Long term Success

Receiving high quality, affordable, comprehensive dental care can change every area of a person's life. Because the health of your teeth and gums have a direct link to your health in other areas of the body, a person is more healthy overall when he or she receives timely, comprehensive dental care. School children who suffer from pain are relieved and can then concentrate in school and improve performance. Improved oral health leads to increased employability and work attendance by lessening pain and improving appearance. Senior citizens have better social lives, less isolation and are able to take in the proper nutrition. Long term, we envision a healthier community.

Program Success Monitored By

When a milestone is achieved by a patient, a specific code is entered into the Dentrix database. A quarterly report is run to measure how many patients are on track with their treatment plan. The CEO and Chief Program Officer discuss the results and make the necessary changes to the program.

Examples of Program Success

Jimmy is a medically retired US Navy officer. He came to Interfaith Dental Clinic as a referral from Veterans Affairs. His front tooth was loose, infected, and causing lots of pain. Unfortunately, Jimmy’s tooth was only part of his health problems. On his initial visit he weighed 486 pounds and his blood pressure was alarmingly high. He was advised by our staff to seek medical attention immediately. At the ER, Jimmy was diagnosed with diabetes and high blood pressure. “I was checking out, I was dying,” Jimmy says. Jimmy is like so many people who are in need. Our patients often do not realize how much their oral health care affects their whole body. They frequently do not understand the consequences of their lifestyle. Interfaith Dental Clinic does not just fix teeth, we teach lifestyle changes. “They provided all kinds of healthy tips and encouragement, while caring for my mouth.” As of today, Jimmy eats well, brushes, flosses, and exercises daily. He has lost more than 140 pounds. His blood pressure is now low, his glucose levels are normal and he off medications. If Jimmy had not come to Interfaith when he did, he likely would have had a stroke or heart attack. “They saved my life,” remarks Jimmy of Interfaith. “It is amazing how that one little tooth would change everything for me .”

Volunteer Program Description

Interfaith Dental Clinic could not operate without the generosity of volunteer dental professionals & non-clinical volunteers. They include; dentists, dental hygienists, dental assistants, specialists and students. In our last fiscal year, volunteers donated 4,541 hours of service, a market value of more than $400,000 to the clinic.

Long term Success

As volunteers work for our program, they become more aware of the great need that exists in our community. As more people become aware, solutions are sought and actions are taken to improve the oral health of the low-income, uninsured members of our community. Because of the generosity of our volunteers, more families and elderly residents are able to gain back their oral health, improving the community's health, employability and quality of life.

Examples of Program Success

Amber’s journey to Interfaith began when she was 13 years old. She was receiving her routine dental exam when a tumor was discovered in her lower jaw. Amber is proof that a dental exam can save someone’s life! She was sent to Nashville to see Dr. Sam McKenna at Vanderbilt to have the tumor removed. Bone was removed from her hip and used as a replacement for her jaw, but she lost five teeth in the process and needed dental implants. Amber’s insurance company would not cover this cost because it was considered cosmetic dentistry. Her family saved and they were able to pay for phase one of her treatment but not phase two, which meant Amber went for 10 years without teeth in the lower left side of her mouth. Needless to say, this was very difficult for a teenager. She learned to adjust way of eating, talking and smiling. Unfortunately, Amber developed an infection in her jaw and went back to Dr. McKenna hoping for a miracle. Dr. McKenna, avid IFDC volunteer knew that she would be an excellent candidate for the Oral Surgery Program. “My experience has been amazing from the get-go!" states Amber. Interfaith Dental Clinic has been amazing and I am so grateful for their hard work and dedication. They enlisted the volunteer help of Rutherford County Oral Surgeon, Dr. Walter Chitwood to oversee and assist with my care. With his time, effort and guidance, together they transformed my smile and changed the rest of my life. No one could ever realize how truly grateful I am! After waiting ten years, this was a tremendous blessing and an answered prayer.”

