Healthy Workforce Initiative Request for Proposal Grant Writing Consultant(s) Williamson Health & Wellness Center, Inc. (“WHWC”) is seeking proposals from qualified firms for grant writing consultants with experience with non-profit grant writing. Copies of this Request for Proposal (RFP) and any related documents are available on the Williamson Health & Wellness Center website at www.williamsonhealthwellness.com The deadline for receipt of sealed proposals is December 14th, 2016 at 3:00 p.m. EST. Section 1 Project Summary 1. Request for Proposal Summary WHWC is seeking proposals from qualified firms or individuals for grant writing in accordance with the Scope of Work specified in this RFP 2. Entity submitting RFP The terms “vendor”, “firm”, “consultant”, “company” or “contractor” used in this RFP or any subsequent documents or communications related to this RFP are interchangeable and mean the entity submitting a proposal and seeking to enter a contract for the goods and/or services requested in this RFP. 3. Description of Williamson Health & Wellness Center, Inc. Williamson Health and Wellness Center (WHWC), is a non-profit community health center located in Williamson, WV to serve a predominately low-income, uninsured general community population as well as public housing residents that reside throughout Mingo County. WHWC was founded in 2011 and, in 2014, began to fulfill the need to provide comprehensive primary care services to one of the unhealthiest and most distressed communities in the entire state of West Virginia. WHWC fulfills its mission of building a culture of health through innovative programs and partnerships that link community health improvement with economic development. In 2014, WHWC was recognized as a Robert Wood Johnson Foundation Culture of Health prize winner. This nationwide recognition allowed the organization to tackle two major challenges affecting healthy equity – access to health care and building a local thriving economy. WHWC has been successful at bringing together cross-sector entities including elected officials, public agencies, private businesses, non-profits, health care providers, housing authorities, economic and workforce development, diverse funding agencies, and educational institutions to develop a shared vision for addressing health equity in central Appalachia.

Section 2 Scope of Work The primary responsibility of the contractor will be to write and prepare federal grant proposals for WHWC. The consultant will also be asked to research and write other funding proposals to state agencies and private funders to support the Healthy Workforce Initiative. Specifically, the contractor will: 1. Identify and report potential funding opportunities to the WHWC Grants Committee 2. Prepare proposals and supporting documents for diverse funding opportunities 3. Submit formal proposals and supporting documents to diverse federal agencies, state agencies and to private foundations 4. Develop a framework with the WHWC Grants Committee for grants reporting, grants tracking and help to maintain quality reporting and continuous improvement on grants related and other fundraising efforts. 5. Comply with record keeping guidelines as required by the WHWC Grants Committee and the Standard Operating Procedures for grants preparation and administration The grant writing team will work in partnership with the WHWC Development Committee and WHWC Grants Committee to bring together coherent proposals that meet the mission of the organization. The WHWC Development Committee guided by the Chief Executive Officer will assist with project planning and vision. The WHWC Grants Committee will serve as a reader for proposals and will help secure letters of support from partners and appropriate approvals from the WHWC Board of Directors, Chief Executive Officer and Chief Financial Officer prior to proposal submissions. All grant preparation and administration will be guided by the WHWC Standard Operating Procedures for grant writing and administration.

Section 3 Proposal Requirements and Submittal Proposals must conform to all requirements stated below, and elsewhere in this RFP. Disregarding these requirements may result in disqualification of the proposal. Before submitting a proposal, each firm shall familiarize itself with the entire RFP. All proposal materials must be placed in a sealed package clearly marked “Healthy Workforce Initiative: Grant Writing Services” with the firms name also visible on the outside of the package. It is the responsibility of the firm to make sure that proposals are received by the due date and time stated on page 1 of this RFP. The firm is responsible for delivery of their proposal notwithstanding any claims of error or failure to perform by a mail courier or other package delivery service. No proposals or proposal modifications may be submitted by email or fax. The proposal package should include one (1) full proposal, preferably paper-clipped so that additional copies can be made, with the following information: 1. Contact information for the applicant including email, mailing address and phone numbers. 2. A brief section to showcase of your background and qualifications and those of your grant writing team, if applicable. 3. A summary of related experience preparing, submitting and securing grants for non-profit organizations. 4. A description of cost including your proposed pricing structure for grant writing services. 5. References from other organizations you have worked with in the past including contact information. 6. A short writing sample of up to one page from a proposal submitted in the past.

Section 4 Selection and Contract Award Proposal shall be evaluated based on the requirements set forth in the RFP. Selection of the firm(s) will be at the discretion of WHWC and will be based on the proposal that WHWC deems to be the most responsive and responsible and services the best interest of our organization. Proposals will be reviewed by a selection committee and will be evaluated based on the following criteria Evaluation Criteria (100 points total) 30 points

Qualifications of the consultant(s)

Successful track record of the consultant or firm

30 points

Quality of the writing sample

Award Notice The notice of award will be made to the consultant who best meets the needs of WHWC based on the evaluation criteria. Each vendor who submits a proposal will be notified of the selection results.
