Request For Proposals (RFP) Asbestos Abatement P57-400-100000014000, 15000, 16000, 17000, 18000 Lime City Road (.1 miles North of Route 20) (41.536758, -83.568213) Perrysburg Township is seeking a qualified environmental professional or qualified firm to conduct asbestos abatement of the structure on the aforementioned property; the structure was once the site of Lime City School (for site photos refer to Attachment B). A predemolition asbestos survey was completed by Patriot Engineering & Environmental, Inc. and the results are included as Attachment A of this RFP. Scope of Services: Contractors interested in obtaining a contract with Perrysburg Township for providing asbestos abatement shall prepare a written bid to include, but not limited to, the following:  A quote on the removal of the asbestos identified in the asbestos survey report (Attachment A).  Removal and disposition of asbestos shall be in accordance with industrial standards as well as all Federal, State, and local regulations.  All materials identified as asbestos containing must be removed and disposed of by properly trained personnel. Insurance The successful contractor shall provide a certificate of insurance to Perrysburg Township to prove that the contractor has liability coverage for not less than One Hundred Thousand Dollars ($100,000) for property damage, Five Hundred Thousand ($500,000) to any person for any number of claims arising out of a single occurrence for all damages other than property, and an aggregate of One Million Dollars ($1,000,000) for all claims arising out of a single occurrence. The contractor must also provide proof of Workers Compensation insurance or proof that the contractor is otherwise fully complying with the Workers Compensation laws of the State of Ohio. Declaration Regarding Material Assistance/Nonassistance to a Terrorist Organization (DMA) Affidavit Ohio Revised Code Section 2909.21 Terrorism requires that any contract that will result in a bidder receiving funding in an aggregate amount greater than $100,000 annually shall certify that it does not provide material assistance to any organization on the United States Department of State Terrorist exclusion list. Included with the bid, the bidder shall complete the DMA Form. Any suggested additional services may be listed separately with reasons for suggestion clearly stated. 2. Provide a written statement outlining the timeline as well as an estimated completion date. 3. Please list clients, including name and addresses, for which your company has completed similar work in the last two years. 4. Include a brief background information on your company, including the closest office location to the aforementioned property site for this project. The following presents details of the scope of work performed. ASBESTOS BACKGROUND INFORMATION The pre-demolition asbestos survey was performed on April 18, 2013 by Susan Blackmore (License #ES35555 – expiration date of November 13, 2013). Suspect asbestos containing materials (ACMs) were collected at the request of Mr. Celley, and all work was performed in accordance with the following regulatory statutes and memorandums: 

The Occupational Safety and Health Administration (OSHA) - Safety and Health Standards for the Construction Industry 29 CFR part 1926.62, Subpart D. Ohio Administrative Code (OAC) Chapter 3745-20, Asbestos Emission Control.

ASBESTOS INSPECTION PROTOCOL The identification of ACMs was accomplished through a visual evaluation of suspect building materials, bulk sampling, and laboratory analysis. Personal Protective Equipment (PPE) and wet sampling methods were utilized throughout the inspection process and all suspect materials were examined to determine condition and friability. The scope of work for the pre-demolition asbestos survey included the following activities:    

Conducting a visual site survey to determine potential ACM and to define homogenous areas. Determining an accurate number of bulk samples to be collected based on information obtained from the visual survey. Collecting bulk samples from suspect ACM based on the visual survey. Laboratory Analysis.

Following the completion of sample collection activities, the bulk samples were delivered to a qualified laboratory along with the appropriate chain-of-custody (COC) documentation. The bulk samples were analyzed utilizing U.S. EPA “Interim Method For The Determination of Asbestos In Bulk Insulation Samples” (Method 600/R-93/116). According to the U.S. EPA and OSHA, “Any material or product containing greater than one percent (>1%) asbestos is designated as an asbestos containing material”. ASBESTOS SAMPLING Twenty four (24) bulk samples were collected and submitted for multi-layered Polarized Light Microscopy (PLM) laboratory analysis of twenty-eight (28) layers. Laboratory analysis of the samples indicates that the following materials are asbestos containing: 

Red/brown 9”x9” floor tile as found on the first floor – 4% Chrysotile and approximately 5,000 square feet (SF)

Black mastic associated with the red/brown 9”x9” floor tile as found on the first floor – 5% Chrysotile and approximately 5,000 SF Window glazing as found on the exterior windows – 2% Chrysotile and approximately 500 linear feet (LF) Thermal system insulation as found on pipe runs within the basement – 15% Amosite and approximately 800 LF Thermal System Insulation (Air Cell) as found on a vertical pipe within the basement – 40% Chrysotile and approximately 5 LF Thermal System Insulation as found on an elbow within the basement – 5% Chrysotile, 10% Amosite and approximately 5 elbows. Thermal System Insulation as found on an elbow within the basement – 10% Chrysotile, 10% Amosite and approximately 5 elbows.