Student Mentorship Program Description

We currently collaborate with the Vanderbilt Oral Surgery Department to provide basic and complex oral surgery with IV sedation for our patients 3 days per week in our facility. Oral surgery residents voluntarily perform this service on rotation as part of their training at the Interfaith Dental Clinic. In this fashion, patients are provided care by oral surgeons with the highest of skill levels, reducing post-op pain, complications and further infection. We also partner with other teaching facilities such as Volunteer State Community College, TSU, MeHarry, Kaplan and Remington, placing students for training. Interfaith Dental Clinic is certified as a training site for a post-graduate Advanced Education in General Dentistry Residency. Interfaith Dental Clinic will host two dental residents from NYU Lutheran Medical Center this year, providing training and experience in comprehensive clinical care and management of the medically compromised patient. Residents will gain exposure to and experience with complex treatment modalities in all disciplines of general dentistry. Interfaith Dental Clinic provides an opportunity to enhance the resident’s level of confidence and competence with advanced clinical instruction in order to integrate clinical practice and exposure into dental public health related activity.

Through our collaborations with local dental educational institutions, the Interfaith Dental Clinic is helping students gain valuable hands-on education, knowledge and experiences that will make them better dentists, specialists, hygienists, and assistants.

The students gain exposure to and experience with general dentistry and learn about the complexities of caring for the under served. Some choose to go into public health service as a result of what they have seen at the Interfaith Dental Clinic. Others continue volunteering in their community after seeing what a profound difference they can make.

Interfaith Education Center for Community Dental Care Description

In order to effectively address the needs of our community and to effect broader community change, Interfaith Dental Clinic and its community partners need to expand our core group of specially, holistically trained dental professionals. We have identified a compelling opportunity that we feel will enable us to address unmet community need, and at the same time build additional financial support for the clinic and engagement with the community of dental professionals in our area. Through the implementation of the Interfaith Education Center for Community Dental Care, IFDC will dramatically expand its impact through hands-on training of dental professionals in our existing clinics. Dental professionals will develop their skills while providing quality care to those in our community who are least able to afford it. These programs will expand our service to the community, as our students and residents will provide professional services in-kind as part of their training. The Education Center will emphasize and enhance our holistic, compassionate care model into all of our training programs for our students.

Long term Success

The long-term results of the Interfaith Education Center for Community Dental Care will be educating the next generation of oral healthcare workers. Through evidence based research and collective experience we will move health professionals to embrace holistic paradigms of healthcare and vocational commitment of care to the underserved. All staff will be engaged in further carrying out this task at every level of contact resulting in an increase in the physical well-being of Middle Tennessee residents, an increase in employability, a decrease in work and school hours missed, and a healthier community as a whole.

Program Success Monitored By

With the additional care provided through the Education Center students/trainees, we expect our patient care capacity to grow more than 50% over the upcoming three years (these results will be tracked through Dentrix dental office software and evaluated quarterly). Utilizing both didactic and clinical training, we will graduate well-prepared and highly sought-after dental professionals with a rare blend of clinical skills, compassion and patient centeredness. Education Center trainees/participants will be given pre and post assessments to evaluate cultural sensitivity and behavioral health modification, as well as utilization of a loving care model. Exit surveys of all graduates will be given, assessing their future career plans, placements and goals. Follow-up surveys will be given annually to track their long term career paths. Results of the assessments will be used to adjust curricula as needed.

CEO Comments Interfaith is unique in that we dedicate the time and resources to provide individualized, patient centered care at every appointment. At each encounter, our patients receive oral health education, oral hygiene training and instruction and motivation for self-improvement from qualified providers. It is our belief that to eliminate our patients’ oral disease and improve their overall health, efforts must be devoted to each of these priorities to achieve sustainable success. If a patient needs additional counseling in any of these areas, we have a dedicated Oral Health Educator available to meet with the for more extensive, individualized consult. There is a severe need for programs like ours that provide fully comprehensive dental care and include IV sedation and dental specialty care. Many clinics serving the poor are left with pulling teeth as the only costeffective option for them and the patient, leaving holes in mouths and lowering employability and the ability to eat nutritiously. Grossly decayed and missing teeth contribute to the isolation of individuals preventing them from fully realizing their potential and becoming active members in their communities. Interfaith Dental Clinic is the only option for patients who need to not only become oral disease free but to transform their overall physical, mental and social health.

