President’s Message As a healthcare provider and leader in our community, Touro understands the importance of healthcare and health benefits. Thanks to the good stewardship of the employees who are using Touro healthcare services and the overall performance of our plan, Touro has been able to maintain our current level of benefits. This year, our entire country is presented with the challenges associated with the implementation of the Affordable Care Act (ACA), effective January 1, 2014. Although portions of the employer health insurance mandate of ACA have been deferred until 2015, significant new additional fees and costs associated with employer-sponsored coverage will take effect on January 1, 2014. As a result of the ACA implementation, Touro and employers nationally are dedicating their resources towards medical coverage. I am pleased to announce that Touro health benefit premium will remain unchanged for 2014. However, the dental and vision benefits will be changing and Touro will no longer subsidize the premiums. Employees electing dental and vision coverage will pay 100% of the premium. All other benefits and rates for life insurance, disability and cancer coverage will remain the same. At Touro, we have a strong desire for our employees to be healthy, happy and productive and we believe those simple principles are cornerstones of a successful organization. Taking care of our patients is what we do every day, but it’s equally important that you take good care of yourself and your family. Your health and well-being are critical to our success! Effective January 1, 2014, Touro’s health plan will include annual preventive service checkups at no cost to you. This means preventive office visits for you and your dependents and any related readings and interpretations will be covered at 100%. No out of pocket expense to you or your dependents for wellness related prevention services. Do your part to stay healthy and help keep our health plan costs affordable through preventative visits and services. Touro will continue to offer the Base and Enhanced medical plans. Remember to always think Touro and use Touro services when provided. Touro is committed to providing a high quality level of benefits to our employees at a cost that is competitive in the local healthcare market. Benefits announcement 2014 Open Enrollment is November 11-22, 2013 Each year eligible employees have the opportunity to elect or change their benefits during Touro’s annual open enrollment period. A series of educational meetings to discuss employee benefits and the enrollment process have been scheduled. We encourage you to take the opportunity during the open enrollment period to attend one of the meetings in order to make an informed decision regarding your benefits. Benefit Changes 2014 Changes That Affect You. • • • • •
UMR will be issuing NEW medical cards to all participants. CHOICE PLUS Options Network has been added to our plan which will provide greater member discounts. The Medical Plans will include improved wellness benefits such as waived wellness office visit co-payments and related readings and interpretations covered at 100%. UMR will be the NEW administrator of the Flexible Spending Accounts (FSA) and will be issuing new FSA debit cards. AlwaysCare is our NEW fully insured vision provider and new vision cards will be issued. Assurant will remain the provider for our NEW fully insured dental benefit. Two dental options (low and high) will be offered.

Use a United Health Care (UHC) Urgent Care Facility. Urgent Care co-payment is $20.00 under Touro Plans. Visiting an Urgent Care Facility offers members a fixed co-payment of $20.00 and covers all other related expenses at 100%. Common conditions treated at an urgent care facility are sore throats, the flu, respiratory infections, ear aches, small cuts, sprains and minor broken bones.
NEW! Wellness prevention services are covered at 100%!
Be sure to get your annual preventive service check-ups. Don’t let attempts to save money harm your health in the long term. Wellness services performed at Touro are 100% covered. Effective 1/1/2014, wellness related office visits will be covered at 100%. This means the office visit co pay of $20.00 and any related readings and interpretations will be covered at 100%. No out of pocket expense to you or your dependents for wellness related prevention services.
Preventive check up pays you $$$. If you are enrolled in the ALLSTATE voluntary cancer coverage, Allstate will pay a $50.00 wellness incentive per employee/dependent that gets an annual preventive check up.
Consider a mail order pharmacy. If you take routine prescriptions, Caremark’s mail-order service can save you money. You could get a three-month supply of your medication but only pay for a two-month prescription. So, if your family spends $50 per month on prescriptions, you could save about $200 per year.
Go generic. Generic drugs cost significantly less – just $10 per prescription. If all of your prescriptions are generic, the $100 prescription annual deductible is waived per employee/dependent.
Take advantage of tax-free benefits. Money in a Flexible Spending account (FSA) is exempt from federal, most state, and payroll taxes, so your taxable income is reduced and you are able to use those pre-tax dollars for health care expenses. For example: A pre-tax $2,000 FSA contribution not only gives you money to spend on qualified health care expenses, but would result in a tax savings of more than $450 for an individual making $25,000 per year. How to Enroll During Open Enrollment, you have the opportunity to review and make changes to your current elections for all of the benefits offered by Touro Infirmary. 1.
What is the difference between UMR and UHC CHOICE PLUS? United Medical Resources (UMR) is the third party claims administrator hired to review and process employee claims for Touro Infirmary in accordance with Touro’s Health Plans. UHC CHOICE PLUS is the network Touro employees are required to utilize when selecting a physician or provider.

