Haverhill Health Centre provides a range of clinics and medical services in the town, which are detailed below. A major refurbishment of Haverhill Health Centre was completed in March 2013. Lots of improvements were made to the centre to benefit patients, including: This directory has information about health services and some of the other related services which are on offer in Haverhill. ALCOHOL CLINIC Fridays (weekly) All day Provider: Norfolk and Suffolk NHS Foundation Trust GP referral

At this clinic, men aged 65 years and over are screened for abnormal aortic aneurysm (AAA). The ultrasound scan, which takes around 10 minutes, is given as part of an NHS screening programme which aims to reduce deaths from AAA by up to 50%. Men will be invited for screening during the year they turn 65. This service helps people with alcohol problems to begin to address their issues. Specialist healthcare staff will assess the problem, provide short term treatment and help the individual to access other support services. The team works closely with GPs and other health providers.

ANTENATAL CLINIC Daily All day Provider: West Suffolk Community Midwives Self referral

The West Suffolk Hospital and Addenbrooke’s Hospital community midwives, based at the Haverhill Health Centre, are responsible for arranging antenatal care for mumsto-be after they have been referred by their doctor. This clinic offers heart patients specialist diagnostic tests. Patients who have been referred for a test by their GP can access the echocardiogram (cardiac ultrasound) clinic without needing to visit the hospital.

A clinic for people who are suffering from leg ulcers providing treatment and advice.

At this clinic, patients with lymphoedema, which is an abnormal build up of fluid that causes swelling, most often in the arms or legs, will have their condition assessed and an individual treatment plan will be drawn up.

A clinic to provide assistance to those who have suffered from or are susceptible to falls.

This clinic gives people the chance to have their NHS hearing aids re-tubed and maintained, collect new hearing aid batteries and discuss all hearing and hearing aid issues.

This clinic gives people support, advice and treatment within the community.

The heart failure clinic works with patients to improve their quality of life by helping them manage the symptoms of heart failure. The clinic helps educate patients about their condition and the improvements they could gain through diet, exercise and medication.

This clinic offers a wide range of expert specialist advice regarding multiple sclerosis (MS) management, along with support and advice to meet each patient’s individual needs.

NEWBORN HEARING TESTS First and third Wednesdays 9am-3pm Provider: West Suffolk Hospital Via midwife

This clinic uses two quick and simple tests to check the hearing of newborn babies within the first two weeks of their lives. The tests, called the otoacoustic emissions test (OAE) and the automated auditory brainstem response test (AABR) are both painless for the baby and will show whether there is a need for further investigation.

ORTHOPTIC CLINIC Second Thursday 9am-1pm Provider: West Suffolk Hospital GP referral

This clinic sees both children with a variety of eye problems, including eye alignment and eye movement (strabismus), lazy eye (amblyopia), double vision (diplopia) and how the eyes work together (binocular vision). Patients are referred to the clinic by their GP, health visitor or school nurse.

Children aged up to 18 years can be referred to this clinic for rehabilitation from a team of specialist paediatric physiotherapists who are experienced in assessing and treating children to help them fulfil their potential.

Pharmacists are experts on medicines and are able to offer advice about minor ailments such as coughs, colds, cold sores and general aches and pains. There are many medicines and treatments that you can buy over the counter from a pharmacy or some shops and supermarkets. A range of other services are also available at pharmacies, such as; ‘stop smoking’ advice and treatment, chlamydia testing and treatment, flu vaccinations, the ‘morning after pill’ if you have had unprotected sex and asthma checks. MEET THE TEAM General practice is developing and changing all the time and staff are regularly updating and acquiring new skills. Here you will find more information on who’s who and when they can treat you. Receptionists - are trained to help you choose the right appointment with the right health expert. Sharing the reason why you need to come to the surgery will help them find the right appointment for you. Phlebotomists - run the blood clinics, which usually take place each morning before the blood is collected by courier and taken to the hospital lab. Please bring any forms with you and remember to fast if necessary. Health care assistants - support the practice nurses by carrying out blood pressure checks, NHS health checks, ECGs, hearing tests, basic dressings and taking new patient details. They can also take blood and test urine. Practice nurses - provide a range of services such as complex dressings, annual health checks, ear syringing, vaccinations (including baby vaccinations) and travel advice. They can also advise on contraception, stopping smoking and managing long term chronic conditions.

