Getting Your I.D. what counts as identification, what it’s for, and how to get it As you start out on your own you are going to need specific forms of identification in order to do certain things like getting a job, opening a bank account, renting an apartment– and the list goes on. Here’s some info on how to get these important pieces of I.D. Birth Certificate Health Card

• Social Insurance Number (SIN) • Driver’s License

• Passport • Status Card

BIRTH CERTIFICATE Your birth certificate is what proves that you were born in Ontario (or elsewhere) and you’ll need it to get other forms of I.D. • Apply online at www.serviceontario.ca The fee is $35.00 for a replacement birth certificate. If you apply online, you will need a credit card to pay. OR • Print off an application from the website (search “Central Forms Repository”) and send it in to: Office of the Registrar General P.O. Box 4600 189 Red River Road Thunder Bay, ON, P7B 6L8 The fee is still $35.00 but you can pay by cheque or money order made payable to the Minister of Finance. SOCIAL INSURANCE NUMBER A Social Insurance Number (SIN) is needed for getting a job and filing for income tax. You’ll need a birth certificate to apply. Your first card is free. The fee to replace a lost card is $10.00. •Apply in person at your local Service Canada Centre and don’t forget to bring your birth certificate! (check www.servicecanada.gc.ca for centre locations) OR • Print off an application at www.servicecanada.gc.ca and send it to: Service Canada Social Insurance Registration Office P.O. Box 7000 Bathurst, NB, E2A 4T1 You will need to include your completed application, payment and your birth certificate.

Getting Your I.D. PASSPORT A Canadian passport is a good document to have, not only so that you can travel outside of the country but to make it easier to get other pieces of I.D.. Print off an application from the Passport Canada website www.ppt.gc.ca *** If you are under the age of 16, you will need to fill out an application for a child’s passport. You may need to have other forms completed by your social worker or your foster parents. The fee for this type of passport is $37.00. ***If you are over the age of 16, you can complete an application for an adult. The fee for this type of passport is $87.00 Get a passport picture taken (can be done at most photo stores) Have someone be your guarantor (ie. someone who can guarantee your identity) Submit your application either by mail to Passport Canada Foreign Affairs and International Trade Canada Gatineau, QC K1A OG3 Be sure to include your completed application, 2 copies of your photo signed by a guarantor, your birth certificate and another document to prove your identity if you are over the age of 16, and your fee payment.

Submit your application in person to your local Passport Canada office (see website for office locations) Don’t forget to bring your birth certificate and other documents to support your identity, plus your passport picture and fee payment.

HEALTH CARD Your health card (also called an OHIP card) is what you need to show whenever you go to the doctor’s or the hospital as proof that you are covered under the province’s health care system. You will need to renew your card every few years. Apply in person at your local Health Card Services OHIP office. (Check www.serviceontario.ca for office locations) You will need to bring a completed application which can be found at www.serviceontario.ca (search “Central Forms Repository”) plus 3 different pieces of documentation: 1. Proof of citizenship (ex. birth certificate or passport etc.) 2. Proof of residency in Ontario (something with your name and your current address– ex. a pay stub, report card etc.) 3. Support of identity (something that has both your name and your signature– ex. a credit card, passport, driver’s licence, student card etc.)

Getting Your I.D. DRIVER’S LICENSE When you turn 16 you can get your driver’s license which allows you to drive in Ontario. Anytime after your 16th birthday, you can go to your local Drive Test office to take your knowledge test. (www.drivetest.ca) You will need to bring: * One or more pieces of I.D. that includes your legal name, your date of birth and your signature * payment for the license fee ($125.00 for the knowledge test, the G2 road test, and your 5 year license) Once you pass your knowledge test and the vision test you will receive your G1 license. Be sure to ask questions about what you are and aren’t allowed to do with this license. Talk to your social worker about how to access driving lessons to prepare you for your G2 road test which you can take 8 months to a year after you passed your knowledge test. To prepare for your knowledge test, it might be a good idea to buy or borrow a copy of The Driver’s Handbook. This book has all of the rules you need to know to drive in Ontario. It can be bought at Drive Test offices for $16.00. Or you can access it online at: www.mto.gov.on.ca/english/dandv/driver/handbook

STATUS CARD A Certificate of Indian Status or a Status Card indicates that you are registered as a Status Indian under the Indian Act. In order to be eligible for Indian Status, one of your biological parents has to be a fully registered Indian. Print off an application from www.aboriginalaffairs.gov.on.ca/ english/services/status.asp Bring your completed application plus 2 pieces of identification (one must have a photo) to an Indian and Northern Affairs office (check the website) OR Mail your application and photocopies of your identification to Indian and Northern Affairs Canada 8th Floor, 25 St. Clair Avenue East Toronto, ON, M4T 1M2

If you don’t know whether you are “status” or not, ask your worker to help you find out.

Getting Your I.D. WHAT IF YOU LOSE YOUR I.D.? So you lost your wallet—now what do you do? Well depending on what kind of I.D. you had in there you are going to have to make a few calls to get your I.D. back and to make sure that no one steals your identity. (Cause let’s face it– ONE of you is enough in this world!). Contact your bank or financial Institution. Report your lost debit or credit cards and your bank will cancel those cards or freeze your account so your money won’t get stolen. Contact your local police and report your wallet lost/stolen just in case it was turned in. Don’t call 911– look in the phonebook for the number for the police department. Replace your Driver’s License. Sometimes you can be given a replacement license on the spot . You’ll need to contact your local Service Ontario Driver and Vehicle Licensing Issuing Office (usually the place where you got your license in the first place). You’ll need to bring proof of your identity (something that has your signature, official name and your birth date). There is a $10 fee. Replace your Birth Certificate. You will need to apply online for a replacement certificate. See the Service Ontario website for more information on how to do this. www.serviceontario.ca Replace your Health Card. To do this, call the Ministry of Health INFOline at 1-800-664-8988 during business hours or call the ministry health card Ontario inquiry system at 1-800-6648988. Replace your Passport. You’ll need to contact Passport Canada at 1-800-567-6868 to report your lost/stolen passport. You will probably have to fill out an application for a new passport and there might be an investigation which might delay the process– be prepared to be patient. Replace your SIN card. You’ll need to contact Equifax to report your SIN lost/stolen and they’ll direct you on how to get a new one. You can contact them at, 1-800-465-7166.
