The key areas of work: • Setting, monitoring and improving standards for the selection, training and assessment of doctors working in paediatrics and child health, both in the UK and overseas. • Setting, monitoring and administering the College examinations (MRCPCH and DCH). • Providing information and support to trainees and career grade paediatricians. • Developing and monitoring a portfolio of educational programmes and resources. • Setting, monitoring and improving the standards and guidelines for CPD and revalidation of the paediatric workforce, in line with General Medical Council (GMC) requirements.

MRCPCH Examination The MRCPCH examination currently consists of the following written papers: • Foundation of Practice • Theory and Science • Applied Knowledge in Practice which are followed by the Clinical Examination. Applied Knowledge in Practice is one examination split across two papers. The MRCPCH Clinical Examination is a practical examination that is split across 10 clinical stations. The written papers can be taken in your order of preference and simultaneously if you wish, but the MRCPCH Clinical Examination can still only be taken once all written elements have been successfully passed. You are expected to complete the MRCPCH before the end of ST3 to progress into Level 2 training. Ordinary Membership of the College is achieved by successfully completing all examination elements of the MRCPCH. There are three sittings of each type of membership examination a year. The MRCPCH Examinations have recently undergone significant changes as a result of a comprehensive review. The written examination papers have been renamed to Foundation of Practice, Theory and Science and Applied Knowledge in Practice; these have formally been the Part 1a, Part 1b and Part 2 respectively

The DCH is designed to give recognition of competence in the care of children to: • SAS grades in paediatrics • trainees in specialties allied to paediatrics • general practitioner vocational trainees You must have successfully completed the Foundation of Practice paper (or previously completed Part 1a) before sitting the DCH Clinical examination. You may apply to do both at the same time or complete the Foundation of Practice examination first and move on to the DCH Clinical during another application period. The aim of the DCH Clinical examination is to assess whether candidates have reached the standard in clinical skills expected of a newly appointed General Practitioner who has completed a short period of training in paediatrics. The DCH clinical examination is held twice a year. All UK DCH Clinical applications are online.

Recruitment into training RCPCH centrally coordinates entry across ST1-4 on behalf of all four nations, providing a common application form, shortlisting and interview scoring matrices, interview questions, guidance notes and person specifications. This enables us to ensure that we can continue to develop a fair and robust selection process for all applicants.

Consultants can support the RCPCH by producing questions/ scenarios at locally arranged question writing days or at the Examination Boards. Questions are produced for: • • • • •

Foundation of Practice (FOP) written examination Theory and Science (TAS) written examination Applied Knowledge in Practice (AKP) written examination Communication scenarios within the clinical examination Video scenarios within the clinical examination

MRCPCH FOP questions are those conditions that are likely to be seen in six to twelve months of hospital, community or primary care practice. FOP also forms the written element of the DCH membership examination. MRCPCH TAS questions are to test the basic scientific, physiological and pharmacological principles upon which clinical practice is based. MRCPCH AKP questions are to test judgment and experience. The two papers contain case history, data interpretation and photographic questions. Communication scenarios test communication with colleagues dealing with professional errors and giving information. Video scenarios assess skills in dealing with acute cases that cannot be brought in to test.

CESR: Certificate of Eligibility for Specialist Registration SAS doctors working in paediatrics who have not completed an approved CCT programme may still be able to join the Specialist Register through CESR. The CESR is equivalent to a CCT and certifies that you have all the competences defined in the CCT curriculum. The CESR in the CCT specialty of paediatrics is open to doctors from the UK or abroad who have training and experience in paediatrics partly or completely outside of the CCT programme. CESRs in only a sub-speciality of paediatrics may also be available to doctors with training outside the UK. You must submit documented evidence of skills and experience in all aspects of the CCT curriculum. This evidence can be from both in and outside of approved training programmes.

