EFECTOS ADVERSOS DE LOS INHIBIDORES DE LA BOMBA DE PROTONES

Introducción:  En

los últimos años los estudios epidemiológicos que se han realizado sobre los Inhibidores de la Bomba de Protones (IBP), a pesar de su excelente perfil de seguridad han indicado posibles riesgos que pude tener el tratamiento a corto y largo plazo con los inhibidores de la bomba de protones.

Introducción:  Conocer

los efectos secundarios de los

IBPs.  Conocer si existen recomendaciones previas a la utilización del fármaco.  Conocer si deben realizarse controles rutinarios en los pacientes que precisan un tratamiento crónico con IBP.

La disminución del ph ácido gástrico puede favorecer la colonización del tracto digestivo superior por bacterias patógenas que van a producir un aumento del riesgo de infecciones a dos niveles: • Gastrointestinal • Respiratorio

de fracturas de cadera asociadas a tratamiento prolongado con IBP.  Alteración en el metabolismo del calcio asociado a tratamiento de larga duración con IBP.

Metabolismo del calcio: 

Yang 2006: 

Registro recogido de Base de Datos de atención primaria: • Pac con fractura de cadera entre 1987-2003 • > 50 años • Comparaba los pacientes que tomaban IBPs y Anti-H2 frente a los que no tomaban terapia antisecretora



Resultados: • Pac en tratamiento con F antisecretores más de 1 año presentaba un aumento del riesgo de fractura de cadera:     



IBPs OR:1,44 95% IC 1.30-1.59 AntiH2 OR:1.23 95% IC 1.09-1.40 IBPs>AntiH2: OR: 1.34 95% IC 1.14-1.38 ♂>♀ OR: 1.78 95% IC 95% 1.42-2.22 A más tiempo de tratamiento y más alta dosis mayor riesgo de fractura.

Vitamina B12: 



El ácido gástrico es necesario para que las enzimas pancreáticas puedan actuar sobre la VitB12 permitiendo que esta se una al Factor Intrínseco y este complejo se absorba a nivel de íleon terminal. El défitit de Vitamina B12 puede originar: 

Vitamina B12:  

Los estudios no dan información concluyente. Pero el uso prolongado (>1año) de inhibidores de la secreción ácida, sobre todo IBP podría dar lugar a un déficit de vitamina B12:  





Ancianos Pacientes desnutridos.

Podrían realizarse controles periódicos en grupos de riesgo que vayan a requerir un tratamiento a largo plazo. Valorar control en estados de hipersecreción ácida como Síndorme de Zollinger-Ellison.

Hierro: 

Se ha observado un aumento de la incidencia de déficit de Hierro en pacientes con hiposecreción ácida:   





Anemia perniciosa Aclorhidria Vagotomía,…

Pero no existen estudios claros que indiquen que un tratamiento prolongado con supresores de la secreción ácida puedan dar lugar a un déficit de hierro sérico, ferritina o índice de la saturación de la trasferrina. Situaciones especiales:  

Síndrome de zollinger-ellison Hemocromatosis.

Hipomagnesemia:   

El mecanismo por el que se produce la hipomagnesemia no esta claro. En ocasiones aparece asociado a hipokalemia e hipercalcemia. Clínica:      



Fatiga Inestabilidad Parestesias Convulsiones Tetania Arritmias cardíacas.

Mas frecuente en paciente en tratamiento con :  

Diuréticos Digoxina



La clínica desaparece con la supresión del fármaco.



Aunque no existen estudios con evidencia en este tema si se aconseja a los pacientes que van a tener un tratamiento prolongados con IBP realizar un control previo al inicio del tratamiento para evitar esta posible complicación. Pólipos fúndicos: 

  

El uso de IBP durante períodos prolongados puede producir pólipos fúndicos glandulares como consecuencia de hiperplasia de células parietales. Siempre asociado a H. Pylori. Desaparecen al erradicar H. Pylori o al eliminar IBP. Pueden dar lugar a displasia en ocasiones excepcionales 

 

Familiares de pacientes con poliposis adenomatosa familiar

No son precursores de adenocarcinoma gástrico. Los IBPs pueden enmascarar síntomas de adenocarcinoma gástrico.*

Conclusiones:  





Los IBP son fármacos en general seguros. Pueden aparecer efectos secundarios derivados de una tratamiento prolongado en pacientes con factores de riesgo. Deben realizarse algunas determinaciones antes de iniciar un tratamiento prolongado con IBP. Sólo deben administrarse en los pacientes que realmente los necesiten y durante el tiempo adecuado.
