This publication describes financial aid and scholarship programs available at the ­University of Utah for resident and non­ resident entering freshmen, two-year college transfer students, and continuing under­graduate students. Inquiries from prospective students, parents, and others are invited. Staff members will be happy to assist you. Please note that scholarships or tuition waivers listed in this bulletin are reserved for undergraduate students who have not yet earned a baccalaureate degree. Privacy Act Notice: The University confidentially maintains Social Security numbers for routine uses, such as facilitating document matching, verifying your identity, and expediting your enrollment and financial aid. Disclosure of your Social Security number is voluntary, but failure to provide your Social Security number may result in delay and confusion regarding your identity. Once admitted, it could result in delay or loss of federal and state financial aid, scholarships, tax credits, student loan deferments, veterans’ benefits, and other benefits under the law. FEDERAL FINANCIAL AID Planning Expenses Individual costs vary according to course of study, type of housing, transportation, resident or nonresident status, room and board, and personal expenses. The Financial Aid and Scholarships office has developed sample budgets to assist you in your financial planning. Allowable expenses are determined from University budgets calculated to permit students to participate fully in normal University life. The budgets are reviewed annually to reflect changes in education-related costs in the Salt Lake City area. Federal student aid consists of grants, loans, and work-study, and is awarded on the basis of need. Some loans not based on need are available. Federal Pell Grants, TEACH, and Federal Stafford Loans are available throughout the award year, depending upon student eligibility. Funds for all other programs are limited, and are generally awarded to students who complete the application process by the on-time completion date. Detailed information on these programs, eligibility requirements, and deadlines are addressed in this section. More information about financial aid is available on the Web at www.financialaid.utah.edu. General information is also available in the Guide to Federal Student Aid, from the U.S. Department of Education (available at the Financial Aid and Scholarships office).

Eligibility A student must meet the following criteria in order to be considered eligible for federal financial aid: • Be admitted to the University of Utah as a degree-seeking student. • Be enrolled for the minimum number of credit hours, depending on the aid program (see program descriptions). • Maintain the standards for Satisfactory Academic Progress (see p. 4) • Have a high school diploma, GED, or equivalent.

FEDERAL FINANCIAL AID • Be a U.S. citizen, permanent resident, or eligible non-citizen. • Be registered with Selective Service, if applicable. • Not owe a repayment of any federal aid to a school previously attended. • Not be in default on a federal student loan.

Applying for Aid In addition to meeting the eligibility criteria listed above, students must complete the Free Application for Federal Student Aid (FAFSA) for each year that aid is desired. Listed below is an option for submitting the FAFSA, as well as instructions on completing the application. • FAFSA on the Web, www.fafsa.ed.gov, is the preferred method for submitting the FAFSA. Applying through the Internet may save weeks in processing time.

Tips in Completing the Application • University of Utah Title IV institutional code: 003675 • University of Utah Financial Aid address: 201 S. 1460 E., Rm. 105, Salt Lake City, UT 84112-9055. • Follow the instructions on the FAFSA application. • Make sure all information is entered correctly on your application, particularly your Social Security number. • Make sure all required signatures are provided on the application. If you file using FAFSA on the Web, you must send in the signature page within 14 days of transmitting your FAFSA. • Do not send tax documents or other materials with your application or signature page. Additional Applications The University of Utah has an online institutional application for financial aid (UFORM) that can be completed online at the following Web site: www.financialaid.utah.edu.

After the Application Is Received The following provides guidance on how processing continues after you applied. It also helps to become familiar with some of the acronyms used in financial aid. • The Student Aid Report (SAR). After your FAFSA is processed, you will receive a Student Aid Report (SAR) by U.S. mail or email. The SAR will list the

information you reported on your FAFSA. If any information on your SAR is incorrect, enter the correct information, provide the necessary signatures, and return it to the central processor. Please contact our office before updating marital status. • Your Personal Identification Number (PIN). To apply for a PIN, please complete the PIN application at PIN.ED.GOV. You may have immediate access to your PIN. However, the PIN will initially have limited use. Please keep a record of your PIN, as it will allow you to make address changes to your FAFSA form, send your information to other schools, or make other changes.. • The Expected Family Contribution (EFC). The EFC is a measure of how much the student and his or her family can be expected to contribute to the cost of the student’s education for the year. Your EFC indicates your eligibility for a Federal Pell Grant and is used in calculating your eligibility for other need-based aid programs. The EFC is also listed on your SAR.