SAS doctors wanting to apply through the CESR route may find it helpful to use the ePortfolio and ASSET systems for paediatric postgraduate training to demonstrate they have all the competences defined in the CCT curriculum. These tools allow you to log your competences electronically. In ePortfolio, you can record reflective learning events, make personal development plans, upload files to the personal library and link all of these to the curriculum. Workplace-Based Assessments (WBAs) are completed via ASSET, which are automatically updated in ePortfolio. Your progress is logged by supervisors in trainer’s reports and confirmed in annual reviews each year. Doctors must register with the RCPCH for use of these College e-systems at: www.rcpch.ac.uk/training-examinations/enrolment-training For SAS doctors not wanting to apply for CESR or not wanting to re-enter postgraduate training should use the online CPD diary and revalidation support systems only, rather than the ePortfolio and ASSET systems.

Recent assessment developments SAS doctors using ASSET should be aware that the College facilitated the creation of a working group which looked at the workplace based assessments offered on ASSET. They examined their suitability and how they could be improved to best assess trainees. The recommendations of this group have been submitted to the GMC. • • • • •

move to supervised learning events (SLEs) for the majority of workplace based assessments (WPBA) the removal of scoring to concentrate on the formative feedback element of WPBAs introduction of case based discussions in safeguarding and leadership the piloting of some innovative paediatric assessments for handover the piloting of adult medicine assessments in paediatrics (ACAT)

These changes will be accompanied by a blended learning programme which will help trainees and assessors understand the rationale and implement the assessments. All training materials will be made available to all trainees, supervisors and tutors The pilot commenced in September 2013 and will run for 1 year after which an full evaluation will be collated and presented to various stakeholders including the GMC.

The work of the RCPCH Simulation Working Group is to create a unified national strategy to enable and enhance the delivery of simulation and technology enhanced learning across all of paediatric training in the UK. The RCPCH Strategy aims to deliver an integrated blended learning approach that is: •

Patient and family–centred and service-driven. The strategy will focus on equipping the workforce with the necessary skills for safe and effective patient care, augmenting supervised clinical practice as part of a blended learning approach that facilitates learning skills in a safe environment.



Educationally coherent. It will identify clearly articulated learning needs mapped to RCPCH curriculum and aligned to service needs. Each School of Paediatrics / LETB will identify leads to champion this work.



Innovative and evidence-based. The aim is to enhance training, informed by the best available evidence.



Delivering high quality educational outcomes. Educational and healthcare providers should ensure that they meet and, wherever possible, exceed agreed standards



Ensures equity of access and quality of provision. Applications should enhance training, improve productivity, reduce duplication and be affordable and cost effective across the health and social care workforce.

Special Interest (SPIN) modules SPIN modules have been developed to standardise the special interest curriculum that were being used by a number of CSACs and special interest groups. The current Frameworks of Competence give both the trainee/ SAS/Consultant and supervisor guidance about the competences that need to be covered. For Level 3 trainees this is in addition to their Level 3 Paediatric training, College approved curricula will be available for the following: • • Diabetes • • Epilepsy • • Safeguarding • • Cardiology • • Allergy • High Dependency Care • • • Nephrology

On completion/evidence of the competences within the modules (signed off by the relevant CSAC and a more experienced colleague), you should be competent to take up a post as a General Paediatrician with a special expertise. It is expected that there will be a requirement in paediatric services for consultants with such expertise and will form part of a Regional Specialty network. If assessments are required these should be taken from the Level 3 assessment strategy utilising the workplace based assessments (WPBAs) guidance to demonstrate competence and recorded within ePortfolio or similar systems. CSACs will give advice on the appropriate spread of WPBAs and other evidence that can be used to confirm competence.

The College Tutor is the RCPCH representative in his/her trust and is a post filled by consultants and SAS doctors. It is his/her duty to keep up to date with RCPCH standards in education and training, new educational initiatives and changes to examinations, assessments and curriculum updates. The role includes: • • • •

coordinating delivery of postgraduate education and training in their trust to deliver both RCPCH curriculum and departmental CPD ensuring a high quality induction programme allocating educational supervisors to trainees ensuring the educational supervision process

The RCPCH holds an Annual College Tutors day in October and has an annual meeting at the RCPCH Annual Conference. It is at these events that College Tutors can share good practice, learn about the College updates in regards to education and training. The College website also houses a web-forum for College Tutors to share their views, concerns or good practice.

