Dear Prospective Contractor: The enclosed addendum has been issued for consideration in the preparation of your response to Florida Department of Environmental Protection (DEP) Solicitation No. 2011012C entitled, “Invitation to Bid (ITB) for Plumbing Services in Dade County to Replace Contaminated Potable Water Wells” issued on September 13, 2010. All responses to the subject solicitation must be received no later than 2:30 p.m. Eastern Time (ET) on Wednesday, October 6, 2010. A response received after the exact time specified will not be considered. Failure to file a protest within the time prescribed in Section 120.57(3), Florida Statutes, or failure to post the bond or other security required by law within the time allowed for filing a bond shall constitute a waiver of proceedings under Chapter 120, Florida Statutes (F.S.). Written notices, formal protests and proceedings must conform with the requirements set forth in Chapter 28-110, Florida Administrative Code (F.A.C.). Protests must be filed with the Procurement Section, Department of Environmental Protection, Procurement Section, Carr Building, Room 235, 3800 Commonwealth Boulevard, MS#93, Tallahassee, Florida 32399-3000 within the time prescribed in Section 120.57(3) Florida Statutes and Chapter 28-110, Florida Administrative Code. Any person who files an action protesting a decision or intended decision pertaining to contracts administered by the DEP, pursuant to Section 120.57(3) F.S., shall post with the DEP at the time of filing the formal written protest a bond payable to the DEP in an amount equal to one percent (1%) of the estimated contract amount, as determined pursuant to Section 287.042(2)(c) Florida Statutes, which bond shall be conditioned upon the payment of all costs which may be adjudged against him in the administrative hearing in which the action is brought and in any subsequent appellate court proceeding.

ATTACHMENT C-1 REVISED SCOPE OF SERVICES Purpose The purpose of this Scope of Services is to outline the work to be performed so that a competitive bid can be made in regards to the installation of potable water service lines in Dade County. Initial Sites The Water Supply Restoration Program (WSRP) plans to have approximately thirty-four (34) homes (see Exhibit I of this Attachment) connected to Miami Dade Water & Sewer Authority (MDW&SA) public water system. All utility, local and state requirements/permits shall be followed. This work will proceed at the time of execution of this Contract and must be completed within four (4) months of contract execution. Additional Sites In the event that additional contamination is found or additional sites are determined to need to be connected, the Department reserves the right to modify the scope of work to include additional sites. Work on any additional sites, which are not included in Exhibit I, shall not be performed unless and until the Contract is amended to include these sites and to provide funding for these sites. Work at any additional sites authorized by contract amendment shall be completed at the rates in the contract. Scope of Work The Contractor shall provide all applicable permits, labor and materials, yard restoration and all other operation/overhead costs required for the complete installation of service lines from public utility’s water meters (meter set costs not included in this work) to residences approved for connection by the Florida Department of Environmental Protection project manager. All work shall meet or exceed all state and local building and plumbing codes. All connections will be coordinated with the appropriate utility provider and the Departments’ Project Manager. All associated costs required to complete this work shall be included in the attached Fee schedule. Each site shall include the following work: • Locate all underground utilities in the public right of way and on private properties. It will be the responsibility of the Contractor to contact the owner/resident to locate buried on-site water lines, electric lines and septic systems. As a standard Plumbing practice, the Contractor should utilize the local ‘Line Location Services for all Public Utilities’ • Furnish and install a 3/4” Copper tubing, type L (for service line lengths up to 150’), one inch (1”) Copper tubing, type L (for service line lengths greater than 150’ up to 250’) and one and a half inch (1.5”) Copper tubing, type L (for service line lengths greater than 250’) including all required fittings to be installed in the most direct/economical path from utility’s water meter to the specified residence. At sites where the well is near the front of the property the most direct/economical service line path will be to the point on the downstream side of where the existing well’s service line will be disconnected. • Furnish and install a ¾” RPZ back-flow device, if one is required by the utility and not already present at the site in question. • Furnish and install a pressure-reducing valve when required due to excessive pressure. • Two (2) appropriately sized brass ball valves shall be installed at each site: one directly after the backflow valve or meter and the other near the new connection to the residence’s plumbing.

Disconnect the existing water line from the well by removing at least two feet (2’) of the plumbing line that was used to connect the well to the residence. In order to maintain any existing irrigation lines originating from the well the point of ‘well disconnect’ can be made as close to the point of entry into the residence. Repair all cut or damaged electrical, irrigation/water and sewer/septic lines. Labor costs can be charged where, after on-site consultation with the property owner, damage occurred to unknown privately (nondisclosed) owned lines. The Contractor shall be responsible for locating and preventing damage to all public utilities. The Contractor and/or the Contractor’s Line Location Service will be responsible for damage to public utility lines. Restore each resident yard to the “as it was” condition. Upon completion of the work, flush all debris and bleed air from the new service line. Show the owner/resident where the new service line and valves are located. Also, inform the owner/resident that the new service line is their property and that they are responsible for all future maintenance repairs of the line.

When crossing a driveway or sidewalk is required the preferred method of crossing is to jet or push the service line under but alternate methods should be considered. As required one of the following will be needed: • • •

The DEP will review its records to identify any contracts the prospective bidder has undertaken during the previous five (5) years (executed during or after September 2005) with the DEP where the bidder was the primary contractor. EXHIBIT I FLORIDA DEPARTMENT OF ENVIRONMENTAL PROTECTION Procurement Section 3800 Commonwealth Boulevard, MS#93 Tallahassee, Florida 32399-3000 September 22, 2010 Questions and Answers for DEP Solicitation No. 2011012C Invitation to Bid (ITB) for Plumbing Services in Dade County to Replace Contaminated Potable Water Wells The following questions were raised by a prospective contractor through correspondence received on the date shown next to the company name. The answers are herein provided for your consideration in responding to the above referenced solicitation. Gobie Plumbing, Inc. – September 13, 2010 Question #1: Re: The electrical grounding requirements. Dade County Plumbing Inspectors are requiring a ground certification by a Licensed Electrician stating that proper grounding is in place. This requirement is necessary when a PVC service line is used. When a metallic pipe is used this requirement is not necessary. How should we address this issue? Some homes grounding may be OK and some may need to be upgraded. Even the ones that are OK are we to include paying an Electrical Contractor for this service? Answer #1:

Re: Damage to underground services or utilities Under attachment C: Scope of work it states that the Owner/Residence is to locate buried on site waterlines, electric lines and septic systems. In the same section it says that the Contractor is to repair all cut or damaged electrical, irrigation, water and sewer septic lines. How are we to deal with situations where the owner did not or could not located these underground items and they need to be repaired? Obviously, minor sprinkler lines or other minor repairs could be taken care of at no additional charge. But for major repairs for items not revealed to the Contractor please clarify how we are to deal with these conditions

Answer #2:

As a standard Plumbing practice, the Contractor should utilize the local ‘Line Location Services for all Public Utilities’. Using that service protects the Contractor from liability in the event they cut any Public utility line. As far as damage to the owner’s buried electrical, irrigation or drain lines, every effort should be made to communicate with the property owner in order to locate all buried lines along the route of the proposed new service line. Assuming the Contractor contacts the property owner and they tell the Contractor where they think the privately installed objects/lines are and then the Contractor cuts into something, there is a provision for additional labor in the Revised Bidder Response Form. If additional hours are needed, the Department’s Contract Manager would confirm with the Contractor and the property owner what occurred so that the additional work can be authorized. Also, see the attached Revised Scope of Services and the Revised Bidder Response Form.
