PAST RESEARCH Projects in the Nuffield Laboratory of Ophthalmology and Oxford Eye Hospital between 1973-2003. When I took up my post in 1973, I had three aims: 1. To develop the clinical facilities of the Oxford Eye Hospital. 2. To foster the growth of applied research into major diseases of the eye; and 3. To maintain and develop the basic science research programme of the Nuffield Laboratory of Ophthalmology. Many of these aims were achieved, by the efforts of, and collaborations with, basic scientists and clinicians within the Eye Hospital and in the Department of Ophthalmology. The appointment of specialists in various areas provided services for cataract, squint, glaucoma and vitreoretinal surgery, external eye disease, oculoplastics, and paediatric and medical ophthalmology, including diabetic eye disease. Comprehensive services were developed by consultant colleagues and their impetus resulted in the provision of anterior segment and fundus photography, ultrasound and electrodiagnosis, fluorescein angiography, laser photocoagulation, and advanced microsurgical facilities. , In the period up to 1973, the Nuffield Laboratory of Ophthalmology was recognised internationally for its outstanding expertise in ophthalmic biochemistry, with a special focus on the crystalline lens and the mechanism of cataract formation. Between 1973 and 2003, this interest was expanded to address disorders of the cornea, tears, lens, vitreous, and retina. The laboratory disciplines included biochemistry, biophysics, molecular biology and neurobiology. Clinical research was developed in the areas of diabetes, corneal disorders, dry eye, glaucoma, cataract and drug penetration and action, as well as in psychophysics. A Clinical Trials Unit was set up in collaboration with the NHS Trust, under the initiative of Mr. Paul Rosen and engaged in the running of clinical trials of ocular surface disease, glaucoma and toxicity monitoring. Psychophysical techniques for the assessment of cataract and glaucoma severity were developed to help in the management of these diseases and new techniques for the detection and measurement of dry eye and the assessment of the optic nerve head were also designed in the department. Some of these areas are elaborated below. The teaching programme over this period included a weekly laboratory seminar and post-graduate teaching programme, with an invited speaker. A monthly Consultation Clinic gave Residents in training the opportunity to encounter both rare and common clinical problems at first hand.

Some of my personal research, carried out in collaboration with colleagues, included: The Tears There was a major tear research programme concerned with the chemistry and biology of the tears, with an emphasis on the meibomian oils and ocular mucins. Our studies provided information as to the structure of the ocular mucins and their sources and the chemistry of meibomian oil in man and animals. Basic information about the melting point of the tear lipids and the effects of these lipids on tear evaporation were accumulated and applied to such problems as dry eye disease and obstructive, Meibomian oil gland disfunction (MGD). A technique was developed to quantify the amount of Meibomian oil in the lid margin reservoir (Meibomatry) as a means of diagnosing MGD. A new approach to the management of dry eye was formulated, based on the use of pseudoplastic solutions that ‘shear-thin’ during lid-globe movement and are therefore tolerated at high viscosity. Sodium hyaluronate is one of these agents. Sodium hyaluronate retention after drop instillation was studied by scintigraphy and a new, non-invasive technique (the non-invasive tear break-up test) was developed to measure tear stability in vivo. Another non-invasive technique was developed to measure the volume of tear fluid held in the tear meniscus (tear Meniscometry). This provides an indirect guide to the total volume of tears available at the ocular surface and therefore is of value in the diagnosis of aqueous-deficient dry eye. More recently, a collaboration with the Mathematical Institute in Oxford allowed us to develop a modelling approach to understand variation in tear osmolarity at the surface of the eye. Tear hyperosmolarity is a key source of ocular surface damage in dry eye disease Cornea I maintained a strong research interest in corneal structure and in the clinical features and pathology of inherited and other forms of corneal disease. This was achieved by a close collaboration with Prof. K Meek and Dr. S. Akhtar in the School of Optometry and Visual Science at Cardiff University, in studies of keratoconus and in the corneal dystrophies. In the Nuffield Laboartoy ,studies were also conducted into the transport properties of human and rabbit sclera, of relevance to drug delivery in the eye. Experimental studies of corneal epithelial healing were carried out in a novel model of impaired resurfacing and gave rise to new concepts as to the role of the basal lamina in recurrent corneal erosion. Clinical studies of steroid penetration into the eye and of steroid provoked pressure rises were also performed. Immunocytochemical techniques were used to investigate the relative contribution of corneal and conjunctival cells to shed cells found in the tears and the influence of contact lens wear. A grading system for quantifying damage at the ocular surface was developed in the 1980’s and is now used on an international basis. Human studies of a number of corneal and other external diseases were actively pursued. I sustained a major interest in the documentation and quantification of the features of the normal and diseased cornea. The Lens When I entered the department in 1973 laboratory had an outstanding reputation in the field of lens and cataract biochemistry. This momentum was maintained by members of the laboratory over the duration of my tenure and it became possible to integrate the lab-based research with research conducted in the clinic. In the laboratory, an interest developed into factors capable of modifying the structure of both the lens crystallins, and the lens fibre membranes, including the role of glycation and other agents causing post-translational modification of the lens crystallins. The role of alpha crystallin as a molecular chaperone in the lens, became a particular focus of research. Post-translational modification of the lens crystallins plays a major role in both nuclear and cortical cataract. The enzyme aldose reductase was another key area. This enzyme was considered to play an important role in the polyol pathway of the lens and was implicated in experimental diabetic cataract and other diabetic complications. The department pointed out discrepancies in the kinetics of the enzyme protein and its assumed mechanism of action, which made its role in human cataract uncertain at that time. I carried out some early ultrastructural studies of certain forms of cataract, but my chief interest was in setting up a clinical cataract unit for the study of the normal and cataractous lens. Mr. Nicholas PhelpsBrown, who joined the Nuffield Laboratory of Ophthalmology in the 1970s, made this possible. He brought with him a Scheimpflug camera of his own design, capable of imaging the crystalline lens in cross-section, at high resolution. He later designed a lens retro-illumination camera and a camera to image and quantify the features of the optic nerve head. The two lens cameras allowed us to characterise both nuclear and cortical cataract in a refined was and to address a range of questions about lens biology and cataractogenesis. This led to a series of important papers on the subject. A clinical classification and grading system for cataract was also developed and the photographic and observational systems were incorporated into clinical studies of human cataract progression and prevention. They were also used to characterise cataract morphology in other studies. A method for photomicroscopy of the human lens was also developed which enabled measurements to be made on individual lens fibres, in vivo. Over two decades, the unit described many new features of cataract and offered novel explanations for their formation. Special interest was taken in the mechanism

behind the increased size of the lens in diabetics and its decrease in renal transplant patients, and fresh insights were obtained into the mechanism of post-vitrectomy cataract and inherited cataract, including its timing and morphology. The Retina The retina and its diseases are not my primary area of interest, but I maintained and interest in inherited retinal disease and took an active interest in an area of Prof.Osborne’s research, concerning the targeting of damage to the retinal neurones by light incident on the eye. According to his mitochondrial theory and personal research, he hypothesised that, in conditions of compromised energy production by the retinal ganglion cells, the absorption of light energy (particularly, short wave, blue light) by the mitochondria of the RGC axons, generates ROS which cause RGC damage, neuronal loss and visual field and visual loss. This hypothesis is of particular relevance to optic neuropathies such as LHON, glaucoma and the vascular retinopathies and is sensible of future interrogation in clinical trials. Memberships
