Home Care Packages Mercy Health Home & Community Care offers personalised solutions designed to support you to live independently at home. Enjoy flexibility, choice and control over how your care and services are provided. From assistance with routine household tasks to more complex care needs, we can design a Home Care Package to suit you. Our care advisor will provide you with all the choices available to access care and services within your Home Care Package budget and guidelines. We will fully support you through the planning and coordination of your choices and assist, as required, with the ongoing management and implementation of these services.

Allied health - Health education programs for you and your carer - Assessment and advice about equipment such as wheelchairs, mobility aids, and communication aids - Podiatry, occupational therapy and physiotherapy appointments in your own home or in the community - Appointments with your local medical practitioner Care advisory - We work with you to identify and develop and your goals - We provide support for you to be an active partner in care planning and making choices about your care - Comprehensive information and support you to access appropriate services and supports - Ongoing access to our skilled care advisors, available by telephone and in person to assist you with further information and referrals.
