Employee Assistance Program (EAP) and Behavioral Health

Preventing disease, and finding it early if it occurs, is important to living a healthy life. Oracle’s coverage includes eligible preventive care — things like yearly exams, lab tests, mammogram and prostate-specific antigen (PSA) screenings — and is 100% covered when you use network doctors.

Master’s-level specialists are available 24 hours a day, seven days a week to help with work/life issues. The EAP provides confidential support for a wide range of personal issues — from everyday challenges to more serious problems, including:

Each of the Mental Health Condition Centers on the website provide information about the symptoms, diagnosis, tests, treatment, prevention and other resources specific to their condition, such as depression, ADHD, alcohol and drug abuse/dependence.

Congenital Heart Disease Services

If you have a complex or chronic health condition, the goal of the nurses is to help you:

Congenital Heart Disease (CHD) treatment usually involves complex surgical interventions. This program provides information and access to the CHD Centers of Excellence network, including recommendations for treatment options and access to physicians with proven clinical experience in treating CHD. Our specialized nurses can also help you find a network medical center for care that has passed quality treatment standards.

}} Stay healthy }} Improve your health }} Live the healthiest life possible A registered nurse dedicated to you (you will work with the same nurse as long as you need) will answer questions about medications, find physicians and schedule appointments. They will provide you with one-on-one guidance and support, direct you to resources and information, inform you of programs or services that are available, and answer your questions and concerns.

Cancer Support Program If you or a loved one is diagnosed with cancer, you may be dealing with lots of questions and difficult decisions. This program covers all types of cancer and gives you and your family a single source for personal support through an experienced cancer nurse. Dedicated nurses can provide information, answer your questions, guide you to a provider in your local community or within the Centers of Excellence network, and also help you deal with the emotional side of cancer.

The nurses are supported by a team of physicians and have access to the latest medical tools and resources available, so you know that you are getting information you can trust. This service is not meant to take the place of your doctor’s care. In fact, it’s meant to support your doctor’s instructions. A nurse may reach out to you directly to invite you to the program or you can call a nurse directly.

Treatment Decision Support

Transplant Resource Services

With Treatment Decision Support, you have access to nurses who can provide information about conditions like back pain that can be treated in more than one way. The nurses can help you understand your treatment options, and connect you with quality physicians. Treatment Decision Support nurses can also explain what you can expect during your treatment.

If you are faced with a possible organ transplant, you can get help and guidance through the Transplant Resource Program. Our Transplant Centers of Excellence network is the nation’s leading network and includes only those transplant programs that have exceeded our strict criteria for transplant excellence. Our nurse consultants provide the information you need to make informed decisions about your transplant care.

Asthma Disease Management The asthma disease management program is designed to help you decide how to best manage your asthma and ultimately improve your quality of life. You will work with a specially trained nurse who will answer your questions 4

Family planning assistance Maternity Support Program

Our network physicians can determine during your pregnancy if you are likely to need NICU treatment. Neonatal Resource Services are fully integrated with the Maternity Support Program.

Pregnancy is an exciting time for mothers to-be. Whether this is your first or third child, it is important you receive the care you need during your pregnancy. With the Maternity Support Program, you will be offered support by an experienced nurse who will provide assistance, guidance and answers to all of your health concerns.

Reproductive Resource Services Reproductive Resource Services (RRS) combines support from specially trained infertility nurses with access to the infertility Centers of Excellence network. Through this program, the infertility nurses can: }} Provide straightforward information on issues surrounding infertility and its treatment, including information on infertility clinics and doctors }} Educate you about the causes of infertility, treatment alternatives, and appropriateness and efficacy of treatment types }} Help you navigate the health care system }} Provide access to the infertility Centers of Excellence network, where each facility meets or exceeds best practice criteria. This means that your doctors are not just experienced, they are experts in their field.

This program gives you personal support before, during and after your pregnancy, including: }} Access to a dedicated maternity nurse }} Support for any special health care needs }} Customized education materials If your pregnancy is considered high-risk, the nurse can help by providing you with specialized support services created specifically for high-risk pregnancies. Our nurses are experienced in high-risk pregnancies and premature births and are available to you throughout your pregnancy to provide answers, support and guidance. To get the most from the program, it’s best to enroll during the first 12 weeks of your pregnancy. However, you can enroll anytime before the end of your 33rd week.

Infertility benefits are not covered through your UnitedHealthcare medical plan unless you participate in RRS.

Neonatal Resource Services The Neonatal Resource Services provide access to: }} A consultation from a neonatal nurse to help you make informed decisions about your delivery options and care }} Some of the nation’s leading Neonatal Intensive Care Units (NICUs) if your pregnancy is deemed “high risk” }} Continued consulting and monitoring during your baby’s NICU stay to answer questions about your health and the health of your baby