What is an Exclusive Provider Organization (EPO-Touro Facility Only)? An EPO directs your medical care to a specific hospital; in this case, Touro. The EPO requires you to utilize all services provided at Touro. For services that Touro does not provide, you will need to seek care at a facility that is in the United Healthcare UHC CHOICE PLUS network, excluding Ochsner and Tulane. Dependents residing outside of the service area will need to select a UHC CHOICE PLUS facility. Benefits are not covered out of the United Healthcare network.
3.
How does the BASE Plan (EPO-Touro Facility Only) work? The Base plan requires you to fully utilize all services provided at Touro. For example, if you are receiving diagnostic testing, lab work or scheduling an inpatient or outpatient procedure you are required to utilize the services provided at Touro, or subsidiary of Touro. Remember to inform your physician regarding the need to utilize Touro. Services performed outside of Touro (when available within Touro) are NOT COVERED.
4.
Will I be required to use Touro Infirmary for all services? Yes. Participants in the Base plan (EPO), are required to use all services provided by Touro.
5.
Are there any advantages of electing the Enhanced plan over the Base plan? Yes. By electing the Enhanced plan, you will have greater flexibility by choosing any provider, in the UHC network or outside of the network. This plan allows you the flexibility to use Touro services or another provider. Keep in mind that any time you choose a provider who is outside of the UHC network, your benefits will be covered at a reduced amount. Remember, services performed at Touro provide the highest level of benefits for you and your dependents.
6.
Does Touro have an arrangement with Children’s Hospital? Yes. Touro does have an arrangement with Children’s Hospital. Children’s Hospital will waive the coinsurance and deductible charges for covered children of employees enrolled in Touro’s health insurance plan. This is for hospital charges only and does not include charges for services that are contracted with an outside vendor. In addition, Children’s Hospital is a preferred hospital under Touro’s health insurance plan. Employees pay a lower co-insurance and lower out-of-pocket maximums for services at Children’s Hospital.
7.
How can I locate In-Network Providers? Check the United Healthcare website at www.umr.com and select United Healthcare Options CHOICE PLUS to find out if your physician is in the UHC CHOICE PLUS network.

What if my physician does not participate under the United Healthcare CHOICE PLUS network? You have two options: Choose a physician who does participate under the UHC network using the UHC website referenced above or ask your provider to consider becoming part of the network. Services performed by a physician who does not participate under the UHC CHOICE PLUS network are not covered.
9.
Do full-time and part-time employees pay the same monthly benefit premiums for health benefits? No. There are two separate monthly premium costs for full-time and part-time employees. The different premium costs are for medical coverage only.
10.
Do full-time and part-time employees pay the same monthly benefit premiums for voluntary benefits? Yes. All full-time and part-time benefits eligible employees will continue to pay the same premium cost for the voluntary benefits, such as dental, vision, life, short & long term disability and cancer coverage.
11.
Are the benefits offered to part-time employees different than full-time employees? No. The plan benefits available are the same. For example, the deductibles, co-pays, and co-insurance benefits are the same for medical, dental and vision.
12.
Will a change in work status affect my benefits? Yes. Touro has two benefit premium rates – one rate for full-time employees and a different rate for part-time employees. If you have a change in work status (i.e. full-time to part-time or vice versa), your medical monthly premiums will change the first of the month following the effective date of the change. You will automatically be enrolled in the same benefit plan; however, the monthly cost will change according to your new work status.
13.
Will I have the opportunity to change my benefit elections or cancel coverage in the event of a change in work status? Yes. You have 30 days from the effective date of the work status change to notify Human Resources of any changes you wish to make. The notification must be in writing and coordinated with the benefits department.
14.
How do I know if my physician has privileges at Touro Infirmary? The only way to address this question is to ask your physician. If your physician doesn’t have privileges at Touro, you will need to select another physician. Ultimately, you want to be sure that the services you are scheduling are covered under your plan.

