NHS North Norfolk CCG’s Response Dear Sir or Madam,

I am writing to you on behalf of NHS North Norfolk, NHS South Norfolk, NHS West Norfolk & NHS Norwich Clinical Commissioning Groups with a response to the information you have requested under the Freedom of Information Act 2000. I have been instructed by the CCGs that they wish to respond as follows:

FORMULARY

1. Which Formulary do you use?

All CCGs commission NHS Anglia Commissioning Support Unit to provide formulary support and have developed various BNF chapter formularies.

2. Which group or committee is responsible for developing this formulary?

The Prescribing Reference Group is responsible for developing this formulary.

3. Who at your organisation sits on this committee?

GP representatives from NHS North Norfolk, NHS South Norfolk, NHS West Norfolk, NHS Norwich & NHS Great Yarmouth and Waveney CCGs, secondary care representatives, the NHS Anglia Commissioning Support Unit Head of Medicines Management and a prescribing adviser from NHS Anglia Commissioning Support unit all sit on this group.

4. Is this a joint formulary with other organisations? If Yes: which organisations do you share the formulary with?

Yes, with the 5 CCGs mentioned above.

MEDICINES MANAGEMENT

5. Is your Medicines Management a joint Committee/Team? If yes: which organisations do you share the committee/Team with?

For NHS North Norfolk, NHS South Norfolk, NHS West Norfolk, NHS Norwich & NHS Great Yarmouth and Waveney CCGs all medicines management is purchased by NHS Anglia Commissioning Support Unit Medicines Management team.

6. Who are the people that sit on your medicines committee/Team?

Not applicable – please see response to question five.

7. Do you commission any medicines management services from, Private providers? If yes: who are those providers?

No medicines management services are commissioned from private providers.

8. Which Area Prescribing Committee covers your organisation?

The 5 CCGs mentioned above in our responses to questions three and five.

Representatives from:       

Each of the five CCGs; 3 acute trusts in area; Norfolk and Suffolk NHS Foundation Trust; Norfolk Community Health & Care NHS Trust; Local Medicines Committee; Local Pharmaceutical committee; and Healthwatch

DRUG AND THERAPEUTIC COMMITTEES

10. Which Drugs and Therapeutic Committee covers your organisation?

The Drugs & Therapeutics Commissioning Board.

11. Who at your organisation sits on the Drugs and Therapeutic Committee?

Representatives from:    

5 CCGs; Hospital trust; 3 representatives from CSU medicines management; and Healthwatch