Award Letters and Missing Information Letters The University of Utah will automatically receive an electronic copy of your application information, as long as you listed our institution as one of your choices on your FAFSA. Once you are an admitted student, we will begin determining your eligibility for federal and state aid programs. Once awards are made, you will receive an email through your UMail account with instructions to access the Web, where you can view your conditional award letter. You can complete your award letter online. If you choose to complete a paper award letter, we ask you to print the award letter, complete the necessary information, and mail or fax it to the Financial Aid and Scholarships office. The fax number is 801-585-6350. Award notifications are sent early in the processing cycle, to allow the student to budget for the academic year. However, as processing continues, the award may be subject to change. You will be informed of any award changes which may occur, as well as the reason for those changes. In case we require additional information, we will inform you by UMail.

Verification 2

Certain applicants are requested to provide documents to verify information reported on the FAFSA. Although a comment on your SAR may indicate that you have been selected for verification, only those applicants who receive a request from the U of U need to submit the following documents: • Verification Form (IVF) • IRS Data Retrieval. You and/or your parent(s) (if applicable) will need to log into your FAFSA form @ www.fafsa.gov and complete the IRS Data Retrieval process (if not already completed). If you and/or your parent(s) are not eligible to complete the IRS Data Retrieval, please submit a signed copy of the 2012 federal income tax forms to the Financial Aid and Scholarships Office. If you did not file taxes, please submit a Non-Filer Statement form, which is located at our website. • Additional Documentation. If additional information is needed, a checklist will be placed on your file and an email will be sent to your UMail account. Staff in the Financial Aid and Scholarships office will review the documents you submit. We will notify you of any changes affecting your aid eligibility.

Dates and Deadlines January 1, 2013: Submit your FAFSA or Renewal FAFSA after this date. April 1, 2013 On-time Completion date: In order for you to be considered an on-time filer, the federal processor must receive your FAFSA, including your signature page if you filed electronically, and your file must be complete in our office by this date. Students who meet this deadline are among the first to be considered for some campus-based aid programs and need-based scholarships. The last day of your enrollment period: Deadline by which the U of U must receive the results of your FAFSA. Allow four to six weeks for the U of U to receive the results after you submit your 2013-2014 to the federal processor. June 30, 2014: The deadline for the federal government to receive your FAFSA. For students attending the U of U, the deadline is valid only for students attending summer term. All other students must meet the deadline of the last day of your enrollment period. If you have been selected for verification: The verification process, including the transmittal of any corrections, must be completed within 180 days of your last

date of attendance, or by August 15, 2012, whichever comes first. To allow verification to be completed, students should submit all verification materials at least 30 days prior to the last day of enrollment. If you have been awarded a Stafford Loan: All eligibility requirements must be met, and your loan must be certified no later than the last day of your enrollment period.

FINANCIAL AID CHECKLIST AND CALENDAR I f you are required to file a federal tax return, complete a student, spouse, and/ or parent U.S. Income Tax Form 1040, 1040A, or 1040EZ as soon as possible after January 1, 2012. Most information on the FAFSA or Re­newal FAFSA is based on tax data. C  heck the Student Aid Report (SAR), which you should receive via email approximately 10 days after submitting your FAFSA on the Web or six weeks for paper application. (For information regarding electronic application, please see section on “Applying for Aid.” Review all information on the SAR and follow the instructions. If you have questions regarding your SAR, contact the Financial Aid and Scholarships office at 801-581-6211.

Awarding of Aid • Federal Pell Grant: Students are first considered for Pell Grant eligibility. (See p. 4) • Other grant aid: After Pell Grant eligibility is determined, students are considered for other federal and state grants. These grants are awarded on the basis of need, and are restricted by funding availability. • Work-Study programs and Federal Perkins Loans: After eligibility for grants has been determined, additional awards are made, according to the student’s preference as reported on the FAFSA. These awards are made on the basis of need, and are restricted by funding availability. • Stafford Loan (subsidized and unsubsidized): After eligibility for all other programs has been considered, Stafford eligibility is determined. Students with remaining need are awarded a subsidized loan; all other students are considered for an unsubsidized loan. Funding availability does not restrict these programs. • Annual award limits apply to aid programs. See more detailed information under each program listed in this bulletin, or refer to the Guide to Federal Student Aid.