Nurse practitioners - run minor illness clinics and are often a good alternative to seeing the doctor. Nurse practitioners can prescribe medication and can therefore see patients with a variety of minor illnesses including ear ache, stomach pain, urinary infections, chest infections, sore throats and sexual health problems. They can also provide emergency contraception.

Minor injuries - surgeries provide a full range of minor injury services during office hours. This includes: injections, cauterisation, aspiration, removal of foreign bodies, joint or muscle sprains, cuts and bruises, minor head injuries and eye injuries. In Suffolk there are local standards in place to make sure everyone can access good dental care. OPTICIANS

Patients qualify for a free NHSfunded sight test if they: • Are aged under 16; • Are 16, 17 or 18 and are in full-time education; • Are aged 60 or over; • Are registered as partially sighted (sight impaired) or blind (severely sight impaired); • Have been diagnosed with diabetes or glaucoma; • Are 40 or over, and their mother, father, brother, sister, son or daughter has been diagnosed with glaucoma; • Have been advised by an ophthalmologist (eye doctor) that they are at risk of glaucoma; • Are eligible for an NHS complex lens voucher - the optometrist (optician) can advise the patient about their entitlement. Patients are also entitled to a free NHS sight test if they receive a range of benefits, including income support, income-based Jobseeker’s Allowance or pension credit or, are named on a valid NHS tax credit exemption certificate or a HC2 or HC3 certificate.

CHILDREN’S CENTRES

DOMICILIARY SIGHT TEST Some people are entitled to an NHSfunded mobile sight test (also known as a domiciliary sight test) in their own home, at a care home with or without nursing or at a day centre. If a patient qualifies for a free NHS sight test, they may be entitled to mobile services if are unable to leave home unaccompanied because of physical or mental illness or disability or, has difficulty communicating their health needs without help. JOLLY GIRAFFE All children can get free NHS sight tests before starting school, which can help diagnose conditions such as squints and reduced vision. Children can continue to get free tests until the age of 16 years. Look out for a Jolly Giraffe poster in your optometrist’s window, pop in and get an appointment for your child.

WELL BABY DROP-IN SERVICE Suffolk County Council’s health professionals run this service, including a weigh and play, from Cartwheels Children’s Centre on Wednesdays from 2pm-3.30pm. They also provide advice about your baby’s wellbeing.

WHEN YOU ARE LOOKING AFTER SOMEONE, WHO IS LOOKING AFTER YOU? Suffolk Family Carers is a charitable organisation that offers free information and support to family carers of all ages. GP link worker, Emily Meadows, works closely with all three GP surgeries in Haverhill to ensure family carers are accessing the support services available to them. A monthly family carer group meets at Haverhill Arts Centre on the last Thursday of each month from 1pm to 3pm. LiveWell Suffolk helps anyone who wants to kick unhealthy habits and start leading a healthier life. They give friendly, practical advice, encouragement and support that will bring about lasting changes to people’s future health and happiness. QUIT SMOKING Trained advisors will support people through the hardest part of quitting with tips, advice and nicotine products or other medication on prescription. They also run a specialist clinic for pregnant women. You can access the free service through your GP or by telephoning 01473 22 92 92. ADULT WEIGHT LOSS Called Fit-Fans, this 12-week programme promotes healthy eating and an increase in physical activity to help people lose weight. Advisors are fully trained and can give you lots of ideas and inspiration to help you find the methods which suit you. Live Well Suffolk’s specialist child advisors have lots of experience in working with children and can give you lots of tips and practical help to break bad habits.

Stepping out in Suffolk is a programme of free health walks which are open to everyone, but ideal for those recovering from illness or who need to improve their exercise levels.

