Certified Nursing Assistant Interactive Skill Review Patryce Smith PROJECT INFORMATION Project Description The academic director of a school that offers certified nursing assistant training has requested the development of self-paced instructional material, to supplement the training of the students.

Client Academic Director for Certified Nursing Assistant Training Program

Problem/Performance Gap The CNA exam has a practical component that requires students to demonstrate a selection of the 22 skills taught during the course of their CNA training. Though students receive hands-on training for each skill, and their understanding is assessed during and after the demonstration, some students still do not pass the practical section of the exam. There are a number of contributing factors, a couple of which are outlined below: 



Skills are typically demonstrated in group sessions. Therefore some students may leave the session with unanswered questions, have restricted viewpoints or are unable to follow along at the pace required by the majority of the students. There is a time lag between the demonstration and the practical exam. It is not uncommon for students to forget steps of a skill between the demonstration and the actual exam. Students therefore attempt to view

EME6930 Class Project Patryce Smith Fall 2009 demonstrations again, look for online videos to review the procedure and/or turn to their notes or textbooks, in order to recall the steps required. These solutions are not always practical nor do they meet the needs of the students. An explanation of the shortfalls of these typical solutions are as follows:  Attend repeat demonstrations. To prepare and attend another live demonstration is time consuming both on the part of the instructors and the students. At times, it will be impractical for students to have access to live demonstrations as often as they would like and at the times they request. Again, live demonstrations still have drawbacks addressed by factor 1.  View online videos. A number of videos of the skills have been produced and shared online. However these videos are typically not instructional and don’t take advantage of the medium to enhance the learning experience for its viewers. Some of the disadvantages from live demonstrations carry over to these online videos, e.g. only one viewpoint is shown. An additional disadvantage of these online videos is that there is no interaction. Therefore unlike in the live demonstration where a student may be able to test his or her understanding by asking a question or listening to the answers of other questions posed during the demonstration, there is no opportunity for feedback.  Use of notes and textbooks. Reading text that describes a technique will not provide the visual cues that students may need to reproduce those steps. Also, unless clearly worded, the meaning of the directions can be misinterpreted. This will result in the incorrect performance of the skill.

FRONT END ANALYSIS The following section provides information from a Needs Analysis. This includes information about the target audience and the tasks they are expected to perform. Target Audience The following provides a summary of the target audience (learners): o o o o o

Learners are predominantly females Learners are between the ages of 18 and 40 Level of education: At a minimum, learners have received a high school diploma or have passed their GEDs. Reading level: Grade 12. Learning styles: A mix of visual, kinesthetic and auditory learners. However, given the field of choice, there is a strong leaning towards kinesthetic. Computer experience: The majority of the learners have had experience with computers, typically with communication programs such as email and internet browsing software. Learners with disabilities, visual impairments or other physical impairments are not expected given the CNA job requirements. Learners also tend to be social learners

Prerequisite Skills And Knowledge The minimal requirements and skill required are listed below: o o

12th grade level reading ability How to correctly greet a patient

The tutorial does not require the students to attend a demonstration prior to taking the tutorial; it is not intended to take the place of practical and handson training.

Application Description The application will provide an overview of the list of skills required for the practical portion of the CNA certification exam. The students will be presented with the steps required to perform the hand washing technique. They will view a video demonstration and will have the ability to interact with the content. EME6930 Class Project Patryce Smith Fall 2009 of the tutorial, the students will be able to take a knowledge check providing them with the opportunity to evaluate their understanding of the information presented.

Task Analysis Hand-Washing Skill 1. You must wash your hands for 10-15 seconds. 2. Use a paper towel on the faucet to turn on the water 3. Wet both hands first up to the wrist, using a scooping like motion 4. Let the water drips from your fingers before attempting to get the soap 5. Keeping your hands over the sink at all time 6. Areas to be wash: Palm of the hands, nail bed, around your cuticle area, up the wrist, back of the hands, back of the wrist, and between the fingers. 7. Turn off the water using a clean paper towel; be careful not to touch the faucet 8. Begin drying your hands from the fingertips to the wrist with clean paper towel 9. Then put on your gloves 10.End of skill Measuring Pulse rate 1. Greet the resident and explain what you here to do: wash your hands and put on gloves 2. Locate the Radial pulse site and count every beat felt along your fingertips for 1 full minute 3. Keeping your thumbs off the wrist (because the thumb has its own pulse) 4. Take off gloves and wash your hands 5. Record the pulse rate 6. End of skill

GOALS AND OBJECTIVES  Performance and learning goals  Learning Objectives Performance/Learning goals This learning solution seeks to: o improve the pass rate of students taking the practical evaluation of the CNA exam. o improve the students confidence in their ability to recall the steps required to complete CNA skills. o provide students with the opportunity to view and interact with the information at their own pace. On completion of the course learners will be able o to complete the hand washing skill o to complete the measuring pulse rate skill o to identify whether the hand washing or measuring pulse rate skills are being performed incorrectly

Learning Objectives A full suite of courses would be required to cover all 22 skills required by the Nursing Board. In this project, the focus will be limited to a course introduction and one (or two) tutorial(s): Hand Washing Skills and (Measuring Pulse Rate)

MODULE 1: HAND WASHING SKILL On completion of this module the students will be able to:  Describe hand washing as a universal precaution to prevent the spread of disease  List and order the steps required to perform the correct hand washing technique  Identify the material required to successfully complete the skill

INSTRUCTIONAL STRATEGIES The following sections summarize the instructional strategies that will be applied during this project. The instructional strategies will  define how the content will be grouped and sequence,  describe the instructional methods or learning activities that will be used, and  describe how assessments will be used to determine if objectives were met.

Sequencing   

Whole – Step by Step. Information will be presented first in whole and then in a step-by-step sequential manner Graphical organizers. Students will receive an overview/graphical organizer of course structure Learning cycles. Utilize Gagne’s nine steps of instruction for each learner.

Instructional Methods and Learning Activities 

 



Demonstrations. Content will be presented using video demonstrations/animations/ text and images. To ensure that the tutorial design takes individual differences into account, video demonstrations will include audio and text as appropriate. Guided Practice. Learners will review material using guided practice and knowledge checks for self assessment Collaboration and Student Reflection. To allow students to collaborate on the topic. Students will be provided with a link to a discussion board set up to discuss each skill. This will encourage further discussion about the topic. It will also allow the instructional designer to obtain feedback from the students on the course, as they discuss course quality, misconceptions. (The discussion board will not be developed as part of this solution) Case Studies. Case studies will be used to motivate the students about the importance of performing the skill correctly.

Then they will select a value, demonstrate what happens to person when the correct pulse is selected. High pulse- XYZ diagnosis – ABC treatment – person saved. If incorrect pulse measured High pulse XYZ no diagnosis patient faints. o Example: Hand washing Infection traced back to improper hand washing. Glossary of terms will be provided

Assessment and Feedback  





Knowledge Checks and Final Quiz. Knowledge checks and a final quiz will be used throughout and within each module. Feedback. Descriptive feedback will be provided for each activity. Feedback to acknowledge correct and incorrect answers will be provided. If an incorrect answer is provided, the student will receive the correct answer on request ( knowledge checks) or immediately (Final quiz). Interactive. The following types of assessments will be used. o Drag and drop – sequencing of steps. o Drag and drop - Matching. o Multiple choice – Matching o Multiple choice Student reflection o Open ended question to allow student to encourage student reflection.