Disbursing Aid • Most federal financial aid is awarded for fall and spring semesters. A student attending summer term will be awarded only for remaining Pell eligibility, if any. The student must request in advance to receive any remaining Federal Stafford award. Summer applications are generally available in March of the applicable year. • A student must meet all eligibility requirements in order to receive a disbursement of financial aid. • Federal and state aid is credited to a student’s tuition account at the University, toward payment of any existing allowable charges, such as tuition and fees. • If your awards are higher than your charges, you should receive a remainder check in

 omplete an online institutional C application for Financial Aid (UFORM) at www.financialaid.utah.edu. Complete, sign, and submit the Financial Aid Award Letter available to you on the Campus Information System (CIS).







the mail at the address you have given the Registrar, within 14 days of the date financial aid has credited your account at the University. If you choose to have your reimbursement transferred electronically to your bank account, please complete a direct deposit application available at the Income Accounting and Student Loan Services Office, or online through CIS or Income Accounting’s website. • If your awards are insufficient to pay all of your charges, you need to pay the remaining amount by the published tuition deadline. • If a student establishes eligibility after the beginning, but before the end of a term, disbursement of aid will occur when all eligibility requirements are met. • If a student does not establish eligibility until after the end of a term, regulations regarding late disbursements will apply. Students in this situation are advised to contact a financial aid counselor. • For questions regarding refund checks, and direct deposit, please contact the Income Accounting Office at 801-581-7344. • For questions regarding disbursement of federal financial aid, please contact the Financial Aid and Scholarships office at 801-581-6211.

Withdrawals and Refunds Students who consider dropping or withdrawing from classes are urged first to meet with a financial aid counselor to determine how aid eligibility may be affected. Information concerning tuition refunds is contained in the Class Schedule under “Refund Policies,” or available through the Income Accounting Office at 801-581-7344. Withdrawals will affect negatively a student’s satisfactory academic progress calculation. See information below for details. Instructions on withdrawing from the University are obtainable from the Registrar’s Office, 801-581-5808, or at www.sa.utah.edu/regist. Only the Registrar’s Office may officially withdraw a student from the University. Returning Aid to Federal Programs Regulations require a student’s eligibility for aid to be recalculated whenever a student withdraws from the University, either officially or unofficially. The recalculation determines the amount of aid a student has “earned,” by prorating according to the percent of the term completed before withdrawing. For example, a student who withdraws, completing only 30 percent of the term, will have “earned” only thirty (30) percent of original aid eligibility. A student who completes more than sixty (60) percent of the term is considered to have “earned” one hundred percent of his or her aid eligibility. Students who have not earned 100% of their aid may have to repay the unearned portion that was already disbursed.

Satisfactory Academic Progress (SAT) Financial aid is contingent upon continued satisfactory progress toward a degree or certificate. The academic progress of students is monitored at the end of every semester. Definitions of Completed and Attempted Credits • A course is considered attempted, but not completed if a grade of I, E, EU, W/D, NC, T (T grades are only unofficial grades for undergraduate students. T grades are official for graduate students), or blank is assigned to that course. A repeated course is also considered attempted, but not completed. • A course is considered completed if a grade of at least a D- is received, or if a CR grade is assigned.

• Audited courses are not acceptable for financial aid purposes. The SAT standards are listed below, as well as requirements for reinstating aid for students who did not maintain those standards. Minimum Cumulative GPA Requirement • Students must maintain a minimum cumulative grade point average of 2.0. • If aid is lost, it may be reinstated once a student regains a minimum cumulative grade point average of 2.0. Maximum Time Frame to Earn a Degree • Undergraduate students are allowed 183 attempted credit hours. • Second bachelor’s students are allowed 61 attempted credit hours. • Master’s candidates are allowed 4 years from entry into a master’s program. • Ph.D. candidates are allowed 7 years from entry into a program. • Aid may be reinstated by graduating from one degree-seeking program and entering a new one. Aggregate Completion Rate • Students must complete at least 67 percent of all attempted credits at the University of Utah and transfer credits. (The calculation for graduate students excludes any course work taken as an undergraduate.) • Aid may be reinstated once a student has completed a sufficient number of credit hours to raise his or her aggregate completion rate to 67 percent. Students have the right to appeal any decision made by the Financial Aid and Scholarships Office. However, the decision of the appeals committee is final, and no exceptions can be made for requests in violation of the federal and state regulations, which govern financial aid. To submit an appeal, please complete the official appeal request form, which is available online at www.financialaid.utah.edu within the Forms and Applications tab.

