Innovation Integration Project Theme Definitions This document explains the terms and concepts for Innovation Integration Project Themes.

PROJECT THEMES A project theme is a collection of related projects. Project themes are used to organize projects across the outpatient clinical enterprise. The Innovation Integration Team has 10 active project themes. 1. 2. 3. 4. 5.

PROJECTS A project has a clear and agreed upon objective, with a defined life span, and is generally something that is new or a one-time effort, with specified requirements. The Innovation Integration Center has included active or planned projects in their project inventory. Projects listed in the inventory aren’t necessarily being worked on by the team. The team should be aware of these projects for reasons of integration or opportunities for building capabilities to support efforts. Using Provider Communication Wizard as a conduit, users can intentionally send documents to providers for his/her review. Docs4U is an interface that is accessed within StarPanel that allows all Vanderbilt providers to view and acknowledge documents. This interface takes the place of traditional faxing within the Vanderbilt Community.

A tool built internally and located within Star Panel in which users can communicate in a secure manner. Widely used within a patient chart to communicate about the patient, but also can be used outside of a patient record. It is also integrated with MyHealthAtVanderbilt, allowing providers and the care team to treat any communication with patients as an automatic part of the patient record. A clinical workflow tool designed to facilitate communication among all members of the healthcare team. OPWB displays numerous aspects of a patient's status including the patient's location, the length of time at that location, and the status of tests to be performed, processes to be completed and/or personnel to be seen in clinic. A tool built internally and located in StarPanel that facilitates the process by which providers communicate clinical information from VUMC to referring providers, primary care providers, and other providers with whom we need to communicate about a patient.

The user-friendly electronic medical record for VUMC. StarPanel allows the users to create a panel of patients. StarPanel integrates patient data from multiple sources, including: laboratory and pathology results, radiology reports and link to PACS, echocardiography reports, discharge summaries, physician notes, list of problems, medications and allergies as well as other documents. StarPanel also contains a powerful messaging system to allow users to communicate regarding patient care. StarPanel also incorporates various whiteboards for patient tracking and data collation. An online on call schedule application. The application automatically sends data to Notify MD regarding provider call schedules. Part of personalized medicine means ensuring that patients receive medications that are right for them. The PREDICT Project empowers clinicians with the genetic information needed to predict and help prevent bad drug side effects. Because each person responds differently to medicines, this knowledge is helpful to prescribe drugs that work better and have fewer side effects. Electronic Prescription Writer application that works within Star Panel. This system provides state-of-the-art integration of prescribing into the workflow of all provider types, and supports a comprehensive set of decision-support alerts and reminders.

Interface within StarPanel that brings all the documentation tools together in one user interface to streamline the documentation process for nurses and the care team.

Combining Management with Building Orders (COMBO)

Combo is a holistic Informatics solution designed to address current clinical workflow challenges as well as new “opportunities” being presented with Meaningful Use and ICD-10 initiatives. This tool will reduce the time it takes VUMC providers to; (1) order prescriptions and diagnostic tests, (2) document the Assessment & Plan, (3) manage patient Problems and Medication lists, (4) submit billing information for visits (i.e.- an electronic equivalent of the “blue Sheet”), and (5) generate the Clinical Summary. The Documentation Completion Interface project is aimed at understanding and defining ways to improve medical documentation. This will be accomplished by developing and implementing this electronic capability in order to improve quality care, compliance, patient safety, and cost (effort). Specifically, the goal is to increase the rate and timeliness of completion of medical documents throughout the enterprise, as well as, providing ways for incomplete documents to be reviewed and executed more efficiently by clinicians. The Clinic Operations Workstream has been established to provide focused attention and accountability for the impact of the implementation of ICD-10 relating to the current processes and supporting technology used to capture and document ICD-9 codes. Current complexities with these processes and tools will create a challenge with the transition to ICD-10. The Workstream will address transition steps by outlining key business activities for staffing, work flows, policies, procedures and training for each clinic and/or department currently using ICD-9 codes. Areas of focus will also include expanding education programs for clinicians regarding documentation requirements, template consistency among specialties, and data format standardization within documentation systems/processes.

Voice Recognition

Problem oriented modular documentation is a new tool that users can utilize to document clinical encounters. Each component is built into the form in a modular format making the tool versatile and intraoperable with other applications. A voice recognition tool used by clinicians to document in StarPanel. A voice recognition software such as Dragon will one day replace most, if not all, traditional transcription services (i.e. dictating into a phone device that is transcribed by a transcriptionist). Voice Recognition has the ability to reduce turnaround time, increase accuracy, and reduce costs of transcriptionists.

