Mission To fulfill Premier Health Partner’s commitment to build healthier communities through prevention, early detection and disease self-management.

Vision To eliminate preventable disease and disability and create and enhance systems, which empower people to live the highest possible quality of life in Premier Health Partners’ geographical market area.

Our Focus Areas Premier Community Health focuses on cancer, diabetes, heart and lung health. Premier Community Health is a 501c3 nonprofit organization. It is a wholly-owned subsidiary of MedAmerica, which is owned by Premier Health Partners. PCH is governed by a community-based board of trustees, which meets quarterly.

From the Executive Director Dear Friends: This will be remembered as a challenging year. Along with the rest of the community, we saw our funding sources shrink and demand for services grow. We had to look for new ways of doing business that used our resources wisely and stretched them as far as we could. During our brief history, we have learned to embrace change. And not just change for the sake of change — but change that makes us stronger and helps us serve this community better. We honed our strategic plan and built a balanced scorecard to measure our success. This year, we had several milestones. First, we were awarded the Alfred P. Sloan Award for Business Excellence in Workplace Flexibility. This award, given by the United States Chamber of Commerce, recognizes employers that value a variety of employee flexibility initiatives that increase employee satisfaction and productivity. In the community, we documented touching about 42,000 lives through our programs and services. Sometimes with a big number like that, it is easy to forget we are serving people, such as the 40 people who found out they had cancer because of PCH services. For the future, Premier Community Health wants to continue impacting the community’s overall health. Through its collaboration with hospitals, other businesses, organizations and even individuals, Premier Community Health strives to better the health of the wonderful communities within the greater Dayton region and ultimately works to improve the lifestyles of those within our area. Awards and Accomplishments

Knowing Your Numbers Could Save Your Life! When David “Chet” McAninch saw an opportunity to have his cholesterol checked at a Premier Community Health screening, he thought he would follow his doctor’s orders and go to the screening. Turns out that was an important decision. “My doctor had been bothering me for awhile to get my cholesterol checked,” said McAninch. “Bottom line, I just didn’t want to go to the doctor.” Lucky for him, during the screening, he learned his cholesterol was alarmingly high. Premier Community Health staff urged him to see his doctor immediately to address this problem. Now, McAninch is making healthy changes to lower his cholesterol. He has the love and support of his family keeping his motivated. “My brother-in-law is a nurse, so I worked with him. He told me I needed to change my eating philosophy.” Chet also has a young daughter he uses to stay motivated in being healthier. “I want to be around to watch her grow up.” Besides your blood pressure, knowing your cholesterol numbers can be one of the most important numbers you can know. Maintaining proper cholesterol levels can help lower your risk of a heart attack or stroke.

Our Impact on the Greater Dayton Region Premier Community Health provides evidence-based programs and services that can make an impact on our community. Most of our programs fall into our four focus areas- cancer, heart, diabetes and lung health. These are the results our programs have given us in 2009.

Cancer Breast Health Programs Mammograms, Pap tests and other services are provided to uninsured women. Part of this program is paid by a grant from the Ohio Department of Health with support from the Centers for Disease Control and Prevention. Mammography Voucher Program • Provided 467 mammograms and found 43 abnormals and 1 cancer Breast and Cervical Cancer Project • Provided 955 mammograms, 766 clinical breast exams, 165 ultrasounds, 78 surgical consults, 85 biopsies with 19 cancers diagnosed • Provided 828 Pap tests, 814 pelvic exams, and 24 colposcopies with 8 cancers/ dysplasia found

My Sister’s Keeper While white women get breast cancer more often, African American women die more often than any other group. This program successfully tacked that problem through engaging programs, free mammograms and a thoughtful ear. • Held 64 parties with 979 participants • Provided 117 free mammograms, paid for 12 co-pays and helped 16 women receive their first-ever mammogram, finding 1 cancer Colorectal Screening Kits Each March, PCH collaborates with Kroger’s Pharmacies, WDTN-TV2, Vectren and others to offer colorectal kits to the greater Dayton region. In 2009, this program: • Distributed 15,000 home testing kits and had 3 abnormal test. Throughout the year, PCH offers these kits at other events. In 2009, we: • Distributed 2,295 take-home colorectal screening kits with 0 abnormal tests

Skin Cancer Screenings In collaboration with the Wright State University Boonshoft School of Medicine Dermatology Residency Program, the hospitals in PHP and the American Cancer Society, a free full body skin cancer screening is offered each May. • Provided 391 full-body skin screenings with 6 basal cell cancers, 1 squamous cell cancer, 1 melanoma cancer and 3 actinic keratosis cancers found • Throughout the year, PCH provided 683 Dermascan facial skin screenings

Diabetes • Provided diabetes education and counseling to 47 people • Provided 532 Hemoglobin A1c screenings

Heart Heart Risk Reduction Program • Served 13,725 individuals with total cholesterol, HDL (good cholesterol), blood sugar and/or blood pressure screenings Stroke Screenings • Provided 324 individuals with screenings to assess their risk of a stroke

Lung Employer Flu Vaccine Program • Provided 2,157 seasonal flu shots and 201 H1N1 shots Tobacco Cessation • Provided tobacco cessation counseling for 74 people

Health and Wellness Health Presentations • Provided 49 free health lectures reaching 1,355 people in the community Senior Health • During May, PCH coordinated a campaign with WDTN-TV2 to educate seniors about the importance of medication safety and to offer a free self-help booklet. The campaign included 318 public service announcements on WDTN Channel 2 • Distributed 16,887 medication safety booklets to senior adults who take two or more medicines • Provided 1,469 senior adults with senior health screenings