Appeals Process Students have the right to appeal any decision made by the Financial Aid and Scholarships Office. However, the decision of the appeals committee is final, and no exceptions can be made for requests in violation of the federal and state regulations, which govern financial aid. To submit an appeal, please complete the official appeal request form, which is available online at www.financialaid.utah.edu within the Forms and Applications tab.

SAT Standard Violations • If you believe that there were exceptional circumstances beyond your control, which caused you to violate one or more of the standards listed, you have the right to submit a written appeal. Such circumstances include the death of a family member, or serious illness or injury to the student or an immediate family member. The appeal must include supporting documentation and be submitted on an official appeal form. Unusual Circumstances Affecting Financial Need • If you or your family have unusual circumstances (such as loss of employment or major medical expenses) that might affect your need for student financial aid, we invite you to discuss your circumstances with a financial aid counselor. The counselor can determine whether your circumstances warrant special consideration, and will provide you with the forms necessary to submit an appeal.

Study Abroad, ISEP, and NSE Programs • Financial aid is available to most qualified students who wish to participate on a University-approved study abroad program. Students on ISEP or NSE programs may also qualify for aid. • Students must meet the same eligibility requirements as do students studying on the University campus. Students are encouraged to visit with a financial aid counselor at least 90 days prior to departure to ensure that all necessary information is collected and on file with the Financial Aid and Scholarships office. • For information on University-approved study abroad programs, contact the Study Abroad Office at 801-581-5849.

Required Information Concerning Drug Abuse

• Standards of conduct that prohibit the unlawful possession, use, or distribution of drugs and alcohol. • Description of the legal sanctions for violation of the above. • Description of the health risks associated with the use of illicit drugs and alcohol. • Description of drug and alcohol counseling, treatment, or rehabilitation programs available to students or employees. • Statements concerning the sanctions imposed on students and employees for violation of the standards of conduct.

Types of Financial Aid GRANTS A grant is money that does not have to be repaid. It is awarded as a means of partially meeting a student’s demonstrated need.

Federal Pell Grant Application—Submit the Free Application for Federal Student Aid (FAFSA) or Renewal FAFSA and indicate on the application that you want your information sent to the University of Utah (step 6 of the application). The Pell Grant is the foundation of all needbased aid. Application for the Pell Grant is required before additional aid can be ­considered.

Teacher Education Assistance for College and Higher Education (TEACH) Federal Supplemental Educational Opportunity Grant (FSEOG) Utah Education Fund (UEF) Higher Education Success Stipend Program (HESSP) Need Based Tuition Assistance Grant (NBTAG) Federal Work-Study (FWS)

For all grants listed above, please refer to the Financial Aid and Scholarship homepage, www.financialaid.utah.edu, and click on Grants & Loans for further explanations.

Bureau of Indian Affairs (BIA) Grant Many tribal organizations offer grants to eligible students. It is best to inquire directly with the tribal organization as to what the eligibility requirements are and to obtain any needed paperwork. Most tribal organizations require that a needs-analysis be done

before funds are awarded. The needs-analysis is completed by the Financial Aid and Scholarships office. Students must complete the FAFSA form in order for a needs-analysis to be completed. It is best to submit the FAFSA form and needs-analysis well in advance of any deadlines, as it may take 2-3 weeks for the form to be processed and completed and is ready for the student to pick-up.

LOANS The University may reject loan applications on the basis of poor credit (credit checks are only associated with Plus loans), or for other reasons as specified in federal regulations. Financial aid administrators may decline to certify loans, or may certify loans for lesser amounts, provided they document the reasons and notify the student in writing.

Federal Perkins Loan Available to—Eligible graduate and undergraduate students with financial need, who are attending half-time (at least six hours per semester for undergraduates and at least five hours for graduates). Application—Have a complete file by the April 1, 2012 On-time Completion deadline, submit the FAFSA, UFORM and any other requested information. Allow for processing time. Provided by—Federal government through participating colleges and universities. Value—Varies, depending on student need and available funds. Repayment—No interest is charged and no repayment is required during in-school deferments. Repayment commences and interest at 5% begins to accrue, nine months (if first-time borrowing began after July 1, 1987) or six months (if first-time borrowing began before that date) after you are no longer officially enrolled at least ­half time. Typically, payments are due monthly. Deferment, forbearance, or cancellation of repayment may be available under special conditions. See section on “Deferment, Forbearance, and Cancellation of Student Loans” below. The maximum repayment period is 10 years. The minimum monthly repayment varies and is described on the promissory note. Special Conditions—An annual orientation and an exit interview are required of Perkins Loan borrowers. For more information about Perkins Loans, contact the Student Loan Servicing Center of the Income Accounting Office on campus, 155 Student Services Building.

