General Changes The Name: Centricity EMR We at MSU’s HealthTeam first transitioned from Logician 5.5 to Centricity 5.6, and now there is one more step in that progression: Centricity EMR 2005.

Centricity EMR 2005 Login Screen The login process for Centricity EMR 2005 remains the same as that of its predecessors: 1. Type your Centricity User ID in the User ID field. 2. Type your Centricity password in the Password field. 3. Click OK . If the Location of Care does not auto-populate after you’ve entered your User ID and then moved to the Password field, the ID that you have entered is incorrect. If all three fields are populated but your password becomes highlighted in blue when you click OK , your password is incorrect and you’ll need to try again. Be sure that the CapsLock button of your keyboard is not on, since your Centricity password is case sensitive.

Once logged into Centricity 2005, be sure to lock the screen if you’re walking away from the computer. Click the Exit button, located on the far right end of the Centricity toolbar. This will prevent unauthorized access to HealthTeam patient records.

Network Connection Recovery In the past, a network failure often lead to chart updates being locked for two or more hours, and there was no way to release those locks. Modifications to the database server used in Centricity 2005 are such that if a network failure results in chart updates being locked, the system will automatically release those locks within minutes, therein allowing users to complete their documentation in a timely manner.

The Out of Office Assistant The Out of Office Pop-Up When a Centricity user attempts to route a document and/or flag to another Centricity user whose Out of Office Assistant is activated, a pop-up box informs the sender that the intended recipient is not presently monitoring his/her Desktop. The sender can then elect to route the document to the originally indicated recipient, or use the Do Not Route button, then determine how best to handle the document or flag.

Activating the Out of Office Assistant Turn on the Out of Office Assistant to let other Health Team members know that you are not in the office. 1. Logged in to Centricity, go to Options | Out of Office Assistant … 2. In the Out of Office Assistant window, click to select “ ~ I am currently out of the office”. 3. Type the content you wish to have included in the auto-reply field. 4. Using the checkboxes in the lower left corner of the window, indicate if you wish Centricity to send an Out of Office reply to ; Documents, ; Flags, or both. 5. Click OK.

NOTE: Activating the Out of Office Assistant does not prevent documents from being electronically routed to your Desktop via an interface. (Sparrow and Ingham labs, MSU Radiology reports, etc.)

Turning off the Out of Office Assistant Once you’ve activated the Out of Office Assistant, the next time you log into Centricity you will be prompted to either keep the Assistant activated, or to turn it off. Yes will deactivate this feature, while No will turn it off.

Desktop Document Views Display similar document types together and designate the order in which they appear by defining the rules in saved, user-specific document views. Once created, those views are available on both your Desktop Documents tab and the Documents pane of your Summary window, and you can easily switch between those custom views and the Enterprise-wise global view. Creating a Document View 1. On the Documents tab of your Centricity Desktop, click the Organize button.

In the Change or Copy Desktop Document View window you can switch between ~ Global Views (those available to all HealthTeam Centricity users) and ~ Personal Views (those visible only to the ‘owner’ of said views, along with others given access to those views). In this window you can also choose to include or filter out documents in the various sub-sections. y Document types y Priority y Locations y Confidentiality types y Status Remove checkmarks from particular document types, and various document properties (such as from a particular location or confidentiality type) as desired. Click the More… button to specify the exact order in which the displayed documents are to be displayed. a. Use the dropdown options in the left portion of the Change or Copy Desktop Document View window to determine the document sort order. b. Use the buttons on the far right portion of the window to change the overall order of document types as they appear in your Desktop view. Example: Select the document type of “Office Visit” from the list of Document Types, then click the Move To Top button to have office visit documentation displayed at the top. Or, place all diagnostic-type documents together. c. Click OK . Now back in the Change or Copy… window, click Save As… to save your selections. In the Save Document View pop-up, type the name that you want assigned to the view that you’ve just defined.

finished, your saved views will be listed just below the global, “All Documents” view in the upper left pane of your Documents window. Move from one view to another as your EMR documentation focus and needs change. Click directly on a view folder to see its entire content, or click the “+” in front of that folder to see specific document types you’ve assigned to that folder’s view.

User-Defined Document Views on the User’s Desktop

Change the Document View on your Desktop Summary Page The documents displayed on your Desktop Summary page are determined by the preferred view associated with your Centricity user ID; however, you can quickly change from one view to another using the small grey arrow located just to the left and above the list of documents. Click the arrow to see and select from the various document views available.

Remove a Desktop Document View You can prevent a document view from being included in the list of view types by removing the ‘preferred’ status of that view. By removing this status, the list is saved for use at a later time. 1. On the Documents tab of your Centricity 2005 Desktop, click the Organize button, on the far right side of the toolbar. 2. 3.