Referral Management The theme, Referral Management refers to the process by which we manage the ordering, scheduling, tracking and routing of assessments from the ordering provider and team through the consultant team and then back to the original ordering provider. Critical referral management refers to the practice of systematically tracking critical referrals in a closed loop system. Critical referrals are those deemed by the clinician to be important to a patient’s treatment, or as indicated by practice guidelines (i.e. referral to breast surgeon for a potentially malignant breast tumor) The projects listed for Referral Management include: Project

Systems Access Management The theme, Systems Access Management refers to management of the complete lifecycle of an interaction with a computing device, starting from the point a device is initialized and ending when a user terminates the session. A view in StarPanel derived from a workflow in which a provider’s support staff populates only the documents the provider needs to review prior to a patient encounter in the outpatient setting. When the provider selects the patient from a list the pre-populated documents are automatically opened and ready for his review. The application is enhanced for the user by using a mobile device such as an IPAD. A device, typically in card form, that is used to authenticate the identity of the device holder/owner. The card is used with a reader that supports the same communication protocol, and can be held within a zero to three inches away from the Reader to initiate the identification event.

StarLite is a view in StarPanel that is generated by using algorithms and log files so that we can decipher which links a user frequently accesses thus removing links that are infrequently or never used. StarPanel will then be more user friendly and easier to navigate.

StarPanel Customized Views (SPCV)

Provides a new view of the StarPanel interface that is scalable to the institution yet meets the specific requirements of individual departments/services in order to increase the efficiency of information review during a patient encounter.

Virtual Clinical Workstation (VCWS)

An operating systems (i.e. Windows XP) running on a remote (server) computer that can be accessed from any location through a thin client device or client software. The thin client device or client software presents the operating systems to the locate screen mimicking a normal physical computer.

Is part of ARRA Stimulus Bill and encourages advancing information technology in health care through the use of electronic health records. Goals of HITECH include: • Improving quality, safety and efficiency • Engaging patients in their care • Increasing coordination of care • Improving the health status of the population • Ensuring privacy and security A clinical workflow tool designed to facilitate communication among all members of the healthcare team. OPWB displays numerous aspects of a patient's status including the patient's location, the length of time at that location, and the status of tests to be performed, processes to be completed and/or personnel to be seen in clinic. Development of capabilities to support the transfer of medical information through interactive audiovisual media, or reach clinical signals for the purpose of consulting, or enhancing clinical decision making for care provided at a distance. Some of the components may include two-way video conferencing, remote monitoring, phone, fax, e-mail, secure messaging, store & forward/interpret, personal health record, remote access electronic health record, health information exchange, and sometimes remote medical procedures or examinations.

An interactive web-based patient portal developed for patients, staff, and providers. Limited access gives patients the ability to message their provider’s office with whom they have had a relationship with. Patients with full access will be able to see lab test results, send and receive secure messages with their doctor's offices, and request appointments. Patients are also able to view some of their personal medical information and review and pay their medical bills online. The development of numerous tools that will foster greater patient engagement and interaction across a set population of patients. Unlike current tools, capabilities fostered through the MHT@V model would be patient centric. That is, ensure patient activation and engagement, communications around patient choice and preferences, and measure adherence and execution of care plan. Patient centric capabilities will enhance system coordination and efficient use of resources while delivering personalized care. National Committee for Quality Assurance (NCQA) Patient Centered Medical Home (PCMH) is an innovative program for improving primary care. NCQA uses an established set of criteria to evaluate the practice’s ability to organize a health care setting that facilities partnerships between patients, their personal physicians, and when appropriate, the patient’s family. Assessment includes the organization’s use of registries to facilitate care, information technology, health information exchange and other means to assure that patients get the indicated care when and where they need and want in a culturally and linguistically appropriate manner.

Tools and solutions designed to improve efficiency of care while collecting pertinent health information at the point of service. Completed forms include, but are not limited to, completion of health assessment forms, quality of life indicators, and numerous regulatory forms such as patient consent forms, and patient bill of rights.

Project providing real-time dashboards (i.e. StarTracker and other outpatient dashboards) which automatically identify data fields within StarPanel (i.e. certain labs, notes, reports or form fields) and delivering them to the user with decision support, where appropriate. These tools are meant to increase the visibility of important data and measures and can form the foundation of population management.

The Center for Health Promotion and Disease Management’s goal is to coordinate effective and timely interventions to minimize disease related illnesses in a chronic population. Additionally, we proactively implement interventions to prevent disease in the healthy population. We accomplish this by communicating directly with the provider, patients, and the payors utilizing evidence-based “best practices” and clinical guidelines. Project providing care coordination for patients insured by Vanderbilt University who are at elevated risk for hospital admissions, ED visits and other major forms of utilization. This system involves use of advanced technology to risk stratify patients and facilitate interactions between the patient, their physicians, and the care coordinators. This system tracks outcomes, patient goals, and barriers, with the primary goal of maintaining patient health. The Physician Quality Reporting System is currently a voluntary reporting program that provides an incentive payment to identified eligible professionals who satisfactorily report data on quality measures for covered professional Medicare Physician Fee Schedule services furnished to Medicare Part B fee-for-service beneficiaries during a specified reporting period