Federal Direct Subsidized Stafford Loan Available to— Eligible undergraduate students with financial need. Federal guidelines determine need. Application—Submit the FAFSA or Renewal FAFSA. The application deadline is 120 days prior to the term for which funds are desired. Provided by—Federal Government. Value—A maximum of $3,500 per year for freshmen, $4,500 for sophomores, and up to $5,500 for juniors and seniors and eligible second degree candidates. The cumulative undergraduate maximum is $23,000. Repayment—Arrangements for repayment must be made prior to graduation or termination of at least half-time study (contact the loan servicer for a repayment schedule). Generally, loans become payable six months after that point. See section on “Deferment, Forbearance, and Cancellation of Student Loans” below. Special Conditions—All recipients of the Stafford Loan must complete entrance counseling and an exit interview.

Federal Direct Unsubsidized Stafford Loan Available to—Eligible undergraduate and graduate students. This loan is not a need-based loan, but is available only to those students who have applied for need-based aid. Application—See Federal Direct Subsidized Stafford Loan. Provided by—See Federal Direct Subsidized Stafford Loan. Value—Varies for dependent undergraduate students according to eligibility. Additional amounts may be available for independent undergraduate and graduate students. Repayment—See Federal Direct Subsidized Stafford Loan. Note that this loan also has the grace period connected with the Federal Direct Subsidized Stafford Loan. Special Conditions—See Federal Direct Subsidized Stafford Loan.

Federal Parent PLUS and Graduate (PLUS) Available to—Parents of eligible undergraduate dependent students, or graduate students. Application—Submit the FAFSA or Renewal FAFSA. The application deadline is 120 days prior to the term for which funds are desired. Also, contact the Financial Aid and Scholarships Office to obtain a separate application for the PLUS loan. Provided by—See Federal Subsidized Stafford Loan.

DEFERMENT, FORBEARANCE, AND CANCELLATION OF STUDENT LOANS Under certain conditions, and as long as your loan is not in default, repayment of the loan may be deferred (postponed), or you may be granted forbearance or cancellation of the loan. Each of the loan programs has slightly different rules guiding these exceptions. If you have questions about the terms of your federal loans, repayment obligations, deferment, or cancellation, check with the organization or institution which made the loan or which holds it. Only that organization or institution can make decisions concerning your loan.

Deferment: Federal Direct Stafford Loans: Deferments are not automatic but must be requested in writing from the organization which holds your loan, and documentation to support that request must be provided. Principal and interest will be deferred for subsidized Stafford Loans, but the student is responsible for paying the interest on an unsubsidized Stafford Loan. Federal Perkins Loans: Deferments are not automatic but must be requested in writing from your institution. The promissory note issued at the time of disbursement delineates available conditions of deferment. The rules guiding deferment of Nursing Student Loans and Health Professions Loans are similar to those guiding Stafford Student Loans.

Forbearance: Forbearance may be granted in the form of a temporary cessation of payments, an extension of the repayment period, or the making of smaller payments than previously scheduled. Interest will usually accrue during the period in which forbearance is granted.

Under special circumstances, forbearance may be granted when the borrower does not fully qualify for deferment. Students apply for forbearance in writing to the organization or school holding their loans.

Cancellation: Federal Direct Stafford Loans and Federal Perkins Loans: Under certain conditions (for example, teaching or nursing at a designated institution), cancellation is possible. Students with Federal Stafford Loans should check with the organization holding their loan; students with Federal Perkins Loans should check with the school which made the loan. For rules guiding forbearance and cancellation of Nursing Student Loans and Health Professions Loans, check with the organization that holds your loan.