The RCPCH CPD scheme enables paediatricians to demonstrate their knowledge, skills and competence for effective medical practice. The scheme is relevant for career grade doctors including including staff, associate specialist, and specialty doctors. The CPD scheme includes access to the CPD online diary which supports: • simple recording of CPD activities in line with RCPCH recommendations • uploading of evidence of CPD e.g. certificates, notes of learning, handouts • creation of personal development plans • production of CPD activity books and CPD certificates for appraisal and revalidation The College is currently developing a web app to support flexibility of access to the CPD diary, due for launch Spring 2014. Registration on the scheme and use of the online diary is not mandatory, but is recommended to help you demonstrate your CPD effectively and succinctly. Access to the CPD diary is a membership benefit, or chargeable to non-members.

CPD activity approvals The RCPCH offers an optional service to doctors and organisations delivering regional or national CPD events or offering e-learning resources to career grade paediatricians in the UK. The approvals process involves: • • •

Research and health policy The key areas of work: • Carry out key projects to look at children’s health services and to encourage and promote research into child health. • Ensuring the insights and experiences of children and young people influence the RCPCH to improve standards of paediatric care and health outcomes for children and young people. • Developing and influencing policy affecting safeguarding, health services and public health for children and young people. • Monitoring and evaluation of workforce trends, through comprehensive data collection, to influence health policy. • Active surveillance of rare childhood disorders through the British Paediatric Surveillance Unit. • Ensuring high standards of clinical practice by producing and promoting high quality child health guidelines to members and facilitating national audits.

Child protection is a high priority for the College and plays a part in everything we do. It is an emotive and potentially contentious subject, but one that is everybody’s business. RCPCH focuses on influencing national policy in this area, developing publications and standards and providing training. As consultants you could encounter child protection cases at any time. Health policy The Heath Policy Team develops and influences policy affecting health services and public health for children and young people. In 2013-14, the team: • published Back to Facing the Future which audited the Facing the Future standards • published care standards for young people held in secure settings • published a position statement on the use of vitamin D • developed an emerging evidence base to understand the reason behind child mortality in the UK Our priorities for 2014-15 include: • developing links with Strategic Clinical Networks • strengthening child safeguarding in the NHS • implementing standards of care for young people in secure settings • supporting the work of the Children and Young People’s Outcomes Forum. • developing new standards for urgent care and treatment outside hospitals • further developing the evidence base about interventions to reduce child mortality in the UK

The Royal College of Paediatrics and Child Health (RCPCH), British Association for Paediatric Nephrology (BAPN) and the British Kidney Patient Association (BKPA) have developed a national online information resource about paediatric kidney conditions for families. The information covers kidney conditions in infants, children and young people up to 18 years of age, including antenatal diagnosis. It is principally aimed at parents and carers, but may also be valuable to young patients (11 years and over). Topics and areas covered have been decided after consultation with parents/carers and nephrology leads. The information is a useful tool for healthcare professionals that can be used when explaining a child’s renal condition to their parents. The website launched in March 2014.

MEDICINES FOR CHILDREN LOGO

Medicines for Children Medicines for Children is a free information website that provides practical and reliable advice to parents and carers of the children you treat - covering more than 100 medicines. Our website features over 160 leaflets – searchable by the drug trade or brand name, or the condition being treated – and a series of videos. Prescribers may use the resource as an aid when speaking with families, or signpost them to the website. Our leaflets can be read and downloaded online at www.medicinesforchildren.org.uk Since March 2011 Medicines for Children has been certified by the Information Standard as a producer of reliable health information for the public. It also features on NHS Evidence. ‘I have recently discovered your information leaflets and found them very user friendly, informative and helpful.’ -Community Paediatrician. You can help us review or write leaflets, or encourage your colleagues to do so. CPD points for reflective learning are available.

