2011 State of the County Health Report The purpose of this report is to relate current information on the Health status of Duplin County including morbidity/mortality data and review priority health concerns selected as a result of a comprehensive community health assessment conducted in 2008. It serves as a yearly update of accomplishments, health concerns and the actions taken to address these concerns. This report was designed to promote an understanding of health needs, and to educate the community regarding health issues that affect Duplin County residents. Data for this report was taken from the North Carolina Center for Health Statistics (NCSCHS), the US Census Bureau and the North Carolina Youth Tobacco Survey (NCYTS), NC Nutrition and Physical Activity System (NCNPASS).

The Africaan Americann and Hispannic populatioons are at inccreased risk for f diabetes.. Undiagnossed and unm managed Diabbetes greatlyy increase thee risk factoors for a variiety of other health issuees such as heeart disease, renal disease and stroke. Page 2 of 12   

Duplin County Health Department implemented a free; ADA approved Diabetes Education Program opened to all residents in 2007. Since that time, approximately 300 people have completed the Diabetes Education Program. Participants receive follow up for six months after completion of the program. The Diabetes program offers residents the chance to receive nutrition counseling from Registered Dieticians at the Health Department. Participants of the program learn how to better manage their diabetes by implementing diet and exercise into their daily living. In addition to the Diabetes Education Program, the Health Department is the medical home for approximately 120 diabetics. Transportation and the lack of diabetic testing supplies were identified as barriers to disease management. In order to assist diabetics in managing their disease, the Health Department has grant funds from Duplin General Hospital to provide $10 gas cards, diabetic testing supplies, and lab work. Diabetics who cannot afford their medication receive their medication through the Prescription Assistance Program and pick up their medication at the Health Department. The Health Department has improved in providing best practices in the management of diabetics. The data reflect an improvement in all areas of care since November 2010. The patients managed by the Health Department had A1cs documented, documented blood pressures, the use of Aspirin in diabetics over 40 years of age, a yearly flu vaccine, a foot exam on each visit, and a renal assessment. A significant number of the diabetics showed an improvement in the A1c values and a decrease in blood pressure. Several initiatives have been introduced by other agencies to improve the management of diabetes. Goshen Medical Center (GMC) completed the ADA certification process and offers the Diabetes Education Program. Duplin General Hospital (DGH) hosts the Diabetic Bus, a grant funded mobile education center that serves the Eastern Region. The Diabetic Bus comes to DGH on a quarterly basis. Duplin County Schools received a grant in October 2011, “Type ll Diabetes-We Are Onto You”. The program targets sixth grade students. Students will have the opportunity to be screened for height/weight, blood sugar, BMI, and blood pressure. The program will focus on meal planning, reading labels and diabetes. Medical providers within the county offer physicals, blood sugar testing and disease management.  

OBSEITY/PHYSICAL INACTIVITY In 2008 Duplin County’s prevalence of obesity in children ages two to four years of age was 18.6%. The prevalence of obesity in children ages five to eleven was 26.5%. In 2011 the Health Department received grant funding from Cape Fear Foundation for a Pediatric Healthy Weight Clinic using the model at East Carolina University School of Medicine. The Pediatric Healthy Weight Clinic, KIDS ON TRACK, started monthly clinics on July 13, 2011 for children through Page 3 of 12   

18 years of age. Due to the referrals from the medical community, the Health Department obtained additional grant funds from Duplin General Hospital Foundation to offer the clinic twice per month. The purpose of the clinic is to provide an intervention for overweight and obese children by a multidisciplinary team. The team consists of a pediatrician providing the medical component, a therapist providing a behavioral component and a nutritionist providing nutritional assessments and nutritional therapy. The participants are scheduled with one or more of the team between clinic visits based on the plan for the child and family. All children are monitored closely to track their progress. The entire family is involved in setting goals and improving the health of the entire family. The Health Department continues to work with the Warsaw Wellness Center, the Faison Wellness Center, and other community organizations to offer a variety of preventative health care programs. In 2010 Duplin Partners for Health collaborated with Duplin General Hospital, Farm Bureau, and other health and community organizations to host a County Health Fair. Residents were able to attend and receive a variety of preventative health screenings. Duplin Partners for Health has provided many enhancements to the Duplin Commons Walking Trail in Kenansville such as benches, a water fountain and outdoor exercise equipment. The last enhancement completed in November 2011, was the installation lights in an area of the trail that was not well lit. The walking trail allows residents and county employees easy access to a safe location where they can increase their physical activity to improve their overall health and wellbeing.

In Duplin County C the majority m of cancer c deathss stem from lung, colon, female breast, b prostaate and pancreas. In 2008-2009 canccer statistics showed the death ratee for cancer is still 203.99 per 100,0000, while the state rate is 190.7. In 20009 the deathh rate for heart h disease was 222.4 per p 100,000 compared too the state deeath rate of 192.2. 1 The death d rate from m strokes hass continued to t decline, hoowever the rate r in Dupliin County off 68.8 is still above thee state rate of o 54.4. In 20010, 149 deaaths were rellated to circuulatory diseaase; 26 were caused byy stroke, 23 by hypertennsion and 58 by heart attaack. Poor diett, physical in nactivity, tobbacco use annd being overrweight/obese are risk faactors for heeart disease, cancer c and stroke. s Duplin County C has one of the higghest rates of uninsured residents in the state. Duue to this facct, many of them cannott afford heallth care. Dupplin County Health H Depaartment workks with patieents to providde them with h appropriatee preventativve screeningss and prevenntion educatiion. In 2010, Duplin General Hospitaal received grrant fundingg from the NC Stroke Caare Collaboraative to offer stroke s screen nings. The hoospital offered these screeenings to Duplin D Countty residents in i all areas of the t county. Through T the screenings, residents’ tootal cholesteerol, HDL, LDL, L triglyceriides, and blo ood sugar weere tested. From October O 1 , 2010 2 to Septeember 30 , 2011, 2 385 reesidents of Duplin County C receiv ved stroke sccreenings. The Health Deppartment alsoo provides reeferrals to thhe s that since the paassing of the North Caarolina Quit Line. A receent report byy NCDHHS showed Smoke-F Free Restauraants and Barrs Law, theree has been a 21% declinee in the num mber of emergenccy room visiits by North Carolina ressidents experriencing heaart attacks.

This rate is slightly higher h than inn the two previous yearss, but it is higgher than thee teen pregnnancy rate for NC, N which iss 33.6 per 1,0000.