EMPLOYMENT Federal Work-Study Available to—Matriculated students with financial need who desire employment on campus. Application—Complete a federal financial aid file by the on-time file completion date of April 1. If not initially awarded, contact the Financial Aid and Scholarships office regarding the Federal Work-Study Request Form. Provided by—Federal/state government and employer matching funds. Value—The total work-study allocation depends on student need, EFC and available funds. Wages are paid from the initial allocation until it is depleted. The hourly wage depends on the established rate for the specific job assignment. Students are limited to working 20 hours per week while classes are in session. Paychecks are issued twice per month. Special Conditions—Work-study jobs are available in many departments and open positions are posted on the University of Utah Human Resource website. SCHOLARSHIPS Scholarships are provided by the University, alumni, and private donors and are awarded on the basis of academic excellence and promise of future achievement. Financial need is an additional factor for some scholarship awards. Selection of recipients is usually made by committee. The University of Utah awards merit-based scholarships and scholarships based on financial need to resident and nonresident entering freshmen and transfer students, as well as to continuing University of Utah students. However, the Scholarships office at the University of Utah subscribes to the national standard of not awarding scholarships/grants to students who have already earned an undergraduate degree. Scholarships or tuition waivers listed in this bulletin are reserved for undergraduate students who have not yet earned a baccalaureate degree, unless indicated otherwise. Eligibility for academic scholarships for students entering college from high school is based on the following: High school grade point average (GPA) for grades nine through eleven, and scores on the American College Test (ACT) or the Scholastic Aptitude Test (SAT). (The University prefers ACT scores but will accept SAT scores.) The ACT and SAT are administered at or near high schools in Utah and other states. Students applying for scholarships should take this test in the spring of their junior year. No upgrades on ACT scores resulting from test retakes after the October 1 testing date will be considered. Current Resident high school seniors applying for Merit scholarships at the University of Utah need to apply for admission to the University by the Admissions priority deadline of December 1. President’s Scholarship invitees will be notified by the end of January.

The deadline for Leadership and Diversity Scholarships is February 1. The deadline for Need based scholarships is March 15.

The University of Utah is required to make certain information available to the public, according to the Higher Education Act, as amended. The information is available at the following web site: www.financialaid.utah. edu then select the U Right to Know tab at the top.

Consideration for entering freshman scholarships will not be given in subsequent years, even if the student should be an entering freshman. Scholarship applications from students already in college, including transfer students, are evaluated on the basis of available funds. A four-point scale is used to

determine the cumulative GPA. In addition, work submitted on some other scale is standardized to the four-point scale by the Admissions Office. Scholarships that consist of full or partial tuition waivers (e.g., the Honors at Entrance Scholarship) are awarded for two semesters per academic year, namely fall and spring. In addition, students may utilize their scholarships during summer terms if they enroll full time (12 hours); however, no student may receive a tuition waiver for more than eight semesters. Students on tuition waiver scholarships are required to take a minimum of 12 hours per semester. Incompletes, repeated hours, withdrawals, correspondence courses, and audited hours are not acceptable when considering renewal of scholarships. In special circumstances, the University will, if necessary, defer scholarships for both humanitarian and academic purposes. Humanitarian leaves of absence are granted for a period of up to two years. An academic leave of absence may be granted for one year, apart from humanitarian service, as long as the former is part of a student’s academic program and endorsed by the student’s department chair. Students must submit a written request to the Financial Aid and Scholarships Office at least 60 days before start of the semester, during which the deferral begins. Scholarships cannot be deferred for attendance at other institutions, except for University of Utah study abroad programs. Students may receive only one University of Utah tuition waiver scholarship. In the case a student receives more than one such scholarship, it will be necessary to indicate a preference for one and to decline all others.

Applying for Scholarships Freshman Leadership and Diversity scholarship application deadline is February 1. Transfer Merit, Leadership and Diversity scholarship application deadline is April 1. The departmental scholarship application deadline for all students is generally February 1. Some departments awarding departmental scholarships use a deadline other than February 1. For specific deadlines, refer to the individual scholarship listings under the “Departmental Scholarships” heading or call the departmental contact listed. Deadline for scholarships based on financial need for all students is March 15.

