Methods of Applying for Services
This program covers acute illnesses and assistance for the health needs to prevent disability for adults 21-64 years. The goal of the program is to reduce costly hospitalization and increase the ability to work. MISP is NOT insurance. It is for adults who suffer traumas or have other emergent needs. Acute illnesses Medical care to prevent disability Prescriptions that are on the Medi-Cal formulary are covered. Dental is available through authorized providers listed in the MISP brochure. 14375 Nason Street, Suite 102 Moreno Valley, CA 92555 1-951-486-5375 Riverside County resident for at least 30 days, ages 21-64, must not have access to health insurance or health plan and not a resident of a skilled nursing home or institution for the mentally disabled. Proof of Riverside County residency for at least 30 days Complete an MISP application Adult between the ages 21-64 Must not be insured or have access to a health plan Must be able to show proof of income Must show proof of identity Must not have a valid Visitor’s visa Must not be in Riverside County for the sole purpose of rehabilitation Must not be institutionalized in a Skilled Nursing Home (SNF) or Institution for the Mentally Disabled (IMD) Call 1-951-486-5375 Persons who meet the eligibility may be granted up to twelve months of eligibility. They must report changes in income, living circumstances or if they apply or become eligible for other health benefits. Failure to do so can result in revocation of eligibility

Coachella Valley Volunteers in Medicine Program Description Services Provided
Provides medical and dental services to low-income uninsured patients in the East Coachella Valley. Preventive and acute care Chronic disease management Minor procedures Laboratory, imaging and other diagnostic testing Gynecologic services / Well women’s preventative care Prescription assistance Basic dentistry including fillings and extractions. No crowns, root canals or dentures Educational programs for disease management.

Must be a Riverside County resident. Fees are on a sliding scale based on proof of income. Call 1-800-720-9553 and set an appointment. There is a $50 charge for the first appointment. Proof of income and proof of Riverside County residency They also accept: Medi-Cal, PPO insurance, MISP, Riverside County HealthCare (RCHC) and Inland Empire Health Plan (IEHP). No waitlist to participate, but you must make an appointment by calling 1-800-7209553 Banning, Blythe, Corona, Hemet, Indio, Jurupa, Lake Elsinore, Mecca, Palm Springs, Perris, Riverside and Rubidoux Monday – Friday 8:00 AM – 5:00 PM.