For full analytical laboratory data and COC documentation please refer to the Attachment A. Site Photographs are provided as Attachment B. CONCLUSIONS AND RECOMMENDATIONS The results of the pre-demolition asbestos survey indicate that the red/brown 9” x 9” floor tile and associated black mastic throughout the first floor, the window glazing on the exterior windows, and thermal system insulation on pipe runs and elbows throughout the basement were identified with a percentage of asbestos that is >1%. The asbestos containing thermal system insulation is considered regulated asbestos containing material (RACM). Therefore, this material must be removed and properly disposed of by an Ohio Department of Health (ODH) licensed asbestos abatement contractor prior to demolition activities. The estimated cost of the thermal system insulation abatement work is $17.00 per linear foot ($13,685) with an additional $1,500 for the abatement work on the elbows, thus totaling an estimated cost of $15,185. The floor tile and the window glazing is considered Category I non-friable materials and may remain within the building during traditional demolition (bull dozer, excavator, etc). In addition, no special considerations for asbestos disposal need to occur. Please note that if traditional demolition does not occur, and if activities such as chipping, cutting, sawing, grinding, or abrading occur on these materials, Patriot recommends utilizing the services of a licensed asbestos abatement contractor to remove and dispose of these materials prior to demolition.

Patriot did not assess the roof of the building and did not sample all of the roofing materials. A sample of the roofing material that was detached from the building was sampled and determined to be non-asbestos containing. The roofing material is considered Category I material that can be left in place during demolition and disposed of as general construction debris. An ODH licensed asbestos abatement contractor must conduct abatement work related to friable materials in accordance with federal and state regulatory requirements. The owner/operator of the building or the demolition contractor must submit a notification to the Ohio EPA and ODH ten business days prior to the start of abatement activities if certain quantity thresholds are exceeded. The ODH abatement notification is required once a total threshold of greater than 50 square feet or 50 linear feet of friable ACM is being removed per project. The EPA notification is required once a total threshold of 160 square feet or 260 linear feet of friable materials are being removed per facility. Based upon the estimated linear footage of TSI, a notification is required. Patriot made a good faith effort to identify RACM throughout the building. The asbestos inspection did not include identification of materials which may be concealed, such as between walls or buried. If suspect RACM not identified in this report are encountered during demolition activities, Patriot should be contacted for inspection and possible sampling for laboratory analysis. Request For Proposals (RFP) Building Demolition P57-400-100000014000, 15000, 16000, 17000, 18000 Lime City Road (.1 miles North of Route 20) (41.536758, -83.568213) Perrysburg Township is seeking proposals for the demolition of the structure on the aforementioned property; the structure was once the site of Lime City School. Structure may be physically inspected on May 21, 2013 between 9:00 a.m. and 3:00 p.m.

Scope of Services: Contractors interested in obtaining a contract with Perrysburg Township for providing building demolition shall prepare a written bid to include, but not limited to, the following: 1. The contractor shall leave the utility shed on the property untouched but shall remove the old school structure including all items within the building, exterior foundation walls (per Option 1 or 2*), removal of all other walls, partitions, beams, or other projections, basement floors and all footings. In addition, all concrete walkways, steps, miscellaneous shrubbery, etc. will be removed. 2. All utilities and wires (gas, electric, water, sewer, and telephone) shall be disconnected per rules/regulations of authorities having jurisdiction. 3. The contractor will be responsible for pulling and paying for all necessary permits 4. The contractor will take whatever steps necessary to monitor the road for debris. 5. Upon completion of demolition, sufficient filling and grading shall be done to bring the area up to a grade level to ensure positive drainage. 6. All rubbish or debris resulting from demolition work shall be removed from the premises during and or upon the completion of work, leaving the site area acceptable to Perrysburg Township. 7. The contractor will provide receipts certifying disposal of materials at a legal landfill or recycling center (where applicable). 8. Owner intends to retain paved parking area for future use. Reasonable measures shall be taken to avoid damage to pavement. 9. Insurance The successful contractor shall provide a certificate of insurance to Perrysburg Township to prove that the contractor has liability coverage for not less than One Hundred Thousand Dollars ($100,000) for property damage, Five Hundred Thousand ($500,000) to any person for any number of claims arising out of a single occurrence for all damages other than property, and an aggregate of One Million Dollars ($1,000,000) for all claims arising out of a single occurrence. The contractor must also provide proof of Workers Compensation insurance or proof that the contractor is otherwise fully complying with the Workers Compensation laws of the State of Ohio. Declaration Regarding Material Assistance/Nonassistance to a Terrorist Organization (DMA) Affidavit Ohio Revised Code Section 2909.21 Terrorism requires that any contract that will result in a bidder receiving funding in an aggregate amount greater than $100,000 annually shall certify that it does not provide material assistance to any organization on the United States Department of State Terrorist exclusion list. Included with the bid, the bidder shall complete the DMA Form. Affixing a signature on the DMA of the bid, the bidder certifies that it does not provide material assistance to any organization on the list, and that failure to complete the form or answer “yes” to any question shall serve for the purposes of this RFP_Bldg_Demolition

affidavit as a disclosure of the provision of assistance to an organization that is listed on the terrorist exclusion list: (http://www.homelandsecurity.ohio.gov/dma_terrorist/Terrorist_exclusion_list.pdf ). Content and Format of Proposals: The bidders interested in this project shall submit one (1) PDF copy of the proposal package to meet the requirements below. The proposal package shall include, but not be limited to, the following: 1. Provide an itemized fee schedule for the requested services; include any and all costs associated with service, materials, permits, etc.) Any suggested additional services may be listed separately with reasons for suggestion clearly stated. 2. Provide a written statement outlining the timeline as well as an estimated completion date. 3. Please list clients, including name and addresses, for which your company has completed similar work in the last two years. 4. Include a brief background information on your company, including the closest office location to the aforementioned property site for this project. 5.