All scholarship applicants must have completed the application process for admission to the University of Utah by their scholarship application deadline. December 1, is the preferred filing date for admission to the University. CAUTION: Please note that processing of your scholarship application depends on interface with the Admissions Office. Consequently, please ensure that both Scholarship and Admission offices have your correct Social Security number and birth date. Discrepancies in information submitted to the Admission and Scholarship offices may result in failing to be considered for any scholarship award. It is the responsibility of the student to ensure that all required documents are on file at the University by the deadline. This includes the sharing of your Social Security number with the Admissions and Scholarship Offices. Incomplete or faulty applications cannot be processed. Students who receive a scholarship, fellowship, or other financial aid administered by the University of Utah are required by state law to certify that they are eligible to receive the scholarship as a U.S. citizen or a citizen of another country who is lawfully present in the U.S. If students do not know whether they are in the U.S. lawfully, they should seek the advice of an immigration attorney before filing a scholarship/financial aid application. No scholarship support for international students is available through the Financial Aid office. Prior to the disbursement of any scholarship/ financial aid, the University of Utah is required by law to verify the recipient’s citizenship status or lawful presence in the U.S. with the Department of Homeland Security (DHS). Once the University submits a student’s information to DHS for verification, this federal agency may have the legal right to share the information with other government agencies. Resident Tuition Waiver Scholarships President’s Scholarship Available to—Select resident high school seniors, who will be entering freshmen and who qualify for the Honors at Entrance Scholarship. Selection is based on academic excellence, leadership, and civic activities. Refer to “Honors at Entrance Scholarship” heading below, for additional qualification criteria. Please note that only applicants, who have completed the admissions process by December 1, by having submitted all required documents, will be considered for the resident President’s Scholarship selection process. Application—All semifinalists chosen from the Honors at Entrance Scholarship pool will be invited to apply. The application deadline is announced to each President’s Scholarship semifinalist. As a semifinalist, it is the responsibility of the student to ensure that all required documents are on file at the University by the deadline. Honors at Entrance Scholarships Available to—Seniors of an accredited Utah high school, public or private, who will be entering freshmen. Recipients are chosen by University committee from a pool of applicants, who qualify under an index that gives equal weight to both GPA and ACT/

SAT scores. The University considers the GPA from grades 9 through 11, and test results from the ACT or SAT, taken in either the junior or senior year. Selection of candidates depends upon ranking based on the University’s Scholarship Index. Application— Complete the admission process no later than February 1. Eligible students are encouraged to apply for admission by December 1, 2012. Due to limited funds, the University can offer awards only to the top applicants whose materials are received on time. Provided by—University of Utah. Value—Depending on academic merit, resident tuition (fees not included) or $2,500 partial tuition, awarded for a maximum of 8 semesters or until the student achieves a baccalaureate degree, whichever comes first. A Cash at Entrance award in the amount of $1,000 will be available to those students, whose academic credentials fall short of qualifying for either the Honors at Entrance or the Partial Honors at Entrance waivers. This Cash at Entrance scholarship is awarded for a maximum of 4 semesters. Grades are reviewed after the second, fourth, and sixth semester. Special Conditions—Qualification as a National Merit Semifinalist (the pool from which finalists and eventual winners are selected) is based on the results of the PSAT/ NMSQT, open to all high school juniors. The test is administered in most high schools in October and must be taken in the junior year. The dollar value of the award includes a $750 annual cash stipend from the University of Utah, and a minimum $500 cash stipend from the National Merit Scholarship Corporation. Alumni Nonresident U Tradition Award and Heritage Scholarship Program Available to—Nonresident high school seniors who will be first-year entering freshmen, and nonresident transfer students who matriculate at the University of Utah seeking an undergraduate degree, and who have at least one parent who holds a degree from the University of Utah. For fall semester 2013, freshmen applicants must be admitted to the University of Utah by February 1 and transfer applicants by April 1.  Freshmen applicants must be admitted with the University of Utah current year’s minimum GPA, ACT/SAT score. Transfer applicants must be admitted with the current year’s minimum transfer GPA. Full-time enrollment (12 course hours per semester) is required. Application—The admissions process must be complete and students specify on the Admission Application that one of the student’s parents is an alumnus of the University of Utah and provide additional details as requested on the form. Application—Submit the Application for Transfer Student Scholarships and one recommendation by April 1. Provided by—University of Utah. Value—Resident portion of tuition (fees not included). The recipient pays the difference between resident and nonresident tuition. The scholarship is normally awarded for one year, but the student may reapply. Number of Awards—10.

