+ How do I become a licensed vendor? 1. Department of Revenue Municipal Services Building 1401 JFK Boulevard Philadelphia, PA 19102  Get your City of Philadelphia Business Tax ID  Get a letter if you are a non-profit  Go to http://www.phila.gov/revenue/tp_registration.html

2. Licenses and Inspections Municipal Services Building 1401 JFK Boulevard Philadelphia, PA 19102  Make sure your planned site is not on a prohibited street  See the prohibited streets list at http://www.phila.gov/li/upddocs/prohibitedstreets.pdf or call 311 to check site with License and Inspections

 Get a Business Privilege License at https://secure.phila.gov/li/licenses  The cost of the license is $300 and there is a $5.95 fee if you buy it online

 Get a Sidewalk Vending License at the Municipal Services Building

1. Fill out a Mobile Vending Application at http://www.phila.gov/health/pdfs/Mobile_Vending.pdf

2. Approval from the health department 3. Schedule inspection 4. Upon passing inspection, you will get an inspection report and License Eligibility Report for retail non-permanent location

5. Take your report to the Municipal Services Building to get your Food License (3112)

B. Whole with Cut Produce 1. Fill out a Mobile Vending Application and commissary application at http://www.phila.gov/health/pdfs/Mobile_Vending.pdf a. Your commissary should be used to clean equipment and utensils, store food and unit, dispose of waste, and repair vendor unit 2. A person needs to be Food Safety Certified and possess a City of Philadelphia Food Safety Certificate a. Find a list of places that offer food safety courses at http://www.phila.gov/health/pdfs/Food_Safety_Brochure_July%202010_2n d.pdf b. Fill out Food Safety Certification Form to get a Food Safety Certificate c. There must be a Food Safety Certified person present when food is handled or prepared 3. Approval from the health department 4. Schedule inspection 5. Upon passing inspection, you will get an inspection report and License Eligibility Report for retail non-permanent location 6. Take your report to the Municipal Services Building to get your Food License (3112)

Whole with Cut Fruits and Vegetables

All preparing and cutting must be done at the commissary: ¬ Keep food safe while you store, prepare and serve it ¬ Wash hands before you work with food ¬ Wear gloves or use utensils when you touch sliced produce ¬ Wash fruits and vegetables in water well before cutting ¬ Pre-package cut produce in single service containers ¬ Cut produce Must be kept at 41° F Check temperature and condition of food often Do not store snacks in ice or water if water can soak through packaging Must be labeled with ingredients and the date and place it was prepared

How do I keep food safe?

What can I do?

¬ Trim and file your nails so the edges are not rough and your nails are easy to clean ¬ No nail polish or false nails unless you wear gloves ¬ No jewelry except for a wedding band ¬ Do not eat, drink or smoke near food ¬ Tell the Certified Food Safety person about any disease or health problem you have

How do I keep food safe? Wash Your Hands

1. Wet your hands with warm, running water 2. Put some soap on your hands 3. Rub your hands together and scrub your bare arms for 20 seconds or more a. Wash under your nails and between your fingers

4. Rinse your hands with warm, running water 5. Turn off the tap with a paper towel and throw it in the trash can 6. Dry your hands with a paper towel and throw it in the trash can