Choose ~ Personal Views from the Change or Copy Desktop Document View pop-up. Select the view you want removed from the dropdown list directly to the right of ~ Personal Views. Place a checkmark by ; Remove as one of my preferred views. Click OK .

Delete a Desktop Document View You can delete a document view that you’ve created and saved in Centricity 2005. 1. On the Documents tab of your Centricity 2005 Desktop, click the Organize button, located on the far right side of the toolbar. 2. Choose ~ Personal Views from the Change or Copy Desktop Document View pop-up. 3. Select the view you want to delete from the dropdown list directly to the right of ~ Personal Views. 4.

Click Remove , located directly below the dropdown list.

Click OK to return to your Documents page, or make further changes in the Change or Copy…. Window.

What’s the difference between “Removing” and “Deleting” a document view? When you remove a view from your preferred view list, Centricity keeps that view and you can retrieve it via the Organize button. When you delete a view using the Remove button, it is deleted entirely from the system and you cannot get it back.

Desktop Groups Early versions of Logician/Centricity only allowed the user to view one Desktop at a time; so, for example, if you were checking for urgent lab results on multiple Desktops, you had to systematically go from one Desktop to the next. Centricity 2005 has a feature that allows you to create a ‘group’, then set your Desktop Documents view such that documents on all of the Desktops of members of that group are displayed in one screen. Furthermore, you can use a combination of your customized and saved Desktop document views and your groups to include or exclude certain document types, therein helping you to focus on the task at hand. Groups can be used to view the flags of multiple Centricity users simultaneously, as well. Creating a Desktop Group 1. Go to your Documents tab of your Centricity Desktop. (You could also start on your Flags tab, but for training purposes, we’ll start on Documents.) 2. Click the binoculars associated with the “View documents to” dropdown field. 3. In the Find Users window, click on the Browse tab. 4. Click on the main folder to expand it; having done so, its contents should be displayed in the right portion of the window. (The illustration here is of the imaginary MHS enterprise, its locations, and its users.) 5. (When applicable, in the HealthTeam’s real Centricity) If only the EMR folder is visible, click on the “+” sign in front of that folder; this expands the EMR folder, revealing the HT folder and clinic-specific folders nested inside. 6. (Also when applicable) If there is a plus sign in front of the HT folder, click on it (the “+”); this expands the HT folder, revealing the various HT clinics. 7. Click on an appropriate clinic’s folder from the left-hand column’s list to display its contents in the right portion of the window. This would typically be your clinic’s folder. 8. Next, click the More >> button.

Click the New Group button, located in the lower left corner of the expanded Find User screen. Type the name you want to assign to the group you are 2 creating in the Add New Group Name pop-up. Click OK . (Optional) Repeat the process of clicking the New Group button and creating additional groups. With your group created and named, build the content of that group. Locate and click to select a HealthTeam Centricity user’s name in the Find User screen. 1 With the desired user name still selected, click the Add Item button, in the lower left corner. Creating your group The Add Item to My Folder pop-up appears, where you will need to identify exactly which group or folder you want to add the selected user name. Click to select the group you’ve created. (The illustration shows the “My Physicians” group selected.) Click OK . A pop-up message informs you that “1 item added to (xxxx), with the name of your group displayed; click OK to close that message box. Repeat the process to add others to your group. Once you’ve identified and added your group members, click on that group in the upper left portion of the Find User window. The members of your group will be displayed in the right pane of that same window. In the lower portion of the Find User window, under “Content of  List”, select ~ Selected folder. Your group name should appear directly to the right of that option, just to the left of the Set button. If it is not, return to the upper portion of the window and select it again.

Using Your Group to View Desktop Documents If you have created a group and set that group as the default viewing mode for your Desktop Documents, you’ll see not only the basic “View documents to” dropdown list, but a second, new dropdown field that is comprised of the members in that group.

When your group is in the “View document to” field, all documents on the group members’ Desktops appear together on your screen.



Use the top dropdown list to switch between your group or groups, your My Folder (if you’ve established content for that feature), Desktops of Centricity users that you’ve most recently viewed, and your own Centricity Desktop.



Also use the top dropdown list to return the view to only Documents on your Desktop.



Use the second dropdown list to change your view from the entire group to a particular member of that group.

Modifying Your Desktop Group To make changes to your Desktop Group, click the binoculars just the right of the “View documents to” field on your Documents tab. Click the More >> button, in the lower left portion of the Find User window, and make the desired changes. Such changes may be to add other members to a group, create additional groups, change the name of a group, or delete a group entirely.