Western Undergraduate Exchange (WUE) The Western Undergraduate Exchange (WUE) is a program of the Western Interstate Commission for Higher Education (WICHE). Available to—First-time entering freshmen only. Transfer applicants and continuing non-WUE University of Utah students are not eligible for consideration. A limited number of awards will be made, based on academic merit. Applicants must be permanent residents of Alaska, Arizona, California, Colorado, Hawaii, Idaho, Montana, Nevada, New Mexico, North Dakota, South Dakota, Oregon, Washington, or Wyoming. Departmental Scholarships To qualify for a departmental scholarship, an applicant must be admitted as a matriculated (degree-seeking) student and have indicated an intended major. The number of scholarships awarded, their value, and special application requirements vary among colleges and even departments within a college. Read instructions carefully for application deadlines and special requirements such as auditions, tryouts, or portfolios. Finally, check with individual departments for other special requirements. Submit a separate application, with other required documentation, for each departmental scholarship for which you would like to be considered (unless specifically instructed otherwise). Submit all application materials for departmental scholarships directly to the department.

TUITION WAIVER SCHOLARSHIPS Students may receive only one tuition waiver scholarship sponsored by the University of Utah. Students who are awarded more than one such scholarship, must indicate a preference for one and decline all others.

Departmental Scholarship Available to—Outstanding Utah resident entering freshman and transfer students who have declared a major. Also available

to continuing University of Utah resident undergraduate students who are eligible for resident tuition, who have declared a major, and who are nominated by their departments. The Civil and Environmental Engineering Department offers a number of scholarships to entering freshmen, continuing, transfer, and graduate students. The exact number and amount of each scholarship is based on the income earned from the Department’s endowed scholarship accounts and the number of applicants and their qualifications. Persons wishing to be considered for a scholarship must submit a Civil & Environmental Engineering Department Scholarship Application. Scholarship applications available through the University of Utah Financial Aid and Scholarships Office will not be accepted. ELECTRICAL AND COMPUTER ENGINEERING DEPARTMENT The Electrical and Computer Engineering Department has scholarships available each year for entering freshmen and for continuing and transfer students. These include tuition scholarships for Utah residents and the scholarship funds listed below. Academic excellence is required, including a high GPA in technical subjects. Application deadline is February 15. Continuing and transfer students should apply using the application form available on the Electrical and Computer Engineering Department’s website www.ece.utah.edu/ scholarships. Entering freshmen are welcome to apply but should be aware that department scholarships are rarely awarded to entering freshmen because of uncertainty about commitment to the EE or CE major. Partial tuition scholarships are generally, but not always, granted to members of the Utah Ballet Company and Character Dance Ensemble. Students may be selected for membership in these performing groups after a minimum of one year of study in the Ballet Department. Departmental scholarships may be awarded to students, who are incoming freshmen and/or transfer students placing in the upper division technique levels. A limited number of one-year tuition scholarships for up to 18 credit hours per semester are awarded to residents of the State of Utah, with a cumulative GPA of 3.5 or higher. All scholarships awarded are for one year. Students will be considered for scholarship renewal during the spring practicum exams. All students who audition are considered for scholarship. MISCELLANEOUS SCHOLARSHIPS Descendents of Disabled Miners Scholarship Available to—Children and/or grandchildren of disabled miners. Application—Applications are available on the web.  Please fill out an Application for Scholarships Based on Financial Need located on the U of U Scholarship website under Printable Applications.  Please indicate “Miners Scholarship” on the line provided for that purpose. In addition to the scholarship application, please submit a statement (“To Whom it May Concern…”), in which mention is made of the name of the relative who worked in the Utah mining industry, and the circumstance of his disabling injury, illness or disability that resulted due to the person’s work as a miner. Value—Four-year scholarship, which covers tuition and books, no fees. Number of Awards—Two. One scholarship is offered through the College of Nursing or the College of Health in a field related to the health care of miners (i.e. Physical Therapy, Occupational Therapy, Audiology, etc.) The second scholarship is available for study in any of the other colleges at the University of Utah.

Elks Scholarships Available to—Outstanding students who demonstrate financial need and superiority in extracurricular activities. Application—Apply to the local chapter of BPO Elks of the USA. Submit the Elks Foundation Application and a copy of the FAFSA. Value—Varies. Number of Awards—Varies.

Intellectual Explorations Study Abroad Scholarship Available to—Matriculated full-time students who have completed 24 semester hours in residence (excluding credit by examination) at the University of Utah, with a 3.0 or higher GPA, prior to departure for study abroad. Priority is given to students who will be sophomores or juniors during their study abroad program, whose proposed study abroad program will contribute to the

completion of general education requirements, and who have not previously received a study abroad scholarship through the University of Utah.