A variety of free programmes are available, including one-to-ones and group sessions, such as ‘Alive ‘n’ Kicking’. This programme is aimed at anyone aged two to 18 years and includes nutrition advice for all the family and fun sociable activities for the children. HAVERHILL LEISURE CENTRE (run by Abbeycroft Leisure)

Community health coaches help people develop personal health plans and provide follow up support on practical lifestyle improvements, which will have a positive impact on health.

‘Get healthy, get into sport’ is a scheme to help people become more active and healthier. It provides practical and financial support to those that need it most. The scheme is accessible through Live Well Suffolk’s community health coach programmes, smoking cessation support and weight management courses. The support on offer is varied and depends entirely on the individual’s needs. Possible methods could include funding initial sessions at a sports club, subsidising transport or childcare, or even a representative from Live Well Suffolk attending the first session with the client.

Age UK Suffolk is currently working to develop befriending schemes in St Edmundsbury. This is a free service which sees people older people who feel lonely to be matched with a volunteer who will visit them or phone weekly so that a friendship can build up. You can refer yourself, a relative or friend can refer or a health professional can too.

Sue Ryder can help by providing a listening ear 24 hours a day. During office hours you can telephone for further practical support and information about dementia, along with details of other services in your local area that can help. The Dementia Helpline is open to anyone living in Suffolk. All calls are confidential. The Dementia Helpline is run by Sue Ryder, working in partnership with Suffolk County Council, Suffolk Family Carers and the Alzheimer’s Society.

ST NICHOLAS HOSPICE CARE AT THE BURTON CENTRE St Nicholas Hospice Care opened its first outreach centre at the Burton Centre on Camps Road in May 2014. People and families facing lifeshortening illnesses such as cancer, motor neurone disease, Parkinson’s and end-stage heart failure, can access a range of outpatient services at the centre. Services on offer include counselling, bereavement support, information and advice, as well as complementary therapies and a pastoral network. Appointments for doctor and nurse-led clinics and physiotherapy sessions are available for hospice patients. The centre offers a warm welcome in a relaxed and informal setting, with a drop in service for anyone looking for advice and information about living with a life-shortening illness. Opening times are Monday to Friday from 9am-5pm.

TRAVEL VOUCHERS You can apply for travel vouchers if you are eligible for a bus pass and live in a remote area, or cannot benefit from free bus travel because of a disability. Travel vouchers can be used to help pay for most community transport, taxis or private hire vehicles. The value of these vouchers is £100 per year and you can apply by telephoning 0845 600 0659. Please note, you cannot hold both travel vouchers and a free travel bus pass. You would need to choose between your bus pass or vouchers. Patients on pension guarantee credit can claim necessary travel costs to receive NHS treatment whilst under the care of a consultant or, in some cases, following a referral for tests by a GP or dentist.

The non-emergency patient transport service is a free service available to some people. Telephone 0845 850 0774 to see if you qualify. The service is available from 8am to 6.30pm on weekdays and 8am to 12noon on Saturdays. If patients experience any problems they can get assistance and advice by contacting the Patient Advice and Liaison Service (PALS) on 0800 389 6819.

If patients are not on pension guarantee credit but their income is low and their savings are less than £16,000, they can apply for help with travel costs to hospital under the NHS low income scheme. Telephone 0845 850 1166 for an application form.

HAVERHILL TRANSPORT

COMMUNITY TRANSPORT Three Counties Transport (3CT) operates a community transport service in Haverhill and the surrounding villages in Suffolk, south Cambridgeshire and Essex. 3CT has five wheelchair accessible minibuses, as well as a wheelchair accessible car service and a community car service. These services are available to people who either do not have their own transport, or cannot access public transport. 3CT can transport people to medical appointments. 3CT also operates a dial-a-ride service to transport people for appointments or shopping. Suffolk Links is a Demand Responsive Transport service which provides connections to bus and train links in rural areas. It serves an area that covers the villages between Haverhill, Bury St Edmunds and Newmarket.