To delete a Desktop Group, click the binoculars just the right of the “View documents to” field on your Documents tab, then click the More >> , in the lower left portion of the Find User window. Select the group that you want to delete from those appearing in the list in the upper right window pane. Click the Delete Item or Group button, located in the lower left corner.

Close the Find User window by once more clicking OK .

Routing Multiple Documents In the past, you could only route a single document from your Desktop at a time. Now, you can select and route multiple documents from the Desktop simultaneously. The same applies on the Documents tab of patient charts. Routing Sequential Documents Simultaneously 1. Click on the first document that you want to route. 2. Hold down the [Shift] key of your keyboard. 3. With the [Shift] key down, use the mouse to click and select the last of the documents that you wish to route.

Once the documents are highlighted in blue, release the [Shift] key. Click the Route button, above and to the right of the listed documents; this opens the Route Documents window. If necessary, change the routing recipient or add another recipient by using the New… , Remove or Change…. Button. Click Route to route the documents to the designated recipient(s).

Routing Non-Sequential Documents Simultaneously 1. Click on the first document that you want to route. 2. Hold down the [Ctrl] key of your keyboard. 3. With the [Ctrl] key down, use the mouse to click and select the remaining documents that you wish to route. 4. When you have selected all the documents that you wish to route, release the [Ctrl] key, then continue the routing process as described above.

NOTE: You can only route or remove multiple documents concurrently. If you select multiple documents, the Open, Edit, and Sign buttons are disabled.

Chart Document Views Creating and using chart document views in patient charts is much the same as the document views on your Centricity 2005 Desktop. Create a Chart Document View 1. On the Documents tab of a patient chart, click the Organize button.

In the Organize Chart Documents window, select ~ Personal Views. Add or remove checkmarks (;) in the options provided in the various sub-sections; such sections include: y Document types y Locations y Confidentiality types y Status Choose the desired Sort By attribute in the respective option in the lower left corner. Click Save As… . In the Save Document View pop-up, type the name by which you want your document view to be saved.

Click OK once again to return to close the Organize Chart Documents window, or remain in the Organize window to create/modify another document view.

Click directly on your custom document view to see all documents that meet the criteria you’ve defined in that view. Click on a specific document type included in a particular, expanded document view.

Remove a Chart Document View Hide a document view from the patient Documents tab by removing the ‘preferred’ status of that view. By removing this status, you can keep the view for future use. 1. On the Documents tab of a patient chart, click the Organize button, on the far right side of the toolbar. 2. Choose ~ Personal Views on the Organize Chart Documents pop-up window. 3. Select the view you want to hide from those included in the View Name dropdown list. 4. Place a checkmark by ; Remove as one of my preferred views. 5. Click OK , at the bottom of the window. Alternatively, remain in that window to create a new chart document view, or to modify an existing view. Delete a Document View Delete a Chart Document View from your ~ Personal Views list by clicking the Organize button, selecting the document view you want to delete from those included in the View Name dropdown list, then clicking Remove . Use caution when completing this task, though, as there is no way to ‘undo’ it; if you remove the wrong document view, you cannot get it back.

Flowsheet View – Showing or Hiding Removed Observations In previous Logician versions, removed observations always appeared on the flowsheet view of patient charts. In Centricity 2005 you have the option to show or hide such values. 1. On the flowsheet tab of the patient chart and with the Removed value in Logician 5.6 desired flowsheet view chosen from the View dropdown list, click the Organize button. The Organize button is located on the far right side of the charting window.

In the Organize Flowsheet Layout screen, place a ; checkmark by “Show removed values” to have removed observations displayed in the flowsheet. Alternatively, remove the ; checkmark, and removed values will not be included in the view. (Optional) place a ; checkmark by “Save as preference” if you want Centricity to remember your choice(s) made in this screen. Click OK (or press the [Enter] key).

NOTE: Any additional changes you make in the Organize Flowsheet Layout window will be included in your default flowsheet layout if you opt to “Save as preference”.

Chart Updates Allergies and Adverse Reactions Centricity 2005 enables us to distinguish between allergies and adverse reactions to treatment. It also provides room for longer reaction descriptions. Access the Change Allergy or Adverse Reaction screen during a chart update by either clicking on the Allergy button of the charting toolbar, or via an Update Allergy button on an encounter form (such as a Nurse Intake form). Update Allergies or Adverse Reactions •

If an allergy or adverse reaction has already been documented in the patient chart, the option to indicate that the patient has no known allergies or adverse reactions is deactivated.