Are all Crescent City Physicians under the UHC CHOICE PLUS network? Yes.

Are all physicians with privileges at Touro under the UHC CHOICE PLUS network? Not necessarily. You should inquire with your physician or contact UMR customer service to verify if they are innetwork.

Can I consult with someone at UMR regarding my decision? A customer service representative at UMR is available for questions regarding your medical benefits. Simply identify yourself as a Touro Infirmary employee at the beginning of your call. The number to call is 1-800-826-9781.
18.
Should I notify my pharmacy and physician of my benefits plan with Touro? Yes. On your next visit to the pharmacy or doctor, simply present your UMR ID card. This will allow the provider to correctly bill UMR for the services you have received. It’s important to inform your physician of the requirement to utilize Touro as a Base Plan participant.
19.
How do I find out more details regarding my medical benefit plan? You have several options: Contact UMR Customer Service at 1-800-826-9781 or visit www.umr.com. Contact the Human Resources Department at 897-8340, 897-8440 or 897-8428 or review the information provided on Lawson Employee Self Service, inside.touro.com.
The Importance of Updating your Other Insurance Touro’s group health plan requires that you notify UMR if you and/or your dependent(s) have medical coverage under another group health plan. This information is needed so UMR may coordinate coverage with that plan and avoid the delay in processing your medical claims. For example: • A person has health insurance coverage through two different employer plans. One person is the member and one is the spouse/dependent • Coordination of benefits makes sure the primary plan pays first on a claim • The secondary plan will then coordinate with the primary plan and may pay an additional amount
Updating Other Insurance Information Other insurance information is updated annually to ensure proper coordination of benefits. Even if you do not have other insurance, it is critical to let UMR know that you have no other coverage. Updating other insurance information is easy. You can visit www.umr.com and log into your member account. You may also speak to a customer service representative during regular business hours by calling 1-800-7839281.

EAP Program The Employee Assistance Program (EAP) provides free confidential, professional assistance to help employees and their families resolve problems/issues that affect their personal lives or job performance. Besides being confidential, the program is voluntary – it is designed to allow the employee or family to seek help on their own. What kinds of problems will the EAP help with? The program deals with human problems – the kind that affect an employee’s personal well being and their ability to perform on the job. Who is eligible for this program? ALL Touro employees and their family are eligible. What is the cost? The first five visits are paid for by Touro. If further assistance is necessary, either your insurance will take care of it or if it is not covered under insurance, the counselor will work on a payment plan with you. Is it confidential? Yes, any issues discussed with your counselor are confidential. Neither your employer nor co-workers will have any knowledge of your request for help.

Notice that this Touro Infirmary Employee Health Plan is a Grandfathered Health Plan This group health plan believes this plan is a “grandfathered health plan” under the Patient Protection and Affordable Care Act (the Affordable Care Act). As permitted by the Affordable Care Act, a grandfathered health plan can preserve certain basic health coverage that was already in effect when that law was enacted. Being a grandfathered health plan means that your plan may not include certain consumer protections of the Affordable Care Act that apply to other plans, for example, the requirement for the provision of preventive health services without any cost sharing. However, grandfathered health plans must comply with certain other consumer protections in the Affordable Care Act, for example, the elimination of lifetime limits on benefits.