UK-WHO Growth Charts The RCPCH have now produced a complete suite of charts (supplied in the UK by Harlow Printing) for all uses ranging from child health surveillance to detailed monitoring of children with health problems. The RCPCH have also developed additional charts which measures the stages of puberty in children 2-20 years, a boys and girl BMI lookup 2-20 years chart and has overseen the development of the Down syndrome 2-18 growth chart which is also available for healthcare practitioners use. Contact and further information Visit: www.rcpch.ac.uk/growth-charts Email: [email protected] Urgent and emergency care patient reported experience measure (PREM) The RCPCH with Picker Institute Europe have developed an evidence based tool to measure the experience of paediatric patients 0-16 years in all urgent and emergency care settings including; GP practices, out-of-hours centres, A&E departments and the ambulance service. The survey covers all aspects of urgent and emergency care where the patient is able to access a clinician on the same day (ambulance, GP, A&E and out of hours services). Workforce Strategy & Planning The College agreed a comprehensive and cohesive workforce strategy in Autumn 2013 which fits in with the College’s policy priority “A better NHS for children and young people”. This strategy sets out a vision under the four main priority headings • • • •

The strategy uses these 4 principles to identify what we have previously or are currently doing to contribute to these aims and describes how we intend to achieve the vision for each strategic priority. The proposed strategy requires strong commitment and leadership across the College, not limited to those currently dealing with workforce planning. www. rcpch.ac.uk/workforce RCPCH Workforce Census The biennial workforce census is a vital resource for the College in underpinning this strategy and for influencing workforce planning. Not only does the census provide a unique breakdown of the number and characteristics of the paediatric workforce, it categorises the types of children’s services provided in general hospitals, community and specialist services, and allows us to compare with provision against service standards, particularly Facing the Future. What happens to new CCT holders in paediatrics after they receive their certificate? The College is undertaking regular monitoring to look at the career outcomes for paediatricians shortly after their recommendation for CCT. The latest survey involved those who obtained their CCT/CESR in 2011 and 2012. The findings showed that: • 96.2% of the 2011 cohort and 81.2% of the 2012 cohort currently work as consultants • One in 10 respondents are currently working overseas • Overall, full time respondents have on average 1.90 SPAs – lower than the overall consultant workforce. • 18.4% have contracts with 1 or less SPA; 57.9% in Scotland. • 72.3% of consultants in the 2011 cohort and 63.1% of the 2012 cohort had substantive contracts; this compares to 94.9% of the whole consultant body. • Respondents wanted to see more management (21.2%) and leadership (10.9%) CPD courses/e-learning. • 46.0% wanted support from the College during transition in leadership and management skills development and 41.0% wanted a College-facilitated network for new consultants. Recommendations based on these findings have been used to formulate the College’s workforce strategy. The survey will be repeated to provide up-to- date information for trainees, to highlight issues around the demand for trained paediatricians and to assist in the development of training and education programmes.

Working Time Regulations Compliance Survey Each winter, the College surveys Clinical directors in all acute and neonatal units in the UK to assess compliance on paper and in practice with European Working Time Regulations, rota gaps due to out of programme reasons and failure to recruit, the extent of consultant working on resident rotas and how concerned Clinical Directors are about their unit’s ability to cope. The findings in 2012/13 showed that: • 12% of tier 2 and 1.4% of tier 1 rotas were noncompliant in practice • Vacancies and gaps account for 15.2% of tier 2 posts and 6.8% on tier 1. Approximately 55% of these vacancies are covered by locums. • 72% of Clinical directors are very or moderately concerned that service will not be able to cope during next 6 months. All those who began paediatric training in 2007 form a cohort which the RCPCH is studying. They were surveyed at the end of the first year of training and after the majority had completed ST3 and ST5. Analysis of the cohort at ST5 is due to be published later in Spring 2014. Some of the main findings from the earlier studies show that: • Nearly 30% of ST1s felt forced to make their specialty choice early. • 71.2% of respondents are happy choosing paediatrics as a career. • 30.5% were not confident of obtaining their chosen post. We also conducted a follow up survey of the leavers from part 2 of the study which found: • Respondents told us work/life balance, rota intensity and structure of the training programme were common reasons for leaving paediatrics. • All respondents would recommend paediatrics to other doctors or medical students.

MMC Cohort Study: Are we training the right workforce?

Child Health National Audit Programme As part of its work programme to drive improvements, the RCPCH currently facilitates three national audits funded by the Healthcare Quality Improvement Partnership (HQIP): • Epilepsy12 • the National Neonatal Audit Programme • the National Paediatric Diabetes Audit These national audits engage relevant paediatricians and teams across the UK to review and measure current practice against national standards. Data is collected, analysed and fed back so that teams can view their results alongside others and identify areas of improvement. The RCPCH has also developed an e-learning clinical audit training resource for trainees and those with an interest in clinical audit.