To document that you have reviewed the patient’s allergy list during a chart update and that no changes are warranted, click to check ; Allergy and adverse reaction list reviewed during this update.



To add a new allergy or adverse reaction, click the New… button, located in the lower left corner of the screen.

New Allergy or Adverse Reaction Screen 1. Select either “Custom Allergies” or “Food Allergies” from the Custom List dropdown field. The selection you make here determines what options appear in the Reaction to: dropdown list. 2. Choose the appropriate classification (Drug, Food, or Environmental) in the Classification field. 3. Pick the element in the Reaction to: field from the dropdown options, use the Reference List… button to locate and select a particular medication, or type it directly in that field. 4. Indicate if the reaction was ~ Critical or ~ Non-critical. 5. Include a description of the reaction, if desired. 6. Document the date of the reaction, if known, in the Onset Date field. 7. Click OK (or use the [Enter] key). 8.

Click OK once again to close the Update window, or continue documenting additional allergy/adverse reaction information.

L An allergy is a hypersensitivity to some environmental agent, food, or drug that can result in minor reactions or life-threatening emergencies. A critical adverse reaction is any undesirable or unwanted consequence of a preventative, diagnostic, or therapeutic procedure or regimen.

Favorite Forms Pane You can create a list of ‘favorite’ forms and text components that you occasionally add to Centricity 2005 encounters. This list is then displayed in the left column of your chart update.

Easy access to additional forms during an update

Add Forms to your Favorites List 1. With a chart update in progress, right-click directly in the white Favorites space, in the lower left corner of your update window. 2. Click with the left mouse button on “Add Fav Form/Text component…” from the pop-up. This opens the Find Components screen. 3. On the Browse tab of the Find Components screen, locate the desired form. 4. Select the form from the right pane of the Find Components window by clicking directly on that form. 5. Click OK (or [Enter]).

Remove Forms from your Favorites List To optimize the usefulness of your Favorites list, keep it clean - remove forms and text components that you find you don’t frequently use. You can still access them at a later date via the Forms Component button of your charting toolbar. 1. Right-click directly on the form that you want to remove from your Favorites list. 2. From the pop-up menu, select “Remove Fav selected item”. Using the Favorites List Place your cursor in the position of the chart update where you want the added form to go. (Ex: if you want the form to be added at the bottom of the visit, put your cursor at the very bottom, beneath the last form currently in the update.) Then double-click on the form in your Favorites list. Alternatively, right-click on the form and choose “Insert into note” from the pop-up window.

Viewing Inactive Problems Traditionally you’ve had to leave the Update Problems window during a chart update and go to the Problem tab of the patient’s EMR chart to view inactive problems. In Centricity 2005 you can access the content of the patient’s inactive problem list right in the Update Problems window. Do so by selecting ~ Inactive, located in the upper right corner of that window.

Reactivation of old Problems Set the status of a problem that has previously been resolved or removed in a patient chart to ~ Active. 1. With a chart update in progress, click on the Problems button of the charting toolbar. Alternatively, access the Update Problems window using a Problems form button. 2. Select ~ Inactive. 3. Click directly on the problem that you want returned to the active problem list. This, in turn, activates the Reactivate button, located in the lower portion of the Update Problems window. 4.

When you reactivate a problem, Centricity removes the original End Date recorded for that problem and moves the problem to the active problem list. The Onset Date remains as originally documented. Once the chart update has been signed, the reactivated problem appears as a typical problem when ~ Active Only is selected on the Problem tab, but the historical information about that problem is displayed if you select ~ All.

When might you Reactivate a problem? When the new diagnosis of the problem is directly related to the original diagnosis and onset date. The clinical history of this reactivation is grouped with the original problem.

New Occurrences of old Problems You can use an inactive problem on the patient’s problem list as a template of sorts, upon which you build documentation of a new problem added to that patient’s chart. When you begin a new occurrence of an old problem, that new occurrence will have a history entirely independent of the original. 1. With a chart update in progress, either click the Problems button to open the Update Problems window, or access this window via a Problems form button. 2. Select ~ Inactive. 3. Click on the appropriate problem in the upper portion of the Problem list. 4. Click New Occurrence… , located in the lower portion of the Update Problems window.

A New Occurrence of Problem window opens. There you determine the Type of Problem, then define the Problem as applicable to that instance. 6. Click OK , then OK again. NOTE: The Onset Date of a new instance of an inactive problem is automatically assigned the date of the chart update in progress; however, you can change the Onset Date in the New Occurrence of Problem window. You can also assign an End Date or Duration there.

When might you use the New Occurrence tool? When documenting a new diagnosis of an inactive problem with a new onset date. This might be a discrete instance of a recurring problem, such as UTI or Otitis Media.