Experience Dr. Rhonda Switzer-Nadasdi was born in Treherne, Manitoba, Canada and graduated from the University of Manitoba with a Doctor of Medicine in Dentistry degree in 1991. She completed a post-graduate residency program at the VA Medical Center, Louisville, KY. Since then, Dr. Switzer-Nadasdi has spent her entire dental career overseeing dental programs for the poor. She served as the head of dentistry at a fly-in health center in the sub-arctic location of Churchill, Manitoba (the Polar Bear Capitol of the World) from 1992-94. She also worked as a staff dentist for the Department of Health in Hamilton, Bermuda in 1994-95 where she treated children of low-income families, people with mental disabilities and people in nursing homes and prisons. Dr. Switzer-Nadasdi became the first employee of the Interfaith Dental Clinic in Nashville, Tennessee in 1995 and has served as leadership since. During her tenure, the clinic has grown from a two-chair operation located in a church basement to a 2-clinic, 15-chair facility with a staff of 35, caring for more than 2,700 patients annually. She was recently inducted into the International College of Dentists, an honor reserved for 3% of dentists worldwide. Dr. Switzer-Nadasdi is the current President Elect for the Nashville Dental Society and will proceed to President next year. She chairs the Council on oral health initiatives of the Tennessee Dental Association, founded and chairs the Middle Tennessee Oral Health Coalition, and now has a seat on the Council on Access and Interprofessional Relations of the American Dental Association.

Senior Staff Dr. Laurie Carlisle DMD Title

Chief Program Officer

Experience/Biography

Dr. Carlisle joined Interfaith Dental Clinic in June of 2010. She is a graduate of the University of Louisville School of Dentistry and completed a year of residency in advanced general dentistry at the University of Mississippi Medical Center in Jackson, MS. During her residency, she performed full mouth rehabilitations, and completed rotations in emergency medicine, general anesthesia, pediatric dentistry, and advanced general clinical dentistry. Dr. Carlisle is a member of the American Dental Association, Tennessee Dental Association, and the Nashville Dental Society. Dr. Carlisle is also a finalist for the 2014 Emerging Leader Award.

Dr. Elizabeth Jackson DMD Title Experience/Biography

DDr. Elizabeth Jackson joined our team as the Clinic Director of our Rutherford County Clinic in July 2013. She graduated with her degree in Biomedical Sciences from Auburn and earned her D.M.D in 2007, from The University of Alabama at Birmingham. While in dental school, Jackson had the opportunity to participate in an exchange program witih Japanese Students from Asahi University. Today she is very involved in organized dentistry and some of her memberships include, the ADA, the Tennessee Dental Association and the Seattle Study Club.

Mrs. Andrea Stilwell Title

Chief Officer of Education

Experience/Biography

Andrea joined our team in June 2014. Prior to that, she worked for Bank of America as part of the Local Market Delivery team delivering the company’s Corporate Social Responsibility and other initiatives across three states. She began her career in the nonprofit world, working in various roles for two Big Brothers Big Sisters’ organizations in Indiana. She graduated from Ball State University with a degree in Political Science in Public Relations. She also holds an MPA from Indiana University, with concentrations in Nonprofit Management and Policy Analysis. Outside of work, she is an active volunteer with several Nashville nonprofits, most notably Junior League and reading each week to the Pre-K students at Caldwell through the PENCIL Foundation.

Mrs. Beverly Grant Title Experience/Biography

CEO Comments The patients that we care for have advanced oral disease processes that have often gone on for years. The care is expensive to provide and requires advanced skills by the dental team. Only the most skilled, qualified and compassionate providers are hired or approved as volunteers. We set our goals in all areas of clinical care and safety above the standard. As stewards of community gifts, the clinical team and the patient work together to eradicate and prevent future disease. Each patient is cared for individually, with respect, dignity and love. Interfaith Dental Clinic is the perfect balance of clinical excellence and compassion.

Organization Comments All of our adult patients are low-income and uninsured. The state of Tennessee provides some financial support to care for this group but it equals only 2% of our operating budget. Twelve percent of our patients are children, but only half of these children are insured for dental through TennCare or CoverKids. Interfaith Dental Clinic is

almost solely supported through the consistent generosity of local citizens, their companies and foundations.