The British Paediatric Surveillance Unit (BPSU) enables doctors and researchers to find out how many children in the UK and Republic of Ireland are affected by particular rare diseases or conditions each year. The Unit was set up in 1986 as a joint activity of the Royal College of Paediatrics and Child Health (RCPCH), Public Health England (PHE) and the Institute of Child Health (ICH) to support research into rare childhood disorders. The BPSU enables paediatricians to: • participate in the nationwide surveillance of infections and infection related conditions • promote the study of rare childhood disorders • provide a mechanism by which ‘new’ diseases can be detected so that early investigation can take place. RCPCH research ethics guidance update The Guidelines for the ethical conduct of medical research involving children (2000) have now been updated and will soon be available online (currently in press). The revised guidance includes information on research involving pregnant women, vulnerable children and emergency situations. The ethical considerations faced by UK paediatricians involved in research in other countries have also been considered. The guidance was developed for individuals involved in the planning, review and conduct of research in children, and takes into account the understanding of children’s interest, legal requirements and the regulation of research. This guidance along with the paediatric route maps that have been developed by the RCPCH, RCN and MRC can be used as: • a resource when planning and conducting research in paediatric patients • a document to maintain abreast of the current legislation on consent and assent practices and how these can be used in your trust or department • a reference of considerations when designing methodology.

Children and young people’s participation and advocacy Our vision is that children and young people are recognised as key stakeholders influencing health services and standards of healthcare to improve health outcomes for children and young people across the UK. ‘We don’t want a “good enough” health service- we need the best possible health service!’ Our objectives: • ensure RCPCH is recognised as an expert in relation to the safe, ethical and meaningful participation of children and young people in health matters impacting their lives • support paediatricians (and wider health professionals) facilitate the participation of children and young people to advocate for better child health policies, legislation, service design and child health standards across the UK and wider • ensure children and young people influence the training, professional development and assessment of paediatricians in the UK • facilitate opportunities that make sure children and young people, from diverse backgrounds, are able to challenge standards of health services delivered to them. The UK Parent and Carers Group ensures the insights and experiences of parents and carers influence the work of the RCPCH.

The Royal College of Paediatrics and Child Health is working with partners in the UK and overseas to improve the health and wellbeing of all children. We have partnerships with paediatric associations, universities and colleagues overseas in coordination with the RCPCH International Regional Advisers. The RCPCH is keen to support members as they undertake international work. This section describes international work currently being undertaken by the College. Global Links Volunteer Exchange Programme The Global Links programme gives the opportunity for experienced RCPCH members interested in Global Child Health to work overseas for 6-12 months in low-resource settings. RCPCH, in partnership with the national paediatric associations in East and West Africa, identifies hospitals where UK and national staff can work together to address the most pressing child health issues in the communities they serve. • flights, accommodation, insurance, pension contributions, pre-departure training, peer support and a modest overseas living allowance will be provided by the programme • you will work where your clinical skills and professional competencies can be passed on to local hospital staff through training and professional development initiatives • you will join other consultants and retired consultants already working within the Global Links programme • we are now recruiting for placements to begin in 2014.

Invited Reviews RCPCH’s Invited Reviews support healthcare organisations and clinical teams in resolving concerns about paediatric service provision, safety, training, compliance, and proposals for paediatric reconfiguration or service design. We can help managers work with clinical colleagues to design sustainable services that improve outcomes for babies, children and young people with compliant, effective working arrangements for clinicians. Our Invited Reviews fall into four main categories: • Service reviews address concerns around safety, quality or sustainability which require a swift and expert view • Design reviews use detailed analysis of whole-service provision perhaps leading to structural service change • Training reviews consider deployment of training places in the light of service pressures • Individual Performance reviews focus on clinical practice within the context of the service. We welcome enquiries from healthcare organisations, commissioners or service planners and would be happy to discuss informally in confidence whether an Invited review could meet your needs. We’re keen to hear from members with experience to offer the programme as expert reviewers. We offer induction training, support and backup, and the opportunity to develop skills through sharing and learning from colleagues.