No more “Minor Diagnosis” The Problem type of “Minor Diagnosis” is not an option in Centricity 2005; however, the functionality that was incorporated in this Logician 5.6 problem type is now included in the basic “Diagnosis” problem designation. Existing “Minor Diagnosis” problems in Health Team patient charts will remain as such, but that same designation will not be an option for subsequent documentation. 1. With a chart update in progress, either click the Problems button to open the Update Problems window, or access this window via a Problems form button. 2. 3. 4.

In the Update Problems window, click on New , located in the lower left corner of that window. Select the appropriate problem type in the Type of Problem field. (“Diagnosis of” is shown in the illustration here.) Use the Reference List… button, in the upper right corner of the New Problem window, to locate and select the appropriate problem from the complete Centricity problem list. Alternatively, with the Custom List dropdown pointing to your discipline’s custom problem list, use that dropdown arrow () to locate and select the desired problem. With a Problem selected, modify the Description and Comments fields as needed. Modify the Onset Date, End Date, and/or Duration, as needed. Click OK to return to the Update Problems window. Or, if you want to add another problem, click Save & Continue , then repeat the process with the next diagnosis.

Once the chart update has been signed, the newly added diagnosis is displayed on the Problem tab of the patient chart. Any comment that you included with the diagnosis is included with the actual diagnosis, on the left side of the screen.

Duplicate Problem Notices A new feature in Centricity 2005 helps reduce the number of duplicate entries on patient problem lists. When problems are added, Centricity checks to see if those problems already reside on the patient’s active or inactive problem lists. Adding a Problem already on the Active Problem List If you attempt to add a problem already on a patient’s active problem list, The Duplicate Problem(s) Found window opens. There you can opt to add a New Assessment to the existing problem, view the Details…. of that problem, or actually add a duplicate of that same problem by clicking the Continue Adding Problem button. Adding a Problem already on the Inactive Problem List If you attempt to add a problem already on the patient’s inactive problem list, the Duplicate Problem(s) Found window opens, where you can click the Reactivate button to reactivate the original occurrence of the problem. Alternatively, click the Continue Adding Problem button to add an independent, new instance of the problem. Problem List Management Organize patient problems in the Update Problems screen using the arrows and Dim button in the right portion of that screen. The up () and down () arrows move highlighted problems in the respective direction on the list, while the arrows pointing right () and left () indent or remove indentation of selected problems. The up arrow with a line under it moves the selected problem to the very top of the patient’s problem list, while its counterpart down arrow with a line moves a problem to the bottom of that list. Select a problem, then click the up arrow with a line to move it to the top of the list. Use the basic arrow up to move the problem up one space in the list. Use the basic arrow down to move the problem down one space in the list. Click the down arrow with a line to move it to the bottom of the list.

Enhanced Medication Documentation The allergies and adverse reactions documented in patient charts are displayed in both the New Medication and Prescription windows.

Additionally, when working in the New Medication window, if the program identifies documented allergy or adverse reactions, assigned icons are displayed in the Define Medication portion of the screen. Then, if you rest your mouse on the displayed icon, details of the reaction are displayed; if you double-click directly on the icon, the full Check Medications for Interactions window opens. Ending an Update with Prescriptions The options available when ending a chart update have been expanded in Centricity 2005. If an update includes a prescription that is set to be faxed or printed, but it has not yet been done when the user ends the update, he or she can have that prescription faxed or printed automatically when the Sign Document or Hold Document button is clicked. 1. Document a presription in a chart update, selecting the appropriate medication and providing its instruction set, quantity, and number of refills. 2. In the lower right portion of the medication window, choose the desired option from those available in the Prescribing Method dropdown list. In this case, we’ll choose “Fax to Pharmacy.”

Click the OK button to close the Medication window.

Back in the Update Medication window, you have the option Print RX ; however, you are not ready to print it at this point, so just click OK to close the Update Medication window.

Complete documenting in the patient chart, then click the End Update toolbar button. In the lower section of the End Update window, there is a checkbox that, when checked, instructs Centricity 2005 to automatically print or fax the prescription when the current user chooses to Hold Document or Sign Document .

If you want the prescription to print or be faxed to the designated pharmacy at this time, put a checkmark in the box to instruct Centricity to “; Sign clinical list changes. Pending prescriptions set to print or fax will be completed automatically.”

Alternatively, if you do not want the presription to print or be faxed at this point, there should be no checkmark in the field for “… Sign clinical list changes. Pending prescriptions ….”

If further documenting is required in the chart update, click Hold Document ; otherwise, click Sign Document .
